Employee was building drill bench, pushed thru the berm at the edge of the highwall and traveled over the highwall.

WHILE CHANGING A TRACK ON A DOZER, THE CHAIN HOOK SLIPPED OFF THE BOGIE & STRUCK THE EE IN THE HEAD AREA RESULTING INS SIGNIFICANT TRAUMA TO THEHEAD AREA. THE EE WAS TRASNPORTED TO THE UNIVERS ITY OF UTAH MED CTR. EE PASSED AWAY 9/25 AS A RESULT OF THE INJURIES SUSTAINED IN THIS ACCIDENT.

Caught in, under or between a moving and a stationary object

High winds blew the doors on the back of the Drag Line house open. The employee and two others decided to close the doors. They successfully closed the right door and were attempting to close the left door when a gust of wind hit the door and flung it open blowing all three of them back with the door. The employee was pinched between the door and the guarding around the Gen-set.

Struck against a moving object

The 103 Dragline at Ramp 53 contacted the cable handler #768 and was struck by the left rear house of the dragline while the dragline was in full swing. As a result, the cable handler was tipped onto its side. The cable handler operator was transported to the Dr. Office for further medical evaluation. On 10/13/25 the MRI confirmed that the employee would need surgery.

Employee was climbing off the dozer that was located on the deck of the lowboy trailer while stepping backwards when they fell off the trailer approximately 3 to 4 feet to the ground onto their back. This resulted in pain to the right elbow, and the wind was knocked out of them. The employee was transported to a higher level of care for further evaluation.

While positioning a barrel of solvent in the lube room of the 102. The employee's finger got caught between the barrel and the air compressor bank pinching the Left hand pinky finger crushing it between the barrel and the compressor bank.

An employee stopped for lunch break at the 2 high wall rock run. Employee climbed down the ladder grabbed both chock blocks, was walking to set one chock block on the left front wheel. The employee slipped on the ice and landed on their arm.

Employee noticed a hose blew apart down in Ramp 58. EE went down to the pump, shut the pump off and started to disassemble. The hose was not repairable, so EE went to get a different hose to replace it. When starting to pull the hose apart EE felt a poke through the glove. After removing the glove, the employee found a laceration on their left middle finger.

Employee was performing an oil change on 1774 after removing the first oil filter. Employee began to remove the second filter the employee felt 3 pops in their right arm between the bicep and forearm. Employee stated that the filter could have been stuck or had been put on too tight. MRI was done and a torn muscle was found.

Employee was carrying a receiver plate and tripped on a mixed gas hose and fell with the plate and the plate contacted the employee on the left eye brow. This caused a laceration over the left eye brow needing stitches.

Employee was opening the back doors of the dragline and placed hand on the lower rail of the door. When the door swung open, EE's left hand was located in a pinch point between the door and the back house fan.

Employee was doing audio test on 2-17-2022 and has showed employee had sustained a STS in left ear. Employee was scheduled to re test on the 2/23/22 and it confirmed that the employee has sustained a permanent STS in left ear, from the baseline in 2017.

Employee was working on 308 drill outside the shop. EE was standing on the mast loosening a hydraulic hose, after they broke the hose loose, they stepped down with right foot, left foot was caught under another hydraulic hose and right foot landed on uneven footing causing them to roll the ankle. Resulting in a small fracture to the right foot.

Employee was changing out a transfer chute wear plate. The employee was directing the crane when the wear plate became hung up. The employee freed the wear plate and as the plate settled the employee's right wrist was caught and bent backwards. The employee was referred to University of Utah. A diagnosis from the employee was received on 03/29/2021 after EE's surgery.

Struck by falling object

The Skid Steer bucket slipped off the attachment shoe edge and fell approximately 1.5 foot to 2 feet to the floor striking the employees foot behind the hard toe.

Employee arrived at R1 drill pad work area. Exited 2470 pickup, went around to get lunch box out of the passenger side. Grabbed lunch bucket with left hand, holding the door with right hand. The wind pushed the door shut and right middle finger was struck between the door and the frame. The finger was lacerated resulting in sutures.

Employee was climbing up the off side boarding ladder on 222 track hoe when, they slipped hitting right shin on a rung of the ladder causing a laceration on shin which required sutures.

