Caught in, under or between running or meshing objects

Injured employee was spinning a pulley sheave by hand to remove an impeller and caught two of EE's left fingers in the belt. EE's left middle finger was badly lacerated and EE's left ring finger sustained a fracture.

Employee was standing at regrind area when dust/debris was blown everywhere from the wind causing the dust/debris to enter employee's right eye. On 5/17/23 employee was prescribed oral antibiotics and eye drops. Employee has been referred to a specialist.

Employee was walking down ramp carrying a hose when EE lost their footing and hit left knee on the ramp. This caused a laceration. Wound closed with 17 stitches.

EE was performing housekeeping. EE was chipping a build up of dirt with a pry bar & struck the back of hand against a stairway edge. The edge of the metal stairway cut through EE's leather glove & created a puncture wound on the back of EE's left hand. Sutures were used to close the 1/2 inch wound.

While looking for pieces of scrap metal at the cement port on letter belt B at the crusher, employee grabbed a piece of metal and swung arm back to throw and right pinky finger made contact with corner of sign causing a laceration requiring stitches to finger.

Employee was complaining about kidneys hurting and not feeling good. Was treated for dehydration.

While employee was throwing a hanger bolt into discard bin and reported a shoulder sprain. Employee was treated as first aid but after MRI determined surgery was needed. Fist aid until 07/09/2019 at which time employee became a lost time injury.

Employee was installing a throat liner on the discharge end of rod mill. While holding the cable with right hand, the liner spun and pinched left ring finger causing laceration.

While employee was washing lime off of light vehicle #165, tripped and fell over a piece of wood causing a laceration to right ring finger that required stitches.

This injury initially was a F/A from 1/26/2017 until the thumb was x-rayed and a fracture was seen on 1/31/2017 at which time it became reportable. The employee was placing a guard on the west side of 7 belt on the short head crusher. EE put one side down and while placing other side the guard pinched thumb between the guard and support beam.

While climbing up the ladder of a loader, employee slipped and caught themself with right arm. Complained of right shoulder pain. Was treated as a first aid, was determined later by MRI employee needed surgery to repair a rotator cuff tear in shoulder. Employee lost no time or had restrictions until 5/4/2017.

Employee was working on 10 tripper and started experiencing cramps in legs and vomiting. Was treated for heat exhaustion at emergency room.

While removing a bearing cap on a 4-15/16"" bearing that was about 4' above the #10 tripper, employee was working from floor level as the cap was lowered. It slipped from hands, striking employee in left leg causing laceration.

EE was removing a drain plug at the bottom of hydraulic tank for a loader. When the plug broke loose the employee felt a popping sensation in shoulder. This injury has been treated as first aid and employee was sent for an MRI when it was determined that surgery was needed. On 04/14/2016 were ee surgery and the first day of lost time.

He has been treated for first aid since then and was out for personal illness for almost a year. Only now receiving any treatment for work injury. He had exploratory surgery and first day of lost time is 03/24/2016. The validity of this injury is in question.

The employee was lining up a liner onto a bowl for crusher repair. The liner made contact with the bowl, it slid down the bowl adjusting itself. The employees foot was below the liner, the liner came into contact with the steel toe of his boot. The liner slid over the steel toe and made contact with the employee's left foot.

Installing shell liner using air winch attached to shell of mill using an eye bolt 2 rows above row being installed. 2 men pushing liner into place a 3rd lowering it A bar was in the liner so it sat at the correct angle while lowering. When pressure was applied to this bar the supporting sling came out of the air winch hook and the load fell on the workers foot.

Employee performing pre-op on equipment. When stepping down off bumper with dried mud he slipped and fell to the ground. Employee thought he was fine and continued working. The following the day (Saturday) his back started hurting at home and went to the doctor. Testing revealed employee had contusion of the back and one rib with a closed fracture.

EE was walking down stairway to lower level in crusher building. When employee stepped off the step onto the landing, he rolled his right ankle. During the examination of the accident, there was an uneven surface of built up dried mud on the stair landing. The uneven surface was crowned in the middle about 1 1/2"" high & sloped down to both sides, the width of the stair tread.

Descending down ladder, slipped and struck her left side of rib cage on hand rail. Incident was first aid case, X-rays on 9/10/2012, revealed fracture of left rib.

As he leaned forward he accidentally placed his hand on the glass, lacerating the palm of his left hand.

