Struck by falling object

PILLAR WORK IN THE NO.2 ENTRY THE LAST LIFT IN THE RIGHT BLOCK WAS FINISHED AND THE MINER OPR. AND HELPER WAS BACKING THE MINER OUT. A PILLAR FALL OCCURED AND FELL BEYOND THE MINER WHITCH CAUG HT THE HELPER INBY THE CORNER OF THE SOLID PILLAR BEHIND THE MINER.

Struck by falling object

CONTINUOUS MINER EXTRACTED ONE LIFT OUT OF PILLAR, BLOCK #12. AS EE AND C0-WORKER ATTEMPTED TO INSTALL THE FIRST POST, THE ROOF FELL, RODE OVER THE BREAKER POST STRIKING HIM, CAUSING FATAL INJ URIES.
