EE was fatally injured while installing a rib bolt in the 8 Right Crosscut on Unit 1 when a slick-n-side formation fell from the mine roof inby the ATRS of the bolt machine, struck the mine floor and protruded into the employees work area. The employee was struck against the back with the rock and caught between the drill pot/mast area of the Fletcher DDR-13 WT he was operating.

While lining strap hole with drill steel they stepped on loose rock, slipped and struck right wrist on strap edge between glove cuff and Kevlar sleeve, causing laceration. Treated with 7 sutures. Employee was walking down #1 Entry when EE tripped over gob on the mine floor, fell toward the rib, and struck EE's right fifth digit on an installed rib bolt, causing a laceration. Employee was struck by a large section of top rock while kneeling beside a continuous miner during troubleshooting. The rock fell without warning, pinning EE's right leg and fracturing EE's fibula.

Employee was assisting a roof bolter move from Entry #6 to Entry #5 when the bolter slid into a hole in the mine floor causing its rear to drop onto the employee's left foot, catching it between the bolter and mine floor. Employee was transported to a hospital, where employee was diagnosed with two fractures and two dislocated toes in left foot. This is a Lost Workdays Incident.
An unintentional roof fall was discovered at the Winchester Peerless Rachel Mine, 1st Left Panel off 2 West Submains at 9 Break #8 Entry. The fall is approximately 20 ft. long and 7 ft. thick. The fall has been dangered off and MSHA and WVOMHST officials have been notified. No injuries or entrapments occurred from this incident. The fall does not impede ventilation controls.
Employee was cutting sprocket chain that was fouled on the offside drive axle to install a new intermediate shaft when the chain flew apart as it was being cut. The chain struck the employee on the left middle finger, causing a laceration. Employee received 6 sutures to close the wound and was given a full release to return to work on next scheduled shift. No Days Lost
Employee was using utility knife to skin back outer jacket of lifeline to connect to mine phone when the knife slipped, causing EE to lacerate their left thumb. Employee sought medical attention where EE received 3 sutures to close the wound. Employee was given a full release with no restrictions to return to work on the next shift, performing EE's normal duties. No Days Lost.
An unintentional roof fall was discovered at the Sydney Eagle Portal, Joe's Creek Mains between 14-15 Break. The fall is approx. 40 ft. long and 6 ft. thick. The Fall has been Danagered Off and MSHA and WVMHST Officials were notified. No injuries or entrapments occurred from this incident. The fall does not impede ventilation or travel.
Wet, muddy conditions on slope. Employee was leveling a railroad track and was removing the jack used for leveling. When the employee released pressure on jack the bar slipped and contacted EE's left arm causing a laceration
The Sydney Eagle Portal personnel elevator had a malfunction of the door switch, making the elevator inoperable for more than 30 minutes. MSHA and State Officials were notified and have been on-site investigating the event. NOTE: The elevator has been checked by MSHA and State Officials and is now operational. No entrapments or injuries occurred from this event.
An unintentional roof fall occurred on the # 1 Retreat Mining Section, between the #1 and #2 entries at Brk 14 covering up the Mobile Roof Supports (MRS). The affected are was dangered off and MSHA and State Agencies were notified and have been on site. A MRS retrieval plan was submitted and implemented. No injuries or entrapments resulted from this incident.
Employee was walking down track entry when EE tripped over gob along the mine rib causing EE to fall into the rib. As EE was falling, EE stuck arm out to brace fall causing EE to strike right forearm against a rib bolt plate causing a laceration. EE received 6 sutures to close wound and received a full release to return to work on EE's next scheduled shift on 5/28/2024.
While passing through a mandoor due to an inoperable inby airlock door, the outby airlock opened, causing a sudden air pressure change that made the mandoor slam shut, injuring EE's right thumb. EE visited CAMC Surgery on 5/22/2024 and was diagnosed with a displaced fracture of the distal phalanx. EE was released and returned to work on 5/23/2024, performing EE's normal duties.

Employee was walking to the mantrip at the end of the shift when a rock measuring 3 feet, 6 inches wide and 2 inches thick, fell from the mine roof striking EE on the top of the head, back, and landed on EE's left foot / heel. Employee sought medical attention and was taken off work for 6 weeks due to pain in EE's foot / heel. This is being claimed as a Lost Work Days Incident.
Employee unhooked crossbar of CAB U-Hanger to let shuttle car cable pull down out of it when the cable jerked down and one of the hangers, unexpectedly still attached to the cable, contacted EE's left hand as it jerked down, causing a laceration. Employee received 20 sutures to close the wound. This is a Medical Reportable Incident.

