Inhalation of radiations, caustics, toxic and noxious substances

Employee has been diagnosed with Occupational Pneumoconiosis. The Safety Department received notification of his diagnosis on 7/9/2015. Employee retired December 3, 2012.

Accident type, without injuries

An unplanned roof fall occurred outby #5 neutral entry near old #6 Belt spad 3037. The fall was approximately 90' x 20' x 5' to 6'. No injuries were report and no equipment was damaged.

Employee has been diagnosed with Occupational Bilateral Carpal Tunnel Syndrome.

Over-exertion in lifting objects

While Employee was resetting a timber he felt pain in his right shoulder. Employee did not seek medical treatment until 9/27/12 which x-rays and MRI were taken. Employee did not miss any work but had multiple sessions of physical therapy and/or chiropractic treatment. Employee also had a steroid injection guided by x-ray done by an orthopedic surgeon.

As Employee was cutting a piece of belt with a knife; the knife slipped cutting his right thigh above the knee. Employee was treated at Raleigh General Hospital and received one (1) staple.

Struck by flying object

Employee was pulling one loop of feeder cable, approximately 30ft, when nylon rope hanger broke which caused s-hook to fly backwards 20ft in the air striking the right side of Employee's bottom lip and chipped his tooth.

Fall onto or against objects

As Employee tripped over a jack and fell against another jack causing a laceration to his left hand. Employee was treated at Raleigh General Hospital where he received three (3) sutures to his left hand.

As Employee was walking he stepped on a rock which caused his right ankle to roll. Employee was treated at Med Express and xrays were taken.

As Employee was carrying a roll of curtain he tripped on a rock causing the curtain to strike the back of his neck. Employee was treated at Raleigh General Hospital. Xrays were negative but was instructed to be on restricted duty for 10 days.

As Employee was moving a battery lid the lid slipped and fell striking his right foot. Employee was treated at Fayette Foot and Ankle Clinic. Employee had xrays taken which were negative.

As Employee was scaling top in #5R a piece of draw rock fell out between a row of bolts striking Employee on the head and neck. Employee was treated at Raleigh General Hospital.

As employee was moving continuous miner to the intersection of 5 and 6 to make repairs a piece of rock fell out between a row of bolts striking employee on the left arm. Employee was treated at the scene by EMTs and transported to Raleigh General Hospital for further evaluation. Xrays were taken and was negative but was instructed to be on restricted duty for four (4) days.

An unplanned roof fall has occurred at Parker Mains just inby 110 spad in the intake #7 Entry outby seal area. The roof fall was approximately 180' x 18' x 7'. No injuries were reported and no equipment was involved.

While walking around Feeder Employee twisted his ankle on loose gob. Employee was treated at Raleigh General Hospital.

As Employee was supplying bolt machine in #5 crosscut a piece of rock fell striking employee on his left arm causing a laceration. Employee was transported to Appalachian Regional Hospital by EMS for treatment and received nine (9) sutures.

While employee was building a stopping on #2 Section in #5 entry he injured his left little finger. Employee was treated at Plateau Medical Center and was diagnosed with a small fracture and also hyper-extended the growth plate.

As Employee was cutting timbers he slipped and fell striking his left knee on the track. The area he was cutting timbers was wet and muddy. Employee was treated at Raleigh General Hospital where he was diagnosed with a left knee strain.

Struck by flying object

Member was operating the continuous miner when a rock came out of the boom and struck him on the lip. Member immediately ceased work and transported himself to the hospital where he received 4 stitches.

Member was dropping cable, the reel took up on the roof bolter and the cable hit him in the right wrist. Member did not immediately cease to work and got a sling to use for the remainder of his shift. When his shift ended, he drove himself to Med-Express and received medical attention, where he was instructed to take off and rest his injury a few days.

Member was walking beside #4 drive motors when his left foot became stuck in mud causing him to twist his knee. Member ceased work immediately and drove himself to the doctor.

