A dragline oiler had been walking the 1570 dragline. He was last seen near the end of a walkway and was later found between the shoe and the revolving frame. There were no witnesses to the event. The employee was fatally injured.

At approximately 3:30pm employee assisted another employee in changing cutting edges on a dozer. After changing the cutting edges the employee returned to operating the dozer for the rest of the shift. The employee stated that on the drive home, EE began to feel bad. EE's wife took EE to the hospital where EE was treated for heat exhaustion and other personal medical issues.

Employee was disconnecting a caustic line from a gasoline powered pump. After releasing the cam locks on the intake hose caustic sprayed onto the employee's ppe apron from being under pressure. The employee then removed the apron by pulling it off over EE's head causing the caustic to get on EE's chin and a small amount came into contact with the employee's arm.

The employee had exited backhoe to help make a pipe connection and was in the process of remounting the machine. After placing right foot on the step, the employee used the grab handles and right leg to step left leg onto the track. When this happened, the employee felt something pull in right leg.

Per the Dolet Hills incident report, an employee of the Dolet Hills Safety Dept was notified of the fire at approximately 03:00a.m. MSHA was called at 03:13 a.m. Per their incident report, the fire was extinguished at 03:36 a.m. The truck which caught fire was operated by a trucking co.
On 2/2, employee flipped a metal plate (45#) from track. 45 minutes later, EE reported to Supervisor that EE may have messed back up. EE refused treatment & said was okay twice during the shift. Employee had back surgery last year. The mine received doctor's notes on 2/25 that EE's back doctor blamed pain on employee flipping plate. Employee has a stretched nerve in back.

Employee was diagnosed with a fractured wrist from an incident that occurred on 11/11. EE refused medical treatment at the time of incident. The employee was removing a lower roller when a bolt snapped, during loosening, causing the roller to push the hydraulic wrench and employees hand backwards. Employee continued to work until asking to seek medical attention due to swelling.
The employee was using a 10"" X 1.5"" brass bar to bump a bearing race into a final drive hub. The employee unexpectedly missed the race, causing little finger to get pinched between the bar and race. The employee was wearing gloves at the time of incident. The employee fractured the tip of little finger.
Employee was on break in the personal parking lot. EE was leaning on the rear of another employee's personal pickup. EE turned to the right to walk and tripped over the receiver hitch on the pickup. Employee landed on the sidewalk.
Employee slipped on wet surface in well field, landing on tail bone. Employee reported to Supervisor & continued to work. On 9/12, the employee requested a medical evaluation. Employee was released to work without any restrictions. On 9/19 employee experienced swelling in knee. Employee returned to the doctor and was given 3 days of restricted duty beginning on 09/20/2019.
Additional backup support was needed to break loose a drill stem connection that was very tight. The pipe wrench used as backup support slipped and pushed EE's hand into an expanded metal guard that had an exposed edge. The exposed metal cut through the glove and cut the bottom side of employee's right ring finger.
During the process of loading up a 2-inch discharge hose some residual fluid in the hose splashed on ee face from the end of the hose and ran down behind safety glasses into ee eye. The fluid was wiped away and then rinsed with fresh water. There was no pain, discomfort or indication of injury so ee returned to work. Later that evening some irritation was noticed.

Employee was working with three others scraping dirt off the front of a dump bed to prepare for liner replacement. EE turned slightly on the metal surface, when EE slipped, fell backwards and automatically reaching out with EE's left hand to catch self. EE felt pain in EE's wrist. EE suffered a fracture in the left wrist.
Mechanic was called to jump start a dozer. After handing the cables to the dozer operator, the employee turned to the other dozer. After taking two steps, the employees left leg slipped under the employee, causing the employees left knee to hit the ground. The area had been skimmed off. Misting rain had begun 3 hours prior with heavy rain forecasted for later.
A red glow was spotted by the stockpile dozer operator. The Supervisor stopped the belt. The incident was away from any other structures or miners. The fire department was called to put out the fire. The Fire Marshall determined the cause of the fire was a hot spot in the tail end turnover of the belt. A design problem with the turnover take up pulley is the root cause.

