Struck against a moving object

For unexplained reasons, the miner gained access to a secondary road, adjacent from where he was instructed to do road maintenance. Miner removed berm and came too close to road edge, loosing control of equipment and falling through a steep slope, rolling over with the machine. This resulted in miners death. Miner was not wearing seat belt.

A FATA ACCIDENT ACCOURED IN THE BC-03 PRIMARY CRUSHER AREA. THE GEHL MODEL 3825 SKID STEER LOADER OPEARTOR WAS PINNED BETWEEN THE BUCKET ARM AND FRONT FRAME CRUSHIG HIS HEAD.

EE works for Securitas Security Services of PR, a company contracted to provide security services. EE claims that EE fell from a four-track in front of the entrance to the shop. This fall caused EE injuries to EE's head, back and legs.

He was working on the removal of a cover focus. When cover focus loosen pricking his finger between the cover and railing of the manlift.

He suffered a cut on the finger of the right hand. This happened while cutting chain with acetylene the D-11N. He put his hand on the chain that stay suspended in the final drive, but by cutting the link chain moved and pinched his finger.

Employee climbed the stairs on a dump truck that was loading material beneath a storage silo to verify the quantity of material being loaded. While descending down the stairs, employee's wedding ring became entangled in the stairs causing injury to one of employee's fingers.

Struck by powered moving object

While employee was lubricating some part of a Caterpillar Loader 988, he signaled the operator of the loader to move the bucket of the loader. When the operator moved the bucket, the employee got his arm pinched between the loader's bucket and the bucket's boom. Arm got bruised, but apparently no fracture was found.

Allegedly, while employee was tighting a nut in an equipment tire's rim, the wrench left his hand and struck the employee's left eye. The employee received first aid including six stitches.

Employee was removing a hydraulic hose from an excavator. After hose was removed, the employee's hand continued to travel because of the force exerted to remove the hose, hitting the excavator boom's pin.

AFTER GETTING HIS TRUCK LOADED, EE GOT UP ON TRUCK TO COVER THE MATERIAL. HE WAS TOLD BY ANOTHER EE TO COME DOWN AND THEY BOTH WOULD COVER IT. WHILE COMING DOWN, HE FELL TO THE GROUND. HE TOLD WITNESSES THE HE DIDN'T FEEL HIS LEGS, SO HE WAS MOVED VIA AIR AMBULANCE TO THE STATE MEDICAL CENTER.

EE WAS ABSENT THE DAY BEFORE 7-11-02 BECAUSE HE HAD A BACKACHE. THE NEXT DAY HE CAME TO WORK THE NEXT DAY BUT HE STILL HAD THE BACKACHE, WHILE GETTING READY ONE OF THE LOADERS WHICH HE WAS GOI NG TO WELD HIS BACK STARTED TO HURT MORE.

CHECKING A PORTABLE COMPRESSOR THAT HAD TURNED OFF BECAUSE THE COMPRESSOR NEEDED WATER. WHEN TAKING OFF THE TOP, THE EMPLOYEE THOUGHT IT WAS COLD ALREADY BUT THE WATER WAS STILL HOT AND BURNT HIS LEFT ARM.

EMPLOYEE WAS TRYING TO TAKE OUT A PART FROM THE TRUCK WHICH COULD NOT COME OUT. WHEN HE FINALLY WAS ABLE TO TAKE IT OUT HE PULLED IT AND CUT HIS INDEX FINGER OF HIS RIGHT HAND.

HE WAS COMING DOWN THE STAIRS TO DRINK WATER & SLIPPED HURTING HIS WRIST OF HIS RT. HAND.
