Employer was notified by compensation administrator that the employee had a diagnosis of pneumoconiosis on 9-14-15.

Employee was diagnosed with pneumoconiosis on 7-21-15.

Employee was installing a rubrail on the left miner. He was using left hand to push the hoses over so the rubrail would close. As the rubrail closed against the miner it pushed a piece of rock against employees left ring finger and the hoses causing a laceration and a fracture to the tip of the finger

An unplanned ignition of gas occurred in the 7 right face on 6/4/2015. Ignition occurred during loading of the third shuttle car when the head of the continuous miner came into contact with the bottom. The operator shut down the miner and the flame above the ripper head was still present. He reactivated the sprays and the flame disappeared. No injuries were reported.

Employer was notified on 9/16/15 by the compensation administrator that the employee was diagnosed with pneumoconiosis on 5/24/15.

EE was cleaning top of power center. He picked up a bag of rock dust & twisted to his right to drop it on the ground & felt a sharp pain in right side of lower back. He went to the foreman & stated ""I may have hurt my back."" Foreman asked if he needed to go outside. He said ""No, I'll be OK."" He completed his shift. He went to the hospital the following day due to pain.

Employer received a pneumoconiosis claim on 2/19/15 noting an initial diagnosis on 2/10/15.

Employee was pulling a rib in the number 5 to 6 crosscut (near spad 6756). Tripped/slipped while pulling rib and pulled rib on him.

A roof fall occurred in the crosscut one break inby spad 6929, on the K-10 Panel. The roof fall will not be cleaned up. All approaches have been cribbed/timbered and dangered off.

EE was at section feeder running rock through feeder when he attempted to hit steel bar to activate e-stop. The steel bar slid out by and did not hit e-stop. EE hit e-stop cable which caused a rock to flip over and pinned his hand and cut it.

A roof fall occurred in the #9 entry at spad 6879. The roof fall will not be cleaned up. All approaches have been cribbed and dangered off.

Hanging face curtain in #2 entry on 4ft ladder. Ladder leg buckled and employee fell from the 3rd rung on the ladder landing on elbow forcing shoulder upward.

Loading tire - tired to move onto pallet, slipped and hit chin

Bolt man went to walk past the bolter after hanging his curtain and tripped and fell into bolt machine.

Employee was re-entering a miner cable into the machine when he felt a sharp pain in his back.

Employee was cutting the strut on miner with a torch - Flame caught pan jack hose on fire, burning his nose and right ear - Drove himself to hospital

Welding on a Joy 14-9CM when a piece of hot material went into boot causing a burn on the inside of the left calf area causing a burn. Was not reported until 8/18/13.

Miner operator sitting inby red zone on #4 CM when miner swung toward him pinching him between machine & rib.

A piece of draw rock fell out of the top and struck operator on top of the left hand

Victim was getting rock off miner felt pain in lower abdomen.

Next to forklift charger loading bags dust or rock dust right testicle felt like it tore. Another EE was loading the same time as IEE was loading.

Victim was plastering on intake stopping when he stood up he felt a sharp pain in his right thigh.

Working on end loader hit from behind by cylinder and knocked into frame.

Supplying roof bolter, carrying roof bolts from scoop to bolter. Twisted and felt sharp pain in lower right side of back

Cutting belt strings off bottom roller, utility knife slipped and cut top of left hand.

Slipped getting out of 602 Scoop and straddled seat and frame scrape in groin area.

While holding cables to hand up, stepped off small ledge, left knee bent backwards.

He was getting out of shuttle car and tripped over his own two feet, landing on his left wrist.

Bolting with twist steel and it hung up trying to pull steel out of top some how canopy hit him on the back of his head, was nothing wrong with the jacks.

Injured employee was lifting the miner cable up to be hung by another employee. When he said he felt a pain in his neck and shoulder.

Laying track tightening up bolts in fish plates pulled muscle in lower part of stomach.

Was loading oxygen tank - oxygen slipped and smashed little finger on left hand between tand and frame of mantrip.

Employee was walking up steps at office and supply room when he felt sharp pain in lower back and legs - knees.

