Employee was sparging an upcomer trying to unplug the line when hot water belched from the line, contacting the employee on the abdomen and arm. Case didn't become recordable until April 6, when employee returned to work with restrictions from Physician.

Electrician was turning a pulley from belts while observing meter. Middle finger of right hand was pinched between the pulley and the belts requiring 3 sutures in finger.

Knelt down on grating rupturing bursa sack in knee. Medical information not received until 1/8/07.
Unloading drag conveyor when it got hung up. Employee climbed about 4 ft. up ladder to look in conveyor. Other employees bumped conveyor. Upon energizing conveyor the stabilizer to the gear box collapsed allowing box to spin 35-40 degrees. The gear box struck employee knocking him off the ladder. When others found him he was laying flat on back; ladder was still in position.
Employee was pulling top & bottom covers off East Lime Drag. Was standing on ladder positioned to the west side of the drag. Employee was cutting bolts loose & was hammering the dowel pins & bolts out in order to remove the piece of cover. Hammering broke loose some lime & a gust of wind blew the lime dust in the employee's face.
Mechanic was removing a flanged ""Y"" about 1-A Leach Sump Pump. When he pulled the last bolt out the pipe sprung six inches in the vertical direction smashing his finger in between the two pipe flanges.

Employee caught his foot in the edge of the Admix Sump Pump. The sump hole had a small submersible pump that was barely keeping up with the inflow of water which caused a layer of water/mud to hide the adjusted grating. Employee's left foot slipped in hole; his momentum kept driving him forward twisting his ankle and applying pressure in the lower shin area.
Employee was called for OT cleanup; used a firehose. The next morning employee had a sore back with pain that ran down the back side of his right leg.
Shoveling out elevator full of product; product was 370F. Employee's feet would get hot and he would stop shoveling for awhile. Didn't notice anything except a red big toe until several days later when his feet started to hurt and he noticed blisters on little toes.
PAIN IN LEFT KNEE AGGRAVATED BY THE NEED TO CLIMB STAIRS TO DO JOB. HAS GOTTEN WORSE SINCE MOVING TO NEW JOB. HAS BEEN PLACED ON RESTRICTED DUTY.
EMPLOYEE WAS ATTEMPTING TO CLOSE VALVE AND FELT PAIN IN RIGHT SHOULDER BLADE. THE FIRST DAY MISSED FROM WORK WAS 6/13/05, WHEN HE HAD SURGERY ON SHOULDER.

WHILE BLEEDING THE BRAKE LINE ON A CRANE, EE LOOKED INTO THE BRAKE RESERVIOR, AT THAT TIME BACK PRESSURE IN THE LINE CAUSED BRAKE FLUID TO SPLASH OUT OF THE RESERVIOR HITTING EE EYES HE WAS GI VEN A FULL DUTY RELEASE ON 3-21-02 FROM R.S. HO=SPITAL. SAFETY WAS NOT AWARE HE HAD NOT RETURNED TO WORK WK OF 3/25 BECAUSE OF HIS INJURY. LATE DR RPT EVALUATION.

EE HAS HISTORY OF BACK RELATED INJURIES. 4/30/01EE REPORTED HE STRAINED HIS BACK AGAIN. DUE TO HIS MEDICAL HISTORY WE ARE TAKING EE AS A CTS
WAS ON HIS WAY OUT OF THE CHANGE ROOM, SLIPPED ON THE METAL GRATING AND INJURED HIS ARM AND HAND. BROKE HAND 5. IN CHECKING ON REPORTS SINCE I'VE BEEN BACK FROM SICK LEAVE FOUND THIS REPORT WA S MISSED.
EE WAS INVOLVED WITH REPETITIVE TASK MOVING BELT PARTS WHEN HE FELT PAIN IN LOWER BACK. DURING EXAMINATION BY MEDICAL DEPT. EE STATED HE HAD EXPERIENCED SORE BACK PREVIOUSLY. WE BELIEVE THIS I S NOT AN ACUTE INJURY. IT IS CUMULATIVE TRAUMA DISORDER OCCURING OVER TIME.
LEFT HAND WAS PINCHED BETWEEN BUNDLE OF ROOF SU PPORT PANS AND A BRACKET FOR PANS THAT IS ON A BOLTER MACHINE.

ATTEMPTING TO REMOVE TAIL SHAFT ON MINER TAIL. HOLDING PUNCH HAMMER, WHILE OTHER EMPLOYEE WAS HITTING PUNCH HAMMER WITH 4# HAMMER. BRASS HAMMER HIT PUNCH HAMMER GLANCING BLOW, CAUSING PUNCH HA MMER TO COME BACK AND HIT EMPLOYEE IN UPPER LIP.
INJURY WAS REPORTED ON 2/25/00. AT THAT TIME IT WAS NOT KNOWN TO BE MORE THAT A DEGREE 0. AFTER INVESTIGATION IT WAS FOUND WITH MEDICAL RECORDS TO BE A RECORDABLE DEGREE 3.
