Fall to the walkway or working surface

Employee was installing equipment with crane assist. While walking employee tripped over a dunnage block and fell down. Employee sought medical treatment and was Dx with a rotator cuff tear. Reporting within 10 days.

EE pinched right index finger between steel while EE was replacing parts on a crusher. EE continued to work. EE later sought treatment on 3-2-23 in which EE was diagnosed with a fracture & given a finger splint. EE was released back to work.

Employee was using a torque wrench in a tight area during tire repair and the socket came off the nut and finger was pinched by wrench. Stitches were needed on finger.

Fall from machine

EE was performing maintenance while standing on the counterweight of a loader. EE misstepped and fell. EE broke collarbone.

EE was installing a new material deflector plate onto a chute. The chute had been removed from the cone crusher & was in the shop. As EE was putting the strip of metal into place so that it could be welded, the strip rotated in such a way that the EE's thumb was bent awkwardly. As a result, one of the tendons in EE's hand ruptured.

Struck against stationary object

""EYES ON PATH"" Incident - While walking on ground level & looking up to diagnose noise coming from C11 conveyor, the EE stepped on a 3"" rock causing his rt. ankle & knee to roll. The EE sought medical treatment on 9/24/12 & diagnosed with a knee sprain. On 9/28/12, the MRI revealed no deterioration or damage, but the employee will have to go to physical therapy for 2 weeks.

On 8/11/12 the body of a 22-year-old female was found in the S/E sump in the east pit. It appears that this individual trespassed by climbing over the 7-ft chain-linked fence with barbed wire along the east edge of the property parallel to Charles St. Then fell from the highwall. According to Caledonia PD, autopsy showed ""multiple blunt force injuries consistent with a high fall.""

Ee was removing a pin from the rt. front strut of an R85 Haul Truck using a porta-power. The EE placed a 6x6x30 block vertically to act as bracing so the unit would not damage the break rotor. EE took down the unit to compress the ram, and when doing this a gust of wind knocked the block over striking the left thumb. Result was a fractured thumb, splinted and returned to work.

Struck by flying object

On 4/6/2010, the EE had placed the 2"" pump in the North Bay of wheel wash reservoir and after it had drained, the pump sunk into the mud and as the employee pulled up on the hose and strap he felt a sharp pain in his right elbow. The employee went to the doctor on 4/9/10 and diagnosed with a tendon strain and given restrictions.

Ee was attempting to descend off the stacker belt onto the catwalk (approx. 42"") after completing repair work in transfer box at HAC. Ee stumbled when putting left foot onto coveyor frame. Lost grip and fell backwards striking his back onto toprail of guardrail system. Ee went to doctor on 3/9/09 and diagnosed with 3 rib fractures and given restrictions.

Employee was re-attaching a water truck axle to a water truck when the axle started to roll toward the employee. The employee attempted to stop the axle when he strained his right shoulder. EE was given restrictions that affect his work activites on September 8, 2006.

Employee was replacing screen on top deck. While climbing up, left knee gave out. Originally injury was recorded as first aid. On 12/16/2004 the injury was upgraded to lost time reportable when the employee required surgery on his left knee.

Part of the loader operator's duties is to monitor material flow. While adjusting flow, the employee experienced an occupational illness, resulting in a dislocated shoulder. This occurs regularly during recreational activities and on 4/27/04 while making the above operational adjustment.

THREE EE WERE ASSIGNED TO CLOSE THE TEARS ON C-11. DUE TO THE PHYSICAL ATTRIBUTES OF THE CONVEYOR. THE EE HAD TO USE C-CLAMPS TO TIGHTEN THE BELT IN ORDER TO FASTEN THE CLIPS ON AT THE TIME IN CIDENT, ONE OF THE C-CLAMPS FELL OFF AND THE INJURIES EE WENT TO REPOSITION THE C-CLAMP AS WELL AS HIS BODY POSITION. EE WAS SITTING IN EUCLID TRUCK WAITING TO BE LOADED WITH SHOT ROCK. THE DRIVER OF THE LOADER DUMPED THE BUCKET OF SHOT INTO THE TRUCK CAUSING THE COVER LOCATED BELOW THE WINDOW TO BLOW OFF AND BOUNCE THE DRIVER OF THE EUCLID AROUND. THE BOUNCING RESULTED IN NECK INJURY, CERVICAL SPRAIN.

ON 12-11-2000, EE, WHILE PREPARING TO LEAVE WORK, SLIPPED ON SNOWQY GROUND AND FELL ON HIS LUNCH BOX. THE FALL RESULTED IN 3 CRACKED RIBS. EE DID NOT NOTIFY HIS SUPERVISOR UNTIL 12-22-00. A RE PORT WAS MADE OUT ON 12-27-00 AFTER AN INVESTIGATION BY THE SUPERINTENDENT AND THE MAIN OFFICE RECEIVED THE REPORT ON 01-01-01.

EE WAS DRIVING A PICKUP TRUCK TO HIS WORK LOCATION. EE REPORTED THAT DUE TO THE ROUGH ROADWAY, HE WAS BOUCNED AROUND IN THE CAB OF THE TRUCK WHICH AGGRAVATED AN EXISTING BACK CONDITION. EE BEG AN LOSING WORK ON JULY 20TH AFTER INVESTIGATION OF THIS REPORTING IT WAS DETERMINED THAT THE INCIDENT WAS COMPENSABLE.