The diesel mantrip was traveling into the mine with the crew aboard. It fell off the rail jarring the employee who complained with pain in his lower back.

The raw coal transfer bin was stopped up with wet coal and roadway gob. The injured employee was hosing coal and rock from the bin into the feeder when several tons of wet coal broke loose filling the feeder which employee was standing in covering his feet and knees & twisting his body. Did not start missing work until 7/14/2010

EE was taking down overcast, cinder block fell on his thumb.

Employee did not report accident to company untill 6/14/10. Employee stated on 6/10/10 he was lifting oxygen cylinder and pulled muscle in back. Employee work his normal shift on 6/11/10

Getting chair parts off mantrip.

Unloading belt structure from scoop and mashed and cut left thumb.

A continuation of a roof fall located between #1 and #2 entries at spad 2509. Continued into # 1 entry and fell up to spad #2520. Fall measured 16' high X 95' long and approx. 3500' outby section. Fall was breakered off.

Shoveling tail piece lift shovel of coal and felt burning in groin area.

Employee was walking and stepped on a rock or lump of coal and said he injured his left foot. He walked off section by self and said he didn't want no help. After he got outside he walked in bath house, took his boots off. Didn't want no help.

Roof fall in #8 return entry on panel C-2A 150' inby spad 5382 and 90' outby. Heinzmann jacks set and access to fall dangered off.

EE was moving miner cable out of his way when his hard hat fell off. While he was getting his hard hat a piece of rock-coal fell hitting him on the head.

Right of feeder between 5 & 6 entry. He was taking a shuttle car tire off. Shuttle car tire fell across both feet.

Employee was working on miner cat pads when a loose piece of rib peeled striking him on the left shoulder and lower back.

Roof fall C-18 pillar section (intersection) outby spad # 5240. Completion of pillar block 31-32. Pulled miner outby and was starting to move MRS when top started working. Top fell in covering MRS.

The injured was helping hang the miner cable. When lifting, he felt a sharp pain in his lower back.

A rib roll hit him. His right leg was hurting. Didn't see any injuries.

Was moving a bottle of Blue Wolf degreaser. It dropped and splashed in face of worker.

The roof bolt operator was drilling the outside hole, on the curtain side, when a loose coal rib, turned over striking employee on the lower back and left hip.

Victim was dropping cables for a power move when cable struck him on right hand.

Got scoop deck into rib, pushing coal in deck on him.

Muddy unlevel for ladder, also high top ladder slipped out from under him, EE fell to the ground.

A small piece of coal rib fell against right side of roof bolter, leaving a scratch and bruise near rib cage.

EE was moving rescuers on man trip when he felt pain in his lower back. EE does have a prior back trouble.

EE was operating scoop and hit a bump, bounced up and when he came back down in seat, he hurt his lower back (tailbone).

While riding to section on Battery Powered ride, man was riding backwards and not looking where he was going. Struck his hardhat on a roof bolt, causing pains to right side of head and neck.

EE was observing bad top, EE turned ro run and felt sharp pain in right calf muscle.

Hit the panic bar and was not ready for it to be shot and got turned around and sprained wrist.

Injured was helping move the miner, as he was moving the slack of the miner cable out of the roadway, the coal rib turned over and struck the helper on the right hip and thigh.

Employee was walking along track when he stepped on a wet. Crosstie and slipped and fell back hitting his back on the rail.

EE was hanging cable, when he lost control of cable and the weight of cable pushed right thumb backwards, resulting in break of right thumb.

Hit a bump in roadway or off set, hurt his back about mid way.

Miner had backed out of pillar line. MRS jack was being moved when a bolt head broke off letting a rock 67 x 72 x 6 fall and strike MRS operator in the lower back.

Injured was putting waterline together. He was pulling with channel locks. They slipped and hit other waterline which had a jagged edge. When locks slipped, hand caught the edge.

Employee was loading miner cable and twisted right knee.

Injured employee was driving a pin in a jack thru the bracket with a hammer when he struck his right thumb.

Injured was in process of changing out bad 48"" top rollers. Bad roller had already been removed & put on flat car. He was installing new roller, lifting up on top rail when he felt a pain in lower back.

