After mining had ceased and mine entrances sealed, the operator received notice of Occupational Illness on the named EE. After reviewing part 50 reporting and present status of the sealed mine we wish to report the illness claim and terminate employment on date 6/20/16 that mine was reported abandoned to MSHA.

Exiting IT14 loader, when he stepped down onto ground with his left leg under him, his knee buckled causing him to fall to the ground. Employee continued to work in his normal occupation until 7/27/15 and then started missing time.
EE picked up one side of the top roller while another ee was lifting the other side and when they turned to install the top roller in the rail, ee felt a sharp pain in his left calf muscle. EE continued to work in his normal occupation until 6/15/2015 and then started missing time.
Employee was descending down the ladder when he missed the last step causing him to twist his left ankle. This accident resulted in a fracture.
