Employee was backing up to be loaded and backed into the wall and was jarred. After an investigation was completed and medical records were obtained indicating a strain to the neck, shoulder, and contusions to the lower back this incident became reportable as of April 3, 2012.

Employee picked up a used air filter that had a burr on the inside mesh area, causing a laceration to his ring finger on his left hand. He was transported to Boone Memorial Hospital by Boone County Ambulance where he received six sutures to close the wound.

Struck against stationary object

Employee was attempting to close a window inside the portal trailer. The window was stuck and he used his left hand to tap on the window rail causing the window to break causing a laceration to the edge of his hand underneath his fourth finger requiring seven sutures.

Struck against a moving object

Employee was operating a D-11T Dozer #20653. He was backing up and the dozer went up onto a rock. EE was looking out the side window when this happened causing his face to come down into the fire suppression panel and chipping a tooth. This accident is now reportable since he had dental work to repair the tooth.
