Struck by falling object

The employee was on the 5th floor of the screenhouse when the incident occurred. As the employee was looking up, sand particles fell onto the employee's glasses, forehead, eyebrow and left eye. The next day, the employee scheduled an eye exam with eye doctor. The doctor's diagnosis was a corneal abrasion and prescribed prescription eye drops.

Employee was operating a front-end loader out in the pit area. As EE was driving over a bumpy sandstone area and holding unto the window bar with EE's right hand, EE sprained EE's right shoulder. The emergency room doctor stated the employee to off work until seen by Orthopedics.

Struck by flying object

A maintenance employee was sitting within 5 feet of another employee using a sledge hammer on a Dredge Cutter Head tooth. A metal chip from the tooth broke off and struck the maintenance employee on the left leg just above the kneecap. The injured employee was taken to a local hospital for treatment resulting in two days of lost-time.

The employee was walking down a flight of steps and injured the right knee. This was a routine task the employee had done many times without an incident. It turned into a torn meniscus that required surgery. The employee worked up to scheduled surgery on 01/16/22.

The employee was pushing with right foot (up to 20 lbs. of force) on a metal truck inspection ramp connected to two cables with counterweights and heard a pop and felt pain in right knee. After months of therapy and an MRI, it was determined the employee had a torn meniscus. The employee worked up to scheduled surgery on 09/23/2021.

Struck against stationary object

The employee was on top of a railcar closing hatch covers and then proceeded to walk down a ramp and struck head on an overhead I-beam. The employee was wearing a hard hat at the time of impact.

The employee stepped unto a metal cover with left foot and felt a twinge in left knee. This incident was initially just a worker's comp claim since the employee continued to work without restrictions. On 2/16/18, the employee underwent surgery for a torn meniscus.

The employee's right elbow struck an I-beam causing a stinging sensation. This incident was initially a worker's comp claim only and not reported to MSHA. Surgery was performed on 2/16/18 after all other medical treatments failed to eliminate the pain.

At approximately 6:00 pm, the employee was at home after working the day shift, and he felt pain in his left knee. The employee utilized a step-ladder with handrails to transfer materials between the 5th floor where parts were staged, and the 6th floor, where the buckets were being changed.

As the employee was using a metal bar to break up frozen sand stuck in the feeder, he felt pain of the right wrist. The employee was using his right palm to push the bar forward. A 60-year old Bagging Operator with nearly 40 years of experience developed soreness of his right shoulder and doesn't know what specifically caused it. This injury appears to be cumulative trauma disorder where over time, stresses have affected the employee's right shoulder.

The employee was applying pressure to a railcar ramp with his left foot and sustained an ankle sprain.

An employee working on a belt conveyor drive motor change misjudged the distance to the floor while stepping off the conveyor, fell backwards, and sustained fractures to both wrists.

The employee was placing a clear plastic bag liner into a plastic trash can when the incident occurred. As the employee was reaching down toward the ground, he felt shooting pain across his lower back. Prior to this, the employee had transported four 40 lb. plastic Culligan jugs of water to the Bagging, Loading and Screening departments.

The employee turned his body 45 degrees, at which time he felt a snapping sound in his right knee. The worker's comp carrier considers this incident a non-compensable injury since the employee was not doing anything unusual other than turning his body. The employee did mention that he sustained a right knee injury twelve years ago while playing football in high school.

THE EMPLOYEE WAS STACKING 50 LB PAPER BAGS OF SAND ONTO A PALLET, WHEN HE FELT A PULL IN HIS LOWER BACK.

The employee was welding in the shop at the welding bench. As the employee was welding, hot sparks fell down onto his bib overalls and started a fire. The fire burned through the bib overalls and through his jeans before the employee realized his pants leg was on fire. The employee slapped the fire out but not before receiving 2nd degree burns to his lower right leg.

The employee had welded nuts onto a threaded rod and within 10-15 minutes attempted to pick it up with his bare left hand, resulting in 2nd degree burns to his palm area that required medical attention.

The employee was on top of a railcar when the accident happened. As the employee was closing a door, he felt a slight pain in his right wrist. Upon examination, the employee noticed a bulge under the skin near his inside wrist. The diagnosis was a ganglion cyst.

The employee and co-worker were working at the Dredge Hold trying to remove an impeller. He placed his left arm behind the pump housing unit in order to unhook a chain hoist when the pump housing suddenly shifted pinning the employee's forearm between a large inlet pipe and the pump housing. This resulted in a bruised left forearm causing stiffness and swelling.

As the employee was pulling upwards on a large wrench to tighten a bolt, he felt a pain in his back. The employee did not seek medical attention until 6/20/08 and then sought treatment for a pulled muscle in his stomach, not his back.

The employee claimed he injured his back by operating the plant's L-330 Endloader performing routine duties. This accident involves SUSPICIOUS CIRCUMSTANCES because the employee was sent home on Administrative Leave on 10/04/07 due to being caught sleeping on the job (3rd offense). Eight hours after the employee was sent home, he called the plant and claimed to be hurt.

