Fall from piled material

EE was on a 6' berm discussing reclamation project with contractor. When they turned to dismount berm they slipped and fell approximately 4' onto rocks and hard road surface resulting in a laceration to left knee that required 7 stitches to close wound.

EE was watering haul roads and claimed they felt pain in their back and leg. The EE was given 3 days off work which happened to be the same time of a paid holiday weekend with no work scheduled. An MSHA E03 inspection was conducted on 9/20/2022. A citation was issued. This 7000-1 is being submitted in order to abate the citation. The company intends to contest.

EE was traveling up to pit. During this process the shifter in the truck that the EE was operating broke and the truck came out of gear. The truck rolled into a dozer in close proximity.

Struck against a moving object

EE was pushing shot rock. EE observed rock in mirror roll down behind dozer. EE turned to look and see where the rock went as EE was turning the dozer. The dozer trammed over the rock. At this point the EE felt a bump and also felt a pain in EE's lower back.

EE was cleaning out cab of the drill. EE cut EE's hand resulting in a laceration between the left index and middle finger that required 2 sutures.

Employee was working on a 988 wheel loader and while removing the sheet metal, the access door over the pump area fell striking and lacerating left hand.

EE was mounting a Triple Seven rock truck at the beginning of the shift. The ladder was not all the way in the down position. When EE put weight on it the ladder gave down, the EE tried to catch self and twisted back.

EE was knocking pin out of shovel bucket for new tips and adapters when a piece of metal flew off and hit EE in the right side of torso.

EE was on deck of dozer, EE lost footing and started to fall, EE turned and jumped from dozer. This occurred on September 27th but was not reported by the EE until 1:45pm on September 28th.

EE was standing on bucket when it collapsed. EE fell to the ground twisting left knee. NOTE: Ladders are provided. EE chose not to use proper tools for task.

Employee was evaluated by a physician and given a diagnosis of Carpal Tunnel Syndrome with surgical repair being performed. It was made known that work activities were a contributing factor on 4-02-2016. EE's last work shift before surgery was 02/24/2016.

Employee underwent a comprehensive audiological evaluation after an abnormal hearing test. Operator was notified on November 03, 2015that the employee's work history and test results are consistent with noise-induced hearing loss, with the workplace being a significant contributing factor.

Employee was lifting a cylinder into truck with a hoist chain when the chain slipped & cut employees left (#5) pinky finger.

Tailgate on truck would not open. The driver of the truck asked grader operator to push against the tailgate with the front part of the grader. When this happened the EE was holding a rock between the grader and the tailgate his left fingers got trapped between the grader and the rock. E.R. was visted by the EE.

While taking back tire off of grease truck the tire was about to fall and the tire smashed the EE's hand.

The employee was standing on a dozer. He was reaching for a grab handle as he slipped and fell against the tilt jack. The employee then fell to the ground.

While slowly driving a powder truck down a ramp, the engine died as the truck slid sideways on snow and ice. While sliding, the truck slid into an unfrozen area, caught traction and turned onto its side.

Employee is alleging incident occured on 10/28/2006 involving strain on his back. Employee worked 10/29/2006 and left early on 10/30/2006 due to illness. Employee reported to a foreman, other than his own on 11/06/2006, that he had been injured on 10/28/2006. Witnesses claim they did not see anything occur.

Employee was exiting a dozer on the left side, as he stepped onto the step on the push arm (right foot was to the side of the step) he put weight onto his right foot and heard a pop along with feeling a sharp pain. He could not put his weight onto his right foot.

Employee was exiting the right side of a dozer when he lost his balance and fell backwards onto the ground. The employee stated he hit on his left buttocks and right shoulder. He got up off the ground under his own power.

The employee was driving a 473 Truck when he was overcome by the heat. When he got out of the truck he became lightheaded and needed assistance. An ambulance was called to transport him to the hospital.

Employee was bolting an electrical box on an 830 Rock Truck, when a grid box fell from the electrical box, hitting the employee in the forehead and across the nose. Resulting in stitches.....23 to the nose.....14 to the forehead.
