Unplanned roof fall occurred at #18 Break on Old Belt, #1 Entry in intersection. Fall was approx. 20' W. x 20' L. x 8' H. no injuries, entrapment or Eq. involved. No escape way, ventilation or travelway affected.
Claim for Occupational Pneumoconiosis due to exposure to mine coal dust.
He was coming down the hill at Alloy between the 6 and 5 marker and tried shifting gears then lost control of the truck and rolled over around marker number 4.
Unplanned roof f fall occurred at Break #15 (#2 Belt) in the #2 right entry c.c. at spad #82. The fall was approx. 20' L. X 18'W. X 6' H. No injuries or entrapment involved. No escapeways, walkways, equipment or ventilation affected. Jacks have been set in area.
EE was dropping belt magnet. At a point the magnet tilted and EE fell. EE did have a safety lanyard on. After the accident we found the hook on the lanyard to be twisted and bent. At the point where it was hooked. EE received six broken ribs, laceration to his face and head resulting is sutures.
Employee caught his right thumb between roof bolts and drill guide on roof bolting machine while installing roof bolts in #1 entry on #2 section. The result was a laceration which required 8 sutures. No lost days of work resulted and no restricted duty to regular job.

An unplanned roof fall above the anchorage zone occurred on the MMU 006/007 (#2 section) belt line between spad number 1689 and 1717 in the belt #4 entry. the fall was approximately 75 feet wide X 18 feet wide X 8 feet thick. No injuries or entrapment were involved. No escapeway or ventilation involved. Conveyor belt line was under the fall.
EE stated that he had placed date, time and initials on a date board at #7 Head charging station, when he slipped due to wet and muddy conditions. The result was he twisted his upper body around and felt pain in upper right center just below shoulder blade.
Employee was examining his shuttle car roadway prior to operating shuttle car, a piece of rock (approximately 46"" long X 5"" thick X 15"" wide) fell between roof support. The rock struck him on the head and left shoulder.

No injuries or entrapment involved, no ventilation involved. Secondary escapeway blocked. Fall was approximately 7 1/2 ft height x 20 1/2' wide x 40 ft. length.
EE was operating the #2 roof bolter. EE was on the helper side, he turned around scraping his left forearm on a bolt plate cutting his arm.

An unplanned roof fall has occurred at 39 brk. Old southwest mains at spad #1091 #2entry. Fall was apprpx 30 to 40 feet long X 20' wide X 6' high there was no injuries involved and the fall did not affect ventilation or travel ways.
EE was kneeling beside scoop changing a steering jack, when the new jack rolled off top of scoop. He attempted to catch the jack and strained his shoulder. The ee contuinued to work at full duty until 4/25/11 at which time he called the mines and stated he had gone to the doctor and would be off work for a week. This accident became reportable as of this date 4/19/11 days off work.
The fall was on #5A belt brk.#3 one brk. outby spad #1361. The fall was 18 ft. wide, 48 ft. long and 5 to 7 feet high. The fall was of grey sandy shale material. The area was timbered and dangered off. The intersection will be cleaned up and re-bolted, with standing support such as L.Lock cribs, super cribs, reg. cribs, 50 and 100 ton jacks.

Fall was approximately 55 ft long, 18 ft wide and up to 15 ft high. Travelway from the track to section goes through break 47. No injuries, entrapment or equipment involved. No ventilation affected.

An unplanned roof fall occurred in the Alloy mine 1 brk inby ss#66 that measured 28' long x 18' wide x 6' high. No injuries were reported. The area was developed on 6/9/08.
Survey crew was traveling to #1 section when they approached end of the track the mantrip slid into a ballast car. The injured EE riding in front of the mantrip had placed his hand across the back of the seat to brace himself. The batteries rocked forward catching the EE's hand between the batteries and the metal back of the seat.
EE was tightening a bolt on a Fletcher roof ranger II cat bolter when the preto ratchet he was using slipped off the bolt and struck him in the mouth chipping his right front tooth.
An unplanned roof fall occurred at the Alloy Powellton mine in the #6 entry, near spad #717 that measured approximately 30' long x 20' wide x 6' high. No injuries were reported. The area was developed on 3-25-10.
EE scaled a piece of rock from the top in the track entry. When he bent over to remove the rock another piece fell and struck him in the back. EE started missing work on 8/11/10. Could not contact him to find out reason for missing work. Rec. info. from Comp. on 8/20/10 that EE had been off work due to injury. After rec. this info. it is determined to be a lost time accident.
EE was loading trash onto flat car. He threw a wooden pallet onto the car that had a metal strap hooked to it. The strap caught his arm, causing a laceration.
Employee was starting to drill a hole for installing a roof bolt, when a piece of roof rock fell (4"" x 4"" x 1/4"") and struck him on the left cheek which resulted in a laceration. Employee was wearing safety glasses. Employee received two sutures.

Employee was unloading a desk for new bathhouse facility, when he twisted his right knee pulling the desk off a truck. The employee continued to work at full duty until 10/14/2010, at which time he started missing days worked for surgery on his knee. The incident became reportable on this date (10/13/2010).
Employee was loading 16' straps onto the bolt machine. While he was pushing a strap onto another, his forearm came into contact with the side of the strap and caused a laceration.

An unintentional roof fall occured at spad # 410 on Red No. 1 Mains, # 2 entry approximatley 900' from working section. The mine is idle at this time. The fall measured approximately 30' long x 20' wide x 8' high. Fall will not be cleaned up.
This victim was utilizing a hammer & punch to drive a race off a bearing. The hammer deflected off the side of the punch and stuck his left thumb. Three stitches to close the wound.
Victim was raising the tailpiece using a come-a-long and a slate bar. The slate bar slipped and finger got caught between slate bar & tailpiece. Three stitches to close.
The victim was moving the roof bolter boom to put in center bolt. Finger got caught between canopy & boom. Seven stitches to close.

Was attempting to align drill steel with the pot. When he raised the pot he caught the tip of his finger under the steel. No fracture, just a contusion.
This electrician laid his hand on the side of the feeder and a rock flew out and struck the back of his left hand. Injury is described as not a fracture however the bone is ""flattened.""