The employee was crimping hose fitting. As employee adjusted the feral, employee's foot inadvertently stepped on the deadman switch pinching two fingers. The employee was wearing gloves.
EE was dressing up an old road area by pushing dirt into a 3ft. washout when the blade aired out tipping toward the right front corner of blade causing the dozer to stop. Upon the verification that EE was wearing seatbelt correctly and because of EE's tall height and thin build allowed upper body to lunge forward causing head to hit the window latch.
An Administrative Assistant was walking back up the driveway, returning from the mailbox, carrying the mail in their left arm. After crossing the highway, they stepped on a small rock. It caused them to lose their balance and fall towards the right side. They tried to catch themselves with the right hand. Both the right hand and elbow landed on the concrete.
EE was winching a generator into place (about 1 ft.) up to the Landoll Dropdeck Trailer to be loaded onto the trailer by an excavator when the chain connected to the generator in which the winch cable was hooked to broke.
Employee was operating pipe fusing machine. EE placed EE's hand on the hydraulic clamp to readjust. At the same time, using EE's other hand, EE operated the controls to close the clamp. The inside of EE's thumb was pinched between the pipe clamp and clamp support.
Employee was operating a 40 ton articulating truck moving overburden before the dragline begins. As employee entered the dump area, the front wheel went into soft ground or a rut, causing the employee's upper body to sway in the seat. The employee felt a pop in employee's neck.
A low wall in the first box pit fractured letting overburden material sluff into the pit
While grinding a weld off the bed of the dump truck utilizing leather gloves, repositioned ee to a squatting position while grinding in left hand requiring constant hand pressure, lost balance and caught himself with right hand on the deck of the dump truck. The grinder struck the floor and moved striking the top of right hand
End of the shift, EE was climbing down a the equipment ladder of a Cat 777 end dump truck. EE didnt use lunch box rope. When EE climbed down to the bumper, EE moved EE's lunch box from the radiator down to the bumper with EE's left hand. When EE started to climb down, EE lost grip with EE's right hand before EE could gain grip with EE's left hand. EE fell approximately five feet.

Cutting fabric to install in stilling basin. Fabric slipped, forearm contacted blade causing puncture laceration requiring two stitches. Treated at hospital, refused ""post-accident"" drug test. Was reminded of co. drug policy and consequences of refusal. EE still refused. Possibility drug use may have contributed to accident, unable to confirm due to refusal to testing.
The employee was operating #310 water truck. After lunch break, EE noticed that the remote for the water fill had fallen on the floorboard on the right side of the seat. When EE reached for the FOB, EE felt something touch the tip of EE's right little finger, when EE looked down, EE noticed blood. During the investigation with the employee, no sharp objects were found.
Employee had changed oil in D10N bull dozer as employee was putting up the left side engine guard with left hand, at which time the employee felt like left arm got a cramp in it. Employee reported to supervisor that arm was still hurting after about an hour had passed. The employee was taken to the doctor and was diagnosed with a sprain to the bicep muscle in the left arm.
The employee opened the driver's door of a Cat 777 and sat on the edge of the seat. EE's left foot to hold the door opened. To get back into the seat, placed right hand on the top of the door frame to push body around into the driver's position. After moving left foot, a gust of wind blew, causing the door to shut onto EE's thumb
The injured employee was aiding another mechanic with truck repairs. After completing task, the mechanic went to see if parts were available to change out the fender. The injured mechanic decided to go ahead and remove fender. As EE stepped across the fender, the fender brackets broke. The employee slid down the fender, landing onto the shop floor.
Water bottle placed inside a rolled up Hydraulic hose. Employee chose to step in the middle of the rolled up hoses and stepped on the bottle causing EE to fall to EE's knees rolling EE's left ankle.
Employees were transferring brake calipers from one wooden pallet to another wooden pallet. During the process of moving brake calipers one of the employee's finger was pinched between two calipers.

Employee were removing tire ring from haul truck wheel. When the wheel was placed in the upright position the ring shifted. The employee reached to stop the ring from shifting, the tip of his finger was pinched between ring and wheel.
Employee was using a hyd. jack with a 100lbs. metal spacer on top. When jack pressure was released the spacer fell hitting employees hand.
Employee was pressure washing a tractor. The tip of the pressure washer became clogged, and the employee swiped his hand over the tip of the wand and the pressure that was built up in the hose released and discharged water cutting his finger.
Employee was removing a pin from the dragline bucket with a hammer. The injured employee was standing approximately 15 feet behind the area when a small piece of the pin detached and struck the employee standing behind the work site in the forearm.
EE was grinding overhead and the grinding wheel came/broke off and cut through face shield, knocked eye safety glasses out of alignment and hit left eye
Two pumpers were connecting 6"" pipe. The track hoe was connected to the opposite end of the connection. After communicating his intentions, the other pumper began to align the gasket. As he aligned the gasket, the track hoe operator was startled by material movement behind him. This made him hit the machine's swing, causing the two pipes to touch, pinching the employee's wrist.

