Caught in, under or between a moving and a stationary object

EE was operating the roof bolter on the #3 section in the #3 entry just inby spad,3032. EE was placing EE's bolt into the drill pot. EE inadvertently bumped into the rotation lever causing ligament damage to the middle finger and ring finger resulting in sutures on both sides of the damaged fingers.

Caught in, under or between running or meshing objects

EE was operating the roof bolter on the #1 North Section in the #1 entry installing outside bolt. EE slipped, right hand landed between the canopy post and long arm. EE left hand landed on the joystick and inadvertently hit the up pressure causing the long arm to extend pinching index finger and middle finger on right hand resulting in (12) stitches.

Over-exertion in lifting objects

EE was carrying tool bag in the #5 entry on the #1 North Section and felt a pop in lower back resulting in a lower back strain.

Struck by falling object

EE was operating the roof bolter in the #2 entry on the #1 West Section. A piece, rock measuring 12""L x 18""W x 2"" thick slid off the canopy striking EE on right shoulder blade causing a slight abrasion and contusion.

EE was operating a shuttle car on the #1 West Section in the #6 entry an hit a pothole resulting in a strain neck.

EE was spreading calcium in the intake at break #13 on #1 belt. EE had allergic reaction causing a skin rash on both legs

EE was operating the C/M in the #6 (right) entry on the #1 North Section MMU 005-0 right side. While backing the C/M out a piece of rock fell measuring 8"" x5""L x30""w x7"" thick striking EE in EE's lower back resulting in a broken L4 and L5. Along with a fractured pelvis and a slight tear to the outermost spinal area.

EE was operating the #4 roof bolter on the #2 North section in the #2 entry. EE tried to line up the bolt with the hole using the drill pot of the roof bolter. EE inadvertently hit the rotation lever, causing the bolt to spin and wrapped their left hand around the bolt. Resulting in a dislocated small finger and received 4, sutures.

EE was working on the #2 North Section operating the right side C/M in the #6 right entry. While, mining the cut through a piece, of rock measuring 18""x23""x1"" thick striking EE on EE's right calf causing a laceration, resulting in four sutures.

EE was working at the #1 tail piece on the walkway side picking up rocks and inadvertently, a piece of rock measuring 8""x8x1/2""x3""thick bounced off the belt striking EE on the head.

EE was working on the #1 West Section helping the electricians install a bottom pin on the 002 C/M that was located in the intersection of the #6 entry near spad #2311. EE stepped of the miner slipped twisting EE's left knee. Please note the accident occurred 10/30/24 EE could not see a specialist until January 6, 2025 and it was determined by the Doctor EE will be off work.

EE was working on the #1 North Section tramming the miner from the #7 entry to the #8 entry. While tramming the miner the standoff caught a piece of the waterline, the EE hit the side breaker and removed the waterline and pinched the middle finger on left hand between the coupling and the standoff causing a laceration resulting in (4) stitches.

Struck against stationary object

EE was working on the No. 2 West Section pulling cable in the #6 entry. EE inadvertently stepped on a rock, twisting right knee.

Fall onto or against objects

EE was working on the #1 North Section in entry #7 L.O.B. carrying cribs and fell twisting their left hand. Please note the employee was supposed to return to work on 10/12/24 per Dr. Excuse. However, EE missed worked on Saturday unrelated to the injury.

EE was working on the #1 West Section (2) breaks outby the feeder loading crib blocks on the scoop. While loading the crib blocks, the corner rib rolled out measuring 5'x 3'x 1x1/2' thick striking the employee on the right foot resulting in (5) broken bones.

Struck against stationary object

EE went to retrieve pogo sticks for the roof bolter operators and while walking back the EE stepped on piece of rock and sprained left ankle. Please note the EE went to the Doctor on 9/18/24 and was placed on restricted duty by the Doctor on 9/23/24.

EE was building cribs at break #42 outby #1 West Section and some debris got in the right eye of the EE. Note: EE went to the doctor the next day thats why I'm not reflecting a LTA.

Employee was operating the #4 roof bolter in the face of #5 entry on #2 North Section. While roof bolting, the employee inadvertently placed their hand between the roof bolt and drill pot smashing the middle finger on EE's left hand causing a laceration.

Flash burns (electric)

Employee was assisting another co-worker change batteries on the #9 scoop. While changing the batteries the cables were caught between the lifting arm of the scoop and battery causing an arc/ flash striking the employee in the left eye.