Employee was bolting in 3 Left Face, when a rock measuring 3'x1' slid down the ATRS of the roof bolting machine striking employee in the left rib area. Employee finished shift & returned to work on next shift. On Tuesday, December 26, 2023, employee sought medical attention & was diagnosed with a spleen bleed & had surgery that day. This is now being claimed as Lost Workdays.
Employee was exiting the maintenance scoop, and as EE was twisting while exiting, EE felt pain in EE's left knee. Based on the MRI Results received by the operator on 12/13/2023, the employee has a torn meniscus and is off work. This is being claimed as Lost Workdays.
Employee fell off the 3rd step of a 4 ft. ladder while plastering a stopping in an 8.5-ft high area. Employee hit the mine floor but continued working, developing back pain. Employee sought medical care and was taken off work until December 8th pending a follow-up visit to medical center.
An unexpected roof fall occurred on the # 1 Section in # 4 entry at break # 5 at 3:20 p.m. The roof fall is roughly 6 feet thick and 18 feet wide. This occurrence resulted in no injuries or entrapments. The MSHA and WVMHST hotlines were contacted, and the occurrence was reported. The location has been dangered off, and a clean-up plan is being drafted for submission.
On November 21, 2023, the operator was notified by MSHA that the employee had a sufficient degree of pneumoconiosis.
Employee was operating Co. #506 Scoop, was hit from behind by Co. #318 Shuttle Car causing neck injury. Notified supervisor. Treated by EMT-M's onsite; later diagnosed with Cervical Strain at CAMC Hospital. Cleared for work on Oct. 9. Visited family doctor on Oct. 10 due to ongoing pain, confirmed diagnosis. This is being claimed as a Lost Workdays Incident.
Employee stepped off the back of the roof bolting machine when EE stepped on a rock, twisting their ankle. Employee was diagnosed with an ankle fracture. This is a lost workdays incident.
On the date of November 11, 2024, the operator was notified that the employee was diagnosed with hearing impairment due to work-related noise exposure. The employee voluntarily resigned from the company on the date of August 29, 2023.
Employee was walking down a steep grade on the section, to change a broken torque shaft carrying a tool bag on EE's left shoulder and a new torque shaft on EE's right shoulder when EE stepped in a hole in the mine floor causing two fractures of EE's left foot. A closed fracture at the distal fibula and a closed fracture on the cuboid bone. This is a lost workdays incident.
Employee was riding a rubber-tired ride to the section when they hit a bump in the mine floor causing employee to bounce out of the seat and strike head against a roof bolt plate, breaking through the hard hat and lacerating the top of employee's head. Employee received 10 staples to close the wound.
Employee was digging out gob from around the track with a pick hammer when a piece of rock broke off and hit the employee in their left front tooth, causing the tooth to partially break. No Days Lost - Medical Reportable Only.

Employee received 3 sutures to close the wound on the wrist. Employee was given a full release and returned to work on employee's next regular scheduled shift (12/23/2022). This is a medical reportable only report.
Employee was installing a 4 ft. roof strap when the strap fell off of the ATRS of the roof bolting machine, striking them in the left leg causing a laceration. Employee received six sutures to close the wound, received a full work release with no restrictions, and returned to work on their next regular scheduled shift.
Employee was laying track at 5 South brk 24 when a track rail slid out of a scoop bucket landing on employee's foot. Employee sought medical attention at a hospital where employee was diagnosed with a fractured left foot. Employee was given a release and did return to work on next regular scheduled shift (6/15/2022). No Days Lost
EE was installing rails and spreaders over the new 7 South Belt Head when EE stepped on a turnbuckle and twisted knee. EE received an MRI on 3/24/2022 and received the results of the MRI on 3/29/2022 diagnosed with a severely torn meniscus. EE is to remain off work until corrective surgery can be performed. This is a Lost Time Accident.
On January 13, 2022, employee was diagnosed with occupational pneumoconiosis
EE stated that EE was in the process of installing Roof Bolts in the #5 Left entry at 37 brk on the #1 working section. During the process of installing the corner bolts, EE stated EE's glove on EE's left hand got caught between the bolt lip and the chuck causing EE's left hand to wrap around the bolt EE was installing, dislocating EE's left middle finger.

Employee hung a mine phone and when EE started walking away EE heard the phone falling, EE stated EE twisted around to catch it and felt pain in EE's right knee. Employee began missing work on 6/22/2021 to have corrected surgery.
Employee was walking through parking lot when EE slipped on ice and fell on EE's left side. Operator was notified on 6/8/2021 that the employee had a partial tear of EE's left shoulder. Employee began missing work on 6/10/2021 for corrective surgery of this injury.