Roof fall #4 intersection LOB. Fall measured 25' x 20' x 20'. All entries cribbed off leading to fall will not be cleaned up.

Injured operates a 12-12 Joy continuous miner. When moving the cable beside the miner, he struck his left elbow against the cable standoff on the miner.

An intersection in the left return 90 feet outby spad #3024 fell for a distance of 60'x18'x25' high, 9000 ft outby the mains seciton.

Getting into shuttle car, he hit rib with side. Rib roll hit him in right knee.

Victim was cleaning on #5 belt. Piece of rock fell from between roof bolts striking victim in the head.

Injured was traveling the intake escapeway on rubber-tired mantrip. Trip hit a rock and bounced. Injured said he felt pain in lower back (report 2 days late).

Roof fall app 800' outby face in the entry app 200' and #7 entry old works 20x20x21 cribbed all approachis to fall and danger off.

Employee called, said he hurt his back. He was going to doctor. Does not know when he was hurt.

He lost balance while standing in scoop bucket and fell across scoop bucket: contusions of leg and elbow.

Roof fall #6 entry xcut lt and rt to #5 entry and #7 entry in old works. Fall 20' W x 80' long x 14' high. Will not be cleaned up. This part of mine in process of being sealed.

Victim was walking down steps of bath house and slipped and fell on his right hip

Employee was repairing bolter in deck of bolter & another employee was putting supplies on the bolter & he dropped a box of bolt glue on his foot.

EE was operating roof bolter when a piece of rock fell out between bolter. The rock hit the EE on the side of his head and neck.

Running S/car hit bump jam neck into canopy

EE was walking to his shuttle car, when he stepped in a hole and twisted his left ankle.

Cleaning on #14 belt, picked up a full shovel of mud and coal, felt pain in lower right side of back.

The injured was working on #3 SC in the #6 entry, when a rock 1'x8""x4"" fell from the top, sliding down the right shoulder and right side of body. Bruised the right index finger and striking the right leg. No injuries other than the bruised finger and sore right leg were found.

The injured is a roofbolter helper when stepping over the cable he stepped on uneven slick place of mine floor.

A fall outby the face 10.500ft in the intake escaping when roof intersection in #4 entry 1 x cut inby spad #2854 back and falling. this area was supported with 6 in turpike fulley connected with 5 cable bolts in a stay pattern cribs & jacks were installed 20x30x15.

The injured employee was operating a remote continuous miner. A piece of line curtain was burried in gob that had been scooped into a fresh cut. When the gob with the curtain came across the end of the conveyor into s.c. it slung coal & mud into miner operators face, filling his eyes with coal.

Injured was walking beside roof drill, falling over coal & loose material lying on floor. He turned to catch himself on side of the machine, falling & twisting his right leg. After the fall he complained about his upper rt leg and hip hurting from a sharp pain in his hip.

Injured was dragging a dust hose from a track mounted tank duster over & thru a crosscut to #9 belt head to start dusting on #9 belt. He stepped on an uneven ledge in the bottom, turning his right ankle resulting in a broken rt ankle.

When changing a broken bottom belt stand, while lifting with a belt rail as a pry bar to lift up on stand. The injured felt a sharp pain in his lower back.

Making a splice on #4 S/Car beating down rings on splice hammer came off knocked and hit his left thumb and cut it.

Injured employee stated he was on top of miner changing a busted hydraulic hose when he climbed down he hooked his pants leg on cable hook, jerking him sideways twisting his lower back.

A roof fall above the anchor point in the intersection of #6 entry, 1 xcut inby spad #4314, measuring 15' thick, 18' wide and 50' long. Outby the face for a distance of 110 feet 2 xcuts inby dumping point located in #5 entry.

The roof bolt operator was preparing to install a fly board in 2 cross between 7 & 8 entry. A loose coal & rock rib dislodged, sliding down onto his lower left leg, breaking a bone. Rock & rib measured 10' thick, 14"" wide and 27"" long.

Going to change a busted hose on roof bolter. Pulling on hose, came loose, burning in right arm down to end of fingers.

Around 9:30 pm EE stepped in a hole in the inby intersection of the #5 entry and twisted his knee. He finished the shift but as the shift progressed, the swelling got worse and so did the pain.