Employee was lifting hood off of D11 CAT Dozer, Hood shifted and pinched left index finger between the hood and the holding bracket for the hood. Broke index finger.

Employee was sitting in the cab of Company #443 haul truck a 785 Caterpillar haul truck, being loaded with dirt. An unseen rock in the bucket struck the truck bed jolting the employee.

Employee was drilling a 5/8"" holes in 3""x3"" angle iron with a Wilton drill press. The piece pulled up out of the vice and grabbed EE's welding glove on EE's left hand. The drill twisted EE's hand and arm into the bit until it pulled off the glove and the bit broke. This accident resulted in a hairline distal fracture of the left ulna.

Welder was standing on a platform, driving a wedge setting the sidewall on a dragline bucket in the bucket shop. The employee lost balance and put left hand on the dog while swinging the 6 lb. Sledge. The sledge glanced off the wedge and struck the employee's left ring finger.

The employee had right hand on the tugger rope just above a snatch block pulley. The employee wanted to put slack in the tugger roper but hit the in button on the controller by mistake pulling right pinky finger into the snatch block pulley resulting in a fracture to right pinky finger.

Employee was removing a fuel filter from a haul truck with a chain wrench when the wrench slipped and struck the employee on the right side of his jaw chipping his tooth.

Employee was removing a guard on a dozer, when the employee picked up the guard he felt a pain in his stomach. The employee was seen by a physician and scheduled for surgery. During the surgery it was determined that the hernia was caused the injury that the employee sustained on 8/8/15. The company received the surgical report and final diagnoses on 9/29/15.

Employee was moving cones for a scheduled blast, he slipped and fell on some loose material at ramp 60. The employee used his right arm to break his fall and heard a pop in his right wrist area. Resulting in a sprain to his right wrist and elbow.

The employee opened a 5 gallon bucket of Foray fire extinguisher chemical with a box cutter knife. The bucket turned and so did the knife causing the knife blade to lacerate his left ring finger. Six sutures were used to repair the wound.

Employee was operating a haul truck on graveyard, being loaded by LeTourneau #243 in ramp 2. The operator's statement indicates she felt pain in her shoulders and neck while being loaded at approximately 1:30 am.

EE slipped and fell injuring his right shoulder, while pulling a barge pump to sure for repair at ramp 60 pond. His right rotator cuff was torn during the fall.

Employee was operating 625 scraper when he ran over a large rock that was barely covered with dirt. After he passed over the rock, the right front tire of the scraper fell off the rock and dropped suddenly. Employee was thrown upward, and then slammed down, bottoming out the seat. He then experienced severe pain in his lower back.

On 01/10/2014 the employee seen his doctor and it was determined that the employee received an Acute, non-displaced, fracture of his left 8th 9th 10th ribs.

Employee fell backwards from an elevated platform and hit his head on the ground causing a concussion and laceration to his head.

Employee was opening a man door in the shop wash bay. The door was jammed so the employee pulled hard on the door and felt a sharp pain in his left shoulder.

Employee was removing a grate over a sump in order to clean the sump. Employee used a 1/4 inch round stock hook approx. 3 feet long with a 1 inch 90 degree bend on the end of the hook. The grate slipped off the hook and struck the employees right foot just behind the steel toe of his boot resulting in a fracture to his Medial Metatarsal of his right foot.

The employee removed the brackets from the section next to the muffler and was in the process of removing the remaining brackets on the tail pipe section. When the muffler came loose, the bracket pinched the employee's left middle finger against the truck frame.

Employee was putting lifting slings into a storage box in the back of a mechanic truck when the lid fell onto the employees left forearm lacerating the top of the forearm. The employee received four sutures.

The employee was helping install half of a hydraulic to the other half of the hydraulic tank by lifting one side of the tank so a fellow employee put bolts in the tank. While lifting the side of the tank the employee felt a pop in his lower back.

EE was working on Tub of a dragline. He was standing on the 3rd step of a 6' ladder. As he went to step down off the ladder, his foot slipped on the 2nd to bottom rung. When his foot slipped, he struck his chest on the Tub Manway Portal in The Tub.