A roof fall was discovered in the primary escapeway, BRK 4 on 1 East, 3 bolts pulled near area where rock was taken for a potential overcast. The dimensions of the fall were approximately 5' in thickness X 15' in width X 2' in length.
Employee was changing a bearing on a slinger duster with a chisel and hammer when a small piece of metal shot from the bearing and entered EE's right thumb. Metal was removed by a surgeon and sutures were then required at that time due to incision.
Employee was assisting to change a top piece of belt structure when it slipped out of EE's hand and hit EE's right foot resulting in fractures to the foot.
Employee was traveling to the surface on a mantrip after shift when the mantrip came to an abrupt stop due to a broken fish plate. Due to the stop the employee struck face on the mantrip which resulted in 5 sutures to upper lip and a fractured tooth. This is a medical reportable incident only, the employee returned to work on the next scheduled shift.
Employee was struck by a piece measuring approximately 5' in length X 2.5' in width X 8.5"" in thickness tapering down to 0"" while bolting. Incident resulted in a laceration and fracture to the employee's left hand middle finger that required surgery.

Employee was dropping a cable anchor on the Co. No. 309 Shuttle Car, employee stated employee lifted upward with right hand on the bottom of the anchor and felt a pop and pain in lower back. Employee was diagnosed with a lower back strain and placed off work. This incident is being reported as a Lost Time.
Employee was exiting the mantrip after arriving to the slope bottom when EE placed EE's left foot on the barrel connected the throw arm of the switch causing EE's ankle to roll which resulted in a fracture of the distal end of the left tibia. This incident is being reported as a Lost Time incident.
Employee was lifting 16' roof mats onto a scoop when they felt the left shoulder pop twice which resulted in pain and a sprain/strain injury, employee was seen by a specialist on 5/14/2019 at which time employee was taken off work. This is now being reported as a Lost Time incident.

The employee stated ee was in the number three entry and in the process of swinging back in after installing a rib bolt when right knee made a pop sound. The employee was taken via by ambulance to hospital where ee was diagnosed what a torn ACL. This incident is being reported as a Lost Time Accident.
Employee stated was shoveling up some gob on three south belt when employee twisted & felt back pop. The employee sought medical treatment and was taken off work. Employee stated back was hurting before going underground, was denied workers comp on the injury. On December 10, ALJ overturned the decision. The incident is being reported as a Lost Time Incident.
The employee stated EE was in the process of dropping miner cable in the 5 entry on the 1-section and a felt sharp pain in EE's back. The employee sought medical treatment at CAMC General and taken off work. This incident is being reported at Lost Time Accident.
This fall is located in the old works of the mine this fall does not affect any airway of the mine the fall was approximately 6 ft high 18 ft wide 30 ft long the area was breakered off to block entrance to the fall
An employee found this roof fall while travelling airways on the morning of 2-28-18 this area had been noticed beforehand and cribs and timbers where set in this area and route of travel was moved to another entry once the fall was discovered the area was dangered off and 4 breaker timbers where set the fall is apprx.40 feet in length 5 ft thick and covers the entire entry
The employee was in the process of knocking out a 12x8 solid block out of stopping when it struck the right arm causing a bruise. The employee later sought medical attention and was placed and restricted duty for seven days. This incident is being reported as a restricted duty.

On 11/10/2017 employee sought medical attention and diagnosed with an effusion of the left knee. The employee was placed on restricted duty. This incident is being reported as a restricted duty.
The fall is located on the 4 west belt at the number 2 break at spad #4114 the fall is approximately 35 feet long 20 feet wide and 5 to 6 inches thick a plan for cleanup has been submitted to MSHA and has been approved.
Employee stated was working outby the section dropping cable when employee stepped in a hole and felt back shift. Size of the hole was 8"" deep. Employee was placed on restricted duty. On 9-22-17 employee was taken off work by the treating doctor.
The foreman noticed the intersection working in the #3 face ee pulled all men and equipment from the area the intersection shortly after fell in the fall measured about 20'by 20' by 6'thick 100ft inby spad #5801
The employee stated was in the process of picking tool bag up when EE felt something pop in EE's back. The employee was transported via ambulance to CAMC where EE was diagnosed with a pulled muscle and was placed on restricted duty. This incident is being reported as a restricted duty.
The fire boss came upon the roof fall towards the end of shift ee called outside to report the fall and state and federal agencies where notified this fall occurred on the 2 west mains 16 brk. in a neutral entry beside of our track entry. No men where injured and no equipment was involved the fall. The fall is 50ft long 18ft wide 5ft thick.
Employee was installing belt structure when they stepped in a hole dropping a bottom belt roller on their left ankle. Employee later sought medical attention and was diagnosed with a fractured ankle. This is a medical reportable incident.