BEATING ROCK WITH A 3 LB. HAMMER. RIGHT BICEP IN RT. ARM STARTED HURTING.

The injured employee is a belt repair man. He was standing on bottom belt putting a 100' piece of belt in on top with a set of channel lock pliers. He lost grip on belt with pliers and fell back- ward off belt striking a 4' water line with his back when landing.

An unplanned roof fall occured in the intersection of the # 6 entry. 180 feet out by spad # 4154. At the #4154 end of the rail in track entry. Fall measures 18' wide 30' long and 12' to 14' thick.

An unintentional roof fall occured in the Entry Intersection of the #6 Neutral Entry at spar # 4148. The fall measured 17' wide 7 to 9 foot high and 40' long. This fall was 900' out by faces in an untraveled Entry. Fall Breaked off and dangered off.

A rock fall unplanned occurred 90' inby 4171 in the intersection of #8 to 9 face. The fall measured 5 to 6' thick x 18' wide and 20' long. Strata consisted of sand rock layered with streaks of coal and fossilized slate.

Said putting a piece of rIb roll on belt. Twisted back, slipped and fell in mud.

Hanging miner cable.

He said suction hose stopped up going to drill pot on bolter. He layed hose across shoulder and was pulling on it. Said both sides were hurting.

EMPLOYEE WAS OPERATING THE HELPER SIDE OF THE FLETCHER ROOF DRILL. HE FINISHED INSTALLING HIS BOLT, WHILE WAITING ON THE OPERATOR TO FINISH HIS BOLT HE SQUATTED BESIDE THE MACHINE TALKING WITH THE FOREMAN WHEN A COAL RIB FELL FROM NEAR THE TOP MEASURING 6"" X 4'X 2', STRIKING HIM ON THE BACK & SHOULDERS CAUSING NECK PAIN.

A roof fall 12'x17'x40' fell from edge of intersection in #4. #4 Entry. at spad #4087 into x cut toward #5 entry. For 40' crushing the intake stopping. Hinzeman jacks and cribs were installed to braker off fall area.

EMPLOYEE WAS WALKING IN #2 ENTRY WHEN A RIB ROLL HIT HIM AND COVERED HIM UP.

He was bolting top; a rock came down drill steel, hit him on shoulder and right foot.

The #1 mantrip was parked at end of track where men had unloaded. The park brake leaked off. The #1 mantrip ran app 800' down track rounding a curve where it met #3 mantrip traveling toward section with a crew of men. The mantrips struck each other. The impact caused a whiplash effect on men riding in #3 causing neck and back pain.

WHILE RIDING MANTRIP, EMPLOYEE WAS FACING OTHER WAY THAN TRAVEL WHEN HE TURNED AROUND HIS HEAD CAME IN CONTACT WITH OVERCAST, LACERATION OF FOREHEAD.

ROOF FALL IN THE RETURN, OUTBY FACE AREA OF THE 001 MINING UNIT APPROX. 1,000 FT. THE FALL 1 X-CUT INBY SPAD 3966 - FROM X 2 ENTRY ACROSS 3 # 4 OVER TO AND INCLUDING INTERSECTION TO SPAD #3973. FALL SIZE: 15' HIGH X 18' WIDE X 240' LONG.

Employee was removing bolts from a bearing to a dump roller on #8 belt head with a 3/4 drive ratchet and socket. When he applied force to the bolt, the socket came off bolt head. He fell backward jamming his right wrist into bottom.

Letting head down, he had his hand on drill pot. Got fingers between drill pot and drill pot arms.

Roof fall #4 entry 2nd out by face in intake at spad #3901 intersection fell in blocking roadway. Fall area 18' by 18' by 10'. All approach ways to fall breakered.

UNINTENTIONAL ROOF FALL IN LT RETURN AT SPAD #3010 BETWEEN #1 ENTRY & #2 ENTRY. FALL APPROX 20' LONG - 18' WIDE - 7' HIGH. FALL WAS FOUND AT 12:15 PM. FALL WAS SUPPORTED ON ALL APPROACHES AND DANGERED OFF. FALL WAS AT BRK 175 LEFT RETURN.