He was using 6"" boring equipment on the right outboard propel when the event happened. With the equipment on EE misplaced his hand causing it to become wedged between the smooth tube mount and the smooth drive drum. The distance between the tube and the drum is approx. 5/8"". The employee was transported to the ER where he received 7 sutures to his right hand.
Employee was removing the discharge line from an unused 6' water pump. After applying pressure, the hose came loose. The employee lost his balance, falling backwards onto the ground. It was found that sandy materials from the water caused the pipe hard to be removed.
An excavator operator was performing his pre-shift inspection. As he was mounting the 349E Cat excavator, he lost his footing and fell back approximately 1.5 feet landing awkwardly on his left foot spraining his left ankle.
Employee was readjusting a lowboy outrigger board on an empty lowboy trailer. He picked up the board with his gloved right hand and then pushed with his gloved left hand and the pad of his ring finger became pinched/caught between the board and the outrigger stand.
Employee was installing a new OEM pin in the equalizer bar on a Caterpillar D10R dozer roller frame. The employee was using a slide hammer to perform the task. While installing, a small piece of the pin became detached and struck the employee above his knee.
A dozer operator was getting off of a D6T Cat dozer and stepped onto a one foot high wind row and his foot slid down the back side of the wind row causing his right foot to hyperextend.

The dragline crew was re-socketing drag ropes. While inserting the wedge, the employee placed his hands on the upper side to guide it into place. As the wedge slid into place, the pad of the employees right ring finger was pinched between the top of the wedge and bottom of the rope.
The injured employee and a coworker were walking along a dried creek bed holding a pipe that had a roll of barbed wire on it. The injured employee slipped and went down to one knee. When this happened the wire slid down the pipe hitting him in the arm causing three small lacerations.
Employee was replacing cutting edges on a D6H Dozer blade. He was using a 3/4 inch impact with his right hand and his left hand was on top of the blade stabilizing himself. One of the plow bolts spun out and cut his left thumb - through his mechanic impact glove.
Employee stepped on lose rock while checking in vehicle at guard house. Employee's ankle rolled causing a sprain.

Employee was removing c-frame bolts on the lower segment. While swinging a 20lb. sledge hammer, employee made contact with the bolt with the hammer. Upon contacting the bolt with the hammer a pain shot in his upper right arm.
Connecting lift cylinder to blade on D7G Dozer, EE's hand was in path of pinch point while he was lifting up the hydraulic cylinder.
Employee went to a field toilet, when using the restroom he felt a bite like a fire ant in the groin area.
Employee was on the ground on the right side of the dragline preparing to install a cover plate on the propel. Another crew were working on the ""A"" leg platform when a chipping hammer fell from the platform and struck the employee on the arm.
Employee was using 20lb hammer to knock out hoist drum bolts and hit hammer handle on bull gear with pinky finger between handle and gear.
Employee was standing on dozer track checking transmission oil when he had finished he turned to go to the cab, he twisted right ankle.
Two employees were removing the right front suspension cylinder spindle off of a 777C truck. EE was using a 1"" impact wrench. He had 6 of the 9 bolts removed when the remaining 3 bolts broke allowing the spindle to fall. As it fell, the spindle rolled onto EE's left foot.

