An unplanned roof fall occurred on #19 belt BK #131 in the travel way. The fall measured 20' high X 20 wide X 9' high. The fall fell on top of a Brookville 10 ton motor impeding all travel

An unexplained roof fall occurred at break 86 and spad #536. The fall measures 19' wide x 20' long, and 10' high.

Struck by falling object

Employee was resetting #18 tail. He had a chain hoist anchored to an exposed roof bolt. We the employee applied pressure to the hoist, it caused the bolt to break striking him in the back of his head. This caused a laceration around 1 1/2"" which required 7 staples.

An unexplained roof fall occurred in the neutral entry at 16 crosscut on #18 belt. No ventilation controls, equipment, or travel ways were damaged by the fall. The fall was approximately 70' long x 19' wide x 8' thick.

An unplanned roof fall occurred between 16 & 17 break at spad #1700 along 18A belt line. The fall was approximately 20'lenght x 18'width x 4'high. No injuries or entrapment involved. No escapeways, walkways, equipment, or ventilation effected. the are has been dangered off, and jacks have been set.

Accident type, without injuries

Unplanned roof fall occurred on #17 conveyor belt line at #47 crosscut and adjacent neutral entry. Fall in belt entry approx. 100' X 20' wide X 6-8' high. In neutral airway was approx. 400' X 20' X 6-8' high. No injuries or entrapment or equipment involved. No ventilation involved. Escape way and travel way were blocked.

Caught in, under or between a moving and a stationary object

Employee was operating roof bolter, he has just gotten into operators deck and turn machine on, he had to adjust his body position in deck and had hold of canopy. The roof bolt machine move and he caught his three fingers between top of canopy and the mine roof.

Over-exertion in pulling or pushing objects

Employee was pulling on a ventilation curtain that was covered with mud located between #4 and #5 crosscut last open break on MMU 009. He felt pain in his lower back.

Accident type, without injuries

Unplanned roof fall occurred that are approximately 20 ft. wide x 6 ft. High x (from spad 758 to 764). No injuries or entrapment involved, no equipment involved. Blocked Walkway to #2, #3 and #4 DPP. No escape way involved still have flow of air in return. The exact location is 2nd southeast panel at spad 758.

Unplanned roof fall occurred that was approximately 20 ft. wide x 9 ft. High x 20 ft. long, no injuries entrapment, or equipment involved. Blocked walkway #2, #3 and #4 DP. No escape way involved still have flow of air in return. The exact location is #5 panel southeast mains @ spad # 1065.

Employee was installing the outside roof bolt which had a curtain hook on it. The employee stated that the bolt was 3 feet from top and he was trying to position curtain hook while rotating the bolt. The curtain hook caught his finger and glove resulting in tearing his end of his right index finger.

Accident type, without injuries

An unplanned roof fall has occurred in the #6 entry on 2nd panel at between spad #1200 and spad #1210. Fall was approx 160' long X 20' wide X 6' to 7'high. Area was bolted with 6' resign bolts. There were no injuries involved and the fall did not affect ventilation or any travel ways.

Struck against stationary object

Employee was walking across section (seam height approximately 44"") he was bent over at the waist. A roof bolt had been damaged and he was in the process of pointing it out to Supervisor & another employee when his hand struck the plate, resulting in a laceration. He received 2 sutures and returned to work at full duty the same day 4/26/11.

Over-exertion in lifting objects

Employee lifted belt structure to install chain in rail and felt a stabbing pain in his lower back. He continued to work until 2/17/2011 at which time he started missing work days.

Unplanned roof fall was discovered in the Old # 1 Panel in #5 entry. The fall was outby spad #1054 in intersection. Fall was approximately 19 ft in width, 19 ft in length & 10-12 ft in height. Area was developed in 7/2009. No injuries, entrapment or equipment involved. No escapeway, ventilation or travelway affected.

Unplanned roof fall was discovered in the Old #1 Panel in #4 entry. The fall was outby spad # 1073 for 3 crosscuts. Fall was approx. 19 ft in width, 300 ft length and 10 ft in height. Area was developed in 7/2009. No injuries, entrapment or equipment involved. No escape way, ventilation or travel way affected.

Caught in, under or between a moving and a stationary object

Employee was operating shuttle car and was turning into crosscut from #2 to #3 entry. He had his left arm resting on side rail of car and caught his arm between shuttle car and rib line.

An unplanned roof fall was discovered inby spad #1083 on old #1 panel in #7 entry. Fall was approximately 19 ft wide, 30 ft long and 8 ft high. No injuries, entrapment or equipment involved. No ventilation, escapeway or travelway affected.

Struck by falling object

Employee was outby section getting a load of cement blocks to construct a stopping. He turned his head (seam height 53 inches and employee 6 ft 2 inches) and struck a fly pad board that was sticking down slightly. Material from board and roof fell behind safety glasses into his eye. He was given prescription eye drops and released to return to work at full duty.

Member was swinging drill boom in when he started to fall, he was trying to catch himself by grabbing the top of the canopy with his left hand. Member continued to fall into the control levers and raised canopy into roof causing lacerations to his middle finger, which lead to doctors amputating the tip of his middle finger.

Unplanned roof fall was discovered at 47 crosscut along 17 belt in the neutral airway entry. Fall was approximately 20 ft length X 20 ft wide X 4-6 ft in height. No injuries, equipment, ventilation or passageway affected. Area developed 7/08. Fall will dangered off and crib/timbers set around the fall.

Employee was struck in the face by sand hopper on #13 mantrip when guard re-railer came loose. Result was a fracture (minor) to his nose. Medical treatment only, no work days lost.

EE was setting a belt motor, he had a 3/4 "" chain hoist hooked up to a belt hanger and to the motor. The hook on the belt hanger came loose and struck EE in the forehead.

An unplanned roof fall occurred in the Coalburg mine at the #6 brk on #16 belt in the intake that measured approximately 30' long x 20' wide x 4' high. No injuries were reported. The area was developed on 6/17/08.

Headman was lifting an 8' curly beam in the belt entry. He slipped on a piece of belt that was covered with mud causing him to strain his lower back. EE continued to work until 9/11/10 at which time he visited health plus in Kanawha city and was put off work.

Over-exertion in lifting objects

After further investigation it has been determined that a reportable accident occurred to this employee. He was loading a flinger type rock duster with dust on the end of a supply car. When raising the rock dust bags up (40 lbs.) he felt a pulling sensation in upper abdomen. He missed work from June 23, 2010 until July 11, 2010.

An unplanned roof fall occurred, after evening shift left at approximately midnight and was found by midnight crew at approximately 3:20 am. Fall was approximately 50 feet long x 18 feet wide x 6 foot high and went from #6 entry towards #7 entry in the # 1 section at spad # 1055.

Struck by falling object

EE was bolting top when the drill steel became lodged. He dropped the head and struck drill steel to free it, but was unsuccessful. Raised drill pot and engaged rotation to free steel. Drill steel was not properly seated and when is came out of hole struck his left wrist.

Victum was handling wire and the wire sprung back, striking EE in the mouth and broke one tooth.

An unplanned roof fall occurred in the Coalburg mine in the #3 entry, spad # 270 that measued 20' long x 20' wide x 6' high. No injuries were reported.

Struck against stationary object

Member was carrying miner bits to miner. Head was turned sideways due to the low conditions. Left ear hit corner of a belt hanger, resulting in a laceration.

Member was unloading belt structure from a scoop bucket. His hand was smashed between two pieces of belt structure, resulting in a small fracture.