Employee was installing a slide on a set of airlock doors at 1 South break 5. EE had one door tied off while working on the other door and the door came loose smashing other arm between the doors. The employee later sought medical attention and was place on restricted duty.
Employee stated was in the process of stepping up into the motor when foot slipped out from under employee, causing employee to strike left rib cage on the side of the motor. Employee sought medical attention on 3/25/2017 and was placed on restricted duty.

Employee stated was making a belt splice. While attempting to skive the belt the knife slipped out of the belt and caught the employee on the left ring finger causing a laceration about 1 inch long. Employee later sought medical attention and received 3 sutures and returned to work the following day at normal job.
Employee stated EE was in the process of retrieving highline on the # 1 Section when the incident occurred. Employee stated EE's foot slipped of a 3 inch ledge and slid under the scoop bucket, in that process the scoop bucket smashed EE's left foot. Employee sought medical attention and was placed on 1 week restricted duty. This incident is being reported as restricted duty
The fire boss on the owl shift discovered the fall at the start of the shift at approx..11:08 pm, called the mine foreman and the mine foreman called this in around 11:20 no equipment or personnel was involved in the fall. The fall is at 16 break on the 2 west belt.it is approx.. 18ft wide 40ft long and 7ft thick we are in the process of creating a clean up plan.
Employee stated was in the process of throwing a timber out of the scoop bucket, in that process EE lifted the end of the timber up and felt a sharp pain through back and groin area. Employee sought medical attention as was taken off work. This incident is being reported as a Lost Time Injury.
Employee stated was making a belt splice on 2 South Belt when foot slipped causing the knife to come out of the belt causing a small laceration to right leg. The laceration required 4 sutures. Employee returned to work next shift. This incident is a medical reportable incident.
Employee stated EE was walking intake escape way on 2 West Panel, when EE stepped in a hole and twisted left ankle. Employee later sought medical attention and was diagnosed with a sprain and was prescribed crutches and a air splint if needed. Employee return to work on the next scheduled shift. This incident is being restricted duty.
On September 12, 2016 at approx. 2:45 pm we were notified that the employee had been diagnosed with Occupational Pneumoconiosis.
Had a roof fall at spad number 919 at 2 west mains panel in the number 2 entry. approx. size of fall is 40 feet long 20 feet wide and 6 foot thick.
Employee stated that ee was loading a hydraulic pump onto scoop 515 at the end of track on 1 section. Employee stated that ee back started hurting. Employee finished shift returned to work schedule shift. On 9-7-2016 Employee sought medical attention and was taken off work. On 9-8-2016 this became a lost time injury due to missing work.
The employee stated that while in process of skiving a piece of belt the knife slipped out causing a laceration to left hand. The laceration required 3 sutures.
The employee stated that EE had taken a load of gravel towards the #1 section to work on the roadway and was on the way back when EE hit a hole and jarred back. The incident became a restricted duty injury on 7/11/16 due to the employee being placed on modified duty restrictions for a strain.
The employee stated that as EE was lowering drill pot on the roof bolt machine. EE started the rotation and the bolt wrench came out and struck right wrist. The employee sought medical attention and diagnosed with a contusion, however the incident became reportable on 6/30/2016 we EE did not return to work. The mine was idle on the 1st and 4th.
On 6/7/2016 a roof fall was discovered in an area of the mine that is not normally traveled. The fall was located on the 2A West Parallel Submains at break 15 in a cross-cut between the number 6 and 7 entry. The fall area was breakered off.
The employee stated that EE had been cutting and welding on a new rock box for the new 3 South belt head on the 5th. EE stated EE felt fine at the end of the shift and went home and had pain in EE's eyes when EE woke up the next day. EE sought medical attention and was diagnosed with burns due to viewing welding arcs and was placed off work for 1 week.
The employee stated that they strained their shoulder while closing an air lock door to the east mains. The incident became reportable on 6/8/2016 when the employee sought medical attention and did not report to work. This incident is being reported as a lost time injury.
The employee stated that they were cutting the insulation off of a motor tree for a new belt head setup when they accidently cut left index finger. The laceration required 3 sutures, so this is being reported as a medically reportable incident.
On May 10, 2016 a roof fall was discovered in the 2A West Parallel Sub-mains. The fall was located in an intersection in the #5 heading 2 cross-cuts inby spad 4248. The area was dangered off and breakered.
On May 10th, 2016 we were notified that the employee had been diagnosed with black lung.
On 04/19/2016 a roof fall was discovered in the return air course on the 2A west parallel sub mains in the #2 entry at spad number 4243.
The employee stated that while racking a 1000' piece of belt onto a sled the duckbill clamp being used pulled off the side of the belt as it was being pulled and struck left leg. Employee sought medical attention on 4/12/2016 after work and was diagnosed with a contusion and placed on restrictive duty.
On 4/4/2016 a roof fall was discover at brk 33 on the 3 West mains in an area that is not typically travelled. A brow rolled off and pulled permanent support. The area was breakered off.
A continuation of an older roof fall was discovered at Brk 30 on the 3 East Mains in the #6 entry.
The employee stated that they were operating the roof bolt machine and was starting to drill a hole, looked up when a piece of draw rock fell and struck front tooth. The piece of rock broke off one of the permanent teeth. The employee did not seek medical attention, however this is being reported due to the loss of a permanent tooth per Part 50 requirements.
The employee stated that employee was dropping bolter cable and as brought arm downward, elbow struck a sharp edge on a pizza pan that was installed with a rib bolt. The laceration on elbow required sutures and returned at full capacity the next scheduled shift.
The employee stated that they were drilling a hole with the RBM, had made up a bolt and propped it against the tray. As employee dropped mast the bolt fell over and the mast kicked it back towards employee causing the plate to lacerate left jaw. Employee sought medical attention and received 6 sutures and returned to work at normal job.
The employee stated that EE was lifting pizza pans onto the roof bolt machine when EE felt like EE pulled a muscle in stomach. EE later sought medical attention and was diagnosed with a strain and placed on modified duty for 3 days. This is being reported as a restricted duty incident.
On 2/5/2016 at approximately 3:00 p.m. we were notified that employee had been diagnosed with black lung.
On 1/27/2016 a roof fall was discovered at break 15 on the 3 West Panel. It was located in the intersection adjacent to spad 1712 and had previously been cribbed off.
At approximately 8:35AM a fire was discovered on an unattended Polaris EMU that was located at the bottom of the slope in the shop area that was not extinguished within 10 minutes.
The employee stated that EE was going through a mandoor at the return shaft when EE twisted back. EE continued working after the incident until 3/20/2016 when EE sought medical attention and was removed from work. On 3/21/2016 this became a lost time injury due to missing work.