The employee was in the process of opening a coupler on a railcar when the accident occurred. As the employee was pulling upward with his right arm, the coupler stuck causing a sharp pain in the employee's right wrist. The employee chose not to seek medical attention until yesterday, thinking his sore wrist would heal itself.

The employee was shoveling sand from under the south cooler in the drying building when the accident occurred. As he was moving the shovel handle forward, the shovel hit a hidden peice of metal under the sand making the employee's body jerk. The impact caused a sudden twinge in the employee's back. He worked the remainder of his shift & sought medical treatment on 6/26/07.

The Employee & Co-worker were rolling up a replaced conveyor belt causing a lot of pushing and pulling action. The EE felt no pain during this process. After the Employee went home and sat down, he noticed pain when he tried to stand up. The employee was placed on a 10 lb weight restriction for six work days.

The employee was sitting on a wooden bench in the change room prior to the start of his shift. He was talking to co-workers; he felt severe pain on the left side of his back.

The employee was placing bags on the spouts of baggers #1 & #3 when the bag he was placing on Bagger #1 did not open properly & his hand slipped forward & contacted the exhaust end of the spout. With use, the spouts get worn with sand running through them & a sharp edge forms. It was this edge that the ee's left hand had contacted resulting in the cutting of the thumb.

The employee was helping to move the plant's drill rig from one hole to the next by pulling on a 2"" diameter water hose. The employee noticed a pain in his lower right abdominal area. The employee chose not to report it for 10 days.

He was sitting on the bow of the jon boat attempting to pull the starboard swing cable back over the sheave while his heels were positioned on either side of the sheave to hold the boat in place. While the employee was pulling on the cable, the jon boat backed away turning his right leg.

EE was stooped over working in an area w/limited headroom, pulling on a firehose which he was using to wash down the area. He felt pain while pulling the hose. As he exited the area & straightened his back, pain increased. He sat down for a few minutes in hopes the pain would subside. EE was with the witness to remove a pallet of channel iron from the dryer room. As the witness was operating the bobcat, 1 piece of channel iron slid from the pile. The EE bent down with his back to re-position the piece of channel iron against the wall.

EE was walking down a hill on a snow covered roadway to the maintenance shop. As the EE was halfway down the roadway, his feet slid out and the EE hit the pavement with his back. As the EE got up, he felt a twinge and burning sensation in his back.

THE EMPLOYEE AND HIS SUPERVISOR HAD JUST CHECKED THE DRAIN OF WET SCRUBBER (WS-12). AS THE EMPLOYEE WAS WALKING BACK TO THE MAINTENANCE TRUCK, HE SLIPPED ON SOME ICE ON THE EDGE OF THE CONCRETE PAD THAT SUPPORTS THE DEMISTER TANK. HE FELL TO THE GROUND TWISTING HIS RIGHT ANKLE.

THE EMPLOYEE WAS TRYING TO REMOVE A TIRE FROM A FORKLIFT. HE WAS SITTING ON A ROLLED-UP CONVEYOR BELT FACING THE REAR LEFT TIRE. HE NOTICED TWO DIFFERENT-SIZED LUG NUTS BUT DIDN'T CONSIDER THE TIRE HAVING A SPLIT RIM HELD TOGETHER BY 6 STUD NUTS. AS HE HAD 2 REMAINING STUD NUTS TO REMOVE THE TIRE RIM SHOT OUT HITTING HIS ARMS. THE TIRE HAD APPROX. 100 LBS OF PRESSURE.

IN 33 YEARS OF EMPLOYMENT, THE EE HAS WORKED HALF OF THIS TIME AS A BAGGING OPERATOR. IN THIS JOB, THE EE HAS HAD TO GRAB AND STACK 100 LB AND 50 LB BAGS ONTO PALLETS. IN THE LAST 5-10 YEARS, THE EE HAS EXPERIENCED TENNIS ELBOW IN BOTH ARMS. THIS HAS BEEN A RECURRING INJURY WHICH HAS RESULTED IN A TEMPORARY LOSS OF GRIP STRENGTH.

THE EMPLOYEE WAS PREPARING TO POSITION A PALLET IN ORDER TO STACK 100 LB BAGS OF SAND WHEN THE ACCIDENT OCCURRED. HE HAD PICKED UP A WOODEN PALLET FROM A STACK OF 3 PALLETS ON A CONCRETE FLOOR AND SLID IT VERTICALLY ACROSS THE FLOOR 6', THENLOWERED ONE SIDE TO THE FLOOR WHEN HE FELT A SHARP PAIN IN HIS LOWER BACK.

EE WAS SUPERVISING 3 MAINTENANCE WORKERS TO REPLACE AN IMPELLER ON PUMP. PUMP WAS LOCATED IN A CONCRETE CHAMBER BELOW GROUND LEVEL. ONE OF THE WORKERS HAD ATTACHED THE CHAIN WITH A NUT AND BOL T FROM THE 1-TON HOIST TO THE TOP CENTER LIFT RING OF THE PUMP HOUSING IN ORDER TO TIP THE HOUSING AWAY FROM THE PUMP. WHEN IT DID SO, THE 120# HOIST FELL APPROX. 12' DIRECTLY ON TOP OF SUPER