Shoveling belt when he pulled something in back.

EMPLOYEE WAS CLEANING AROUND #8 BELT HEAD THAT WAS GOBBED OFF AND TRIPPED OVER TURNBUCKLE AND FELL TWISTING BACK AND LANDING ON LEFT SHOULDER & ARM. WENT TO HOSPITAL - TURNED IN A PREVIOUS CLAIM ON BACK & SHOULDER, AGGREVATING PREVIOUS INJURY.

Roof fall in intersection across from spad 1575 (to the rt of spad #5 entry)fall is approx. 15 wide x 22' long by 6-8' high will not be cleaned up-will be timbered off. knocked out part of stopping developed approx. March of 2002

Bolting top in face when he lowered head on bolter a piece of drill steel fell out of hole and hit him on lower left arm, causing a bruise and contusion.

Miner operator was loading end of 8 rt on main section 1 break in by spad 3748. Miner operator was loading gob and became pinned against rib approx 3' inby standoff. Injury to pelvic area and legs.

ROOF FALL ON OLD 1ST LEFT PANEL #4 ENTRY - X-CUT TO THE LEFT OF SPAD 1430 LOOKING TO OLD FACES. FALL IS APPROX. 80' LONG X 20' WIDE X 10-12' HIGH. NO VENTILATION OR TRAVEL WAY DISTURBED. DEVELOPED APPROX. MAY 2002.

EE WAS HELPING PUT STRUCTURE BACK IN BELT AND WAS PRYING BELT UP WITH A 4X4 POST AND IT BROKE AND HIT LEFT KNEE ON BOTTOM AND 2ND EMPLOYEE THAT WAS HELPING HIM FELL ALSO AND PARTIALLY LANDED ON HIM

Employee was operating a scoop on the roadway on 1st right parallel with A belt and hit a hole in the roadway. He jammed his head and neck into the canopy, having pain in neck on the right side.

Employee was leaning on scoop bucket loaded with block and the bank broke and caught his hand against the scoop. Bucket and block mashed his right hand, breaking index and little fingers.

Roof fall 1 break outby spad 3048 between 5-6 entries. Fall is 40' long x 11' high x 19' wide. Above anchored torque tension bolts used (6'). Will not be cleaned up - cribbed off. No ventilation disturbed.

Roof fall 1st Rt section LOB 6-7, fall is above anchorage - 6' torque tension bolts were used. Developed week of 4-11-05 to 4-16-05 fall is approx 8' high by 18' wide by 30' long. 1 brk in by spad 3717 in #6 entry.

Employee was getting bottle of oxygen and stepped in hole & twisted left knee while helping to replace wheel unit on #3 s/car.

According to worker's compensation, employee stepped on a crib block which turned over and he fell forward on his arm and knee.

Roof fall #1 entry return (spad #3024 #2 entry). Fall was approximately 20' x 20' x 15' & about 1000' from face.

Employee was operating the roof bolter of the 000-3 mmu (1st right). At 9:00 pm he came to the section boss and stated he had been struck by a rib roll. He was bolting at the mouth of the #2 entry. He was taken outside and by ambulance to a hospital.

While mining face #5 entry rib rolled over and struck employee on right shoulder. After exam was made he wanted to go get xrays done.

7-8 crosscut and 6 rt ee was moving miner cable and water line in 7-8 crosscut and felt pain in lower back and continued working and 2 hours later in 6 rt he was transferring plates on bolter and reaching far over bolter and lifted plates and back gave out after feeling something pop and he went to the ground.

EE WAS STANDING AT THE REAR OF THE ROOF BOLTER WHEN CAP COAL AND ROCK FELL, STRIKING THE BOLTER AND SLID OFF AND STRUCK LEFT FOOT CUTTING AND MASHING 4 TOES. EE WAS SENT TO RALEIGH GEN. ROCK WAS APPROX 3' WIDE, 4' LONG, RANGING FROM 18"" UP TO 4"" THICK. TRANSFERRED TO MORGANTOWN TO SEE SPECIALIST.

