Caught in, under or between a moving and a stationary object

EE was flipping a rock from off the top of the C/M into the conveyor boom. In the process of flipping a rock into the conveyor, the rock tilted and pinched EE's right index finger against the mine roof, causing a laceration to the right index finger.

Over-exertion in pulling or pushing objects

EE was installing a flyboard in the No.4 entry of No.1 section. EE pushed the board forward before finishing bolting the board in place. EE was pulling the board into position and felt something pull in their right arm.

Struck by... (Not Elsewhere Classified)

EE was cutting the belt to make a splice, and the knife went through the belt, cutting EE on right leg just above the knee.

Accident type, without injuries

A non-injury roof fall accident occurred on 09-26-24 at approx. 5:30 pm. The fall occurred in the #6 entry in-by (SS #6039) at the #12 crosscut on the #1 working section located on the #5 North Panel. The roof fall fell above the anchorage. The roof fell measured approx. 5' thick 18' long and 15' wide. The fall was cleaned up.

Struck by falling object

EE was at the Power Center and heard the breaker knock on the right-side roof bolter. EE stated that they went to check the cable. EE states that they saw a piece of draw rock on the cable. EE states that they bent over to remove the rock off of the cable when another piece of draw rock fell striking them on the back of the right hand causing a fracture to one of their fingers.

EE was throwing timber butts into a scoop. EE received injuries to right foot/big toe due to the scoop bucket.

A non-injury roof fall occurred between break No.27 and No.28 of the No.4 North Panel in-by spad No.5599 in the No.4 entry. The fall does not extend into any intersections. The fall is estimated to be 7' thick.

A non-injury roof fall occurred between break No.27 and No.28 of the No.4 North Panel out-by Spad No. 5604 in the No.6 entry. The fall is within the heading and does not extend into the intersection. The fall is estimated to be 7' thick.

A non-injury roof fall occurred between break No.27 and break No.28 of the No.4 North Panel approximately 180' in-by spad No.5573 in the No.5 entry. The fall is within the heading and does not extend into the intersection. The fall is estimated to be 7' thick.

Struck by falling object

EE was bolting in the No. 7 right. EE put the steel against the top to start drilling. A piece of draw rock fell out around the bolt hole and hit the EE on the left hand causing a laceration 7"" long. The rock scraped down the left arm and hand causing a laceration. The EE received 10 sutures to close.

A non-injury roof fall was found on 12/05/2023 while conducting an airway examination. The fall is not in a travel way, airway, or an active section. The fall was located in the No.9 entry on the Stowe Submains in between survey stations 4648 and 4661.

A non-injury roof fall occurred on Nov. 6th, 2023 1 break-in-by spad #3967 in the intersection between seal #11 and seal #10 in the #4 entry.

Caught in, under or between a moving and a stationary object

EE was helping the right-side pin crew install a curtain board. EE was holding the board with right hand and had left hand against the top. EE told the operator to set the T-bar when they were ready. The operator raised the T-bar up and the EE left their left hand against the top and when the T-bar was raised it pinched the left hand and fingers against the top.

On October 20, 2023, a non-injury roof fall occurred at approx. 8:00 am at Break 22 on 1C Belt in the #5 entry inby spad 4536. The fall is estimated to be 8 high, and 20 long in the intersection of our track/belt entry.

A non-injury roof fall was discovered from 20 ft. inby the rib line at spad no 4671 in the No 5 entry extending inby to 20 ft outby the rib line at spad no 4670 of the Stowe Submains between crosscuts 30 and 31. It measured approximately 60 ft long and 5 ft thick. The fall was cleaned up and supported in accordance with roof control plan.

A non-injury roof fall occurred on the 1D Conveyor Belt in the intersection located at Survey Station No. 4607, Crosscut 2, on the Stowe Panel No.1. The roof fall was approximately 30'L X 20'W X 7'H.

A non-injury roof fall occurred on the NO.1 section between #5 and #6 intersections located two crosscuts in-by SS# 4612. The roof fall was approximately 90' long X 20' wide X 10' high. This fall has been cleaned up.