Employee using impact wrench to remove bottom roller. Loader cribbed with 1' of clearance. He loosened what he believed was the next to the last bolt, the roller dropped. Impact wrench pinched employee's finger against track. It was found that employee had completely loosened the other bolt, but it never fell so he thought it was holding.
EE was holding the front arm of the 16G Motor Grader. Another contract employee was tapping a bushing in with a 3 pound hammer. A piece of the bushing broke off and punctured EE's left arm (inside elbow).
Windy conditions possibly caused a brush pile to reignite and embers were blown over a fire lane. The under brush of 1 acre of wooded area was burned. No miners were in the area.
Employee was working on revolving frame and became ill. He was taken for medical treatment and treated for heat exhaustion.
A fire was started beside Hwy 1 by a fallen public distribution utility line. Due to dry conditions, it spread onto mine property. Local fire department was called to the scene by someone traveling the highway. 0.91 acres were burned. No miners were in the area. MSHA was notified within 10 minutes of recognition.
Employee was pulling slack on 3/4 reaving. Lost his grip and cable came up and hit in the groin area. This made him sick at his stomach and he threw up. He appeared to be over heated as well. Employee was moved to a shaded area to cool down. He was then taken to the hospital for evaluation and received IV fluids. Employee was back to work next shift.
The operator was unloading the Cat 324D track hoe from the low boy trailer. When the operator got the machine to the back of the trailer he lifted the bucket off the ground and the track hoe tipped over on the boom side of the machine. The low boy was parked on uneven ground.
Employee stumbled before exiting restroom door. He landed with a closed right hand hitting the bottom of the door jam. Upon investigation, no tripping hazards were found. There were no witnesses to the incident.
Employee was placing 105 lb. Spreader Bar & Dump Block pins into pin hole and the pin slid into place faster than expected crushing employee's left tip of ring finger between the pin head and the dump block..
A parish approved control burn had been conducted when a small secluded area in the mine away from mining activities became ignited. The fire consisted of only brush being burned in a 2 acre area. The fire was extinguished in 1 hour and 1 minute.
Dozer operator was dismounting dozer, when he stepped backward off dozer blade arm, he felt pain in groin area.
Working on D7E SCG115 in area N. Task was to inspect for an oil leak. Lowered belly pan under front lower radiator. Belly pan bolts snapped under weight of mud collected in pan. Belly pan fell on Kevin's right knee, caused a sprain to right knee and ankle.
After placing a trash bag into a drum, the Oiler pressed down on the plastic bag to deflate. A small wire pierced through the bag and his glove. It made a small mark on his middle right finger. Two days later he experienced pain and swelling in the finger. Treatment for infection was given at the local ER. No work restrictions were given to employee.
Employee was grinding in a small compartment when a flying particle entered his right eye.

Crew decided to place a rope spool on its side. As crane operator lifted, per signal person, top of load began to slighly lean towards the crane. Load was steadied by tagline. As he attempted to exit area, the bottom of load slid towards outrigger, hitting him in back of leg. The motion pushed his leg against outrigger, causing it to be pinched between the two.
The employee was relocating an empty aluminum crate when the incident occurred. As he released the crate, the back of his middle finger came into contact with the aluminum ID tag. He received a minor cut on the back of middle right finger. It was determined by the ER doctor that three stitches were required because of the location of the cut to the first joint of the finger.
Employee was walking from the tractor to his personal vehicle at quitting time and mis stepped causing him to land improperly on his foot. The area was free of all tripping hazard during the time of the incident. When the employee's foot made contact to the ground, it was angled and the force causing a break in his right ankle.
Installing a carrier assembly in the right rear final drive. After I got the carrier assembly on the final I used the boom to hold the carrier assembly in place as I worked the carrier assembly inward. After the carrier assembly was bolted up I went to retract the boom and hit the work switch on the boom control. It swung the end of the boom into my face.

While pulling a length of 3/4"" hose, across the ground, the employee lost his balance. He raised his right arm to steady himself and partially dislocated his shoulder. The employee's right hand or arm never came into contact with anything.
EE claimbing into DL tub, stepped on weedburner he had left in floor and twisted ankle. Mildly distracted fracture thru the tip of 5th metatarsal.

Employee was cleaning up around the shop when he needed to move a rolling toolbox to sweep under it. As the mechanic was moving the box, it rolled into a building column, striking his left thumb. The employee was taken to the ER where he was diagnosed with a small fracture to his left thumb. His thumb was cleaned and he was released.
A small portion of the lower high-wall near the coal seam sloughed off. This caused the employee tending a six inch diesel pump in the center of the pit (approximately 50 feet from the high-wall) to turn suddenly and twist his right knee. He was taken to the ER and evaluated as having a sprained knee and released.
Employee was removing a pump (75lbs). from 785 hauling truck. no strap was used to secure to secure the pump. He had oily hands. The pump slipped, striking left side of head knocking him to the ground.
While rolling up a length of 1 1/4 inch rubber hose at a remote fueling location, the mechanic stumbled and fell. While attempting to catch himself with his right hand, a small rock punctured his palm. Closing the wound required three stitches and a tetanus shot was given.
The hydraulic take up malfunetioned causing the drive pully to slip and create enough heat to ignite the belting on start up. No injuries were involved with the event. The fire was completely extiguished in 41 minutes from discovery.
While removing a drag drum bearing cover the cover slipped causing the employee to twist his back. MRI received no disk damage. A strained nerve is the diagnosis.
EE was struck in the chin by a sliver of metal from a socket wedge being driven out by a sledge hammer appx. 10 ft away. Sliver was removed and chin stitched.
EE was installing the master pin in a set of rails on a truckhoe.This requires driving the pin in with a sledge hammer,as the employee was doing this,a small piece of metal chipped off the end of the pin,hitting the EE on the side of his right leg.EE was taken to the ER requirinf 4 stiches. All proper PPE was being used.
During a grinding operation, a welder removed his full face shield wiped his forehead with his shirt sleeve. He felt something in his left eye. He was still wearing safety glasses. The eye dr removed an embeckled small metalic particle from his eye.

