Employer was notified by compensation administrator that the employee had a diagnosis of pneumoconiosis on 9-14-15.

Employee was diagnosed with pneumoconiosis on 7-21-15.

Employee was installing a rubrail on the left miner. He was using left hand to push the hoses over so the rubrail would close. As the rubrail closed against the miner it pushed a piece of rock against employees left ring finger and the hoses causing a laceration and a fracture to the tip of the finger

Accident type, without injuries

An unplanned ignition of gas occurred in the 7 right face on 6/4/2015. Ignition occurred during loading of the third shuttle car when the head of the continuous miner came into contact with the bottom. The operator shut down the miner and the flame above the ripper head was still present. He reactivated the sprays and the flame disappeared. No injuries were reported.

Employer was notified on 9/16/15 by the compensation administrator that the employee was diagnosed with pneumoconiosis on 5/24/15.

EE was cleaning top of power center. He picked up a bag of rock dust & twisted to his right to drop it on the ground & felt a sharp pain in right side of lower back. He went to the foreman & stated ""I may have hurt my back."" Foreman asked if he needed to go outside. He said ""No, I'll be OK."" He completed his shift. He went to the hospital the following day due to pain.

Employee was pulling a rib in the number 5 to 6 crosscut (near spad 6756). Tripped/slipped while pulling rib and pulled rib on him.

A roof fall occurred in the crosscut one break inby spad 6929, on the K-10 Panel. The roof fall will not be cleaned up. All approaches have been cribbed/timbered and dangered off.

EE was at section feeder running rock through feeder when he attempted to hit steel bar to activate e-stop. The steel bar slid out by and did not hit e-stop. EE hit e-stop cable which caused a rock to flip over and pinned his hand and cut it.

The injured employee was hosing coal and rock from the bin into the feeder when several tons of wet coal broke loose filling the feeder which employee was standing in covering his feet and knees & twisting his body. EE was moving miner cable out of his way when his hard hat fell off. While he was getting his hard hat a piece of rock-coal fell hitting him on the head.

Injured was traveling the intake escapeway on rubber-tired mantrip. Trip hit a rock and bounced. Injured said he felt pain in lower back (report 2 days late).

The injured employee was operating a remote continuous miner. A piece of line curtain was burried in gob that had been scooped into a fresh cut. When the gob with the curtain came across the end of the conveyor into s.c. it slung coal & mud into miner operators face, filling his eyes with coal.

Injured was walking beside roof drill, falling over coal & loose material lying on floor. He turned to catch himself on side of the machine, falling & twisting his right leg. After the fall he complained about his upper rt leg and hip hurting from a sharp pain in his hip.

Injured was dragging a dust hose from a track mounted tank duster over & thru a crosscut to #9 belt head to start dusting on #9 belt. He stepped on an uneven ledge in the bottom, turning his right ankle resulting in a broken rt ankle.

A roof fall above the anchor point in the intersection of #6 entry, 1 xcut inby spad #4314, measuring 15' thick, 18' wide and 50' long. Outby the face for a distance of 110 feet 2 xcuts inby dumping point located in #5 entry.

The roof bolt operator was preparing to install a fly board in 2 cross between 7 & 8 entry. A loose coal & rock rib dislodged, sliding down onto his lower left leg, breaking a bone. Rock & rib measured 10' thick, 14"" wide and 27"" long.

Going to change a busted hose on roof bolter. Pulling on hose, came loose, burning in right arm down to end of fingers.

Around 9:30 pm EE stepped in a hole in the inby intersection of the #5 entry and twisted his knee. He finished the shift but as the shift progressed, the swelling got worse and so did the pain.

The injured employee is a belt repair man. He was standing on bottom belt putting a 100' piece of belt in on top with a set of channel lock pliers. He lost grip on belt with pliers and fell back- ward off belt striking a 4' water line with his back when landing.

An unplanned roof fall occured in the intersection of the # 6 entry. 180 feet out by spad # 4154. At the #4154 end of the rail in track entry. Fall measures 18' wide 30' long and 12' to 14' thick.

An unintentional roof fall occured in the Entry Intersection of the #6 Neutral Entry at spar # 4148. The fall measured 17' wide 7 to 9 foot high and 40' long. This fall was 900' out by faces in an untraveled Entry. Fall Breaked off and dangered off.

A rock fall unplanned occurred 90' inby 4171 in the intersection of #8 to 9 face. The fall measured 5 to 6' thick x 18' wide and 20' long. Strata consisted of sand rock layered with streaks of coal and fossilized slate.

Employee was removing bolts from a bearing to a dump roller on #8 belt head with a 3/4 drive ratchet and socket. When he applied force to the bolt, the socket came off bolt head. He fell backward jamming his right wrist into bottom.

Employee was leaning on scoop bucket loaded with block and the bank broke and caught his hand against the scoop. Bucket and block mashed his right hand, breaking index and little fingers.

Roof fall 1 break outby spad 3048 between 5-6 entries. Fall is 40' long x 11' high x 19' wide. Above anchored torque tension bolts used (6'). Will not be cleaned up - cribbed off. No ventilation disturbed.

Employee was operating the roof bolter of the 000-3 mmu (1st right). At 9:00 pm he came to the section boss and stated he had been struck by a rib roll. He was bolting at the mouth of the #2 entry. He was taken outside and by ambulance to a hospital.

While mining face #5 entry rib rolled over and struck employee on right shoulder. After exam was made he wanted to go get xrays done.

Over-exertion in lifting objects

7-8 crosscut and 6 rt ee was moving miner cable and water line in 7-8 crosscut and felt pain in lower back and continued working and 2 hours later in 6 rt he was transferring plates on bolter and reaching far over bolter and lifted plates and back gave out after feeling something pop and he went to the ground.
