Accident type, without injuries

A non-injury roof fall occurred on 10/15/2025 at 11:54 am. The fall occurred in the #9 entry at the cc17 intersection in-by Spad No. 2321 on the 1E mains (5 belt) panel in the primary escapeway air course.

Struck by falling object

The EE stated that they were on the offside (Right) of the right bolter on No.1 section. EE and their bolt partner were bolting in the No.6 Right Crosscut. The EE stated that the drill steel got stuck in the top. EE had their hand on the drill pot, and a piece of draw rock fell from the top and hit their left thumb.

Struck by falling object

The employee stated that EE was on the operator side of the right bolter on #2 section. As EE was backing out of 6R, EE spotted a piece of draw rock. The rock looked as if it went back into the rib, so they chose to bolt it to the top rather than try and pry it down with a slate bar. When EE pulled the starter out, the rock fell and broke on the mast, striking EE's left forearm.

A piece of draw rock fell, striking the EE on EE's right 5th (pinky) finger, causing a laceration which required sutures to close.

EE dropped a bearing carrier on EE's right foot, causing a fracture to the top of EE's foot.

Caught in, under or between a moving and a stationary object

EE was attempting to unload supplies from the flat car. EE noticed that EE needed to adjust the forks on the forklift. EE was wiggling the forks and trying to spread them apart. EE stated that EE lost footing and EE's hand slipped behind the fork when it slammed mashing EE's right ring finger.

A non-injury roof fall occurred at approximately 1:00 pm on the #1 working section in the #2 entry. The fall occurred approximately 150' in-by survey spad 3609. The fall occurred as the section was retreat mining this entry.

A non-injury roof fall occurred on 07/17/2024 at approximately 8:45 a.m. The roof fall occurred on the No.1 working section. The section was retreat mining in the No.9 entry. The fall area was just inby the No.16 crosscut near the No.3753 survey point. The fall was approximately 6' high X 15' long.

Roof falls. The first is in the number 3 entry going from the inby corner of the intersection at brk 13 (spad # 3535) to the outby corner of the intersection at brk 14 (spad #3453). This fall is approximately 100' long x 20' wide x 8' high. The second area is in the #4 entry in the intersection at spad #3436 at brk 13. The fall is approximately 20' long x 20' wide x 8' high.

A non-injury roof fall occurred on 10/11/2023 in the No.6 entry 2 crosscuts outby the No.1 section in the intersection at survey station No. 3360 on the SC13 panel. The all approximately measures at 30'L X 20'W X 6'H.

A non-injury roof fall occurred on 09/18/2023 at approximately 1:48 a.m. on the 1E Mains (company 2A) belt line at crosscut 17 survey station 2317. The fall went into the No.4 and No.6 crosscuts. The fall is approximately 50' Long X 20' wide X and 11' high.

A non-injury roof fall occurred at approximately 3:50 a.m. on 09/13/2023 on the 1D belt line in the intersection at crosscut No. 14 survey station 2471. The fall went from the intersection over into the No.4 and No.5 crosscuts. The estimated size of the fall is approximately 40' long X 20' wide X 10' high.

An unplanned roof fall occurred on SC panel 9 at survey station 2473 in Crosscut 15. The fall was approximately 24'L X 24'W X 10'H. This is in the No.4 (neutral/track) entry just out-by the active No.1 section.

EE states that EE was in the process of removing the drill steel. As EE reached to pull the drill steel from the rib a piece of kettle bottom became dislodged striking the EE's hand against the drill pot causing a laceration and fracture.

EE was preparing to work on the panel of the right-side dump S/C. The rub rail was bent against the panel lid. EE jerked the panel lid and it dropped striking EE on the left middle finger causing a fracture and laceration.

An unplanned roof fall occurred on SC panel 10. The fall is in the #7 entry at Cross Cut 32. The fall measures approximately 25'L X 19'W X 10"" Thick. There are no active workings in this area. The fall will be cleaned up.

EE was cutting a water coupling straw in half, during this process, EE cut the inside palm of EE's left hand.

EE had reached down to pull cable slack. EE felt something pull in EE's right forearm.

Truck driver stated that while scaling in at the mine, leaned over toward the scale box bracing to enter load number when left wrist popped causing severe pain. EMT at the mine site wrapped EE's wrist to stabilize it.

EE was hit in the face with drill steel. EE's safety glasses were pushed against EE's face causing the laceration.

A roof fall was discovered 1 crosscut in-by survey station 2320 in the right return approximately 13 crosscuts outby the No.2 working section. This fall does not impede travel or ventilation. We plan on dangering the fall off in all directions.