Scraping bottom from #4 entry to #1 entry. EE was helping with cable on the 14-15 Joy miner. He bent over and was trying to pull cable over one side out of the shuttle car's path. He said he felt like something pulled in his back.

Employee was helping make blet splice on #6 belt, 1 brk out by tailpiece. EE was beating splice with hammer and had a pain in neck, shoulders, and halfway down spine. EE finished shift.

#6 ENTRY ON 1ST LEFT OFF 5 BELT ACROSS FROM SPAD 2540. ROOF FALL IS APPROX 8-10' HIGH X 60' LONG X 20' WIDE, FELL ABOVE ANCHORAGE. DEVELOPED AROUND OCT 2003 6' T0RQUE TENSION BOLT WERE USED. NO VENTILATION OR TRAVEL WASYS WERE INVOLVED. WILL NOT BE CLEANED UP, WILL BE CRIBBED OFF. FALL IS APPROX 1.5 MILES FROM MAIN FACE AND APPROX 1 MILE FROM 1ST RT FACES.

ROOF FALL #3 ENTRY ACROSS FROM SPAD 2854. FALL WILL NOT BE CLEANED UP. IT WILL BE DANGERED OFF AND TIMBERED OFF. NO VENTILATION DISTURBED. DEVELOPED APPROX NOV 2003, FALL IS 60'X20'X8-10', APPROX 8000' FROM FACE AND APPROX 3000' FROM RT FACE

EE WAS GOING INSIDE IN MANTRIP AND BRAKES FAILED. EE JUMPED OUT OF RIDE, JAMMED LT KNEE AND LT HIP WHEN EE HIT BOTTOM. HAD SORENESS BELOW RT KNEE WHERE IT HIT MANTRIP. EE STATED HAVING MUSCLE SPASMS IN HIP AND LOWER BACK.

MAINS SECTION, #8 ENTRY, 5 BRKS OUTBY FACE, 1 BRK OUTBY DUMPING POINT. ROCK FALL 1 BRK ACROSS FROM SPAD 3343. FALL IS APPROX. 20' WIDE X 24' LONG X 10' HIGH. WILL NOT BE CLEANED UP - TIMBERED OFF. MSHA INSPECTOR WAS ON SECTION AND CHECKED IT OUT. DEVELOPED WEEK OF 10/4/04 TO 10/11/04.

Walking toward #9 heading employee's leg went from under him, causing him to fall. He hit his knee on a rock.

EMPLOYEE WAS TAKING LUG NUTS OFF SHUTTLE CAR TIRE AND WAS PUSHING DOWN ON 3/4 DRIVE RATCHET AND A KNOT DELEVOPED IN RIGHT GROIN AREA, BECAME SORE. HE WENT TO DOCTOR WHO SAID IT WAS AN INFECTION. DID NOT EXPLAIN TO EMPLOYEE WHY KNOT DEVELOPED.

EE WAS WALKING TOWARDS THE FACE AREA AND STOPPEDTO SAY SOMETHING TO A FELLOW WORKER WHEN RIB FELL STRIKING HIM ON HIS LEFT SIDE KNOCKING HIM TO THE GROUND. THE COAL RIB STRUCK HIS RIGHT FOO T AND KNOCKED HIM DOWN.HE STATED IT WAS PAINFUL BEHIND THE TOES AND TO THE MIDDLE OF HIS FOOT MAINLY ON THE SIDE. ROOF FALL IN INTERSECTION IN NEUTRAL ENTRY AT MOUTH OF 1ST LEFT SECTION AT SPAD 1403. FOUND WHILE CHECKING PUMPS. AREA CRIBBED OFF. WILL NOT BE CLEANED UP.

STEPPED IN A HOLE CARRYING A CINDER BLOCK. HE TWISTED HIS RT. ANKLE. WENT TO THE DR., HE IS ON CRUTCHES BUT IT IS NOT FRACTURED.

EE WAS PLASTERING A STOPPING BETWEEN #6 AND #7 ENTRY, SPAD 1479-1980. SHUTTLE CAR OPERATOR MADE A WRONG TURN FROM HIS ROADWAY HITTING THE INJURED EE WITH THE SHUTTLE CAR AND PUSHED HIM THRU ST OPPING.