While performing a preshift inspection the hed pulled on the plastic hood latch, the handle broke cutting his finger.
After grinding, the employee wiped his eye with his sleeved forearm. A small metal particle was found by the eye doctor in his eye.
The employee was dismounting a cat d-10-r dozer when he lost his footing and fell to the ground from the blade push arm.
While mounting the crawler type backhoe, the employee slipped from the step to the ground straining his back.
Crew changing-out hoist ropes on 2570 W. Rope slipped free...swung forward and hit EE in the chest... throwing him backwards into the bucket pin. Rope hit EE from shoulder to hip and an abrasion in mid back.
An air motor on a lube pump tube had malfunctioned. The mechanic sprayed oil on the pistons to free them up. When he reattached the air line, the pump started running. His thumb was caught between the piston and the motor housing.
Dewatering well PVC pipe, coupling failed. Holding the cable that lets pipe into well, he had cable in hands and when coupling failed jerked & injured right shoulder.
Employee was dismounting that #1 dragline, descending the steps between the house staircase and the top of the shoe. He slipped and fell, striking his elbow and back.

EE pulled out a rear idler on D10N-6 & had removed the new idler from the crate. He then lowered the old idler into the crate & removed the lifting straps. He then tried to lift the crate w/the used idler, when the crate failed & the idler fell onto his left ankle.
Two mechanics were removing a U-joint from a haul truck and were striking the U-joint with a sledgehammer to remove it from the dragline. One mechanic was swinging the sledgehammer and struck the U-joint when a piece of metal flew off & struck the 2nd mechanic in the right thumb.
Burning cleared brush piles, planned fire got out of control and burned apprx one acre of grass. No injuries and/or property damage.
EE WAS PREPARING TO START THE LUBE TRUCK TO TAKE IT TO THE FIELD. AS HE WAS TRYING TO START THE ENGINE, AN INSECT BIT OR STUNG HIM IN THE LOWER LEFT ABDOMEN AREA. (GIVEN PRESCRIPTION MEDICATION.)

EMPLOYEE WAS WORKING IN THE PIT AS A PUMPER WHEN THE SPOIL SLID ACROSS THE PIT PINNING DOWN THE EMPLOYEE WITH DIRT. THE DIRT PINNED THE EMPLOYEE'S LEG AGAINST A 6"" PLASTIC PIPE CAUSING A BRUISE.
The injured installed a used battery on a 6"" water pump but did not clean/remove corresion from the battery posts. a co-worker turned the key to start the pump. At that time an arc occurred and the battery top failed and the battery cap and other debris flew up and struck him.
Injured was repairing a leak in a discharge pump pipe near a highwall. The pipe was chained off to keep it from going over highwall. The coupler was removed from two sections of pipe and when the injured pulled on the pipe to separate them, the pipe sprang upwards, striking the injured in the upper lip.
Operator of machine working in rough terrain when machine broke over a hump of dirt causing sudden jarring drop causing operator to receive upper back strain.
A FIRE OCCURRED IN THE ENGINE ROOM COMPARTMENT SHORTLY AFTER THE LOADER HAD BEEN FUELED. THE OPERATOR HAD DIFFICULTY AND WAS UNABLE TO START THE FRONT ENGINE. IT APPEARS A FAULT IN THE FRONT ENGINE STARTING SYSTEM MAY HAVE STARTED THE FIRE. THE FIRE ENGULFED AND DESTROYED THE ENTIRE ENGINE ROOM COMPARTMENT.
Employee was driving an empty 110 ton coal hauler back to the pit. He hit a rough spot in the road and when the truck bounced he jammed his lower back.
Injured and a co worker were driving out a pain in the steering boss of a motorgrader. the co worker was helding a track maul against the pin and the welder was striking the maul with a sledge hammer when a piece of metal flew off and struck the welder's right thigh