On 10/23/2015 a roof fall was discovered in the 3 West Mains at break 31 in an intersection. The fall was located in the #5 entry at spad 2196.
On 10/19/2015 a roof fall was discovered on the 3 West Mains at Brk 29 in the #6 entry.

On 10/12/2015 a roof fall was discovered in the 3 West Mains air course where miners do not normally work of travel. The was located at break 31 in a cross cut from #3 to #4 entry.
On 10/8/15 we were informed that the employee had been diagnosed with black lung. His last day of employment was 9/3/2012 at the operation.
The employee stated while he was shoveling the 4 West belt he lost his footing and started to fall, but reached out with his right hand to catch himself. He grabbed a piece of top structure at the wing roller and caused a laceration to the tip of his right middle that required sutures. The incident is a medical reportable injury.
On 9/24/2015 a roof fall was discovered that was not located in active workings of the mine. The fall was located on the 3 East Mains break 30 in the #7 entry. The area was dangered off.
On 9/21/2015 at approximately 9:20 a.m. a roof fall was discovered at break 21 of the 3 West mains between the #6 and #7 entry. The area was dangered off.
On 9/21/2015 at approximately 9:20 a.m. a roof fall was discovered at the mouth of the 3 Right Panel of the 3 West Mains.
On 9/10/15, while the mine was owned and operated by Emerald Processing, LLC, the individual stated while he removing drill steel from a previously drilled hole he experienced pain in his right shoulder. The incident did not become reportable until 11/7/15 when the individual initially sought medical attention, at which time the mine was owned and operated by Kanawha Eagle Mining.
While operating the company No. 405 roof bolter the employee was drilling a 12' hole for a cable bolt in the No. 5 Entry. During the drilling process the operator's Kevlar sleeve was caught between two sections of stack steel where it was bunched up around his elbow and when he engaged the rotation his arm became entangled fracturing his foreman.
On August 26, 2015 we were notified that the employee had been diagnosed with hearing loss.
EE stated that he was reattaching a wire to the starter of the 910 mantrip at the time of the incident. He stated that he had to reach under the shields during the process and strained his shoulder and the pain intensified after servicing the wheel unit of a shuttle car. EE later sought medical attention and diagnosed with a strain. He was placed on restricted duty.
The employee stated that he was adding coolant to the 507 diesel scoop due to the scoop overheating. When he was adding the coolant to the scoop, the coolant boiled out, contacting his right arm. On 8/17/2015 the employee sought medical attention and was prescribed antibiotics for an infection.

