On 5/12/14 an event occurred at this mine which resulted in the entrapment of two (2) miners. Upon subsequent recovery it was determined that both miners had perished. The injured were fatally struck by coal/rock. An investigation is on-going. The causes are unknown.

Accident type, without injuries

An unintentional roof fall occurred along the 4A Roadway at crosscut No. 4. The fall measured approximately 5' thick by 20' long and 18' wide. The fall damaged the inby head wall of an overcast. The mine is non-producing. The fall was found on 06/07/2016 at approximately 6:35 a.m.

Struck by falling object

Employee states he went to the Brody bathhouse after his shift to clean-up. He was attempting to turn the water supply to the sink on when the incident occurred. He placed his left knee on the sink and reached up to turn the valve. The sink fell off of the wall and broke, resulting in a laceration to his left shin. Sutures were used to close the wound.

Over-exertion in lifting objects

Employee stated he was shoveling at the 3-B belt tail and when he bent over he felt pain in his lower back. He was diagnosed with a lower back strain and worked under restricted duty for one (1) day.

Employee stated he was lifting a piece of 3/16"" sheet metal measuring 24.5"" x 8' into place on an overcast when he lost his grip on the sheet metal and it slipped striking him on the lower right arm. This resulted in a laceration, and sutures to close the wound.

Employee stated he was in the process of picking up a 60"" bottom roller that was lying on the mine floor when he felt pain in his groin. On 3/19/15 the employee was diagnosed with a hernia.

Employee stated that he was shoveling along the 4-I Belt and as he lifted a shovel of material to place it on the belt he felt a sharp pain in his groin. On 03/13/2015 the employee was diagnosed with a hernia.

Struck by falling object

Employee stated he was using a slate bar to scale the mine roof on the #4 section in the #3 entry when a piece of rock fell and struck him on top of the head (hard hat). This resulted in a C6-7 disc herniation. This incident became reportable on 3/17/2015 when medical evidence supporting the alleged incident was received. The employee began missing work on 2/24/15.

Over-exertion in pulling or pushing objects

Employee stated he was pushing down on the water clamp that connects the waterline to the section manifold when he felt pain in his groin area. On 2/26/2015 he was diagnosed with a small left inguinal hernia.

Employee stated that while attempting to install a roof bolt into the mine roof, a piece of rock fell from the top and struck him on the right foot. This resulted in a fracture of the 2nd toe.

Employee stated that he was operating the shuttle car on the #4 section when he hit a bump in the roadway and felt pain in his right side. He was diagnosed with a muscle strain and placed on restricted duty until 1/20/2015.

Employee stated that he was in the process of installing the rub rail on the continuous miner when his right index finger was caught between the rub rail and the frame of the continuous miner. This resulted in a crushing injury, which required sutures to repair.

Employee stated that he was operating the 14-man battery powered mantrip on the #3 section. The mantrip went through a crosscut and ran into the beltline. This resulted in bruising and strains to the left leg.

Employee stated that he was supplying his bolt tray and as he turned around he struck his head against the mine roof. This resulted in a strained neck and restricted work days.

Employee stated that he was operating the shuttle car on the #4 section. He was loaded and returning to the feeder to dump when he hit a bump in the roadway which caused him to be jarred into the canopy. This resulted in a strained neck and restricted work days.

Employee worked for Brody, Mining, LLC until 2/22/2014. After ending his employment he was diagnosed with occupational pneumoconiosis.

Over-exertion in lifting objects

Employee stated that he bent over the side of the scoop bucket to pick up two (2) bundles of roof bolt plates when he felt a sharp pain in the area around his navel. This resulted in a hernia.

Employee stated that he was walking outby the section on the roadway when he stepped on a slick rock and twisted his right knee. This resulted in a knee strain and restricted work days.

Fall to the walkway or working surface

Employee stated that he completed performing maintenance work and was stepping off the feeder when his foot slipped, causing him to fall to the ground. This was a same level fall that resulted in a back strain.

Employee stated that he was examining the 2-C beltline when he walked into low top and struck his hardhat/head against a belt hanger that was sticking down from the top. This resulted in a strained neck and restricted work days. The incident did not become reportable until 7/16/14 when the employee was placed on restricted duty.