CONDITIONS WET FOOT SLIPPED FROM STEP AND INJURED LEFT HIP - NO FRACTURES.
A FIRE OCCURRED ON THE OVERLAN CONVEYOR SYSTEM. THERE WERE NO INJURIES RELATING TO THIS INCIDENT. EQUIPMENT DAMAGE INCLUDED CONVEYOR BELTING, STRUCTURE STEEL AND SOME ELECTRICAL CONDUIT.
EE WAS DESCENDING THE STAIRS OF THE 4BELT/2BELT TRANSFER STATION & AS HE NEARED THE GROUND HE LET LOOSE OF THE HANDRAIL. AS HE STEPPED ON THE 2ND STEP FROM THE BOTTOM A WELD FAILED & ONE SIDE OF THE STEP DROPPED 4"" CAUSING HIM TO LOSE HIS BALANCE & FALL APPROX 12"" TO THE GROUND
EMPLOYEE WAS KNEELING DOWNM BENT OVER, TRYING TO PLACE A NYLON STRAP ONTO A SEVEN (7) INCH DIAMETER PIN. WHILE TRYING TO ROLL THE PIN ONTO THE STRAP HE FELT A PAIN IN HIS BACK. EMPLOYEE DID NOT MISS WORK DUE TO THIS INJURY UNTIL MAY 27, 2004
Employee was cutting trees 2"" in diamiter. This particular tree was being cut 6"" above his head. The weight & momentum if the saw caused him to swing down letting the saw come in contact with his leg. Causing him to hit his leg with saw.

EE WAS GETTING OUT OF TRUCK AND CLOSED THE DOOR ON HIS FINGER
THE EE WAS USING A 4 1/2"" PORTABLE GRINDER DURING THE DAY. HE WAS WORKING IN THE CHUTE @ 2A TO 4 CONVEYOR ALSO @ THE TAIL OF #4 BELT. SOMETIME DURING THESE OPERATIONS, 2 METAL PARTICLES GOT IN TO HIS LEFT EYE. HE DID NOT NOTICE ANY IRRITATION UNTIL AFTER SHIFT WHEN HE WAS WASHING HIS FACE. THIS COULD HAVE CAUSED THE METAL TO BECOME STUCK IN HIS EYE. HE WAS WEARING CO. ISSUE SAFETY
EE WAS GRINDING WITH A 4 1/2"" GRINDER. THE EE STATES THAT HE IS UNSURE WHAT HAPPEN. ON FURTHER INVESTIGATION THE GRINDER WAS FOUND TO HAVE A WIRE WHEEL BEING USED TO CLEAN BOLTS. IT APPEARS AS THOUGH THE GRINDER KICKED-BACK. EITHER THE EE ATTEMPTED TO CATCH THE GRINDER WITH HIS FREE HAND OR THE GRINDER KICKED BACKINTO HIS HAND. HE ALSO STATED THAT IT HAPPENED SO FAST HE WAS UNSURE.
WHILE REMOVING THE LOWER INSIDE CABLE PIN, THE EMPLOYEE'S LEFT HAND WAS CAUGHT BETWEEN THE CABLE PIN AND HAND RAIL. WHEN THE PIN CLEARED THE BRACKET, THERE WAS TOO MUCH SLACK IN THE CHAIN OF THE COME-A-LONG ALLOWING THE PIN TO SWING FREE AND AROUND PINCHING HIS HAND AGAINST THE HAND RAIL.