On 7/28/15 at approximately 7:30 a.m. a roof fall was discovered that was not located in active workings of the mine. The fall was located in the 2 West Mains, Break 14 in the #6 Entry at Spad 1066. The area was breakered off.
The employee stated that he got out of his scoop to load 6 crib blocks and when he lifted and turned with the last block he felt pain in his lower back. The employee finished his shift, but later sought medical attention at which time he was diagnosed with a strain. He returned on his next shift under restricted duty.
On 7-17-15 a roof fall was discovered in the South Mains intake air course at spad #320. The fall was not located in active workings. The fall was located in an intersection and measured 20'x18'x6 to 7'.
EE stated while he was installing roof support in the #4 entry he extended his left hand out to place a bolt on this wrench when a piece of draw rock approx 12""x12""x3-4"" T fell striking his hand. When the rock contacted his hand it pushed it down toward the drill pot before he pulled it away striking his finger between the rock & pot. Chipped bone L ring finger. Medical injury.
The employee stated that he and another employee were in the process of loading a scoop with rock dust and during the process the employee got rock dust into his eye. The employee later sought medical attention and was prescribed prescription eye drops.
EE operating the 507 Scoop on the West scoop roadway travelling outby, pulling the old West Belt Head toward bottom of the slope. Between break 32 & 31, chain came loose and EE backed up, was reconnecting belt head when another EE who was operating the 509 scoop struck the belt head pushing it towards the EE. EE was caught between the belt head & scoop, fractured both lower legs.
EE stated while using slate bar to pry piece of rock from mine roof, a separate piece of rock came out of roof in front of slate bar. That piece of rock fell down bar toward EE's face. EE used left hand to block rock. Rock that struck EE's hand was approx. 3'x2'x3"". EE was wearing metacarpal gloves. EE sought medical attention after his shift & was diagnosed w/bruise.
On 3/31/2015 at approximately 10:50 a.m. a roof fall was discovered that was not located in active workings of the mine. The fall was located in the previously mined 6 West Panel in the #4 entry, in the intersection 1 break inby spad 3129. The area was dangered off and cribbed.
The employee stated that he had just completed dumping the metal from the slope belt magnet and as he was raising the metal door back into position, he felt pain in his left shoulder. The incident became reportable on 3/16/2015 when the employee did not return to work.
EE stated that he was cleaning the #3 entry & had made passes up the right side & middle. As they were hanging the cable, the weight of the cable bent him backwards straining his back. He completed his shift & returned to work on the 17th, but due to discomfort sought medical attention after shift. Reportable due to restricted work activity on 2/17/15.
On 2/3/2015 a roof fall was discovered in the 2 West Mains air course where miners do not normally work or travel. The fall was located in an intersection at break 16.
The employee stated that he was exiting the 507 scoop and as he pushed himself out with his arms, his right foot slipped out and he fell towards his right leg. This caused his right knee to bend inwards toward his left leg. The incident became a medical reportable on 9/17/2015 due to the employee receiving physical therapy.
The area in which the fall occurred was in the 5RT. panel. The continuous miner had finished mining in the No. 7 entry when the roof fall occurred.
2014 · 28 incidents
A roof Fall was discovered in the 4 Right Bleeder in an area where miners do not normally work or travel. No ventilation controls were damaged, nor were any travel ways impeded by this fall.
On 11/5/2014 the operator was notified that the employee had been diagnosed with pneumoconiosis. This is being reported as an illness
EE was attempting to replace a 5"" roller in the 4 East take-up by using a slate bar to pop the roller up from the roller bracket. He had both hands & one knee on the bar. When the roller popped out of the bracket it caused him to strike the back of his right hand against the frame of the take-up. He was diagnosed with a hairline fracture on the back of his hand.
The employee stated that he was assisting with the removal of a scrubber hood on continuous miner when the slate bar he was holding came loose and the weight of the bar lacerated the webbing between his thumb and forefinger. The incident is a medical reportable due to the employee receiving 7 sutures.
On 1/7/14 at approximately 9:59am we were notified that the employee had been diagnosed with black lung.
The employee was positioned at the corner of an intersection to stay clear of a shuttle car that was in motion, but as the cable real tensioned to the cable anchor that the employee was standing beside of it caused the cable to strike his lower lip. The employee received a laceration that required sutures and returned his next shift.
The employee stated that he had backed the MRS out of the last lift of the block and a rock was laying in the MRS over the plow. the employee was trying to push to rock off the plow when his hand slid down a sharp edge of the rock cutting through his glove and lacerating the heel of his hand. The laceration required 6 sutures.