Employee was working on the #1 West Section building a stopping between #5 & #6 entry. While building the stopping a piece of draw rock fell measuring 25""x12""x1"" thick striking the employee on the left forearm.

Employee was traveling to the working section and picked up a container of Blue Wolf all purpose cleaner. While, traveling the liquid leaked off from the nozzle causing a chemical burn to the right buttock.

The employee was operating the roof bolter on #1 West Section in entry #8 right. While, raising EE's drill steel against the roof a piece of rock fell the employee was able to move out the way causing a strain/sprain to the right shoulder. Note: The employee was waiting to see a specialist which occurred on 5/14/24.

EE was operating the continuous miner on #1 West Section MMU 001-0. At some point the miner got stuck and the EE hooked a chain a shuttle car and the shuttle car operator tried to pull the continuous miner, and the chain broke. Striking the continuous miner operator on the right side of EE's head near EE's eye causing a laceration and received 40 stitches.

Employee was carrying 2 cans of hydraulic oil; employee slipped catching self, causing a strain to the lower back. Note: Employee did not seek medical attention until 04/17/2024.

Employee was operating the right-side roof bolter on #1 West Section in the #7 right entry. While, inserting drill steel in the pot to install extra bolts a piece, of rock fell measuring approximately 20''x 12''x 5 inches thick striking the employee left hand causing a fracture to the base of 4th distal phalanx.

Employee was working on 1 North Section on the Right side of the section. EE was building a stopping at Bk. 35, while walking with block they tripped and fell and the block landed on their right wrist causing a contusion. Please note the employee was off from 02/19/2024 until 02/29/2024 waiting to see specialist.

EE was roof bolting in the #6 entry on #1 West Mains Section. While, bolting EE noticed some draw, rock so EE proceeded to scale the mine roof with a slate bar. As EE was scaling, a piece of rock fell behind EE, striking EE's left leg causing a broken, Tibia and Fibia.

EE was working on the #1 North Section drilling shot holes with the #2 Roof Bolter. While putting EE's stack steel together EE butted the steel against the roof pushed it causing the steel to strike back fracturing EE's right thumb.

EE was pulling the miner cathead across the power box. As EE was pulling the cathead slipped off the box striking EE in EE's left knee.

Employee pulled Roof Bolting machine into the #4 Left to bolt cut. When rib rolled out from behind curtain striking EE in the left shoulder, knocking EE into the controls.

While conducting the weekly exam in the right return from the #2 Section, the examiner found a roof fall in the #8 Entry at Break 25 and 26 on #4 Belt. All approaches to the roof fall have been cribbed and dangered off. The roof fall will not be cleaned up.

On the 1 North Construction site for the 1 West Overcast, while sliding a 6 inch I-Beam into position employee's left hand was caught between I-Beams

Employee was cutting down some top rock (sandstone), when a piece of carbon broke off a miner bit striking the employee in the face.

The employee had moved the ride and went to the Roof Bolter. The ride rolled down a slight grade and pinned the employee between the front of the ride and the roof bolting machine.

Was mining on the A-Mains #2 Section in the #6 right entry when EE was struck by a rock that had been dislodged from the CM conveyor chain striking EE on the left side of face.

Employee was working on the continuous miner, was removing the side lid covers for the panel. Cover slipped out of one hand causing EE's right index finger to be caught between the cover and the rib.

1 North Panel near spad 391 - while removing a regulator employee tripped and fell onto a concrete block, then a portion of the remaining regulator fell onto employee's back.

Moving feeder on the A-Mains. Raising the feeder off the tailpiece, when a crib lying on top of the feeder got squeezed against the top and shot the crib out the side, striking EE on the left side of head.

Truck driver was putting water in truck. They climbed up on the tire. When climbing down they slipped and fell twisted a knee.

Brk 3 in Roadway on 3 Belt, EE was lifting beam up on overcast. Was placing beam down on top of wall and pinched EE's hand (between thumb and index finger on left hand) between the beam and top of wall

Using an insert puller to remove insert from CM. A pick hammer was placed behind the insert puller to use as a wedge, the pick hammer came out under pressure glanced off another lug and struck employee in the face (under the right eye).

Setting the feeder 2 brks inby #202 spad in #4 entry, blocking the cats to get it high enough to go over the tail piece. Was throwing a crib under the cat on the offside, when EE struck their left hand against the feeder and the end of the crib block.