THE INJURED EE WAS REMOVING BOLTS FROM A P&H LOADER PROPEL TRANSMISSION. HE WAS USING A 3/4"" RATCHET WITH AN 18' HANDLE PLUS AN ADDITIONAL 18"" CHEATER PIPE FOR ADDED LEVERAGE. THE SQUARE DRIV E ON THE RATCHET BROKE OFF ALLOWING THE INJURED TO LUNGE FORWARD CAUSING A LACERATION APPROX 2 1/2"" LONG TO HIS RIGHT HAND.
THE INJURED EE WAS OPERATING THE WATER TRUCK AND WAS USING THE DECK MOUNTED MONITOR WATER CANNON. HE NOTICED THAT THE RADIATOR CAP AP REARED LOOSE AND HE TOUCHED THE CAP WITH THE TOE OF HIS BO OT. THE CAP FLEW OFF AND THE HOT ENGINE COOLANT SPEWED FROM THE RADIATOR SOAKING HIS LEG, ARM AND HAND. THIS CAUSED SECOND DEGREE BURNS.
EE & CO-WORKER ARRIVED IN THE PIT TO REPAIR A LOOSE BUCKET TOOTH RETAINING PIN ON A P&H BUCKET. THEY PARKED THE SERVICE TRUCK PARALLEL TO THE BUCKET. INJURED WAS PERFORMING THE WELDING, HE WAS KNEELING IN FRONT OF THE BUCKET. A HAUL TRUCK WAS PARKED APPROX. 200'FROM THE BOTTOM OF THE RAMP. IT BEGAN MOVING, STRIKING THE SVC.TRUCK THENJACKKNIFING AND STRIKING EE AND THE BUCKET.
THE INJURED EMPLOYEE WAS CLIMBING THE ACCESS LADDER OF CO #253 DART COAL HAUL TRUCK. HIS RIGHT FOOT SLIPPED OFF THE RUNG OF THE LADDER AND HE CAUGHT HIMSELF WITH HIS LEFT HAND ON THE HANDRAIL. THE TWISTING MOTION AS HE SLIPPED CAUSED A MUSCLE STRAIN TO HIS BACK.
THE INJURED EE WAS PREPARING TO OPERATE A CAT D10-R DOZER IN THE PRE-STRIP FILL AREA. IN CONDUCTING HIS PRESHIFT INSPECTION HE STEPPED ON SOME SPILLED OIL ON THE UPPER DECK, SLIPPED AND FELL O NTO THE DOZER TRACKS BRUISING MUSCLES IN THE LOWER BACK LUMBAR AREA.
THE INJURED EMPLOYEE WAS ASSISTING IN REPLACEMENT OF A LOWER FAIRLEAD SHEAVE. AS THE SHEAVE WAS HOISTED INTO PLACE IT BACAME BOUND UP AND AS THE LIFTING DEVICE WAS REPOSITIONED, THE SHEAVE MOV ED UNEXPECTEDLY PINCHING THE VICTIMS RT RING FINGER BETWEEN THE SHEAVE AND FAIRLEAD FRAME. THE EMPLOYEES INJURY WAS A FRACTURE OF BONE, LACERATION AND CRUSHING WOUNDS TO THE RIGHT RING FINGER

A FIRE INVOLVING THE VOLVO FUEL TRUCK OCCURRED. THE SECURITY GUARD NOTED SMOKE COMING FROM THE HEAVY EQUIPMENT PARKING AREA. SHE IMMEDIATELY ATTEMPTED TO CONTACT FORMAN AND INFORM HIM OF THE FIRE. ATTEMPTS FAILED (LATER DISCOVERED THAT THE BATTERY ON HER PORTABLE RADIO HAD RUN DOWN.) ANOTHER PERSON CALLED FOREMAN. THE ENGINE COMPARTMENT & OPERATORS CAB WERE ENGULFED IN FLAMES.
REMOVED LIGHT BAR TO REPLACE BOOM BUSHING. AS HEWAS PLACING LIGHT BAR ON CATWALK IT HIT THE GUARDRAIL & SLIPPED FROM HIS HAND. THE ELECTRICAL CORD SLIPPED THROUGH HIS FINGERS & THE RUBBER PLUG CONTACTED HIS RING FINGER BREAKING THE 1ST JOINT
THE INJURED EE WAS DISMOUNTING A CAT D-7 DOZER. AS HE STEPPED FROM THE TRACK ONTO THE BLAD PUSH ARM, HIS FOOT SLIPPED OFF OF THE PUSH ARM STEP ALLOWING HIS RIGHT SHIN TO CONTACT (SCRAPE) AGAIN ST THE TRACK PAD. THE ORIGINAL INJURY WAS A CONTUSION/ABRASION APPROX. 3"" LONG. 2 1/2 WEEKS POST INJURY THE EE HAS A MAJOR INFECTION OF THE INJURED AREA REQUIRING I-V ANTIBIOTICS (OUTPATIENT)
THE INJURED EMPLOYEE WAS ASSISTING IN CLEAN UP/ HOUSEKEEPING ACTIVITIES IN THE LOWER REVOLVING FRAME OF THE B/E 1570 DRAGLINE. BECAUSE OF THE AWKWARD POSITIONS HE WAS IN, HIS HARD HAT HAD BEEN REMOVED. AS HE MOVED FROM ONE COMPARTMENT TO ANOTHER, HIS HEAD STRUCK A SHARP CORNER OF AN OVERHEAD CABLE TRAY WHICH CAUSED AN APPROX. 3"" LACERATION.
THE EE WAS WALKING ACROSS THE SHOP FLOOR WHEN ANOTHER MECHANIC ASKED HIM A QUESTION. AS HE CONTINUED WALKING, HE TURNED TO ANSWER AND AS HE DID, A SHARP PAIN IN HIS LOWER BACK CAUSED HIM TO ST OP WALKING & BECOME IMMOBILE. DIAGNOSED AS A SEVERE MUSCLE STRAIN/SPASM IN HIS LOWER BACK.
EMPLOYEE WAS OPERATING THE FUEL TRUCK. RAINY/ MUDDY CONDITIONS. AS HE ATTEMPTED TO MOUNT THE REAR DECK OF THE FUEL TRUCK, HIS LEFT FOOT SLIPPED AND HE STRUCK HIS SHIN ON THE EDGE OF THE DECK. THIS RESULTED IN A BRUISED/ SCRAPED AREA APPROX 6"" LONG ON HIS SHIN. SWELLING/EDEMA OF AREA OF TIBIA FIBULA.