Employee slipped off of the lower step on a fuel truck and landed on the door threshold, injuring his right back rib area.

Employee was moving a steel shaft with a forklift. The employee got off the forklift to adjust the cribbing, the left fork kicked up and struck the employee in the left shin. The employee recieved eight stitches.

The employee was attepting to open the door of a water truck and the wind blew the door closed on her right index finger tip. Employee was taken to the hospital and suctures were required to close the wound. She subsequently missed two (2) work days and returned to light duty on 6/11.

The employee was attempting to fuel a piece of equipment when the rear door of the service truck lowered striking him on the back of the neck. Initial examination and xrays were negative, however after returning from scheduled days off the employee complained of pain. The employee was examined by a physician and placed on restricted duty.

The employee was attempting to attach a nylon strap on a dump socket wedge for a dragline bucket rigging when the wedge slipped causing it to graze their left hand. Upon medical evaluation, it was determined that there was a non-displaced linear fracture of the second metacarpal on the left hand. The employee was placed on restricted duty.

The employee was moving along the edge of the left track of a D11 Dozer when he lost his balance and fell to the floor. Upon medical evaluation, it was determined that he had strained his left ankle. This accident has resulted in lost work time.

Employee stepped down an ~6"" step with his right foot and stepped on a 2"" piece of angle iron. He suffered a sprained right ankle. This injury was a First Aid case until 01/20/06. It became reportable due to Physical Therapy Treatments employee received on 01/20/06. Note: This employee is a TXU Mining employee who was on company business at the specified mine site.

The employee has been diagnosed with Carpal Tunnel Syndrome in both wrists and underwent surgury for this condition on one wrist, October 19, 2005. He will have the other wrist operated on, November 02, 2005. This has resulted in a Lost Time occupational illness.

At the time of the incident employee was using an air torque wrench to remove sprocket segments. As he was removing one of the bolts holding a segment in place the wrench slipped from his grip. When he attempted to catch the wrench he accidently squeezed the wrench trigger, activating the wrench and causing the torque arm to swing into and smash the left ring finger.

The employee was reaching for an item above his head, lost his balance and fell to the floor on his right side causing a fracture to a rib on his right side. The employee was released to full work duty.

The employee was cutting an envelope open with a pocket knife when he cut through the envelope and caused a laceration to the inside of his right wrist. This injury required three (3) stitches.

The employee was decending the ladder of a haul truck when he slipped and fell to the ground. He fell onto his left hip. Upon medical evaluation, it was determined that he had a fracture in the left pelvis region. This accident has reasulted in lost time.

This employee has been diagnosed with Carpel Tunnel Syndrome in both wrists. He has underwent surgery for this condition on his left wrist on 11/29/04 and surgery for his right wrist on 12/08/04. This has resulted in a lost time occupational illness.

The employee was dismounting a dozer when his foot slipped off the bottom step. At the same time, the employee was holding onto a grab iron. When the employee's foot slipped, he felt a sharp ""HOT"" pain in his back. This resulted in a strain / sprain in the employee's lower back. The employee was put on restricted duty.

EE WAS WALKING ACROSS A DIRT PAD WHEN A PIECE OF DIRT ENTERED HIS RIGHT EYE. THIS MATERIAL BECAME IMBEDDED IN THE CORNEA AND WAS REMOVED WITH A COTTON SWAB.

At the time of the incident Employee was observing a co-worker hoisting a brake accumulator out of a CAT 988 wheel loader. As the accumulator was being lifted out of the loader it suddenly rotated on the end of the hoist cable and struck employee's right middle finger, pinching it between the accumulator and a handrail.

EE BENT OVER TO INSPECT AN OIL LEAK. AS HE SIMULTANEOUSLY STRAIGHTENED UP AND TURNED AROUND, HE FELT A SHARP PAIN IN HIS BACK. UPON MEDICAL EVALUATION, THE EE WAS DIAGNOSED WITH A STRAIN IN THE LOWER BACK. HE WAS PLACED ON RESTRICTED DUTY.