Equipment operator was preparing to dismount from a dozer. He stepped out of the cab onto the crawler track. As he turned to grab the handhold bar, he lost his balance and fell off the dozer onto the ground.

EE was atempting to open a bottom dump railroad car into the track hopper. When the employee pulled on the handle to open it, she felt the doors did not open, so she decided to close the lever and try again. As she pushed on the lever to the close position, the handle snapped back suddenly catching her right ring finger.

Coming down off a slope at the tailings, employee's feet slipped out from underneath him. He lost his balance and fell backwards, while trying to brace himself, he out stretched his arms back. When he made contact with the ground he felt something pop in his left arm.

He fell through grabbing himself on the handrail and arresting himself with his retractable fall arrest equipment. Saw doctor 05/21/2009, released with restrictions, employer able to comply.

Employee was working, kneeling down preparing to hook up the ""D"" ring on a sling to lift a bearing pedestal when the 10 ton hoist hook assembly struck him on the left side of his shoulder and head.

Employee was working on sump pump using an oxy-acetylene torch to cut off a bearing. A flash occurred causing a ball of fire to come towards him. The ball of fire caused multiple burns to the face, neck and right forearm near the wrist.

Employee was walking down a dirt incline and stepped on some lose material. Her right leg gave way and she caught herself with her right leg. Employee felt a sharp pain in her right lower back that radiated down her right leg. At clinic employee told nurse that pain had been bothering her for the last several weeks.

EE operating the 243 Locomotive. He reported as he was approaching the concentrate hopper cars he went to apply the brakes & they did not work. He hit the hopper cars at approx. 5mph causing him to hit the control panel with his body, two facial cuts below his left eye from his safety glasses & he said his right chest area was sore.

Employee was walking back to control room in the Mill building when she stepped into a divit in the floor of the grinding area and twisted her right knee. Employee was force to work the overtime shift.

Employee reported accident on 8-15-08. On 8-8-08 Employee stated at end of shift that his knee was sore from working on his knees for two 12 hour shifts in a row. He said that no special event took place but that his knee became sore. He thought that it was just sore from constantly working on his knees. On 8-15-08 he felt that it wasn't improving and needed to see the nurse.

Employee was in the process of cleaning the bolt on a 24 inch valve. He reported that he had his left hand on the pipe to stabilize himself. The impact wrench was sitting on top of the pipe and it slipped down were employee was working. the wrench landed on employees left hand pinching his small finger between the wrench and pipe. Employee's finger required 6 sutures.

On 4-21/08. Employee mechanic was working on a motor for the # 5 rod mill in the mill building. He reported a 12 pound hammer fell from above striking him in the left upper arm. His arm was between the hammer and motor coupling. He sustained contusions to the upper left arm. On 4-22-08 employee reported to clinic to see doctor and became a restricted case.

Employee was tighting bolts on A-mill with a 1 inch impactor. The air hose broke of at the male end of quick release coupling and the whip hose hit him on side of face. Employee sustained left facial contusion with whiplash type injury with neck strain.

Employee was standing at the discharge end of #3 rod mill. He stepped foward to grab a discharge box, the grading moved causing him to fall into the hole that was approx.20 inches deep. Employee sustained a sprain to the right knee and leg. Case was first aid, become restricted on 4/7/08 after seeing doctor.

Employee noticed a Skirtclamp that was dragging on a conveyor belt. Employee (while belt was running) pulled pin from clamp to adjust clamp. While adjusting clamp, trailing edge caught belt which caused it to jump foward pinning employees left hand between the skirt board and clamp. Employee sustained laceration with tendon, ligament and nerve damage.

Employee was descending concrete at Admin building. Near bottom of stairs employee stepped on debris twisting her ankle and fell catching herself on handrail.

Employee was helping unplug tail sample pump in the mill. He was up on the pump, had positioned a flex hose over the discharge pipe and was holding it in place. Pump was turned on, he was unable to keep flex hose in place and consequently hot mud sprayed on his right arm and right side of face resulting in first and second degree burns.

Employee reportedly was helping other workers move grinding rods in the mill. The crane pulled on the cables causing the rods to shift and in doing so, the cable and hook pinched employee's right ring finger resulting in an open wound that required sutures to close.

Claims she was bounced around in the cab and when she tried to reposition her body, her foot slipped off the cab frame.