The employee stated that he was constructing a set of overcast steps and while holding one leg of the stairs the other side shifted contacting his right middle finger. This resulted in a fractured right middle finger. This is being reported as a medical reportable.
The employee stated that during a belt move he crossed the belt (locked and tagged out) to get a chain hoist and when he crossed back over he slipped and fell striking his right knee on the water manifold. The employee did not return to work on the 13th. This is being reported as a lost time injury.
The employee stated that the 1 West tailpiece had gobbed out and after cleaning the tailpiece he bent down, picked up a rock, and tossed it onto the belt when he felt pain in his neck. The employee sought medical treatment for the strain and was placed on restricted duty for three days.
While laying track in the track entry of 1 North, employee attempted to split a half header on the rail when he smashed his middle finger on his left hand between the half header and rail
On 7/10/2014 a roof fall was discovered in a travelway entry between break 28 and 29 of the 6 Right Panel of 3 East bleeder. The fall was approximately 6' thick, 18' wide, and 20' long.
On 7/10/2014 a roof fall was discovered in a travelway entry between break 27 and 28 of the 6 Right Panel of 3 East bleeder. Fall was approx. 4' thick, 18' wide, and 20' long.
A roof fall was discovered in the South Airways in an area where miners do not normally work or travel. The fall was located in an intersection 1 break inby spad 301.
On 7/9/2014 the operator was notified that the employee had been diagnosed with occupational pneumoconiosis.
During the preshift examination conducted on 7-6-2014 after an idle period a roof fall was discovered on Unit 3 in the #1 Entry intersection 80' inby spad 2294.
EE was unloading bag dust from one scoop to another and his right knee twisted causing pain. Later in the shift he exited the scoop. While walking around the batteries, his foot got stuck between the side of the batteries and a rock that caused his knee to turn and pop. He sought medical attention and was diagnosed with a sprain/strain.
Operator is submitting this 7000-1 to abate citation #9009049 issued on 6/12/2014 for failure to report. The roof fall occurred over a pressured MRS in the #5 entry, directly inby spad 2071 on the MMU 004-0 that resulted in the piece of equipment being unable to tram itself free without assistance. Do not believe the roof fall meets the requirements as a reportable roof fall.
The employee stated that another employee and himself were carrying a P-50 pump approximately 200 feet. While carrying the pump toward the water hole he strained his lower back. The employee later sought medical attention and this is being reported as a medical due to scheduling follow-up visits. On 7-29-14 the EE began to miss work due to the injury.
Employee stated he was walking the belt entry back to section from locking out the belt head when a piece of draw rock fell from the roof and struck him on the neck and back resulting in a strain of the neck and back. This incident became reportable as lost time injury on 5/20/14 due the employee missing 1 day as a result of the injury.
Operator is submitting this 7000-1 to abate citation #9009048 issued on 6/12/2014 for failure to report. The roof fall occurred from a completed left-hand lift in the #8 entry, directly inby spad 2101on the MMU 004 causing a piece of rock to block the route of the MRS. Do not believe the roof fall referenced in this citation meets the requirements as a reportable roof fall.
The employee stated that he was prying a metal plate away from the feeder conveyor chain with a slate bar when had pain in his shoulder. The employee had a previous injury at another mine that he was being treated for, but this incident is being reported due this occurrence which resulted in a strain and follow-up visits.
On April 23, 2014 we were notified that the employee had a medical examination that indicated evidence of pneumoconiosis.
On 4/25/14 the employee reported that he had a strained groin that was suspected to have occurred on 4/23/14 when pulled cable slack from a shuttle car reel. This is being reported as a lost time injury.
On 3-7-14 we were notified that ee was diagnosed with occupational pneumoconiosis.
EE stated he was knocking the lugs loose from the track rails while retreating the track off the #2 section. During the course of the process ee reported pain in his lower back. This incident became a lost time inj on 3/7/14 when the ee missed work associated with the inj. A sought med treatment on 8th was diagnosed with a strain. EE missed 4 days and retuned on the 12th.
The employee stated he was walking to get on his shuttle car when he stepped in a hole and twisted his left knee causing a strain/sprain. The employee continued to work until 2/24/14. The employee began missing work on 2/25/14 while under doctors care.
The employee stated that he was attempting to push a rock into the throat of a CM and as the back of the rock tipped into the throat it caused the end the employees hand was on to tip up towards the mine roof. The tip of the employees right ring finger was caught between the mine roof and the rock. The employee received a fracture to the tip of his ring finger and five sutures.