EE was rib bolting and had to reach over the drill pot to insert glue and a bolt. While doing so, EE experienced a burning sensation in his lower back.

EE was operating a fork-lift and it slid over, causing it to catch the bottom edge of a spreader stand and hit him in the cheek. EE has sustained a fracture in his face and must be off work for a few days.

EE was turning water sprays on for inspector, in doing so, his shoulder popped out of place.

Member was in the process of packing up material beside the section power in preparation to pull the power center back for pillaring, when a small pice of coal rib fell on the member's left leg.

Member was sitting in mantrip preparing to drive mantrip underground when the member reached up to grab ahold of the hand grab underneath the mantrip canopy to readjust his position in the mantrip and felt a strain in his right arm shoulder area.

Member was in the process of setting timbers near spad 2303, when the rib rolled striking member in the back.

An unplanned roof fall occurred in the main line intake, one crosscut inby spad # 88, in the # 5 entry. The fall measured 19'Long X 19'Wide X 8' in Height. No accidents were reported. This area was developed in May 2008.

Struck by falling object

While loading a shuttle car, a piece of rock came off the chain of the miner, striking EE on the forehead above the right eye. This caused a laceration on the forehead which resulted in 22 stitches.

Accident type, without injuries

Roof fall in the # 3 entry on the # 1 section one crosscut inby spad # 1613. Fall was 50' x 19' x 4'. No Injuries or blockage of air. No equipment involved.

Absorption of radiations, caustics, toxic and noxious substances

EE was mixing B-Bond in a bucket. As EE began dumping the B-Bond out of the bag into the bucket some of the dust got into his eyes. EE was treated by ambulance and later transported to CAMC General Division.

Over-exertion in lifting objects

Attempting to hang a miner cable and water line and it started to fall. He attempted to catch it resulting in a pain in his back. He did not cease work due to the accident nor did he require emergency care.

Bolter operator was assisting electrician checking dust suppression system due to water dripping out of tap. Bolter opr. handed elect. an old resin box to cut a piece to rest his knees on in the wet conditions while checking the system. While attempting to cut the resin box, the utility knife slipped & cut the right middle finger of bolter opr. which required 1 stitch.

Electrician was doing prev. maint. checks on cont miner, when he bent over to check the traction breaker, a piece of rib rock measuring 6' L, 15"" W and 1-4"" T, struck him on the right hip and upper leg. He was taken to hospital where X-ray was normal. He was referred to follow up w/specialist and MRI. Contacted Safey Dept 9/4/09. MRI on 9/2 showed ruptured discs/1 bulging disc.

EE was in process of roof & rib & dust parameter checks when he marked w/paint a piece of rock brow that needed a spot bolts to be installed. He retrieved remote box & was walking to position himself to back continuous miner out for the bolter when piece of rock brow measuring 3.5' lx2.5' wx1' thick struck right shoulder & down back & caused cut to back of leg requiring 6

Member was in the process of taking roof bolter cable down which was hung with nylon rope hanger with metal hooks attached to hang on eye of roof bolt plates. As he was dropping cable the metal hook caught his nose hole causing a laceration which required 2 stitches.

Fall to the walkway or working surface

While moving the section, the scoop was pulling the sc cable and member was attempting to release the diverson valve to allow cable to pull freely. The cable caught his right foot causing him to fall creating a sprain type injury.

He was moving a stop sign out of his way when the sign hit him in the mouth. He was servicing a scoop at the time this happened.

A roof fall was discovered on old #1 section, In entry #5 at spad 489. The size of the fall was 40'x20'x91/2 to 10' thick. MSHA inspector and state inspector were at the mine when roof fall was found.

The EE was sealing a block stopping and exposed his front legs, due to holes in his pants and had an allergic reaction.

Member stepped off school bus that is used to transport members from parking area to portal twisted left ankle. Referred to specialist. Specialist hold member from work.