THE EE REPORTED THAT HE HAS REPETATIVE MOTION INJURIES WITH BOTH WRISTS. THIS WAS CONFIRMED BY A NEUROLOGY EVALUATION. THE EE HAD SURGICAL REPAIR OF THE LEFT WRIST ON 4-24-03, AND SURGICAL RE PAIR OF THE RIGHT WRIST ON 4-14-03. THERE WAS NOLOST TIME DAYS. EE IS ON RESTRICTED DUTY.

UPON MEDICAL EVALUATION HE WAS DIAGNOSED WITH CONTUSIONS TO THE RIGHT SHOULDER AND RIBS AND A STRAIN TO TTHE RIGHT SHOULDER. HE WAS PLACED ON RESTRICTED DUTY AFTER FURTHER EVALUATION WHEN HE RETURNED FROM VACATION.

WHILE WALKING ON SNOW COVERED GROUND, THE EE SLIPPED AND FELL TO THE GROUND. THE EE FELL ON HIS BACK AND HEAD AND WAS UNCONSICOUS FRO A FEW MINUTES. UPON MEDICAL EVALUATION, NO SIGNIFICANT INJ URIES WERE NOTED OTHER THAN CONTUSTIONS. THE EE WAS PLACED ON RESTRICTED DUTY.

EE WAS TRANSPORTING #401 HAUL TRUCK FROM THE FIELD TO THE SHOP AFTER REMOVING THE DUMP BED. THE STIFF SUSPENSION ON THE TRUCK CAUSED HIM TO STRIKE THE BACK OF THE SEAT WITH HIS LOWER BACK. THI S RESULTED IN A STRAIN TO HIS LOWER BACK. EE WAS PLACED ON RESTRICTED DUTY.

EE WAS ATTEMPTING TO DISMOUNT A 789 HAUL TRUCK UTILIZING THE LADDER AND STEPS. AS HE STEPPED FROM THE BUMPER TO THE RUBBER STEPS HE PLACED HIS RIGHT FOOT ON A NUT AND BOLT INSTEAD OF THE RUNG OF THE STEP. WHEN YIS WEIGHT WAS TRANSFERRED TO THE RIGHT FOOT HE FELT A POP. XRAYS REVEALED A FRACTURED BONE IN HIS RIGHT FOOT.

EMPLOYEE WAS PARTICIPATING IN EMERGENCY RESPONSE TRAINING. WHILE RESPONDING TO A SIMULATED ACCIDENT, THE EMPLOYEE GOT DIZZY AND FELL BACKWARDS. AS HE FELL, HE STRUCK THE BACK OF HIS HEAD AGAIN ST A TRAILER HITCH. THIS CAUSED A LACERATION THAT REQUIRED 6 SUTURES TO CLOSE.

CAUSED LEFT RING FINGER TO BE LACERATED.

THIS RESULTED IN EE'S THUMB BEING FRACTURED. ALTHOUGH THE ACTUAL EVENT TOOK PLACE ON 5-5-02, IT WAS NOT DETERMINED THAT A FRACTURE EXISTED UNTIL 5-6-02 WHEN THE EE OBTAINED XRAYS.

EE WAS WORKING ON A COAL HOPPER. THE WIND BLEW A PIECE OF COAL DUST INTO THE EE'S LEFT EYE. THIS RESULTED IN THE REMOVAL OF A CORNEAL FOREIGN BODY AND AN ABRASION TO THE CORNEA OF THE EE'S LEF T EYE. IT IS NOTED THAT THE EE WAS WEARING PROPER EYE PROTECTION AT THE TIME OF THE INCIDENT.

THE EE UNDERWENT SURGE RY ON JANUARY 10, 2001, TO REPAIR THIS INJURY. THIS WAS THE FIRST DAY OF MEDICAL TREATMENT AND THE FIRST DAY MISSED A RESULT OF THE INJURY.

WHEN THE WATER WAGON CAME TO A STOP, THE EMPLOYEE STRU CK THE SIDE WINDOWS, WHICH WAS RESULTED IN A HEAD LACERATION. THE EMPLOYEE ALSO INCURRED TWO MINOR RIB FRACTURES ON THE LEFT SIDE OF HIS CHEST AREA. WAS WEARING HIS SEAT BELT.