Employee was employed by Brody Mining, LLC until 10/28/2012. On 7/10/2014 the operator was notified that the employee had been diagnosed with occupational pneumoconiosis.

Employee stated that he was removing a cable bolt from the tray of the roof bolt machine and as he pulled on the bolt his hand slipped and his right wrist struck the bolt tray. This resulted in a contusion and restricted work days.

Employee stated that he was walking beside the section power center when he tripped over the feeder cable and fell to the ground. This resulted in a sprain/strain of the lower back.

Employee stated that he attempted to trim a tree limb using his pocket knife, when the knife slipped, and cut his left pointer finger. This resulted in a laceration that required 4 sutures to repair the wound. This incident resulted in no lost time.

Employee stated that he was making repairs to a diesel mantrip. After lifting the rear end into place he felt a burning sensation in his abdomen and groin. Medical received on 6/19/14 diagnosed the employee with a hernia.

Employee stated that he dismounted the RBM to man the drill station and as he stepped forward he tripped on a rock. As he fell, he extended his left arm and his hand struck the drill pot. Medical received 05/09/2014 states a fracture to the base of the fifth finger.

Employee stated that he was working along the 3-B belt when a piece of rock fell from the top and struck him on the back of the neck. The rock was approximately 22"" wide and 0"" to 1"" thick. This resulted in an abrasion and one lost work day. This incident became reportable on 5/22/14.

Employee stated that on an unknown date and time he was operating a diesel mantrip when he hit a bump and bounced into the air. This caused him to pinch his left testicle between his leg and the seat of the mantrip. A medical diagnosis has not been received at this time. This incident became reportable on 5/22/14.

Employee stated that he was installing a rib bolt and as the bolt entered the hole it bent and then the rotation was engaged, which caused the bolt to strike/twist his hand and wrist. This resulted in a contusion and restricted work duties.

Employee was walking into the face of the #7 entry. A roof strap was hanging down from the top and as he walked into the entry he struck his right cheek against the strap resulting in a laceration. The incident became reportable on 6/27/14 due to complication and subsequent lost days.

Employee was diagnosed with occupational pneumoconiosis and awarded 10% PPD.

Over-exertion in pulling or pushing objects

Employee stated that he was using a slate bar to straighten the front basket of a mantrip. As he pulled on the slate bar, he felt pain in his right shoulder causing him to lose his grip on the slate bar and fall to the ground. This resulted in a back and right shoulder strain.

Employee stated he was stacking structure on a flat car, when back and shoulders started hurting. This resulted in a shoulder and back strain.

An unintentional ignition of gas occurred in the #6 entry of the #2 section while the 006-0 MMU was in the process of extracting coal. There were no injuries to miners or damage to equipment during this event.

Employee stated that he was walking in the number 7 entry when he stepped on a piece of coal and twisted his left knee. This resulted in a knee sprain.

Employee was replacing cat pads on the continuous miner. As he positioned the cats for installation, his finger was caught between the joint of the pads. This resulted in the amputation of the distal portion of the left little finger.

Employee stated that he was loading crib blocks on the service scoop and as he turned to placed one of the blocks onto the scoop he tripped over a rock and fell to the mine floor injuring his back and right shoulder.

Employee stated that he lifted his personal tool box and then his back began hurting. This resulted in a back strain.

A rock fell from the mine roof near the face of the #5 entry approximately 100 feet inby spad #13280 on the #3 Unit, striking the employee on the head and neck.

Employee stated that he was hauling supplies with the diesel scoop when a plaster bucket fell off of the scoop bucket, hit the roadway, and splashed up into his right eye.

Employee stated that he was using a belt knife to cut a piece of belt rubber and as he pulled the knife slipped and cut him on the top of the left hand. This resulted in a laceration that required sutures.

Employee was installing the cutter head pin on the continuous miner. As he struck the pin with a sledge hammer a piece of the pin broke off and imbedded into his right leg just above the knee. The foreign body was not removed and the wound was closed with sutures.

Employee stated that he was hitting the core breaker pin when a piece of metal went behind his safety glasses and into his right eye. Medical received on 12/19/2013 confirmed a medical reportable incident.