EE WAS HELPING TO REPLACE/CHANGE OUT THE BUCKET ON THE 1570 BE DRAGLINE. HE WAS INVOLVED WITH CHANGING A REPAIR LINK ON THE DRAG CLEVIS WHEN HE REACHED IN TO CHECK FOR PIN CLEARANCE. AT THIS TIME THE CHAIN LINK ACCIDENTLY MOVED AND PINCHEDHIS LITTLE FINGER AGAINST THE CLEVIS. THIS CAUSED A LACERATION WHICH REQUIRED APPROX 12 STITCHES TO CLOSE THE WOUND.
EE WAS INVOLVED IN REPAIR OF THE #183 P & H 1550 LOADING HOE. HE HAD LOOSENED 2 BOLTS ON A COMPONENT AND AS THE THIRD BOLT BROKE LOOSE, HE FELL FORWARD SPRAWLING ACROSS SEVERAL HYDRAULIC HOSES . IT WAS AT THIS TIME THAT HE FELT A SHARP STABBING PAIN IN HIS SHOULDER, ARM AND HAND. INJURY ON 11-22-00 WAS DIAGNOSED AS A HERNIATED DISC BETWEEN 6TH AND 7TH VERTEBRAE.
EE WAS ANSWERING A CALL FROM THE 1570-W DRAGLINE. AS HIS PARTNER DROVE THE WELDING TRUCK TO THE REPAIR SITE, INJURED EE STEPPED FROM THE TRUCK CAB AFTER IT WAS PARKED. AS HE STEPPED DOWN HIS R T FOOT CAME TO REST ON A DIRT CLOD AND THEN TURNED OR ROLLED OVER CAUSING A SPRAIN AND POSSIBLE FRACTURE OF A BONE.
THE INJURED EE WAS STANDING ON THE PIT PUMP SKID PRIMING THE PUMP. AS HE STEPPED BACK & OFF THE SKID, HE HELD ON WITH HIS LEFT HAND. AS HE LET GO OF THE FITTING HIS LITTLE FINGER ENCOUNTERED A SHARP EDGE ON THE PUMP DISCHARGE FITTING CAUSING A LACERATION TO HIS LITTLE FINGER. LACERATION REQUIRED 7 STITCHES TO CLOSE THE WOUND.
THE ARCH ON THE D L BUCKET HAD BROKEN; THE D L OPERATOR STOOD THE BUCKET ON ITS NOSE SO THE BREAK COULD BE REPAIRED. INJURED EMPLOYEE WAS STANDING ON THE INSIDE OF THE ARCH WHEN A LARGE CLOD OF DIRT (APPROX. 200#) FELL, STRIKING HIM ON THE HEAD, SHOULDER AND UPPER CHEST AREA. THEEMPLOYEE'S HARD HAT CAUSED THE LACERATIONS TO HIS HEAD/CHEEK/EYE, BRUISED UPPER CHEST AREA AND
THE INJURED EMPLOYEE WAS ASSISTING IN PULLING OUT A PIECE OF STUCK EQUIPMENT. HE WAS PULLING A LARGEW DIAMETER TOW CABLE FROM THE FOREMAN'S PICK UP. AS ONE END OF THE CHOKER PULLED FREE, IT FL IPPED AROUND PINCHING HIS THUMB AGAINST THE TAILGATE. THIS CAUSED A LACERATION AND CONTUSION TO HIS LEFT THUMB. NAIL HAD TO BE REMOVED.