A non-injury roof fall occurred on 6/6/2022 located in the No.6 entry of the No.1E Mains at 10, spad 2216. The fall was cleaned up and re-bolted.

EE was installing bolts in the No. 9 heading. EE had put bolt and glue in hole and was holding the bolt with EE's right hand while starting it in the hole. Once bolt was in about 1' the bolt hung up/jumped and popped out of chuck striking the EE on the hand.

Fall from machine

EE was trying to pull tarp out and it was hanging up. EE got out of truck, pushed in tarp bar. The bar came back but the tarp itself was wound up in a tarp box. EE proceeded to come up and push tarp when it finally broke loose, EE lost footing and fell to the ground.

SC Panel 3 - Fall in return located in No.5 and No.6 starting 1 break inby regulator line extending into Gob. Also located 1 break inby 1306 spad, extending through right hand break to intersection of number 8 entry.

EE was cutting brush outside around the dispatch office. EE was cutting a small tree with a bow saw when the bow saw slipped and cut the left index finger that required sutures.

EE was making the end on a piece of move belt. EE was using utility knife when the knife slipped cutting the EE on the left knee. The EE received sutures to close the laceration. EE is back to work in normal job position.

Employee stated that EE was attempting to hang bolter cable when the cable pulled unexpectedly causing EE's hand to be pulled into a pizza pan resulting in a laceration to EE's right thumb.

Employee stated was moving the RB Machine through fly pads when a bundle of plates fell into the deck and struck right arm just above the elbow.

Employee stated that EE was cutting a piece of structure off the tail piece, when the structure came loose and struck EE in EE's left leg.

Employee stated EE was taking the rub rail off the CM when EE's hand slipped and pinched between the rub rails causing a small laceration and a fracture to EE's left ring finger.

Employee stated that EE stepped off of the back of the feeder onto a piece of coal which caused pain in EE's right ankle.

Employee stated that EE bent over to pick up a battery jumper cable when EE felt a pain in EE's lower back. Employee was taken off work by physician.

Employee stated that as employee was attempting to cut a piece of belt with a belt knife, the knife slipped and employee cut right thigh. Employee received staples. Employee returned with no restrictions on next scheduled shift 12/7/2018.

Employee was shoveling along belt when a piece of rock fell and stuck EE on the back of EE's hard hat.

Employee was shoveling belt at Brk 7 on the #4 Belt when a piece of draw rock struck them on the top of the head. Employee was taken off work on 11/16/2018 and has not returned to work.

Employee stated was moving cutting slack out of the roadway when a small piece of rib rock fell out from the top of the rib and struck employee on top of right foot. IE sought medical attention on Friday 10/12/2018 where IE was diagnosed with a fractured bone in foot and was taken off work by IE's doctor.

Employee stated that ee went underground to make the face area. On ee way back outside on the number 5 belt line ee fell hurting back and left arm

On 12/14/2017 a fall occurred at this mine. The fall occurred at 7 cross cut on #3 belt in the right return. This impeded ventilation for the mine.

employee stated that employee was changing a bottom roller on the overland belt. standing in mud employee's foot got stuck in mud employee went to turn and walk and felt something pull in right knee.

Employee was waiting on a coal Hauler to return to the miner and had backed the miner up in supported top to get a snack and drink from their dinner bucket that was on top of the miner. When employee was stepping away from the miner a section of draw rock fell out striking them in the back and lower left leg.

The employee stated was bolting in #7 heading, slipped on some Oil on the bolter hitting right knee on the bolter frame.

Employee stated was bolting in 1 left, was drilling with bolt machine when rock fell between bolts and ATRS hitting the employee in the right shoulder and arm. This occurred inby spad 434.

Employee stated EE was climbing into the Roof Bolter when a piece of draw rock fell striking EE in the head and neck area cutting left ear.

Employee stated was attempting to place the bolter cable onto the scoop when a loop of cable struck EE in the mouth busting lip and chipping tooth.

Employee stated that EE was tightening some bolts on the shaker system. While tightening the bolts, the wrench slipped and EE hit right elbow on a piece of the housing on the shaker system. Employee did not start missing work until 6/26/2017.

Employee stated that employee was attempting to unload stopping plaster, when employee stood up with a 50 lb. bag of ""B- Bond"", employee felt a pain in employee's back.

Employee stated that EE was walking at the drift of the track entry when EE tripped over a crib block and felt a pain in lower back. Employee did not start missing work until 5/18/2017.

Employee stated, employee was walking on the off side of the Belt. While walking the employee tripped over a crib block, and falling into belt chain, cutting over employee's left eye.

Employee stated that while tramming the Fletcher walk through roof bolter, a T3 strap fell from the side of the roof bolter and struck left foot.