Employee was making a splice in the shuttle car cable and as he was striking the knocker he accidentally struck his left thumb with the hammer. This resulted in a laceration that did not require sutures.

Employee moved the RBM into the 3 left crosscut and dismounted the operator's deck. As he was exiting the deck of the RBM, the continuous miner trammed into the flypads directly behind the roof bolter, striking the employee and knocking him to the mine floor. This resulted in multiple sprains/strains of the upper body.

Employee stated that he was pulling back on a roof bolt to line it up with the drill pot. As he applied pressure on the bolt he felt something pull in his right shoulder. This resulted in a right shoulder strain.

Employee stated that he was pulling a water hose through a crosscut to wash off the duster batteries. As he walked through the crosscut he tripped over a rock dust hose and fell to the mine floor injuring his shoulder. Medical received on 08/08/2013 diagnosed the employee with a left shoulder sprain.

Fall from piled material

Employee stated that he was holding timbers that were being cut by two other employees by sitting on top of the timbers. As he dismounted the stack of timbers, he got his foot caught in the bindings that hold the timbers together and fell to the ground. Medical received on 08/15/2013 diagnosed the employee with multiple sprains/strains.

Employee stated that he was pulling a bag of garbage out of the trash can in the bathhouse and strained his left shoulder. Medical received on 08/28/2013 diagnosed the employee with a left shoulder strain.

Employee was walking down a grade on the section and slipped and fell backwards. This resulted in a strain to his left leg.

Employee stated that he was picking up a piece of belt structure on the mine yard and hurt his back. This resulted in a back strain and medical received on 08/15/2013 confirmed that the employee was receiving medical treatment.

When the boom of the machine swung, the plates and pans fell off and struck the employee on the side of the rt. knee. Employee was diagnosed with a strain and began missing work on 3/8/13.

Employee was installing roof bolts in the #4 entry on the #1 Section when a piece of rock approximately 31"" x 24"" and 1/4"" to 4"" thick fell from the top and struck him on the right shoulder. This resulted in a fracture to the right 5th rib.

Employee stated that he was down on his knees with a come-a-long ratchet wrapped around a pipe when he used one hand to lift the pipe and the other to pull the ratchet from under the pipe. This resulted in a lumbar spine strain.

Employee stated that while installing I-beams along the #4B Belt he was struck on the head/back by a beam when the employee holding the beam lost his balance and dropped the beam. Employee began missing work, complained of back pain, saw a doctor on 01/31/13, and medical received on 2/7/13 stated ""mild depression of the T5 & T8 vertebral bodies, with no acute fractures seen"".

EE was hanging the roof bolter cable when he finished he walked along the side of the bolter to check his curtain. Rock fell in front of and over the ARTS of the bolt machine pinning ee to the ground and landing on his right foot.

Employee stated that while cleaning the No. 4A Belt Head, he stepped on a turn-buckle and twisted his right ankle. Employee began missing work on 1/17/13 due to a sprain.

The employee stated he caught his right index finger between a ratchet and miner frame causing an open wound that needed sutures. He was tightening ripper motor mount bolts.

The employee stated he stepped off the walk-through RBM and miss judged the distance causing him to twist his ankle. He is missing work.

The employee explained he reached over and picked up a roof bolt and pie pan out of the roadway and pain shot down his hip and left leg. The initial WC BI-1 shows prior surgery prior injuries on his lumbar spine and not acute injury.

The employee stated he jumped off the Phillips s/c landing on the mine floor after making repairs. He complains of pain in lower back down to left knee. He is missing work .

Upon an investigation the D-8R stockpile dozer was shoving coal into No.1 feeder and when backing around stacker the machine went down into small void at feeder No.2. To prevent a re-occurrence spot lights were installed, upon dozer operator's request, for visibility when operating at and near feeders, and changing operating procedures to prohibit backing passing feeders.

The mine received a phone call on 5/27/10 from the employee who stated his neck had pain radiating down through his shoulder. He stated he did not file an accident/illness prior leaving the property. Medical received on 6/3/10 states there is no acute injury but chronic disease.