A roof fall was discovered at approx. 11:10p.m. in the 2 West Mains travel way. The fall occurred in the #3 entry of Spad 1062 and measured 40' long X 20' Wide X 8' long.
The employee stated that he was walking by the power center and tripped over the continuous miner cable. As the employee fell, he stuck his hands out to try and brace his fall. However, his left hand struck a metal ear on the outer edge of the platform, puncturing employee's left hand, resulting in three sutures.
A roof fall was discovered at approximately 2:00 pm in the 2 West Mains primary escapeway. The fall occurred at break 15 in the #6 entry of the 2 West Mains. The fall was approximately 35' long, 18' wide and 6' high.
The employee stated he was lifting muddy line curtain and felt pain in his left shoulder. The employee sought medical attention and received physical therapy, but did not miss any work. After further consideration, this is being filed as a medical reportable.
The EE stated that while dismounting the rear of the 404 roof bolter, he stepped into a small hole of water that contained a hidden piece of rock. While dismounting, he stepped onto the rock with his right foot. The rock was slicken sided which caused his foot to slip off the rock twisting his ankle.
A roof fall occurred in #7 Entry of the 2 West Mains panel above anchorage. The fall was located at spad 1109.
The employee stated that he was operating a shuttle car when the car slid into rut causing his head to contact the shuttle car canopy. The employee had recently returned to work due to a medical issue with his neck, but stated he felt pain. After further consideration and because the insurer found the injury compensable, this is being reported as a lost time injury.
The employee stated that he and two other employees were lifting miner cable to place in hangers outby the face of #5 entry when he felt pain in his back. The employee sought medical attention and was diagnosed with arthritis and returned to work. After further consideration and because the injury was compensable, this is being reported as a lost time injury.
The employee stated he was bolting in #7 entry and had been experiencing back pain for a week after bending a bolt. The employee stated he was being treated for a back injury through a previous employer at the time of the injury. After further consideration and because the insurer found the injury compensable, this is being reported as a lost time injury.
On 4-18-13 a roof fall was discover in the intake aircourse on the 2 South Mains at spad 1107. The intersection fell above anchorage that this location up to 6' high.
On March 25, 2013 employee was awarded 10% pulmonary function impairment attributed to occupational pneumoconiosis.
As EE was installing roof bolt in #7 right on #1 unit, a piece of slip rock approx. 2 1/2"" long x 2 1/2"" wide x 2"" thick fell out from above the fly board & struck him on the face, neck & his lower bicep. He received a laceration to his neck & abrasions to his bicep & face. He left in an ambulance & returned to work his next shift. This is being reported as a medical incident.
Employee has been diagnosed with 5.5% permanent partial disability due to hearing loss.
IE was assisting another with a roof bolt.IE was holding support EE was securing it with a bolt,he placed his right hand on the mast.Once EE installed bolt he dropped the drill pot smashing IE's fingers in the mast.EE suffered an amputation of his right middle finger just below the second knuckle and an amputation of the right ring finger just above the nail bed.
EE stated he was walking by the scoop and stepped in a hole causing him to twist his right ankle. EE finished his shift on 1/28/13 and reported for work on 1/29/13 but was unable to work the full shift. EE sought medical treatment and was placed off work for a strained knee.
The EE stated he was walking beside the parked CM when he stepped on the miner cable and his left foot slipped of the cable causing his right leg to land awkwardly and his knee twisted. EE started to miss work to the injury on 1/31/13 and RTW on 2/4/13.
The EE stated he was walking along #2 East Belt when he slipped in a hole of water and strained his left knee. EE started missing on 1/30/13 and RTW 2/1/13.

EE stated that he was bolting in eight right, when a rock fell from the top and hit his right shoulder and right hip.
The employee sought medical attention for the onset of back pain on 12-13-12 and on 1-2-13 medical information provided to the operator denotes degenerative changes to the lumbar region.The ee stated that he felt pain in his lower back while loading roof bolt glue.
EE stated that he was putting up a bolt in the face of #7 entry as he touched the top with his drill steel, a rock fell from the top, striking his upper arm. The EE suffered a laceration that required sutures.
The employee stated, while traveling to the section a roll of plastic mesh entered the cab of the mantrip and struck him in the back of the head. The employee suffered a cervical strain and contusion.
EE reported to mine's compensation provider that he was struck by a block that fell from a stopping on 8/21/12. EE did not report the incident at the mine & was laid off on 8/22/12 during a work reduction. The EE sought medical the day he was laid off and diagnosed with a strain. This incident report is being filed to abate citation 8153546 as a medical reportable incident.
Injured was using a cutting disc with 4"" grinder. Grinder kicked back and cut injured's left forearm.
The employee stated that he was helping move a roof bolter from #3 entry to #2 entry when the cable reel engaged and caused the RBM cable to strike him in the back of his head. Employee started missing work on 4-20-12.