EE was bolting top when a piece of rock slid off of the operator's canopy and struck EE in the left foot. The EE states EE twisted EE's knee pulling EE's foot out from under the rock.

A non-injury roof fall occurred on the No. 1 Section in the No. 7 right crosscut into the No. 8 intersection on the Stowe Submains at crosscut 27 in-by the loading point. The fall measured 25' long by 20' wide by 5' high.

A non-injury roof fall occurred on the No. 1 Section at crosscut 23 in the No. 4 Entry, two crosscuts out-by the loading point. The roof fall measured 6-8 feet thick.

Struck by powered moving object

EE was loading a S/C with a C/M Machine in the NO.1 heading. The EE thought the S/C had left and stepped out to make sure the C/M was still on center. During this process the S/C struck the EE.

Accident type, without injuries

A non injury roof fall occurred in the No.3 Entry at Survey Station 3980 on the Cartwright No.3 panel. The roof fall was approximately 60 feet long by 18' to 25' wide by 6' high which begins in the No.3 intersection and travels into the left adjacent crosscut. The fall will not be cleaned up.

Over-exertion in lifting objects

EE was changing out planetary on C/M Machine. EE bent over to pick up Sun Gear and felt pain. EE continued through 07/22/2022. EE started missing work on 07/25/2022.

Accident type, without injuries

On June 30, 2022 a roof fall measuring 30'L X 14'W X 7'H occurred on the No.2 section while mining pillars jut in-by survey station No.3500 in the No.6 entry. The fall was cleaned up using an MSHA and WVOMHST approved cleanup plan.

Fall to the walkway or working surface

EE was positioned in the 4 left intersection on 2 section. A scoop was in the process of pulling cable in the outby direction down the No. 4 entry. The cable came across the intersection where EE was standing causing EE to fall. EE struck EE's face which resulted in a laceration to the lip and nose.

On June 16th, 2022, the pre-shift examiner was conducting the pre-shift for the Owl Shift on 1 Section. While examining the intake to the refuge chamber, a roof fall was discovered between the #5 and #6 entries between Spad 3816 & 3826. The fall is dangered off from all approaches & is currently reported on the 75.1200 map located in the meeting room.

Caught in, under or between a moving and a stationary object

EE was pulling the pin that connected the tow bar from the supply motor that connected a low boy flat car. The lowboy was tied off on one end. The flat car begin to roll as the employee was trying to uncouple the car resulting in fractures and lacerations to the left middle and ring finger.

Struck by rolling or sliding object

EE was mining in the last left side lift of the No.3 entry (Inby Spad 306/Between Pillar blocks 94 and 95), when the right rib rolled out striking the EE in the back of the legs.

Struck against a moving object

EE was operating a rubber tire ride and was in the process of traveling to the No.2 section to help work on a C/M. EE turned up in the No.6 entry when the front tire of the ride hit a rock and spun the steering wheel around striking the EE in the hand cause a fracture to the top of the EE's hand.

Accident type, without injuries

A roof fall was found on 10/27/2021 during the weekly examination this fall is located in the Right Fork No.1 panel. This fall has been dangered off from all approaches.

Fall onto or against objects

EE bent down to skive belt and lost EE's footing. EE slipped while EE had EE's knife in hand. The knife cut EE's inner thigh as EE was trying to catch self from falling.

Struck by rolling or sliding object

EE was in the NO.8 entry operating the C/M. A piece of coal rib rolled out striking EE in the lower left leg.

Fall to the walkway or working surface

EE was exiting a scoop and slipped causing EE to strike knee on the mine floor. This incident was not reportable until 03/17/2021 when the EE was placed on restricted duty.

2020 · 2 incidents

Fall from machine

EE was mounting a 930 G loader. EE was ascending from the first to second step. During this process EE fell hitting their left leg. EE continued to work at full job assignment until February 26,2021. EE is now being reported as a NFDL incident due to EE currently being off work.

Struck by falling object

EE was carrying miner bits to the right side of the miner approximately 50' in-by survey station 728 when a piece of draw rock fell striking the EE on the top of the left foot.
