Struck by flying object

03/18/2025 Employee was standing near the Deister screen when the wind blew dirt into the employee's right eye. First Aid measures were provided on site. The employee was seen by a Banner Health clinic on 03/18/2025. Prescription eye drops were prescribed by the physician. The employee did not have any lost time or restrictions.

The injured employee was performing maintenance on a bearing for the P-1 conveyor when right foot slipped into a gap between the conveyor frame and the concrete retaining wall. The injured employee's body turned to one side causing a twisting injury to the right knee.

On Friday, October 19, 2018, a welder/mechanic was walking through a shop when they tripped on a welding cart outside of the welding bay, resulting in a fall. The EE fell onto a piece of angle iron that was on the weld cart, causing a laceration to their hand. The EE was taken to the emergency room where they received 6-8 stitches.

An employee was attempting to connect a light plant trailer to the hitch on a pickup when EE used a bar vise to strike the tongue of the trailer hitch, missing and pinching EE's finger.

This employee and another were welding in the welding shop. Employee went home and started experiencing pain in both eyes. He went to the doctor at 12:00 am and was told he had flash burns to both eyes.

Folding D-10 dozer track for shipment. While folding tracks master link folded under to far. Employee straightend master link with right hand. At this point his left hand was between track pads. The tracks flatened cutting and breaking three fingers on his left hand.
