While tramming outby in the #3 entry, a wood plank, 3"" thick x 9"" wide x 12' long entered the cab of a Fairchild scoop and fatally injured employee.

Caught in, under or between a moving and a stationary object

He was bolting 7-8 crosscut. He let the head down and said the joy stick stuck causing the head to come down on his foot and ankle. It caused a sprained ankle.

Inhalation of radiations, caustics, toxic and noxious substances

He was loading gob out with the miner. He was also watering down the gob. He bent over to pick up the miner water line and inhaled dust.

Struck by falling object

while roof bolting a piece of head coal fell out between bolts hitting him in the left calf. cutting him.

Accident type, without injuries

A roof fall occurred 7 block zone 3 belt entry measuring approximately 50' long 16' wide and 7' above coal seam.

Struck by flying object

he clamped a 1"" airline onto a nipple on the main airline. when he turned the air on the 1"" line blew off. the end of the 1"" line with the clamps hit him in the nose cutting it.

Over-exertion in lifting objects

He was carrying a box of resin from back of bolter to front of bolter. He felt something pull in lower back. [He never went to doctor or missed work until 5-21-14].

Struck by flying object

Working on scoop rear end. He was watching another EE take off snap ring. Snap ring flew out went under glasses and hit him in the eye.

riding rubber tired mantrip from end of rail to section. went over a hump his head hit a roof bolt. cut his forehead.

walking from office to pit mouth. walked down hill got on flat at bottom slipped on snow and ice causing him to fall hitting his head, back and right side of body.

They were finishing a belt splice. EE was pounding the splice making sure all nails and splices were down. EE was feeling splice to make sure ends were down. EE got hit on the left ring finger with hammer.

Lifting and carrying Omega blocks. Felt pain in lower right part of stomach and a knot came out.

Struck by falling object

While installing fly board, he put his hand out to grab drill steel. A piece of rock fell hitting right pinky finger between rock and bolter head cutting finger.

Going thru manual airlock doors. While going thru outby set, the inby set was partially open. His right hand got pinched between the door and the frame. Cut middle and ring finger on right hand.

Lifting a wooden bridge plank when he felt something pull in upper back of right leg. (torn ligament)

He was cutting bands off of bundle of bolts. He turned and his right arm hit band cutting his arm.

Loading roof bolter. He broke band holding the bundle of bolts together. When the band broke it sprung up. The broken end of the band hit his left forearm.

Employee was installing a 6' tension bolt, when he was pushing it up to roof it started to bend and he spun rotation, causing bolt to spring around and hit his arm.

Employee was scooping #4 Hdg. and curtain fell down. Reached up to get curtain and finger got between canopy and roof. Cutting off end of finger.

Was hanging hi voltage cable during belt & power move pulled abdomen continued to work and on 6/6/12 rupture appeared continued on light duty until 7/9/12 when he had surgery on rupture.

Was pulling a roof bolt off bolter and hit elbow. finished shift worked 6,27,28,29,30, never heard from him came back to work the 4th of May never come back in, the 7th called on the 9th had infection in elbow.

EE had complained of his back hurting before going to work, then was hanging cable and got down on his knee and said he hurt his knee. Which he had surgery on it in 2008 from another ee was on light duty for 4 days came back on full duty 3/8/12 and was discharged on 3/26/12.

Fall was approx 20"" long, 8' high, 16' wide. It was in an intersection at 6 block #3 intake escapeway. Escapeway was rerouted from 5 block to 7 block. Fall was timbered off. Everyone was informed and maps updated. There was 6' tension rebar bolts. Cover was 160' to 200'. Date mined was 4/2008.

EE was cutting a piece of belt for skirting in tail piece on #2 belt and had belt laying across his leg.

Struck by flying object

Foreman was pulling S-Car off feeder with a 3/8"" chain with hook's. The chain broke and hook sending 2' of chain 26' and hitting foreman in back.

Intersection at spad #38 fell, 6' high 16' wide and 25' long, per map attached.

Contact with cold

EE was working outside spicing stacker belt and the temperature was below 32 degrees and didn't have anything on his face and allegedly got frostbite.

Employee was roof bolting he bent a bolt and it stuck in chuck when he dropped head bolt sprung out of chuck and hit his forearm.

Employee was hanging miner cable for S-car roadway and when he lifted miner cable above his head got a sharp pain in his abdomen. He worked his reg. job until 1/20/2012 and had surgery for a rupture.

#4 Main's #1 Hdg. pumped water out of old sump area, and found a roof fall. Fall was 30' long 8' high 12' wide, and was supported with 7' tension rebar bolts, had 680' of cover, was mined Feb 09, was 50' inby spad 7281 in #1 Hdg.

EE was pulling a piece of belt with channel locks and they slipped and pulled right shoulder.

Assistant mine foreman was helping mine foreman load a rock duster and pulled right shoulder. Employee worked his regular job until 1/20/2012 and had surgery on 1/30/2012. Returned to work on 1/24/2012.

#1 Track at 5 block crosscut 5 to 4 Spad #56 to left of track fell in. There was arches in #4 at spad #56 and arches on track at 5 Block with a row of 4 of Fiber Cribs. Had 7' Tension Bolts. Fall Length 30' width 17' Height 7 1/2'.

Mine foreman doing weekly came up on a roof fall in old sub mains at #2 Hdg. 33 to 34 blk at spad 1900. Cover is 240'. Support is 7' tension rebar. Length - 40', width - 17 1/2', height - 8 1/2'. Mined June of 2010.

Roof bolter operator was hit on the knee with a small rock which fell from a potted out area that he was bolting.

Bolter operator was trying to align 2nd drill steel up with 1st drill steel and pinched his finger between the 2 steels at the coupler.

EE was throwing salt into the bed of his trailer. EE was inside of salt building getting salt out of the barrel, turned to throw the salt and stepped on loose salt on the floor. Foot slipped and EE tore a muscle in his lower back.

Hanging miner cable and cut finger on a piece of steal wire.

Front wheel on 2 man rubber tired mantrip caught rail in track entry and steering wheel whipped and cut operator's finger.

Supply man cut his wrist on a metal band on a bundle of roof bolts.

Miner operator got between the right rear corner of the machine and the rib while tramming the machine. He said the bits caught the rib, kicking the machine towards himself, smashing his right leg.

Employee using 3/4 drive ratchet to loosen bolts on mine head, ratchet slipped handle smashed finger between ratchet and frame of miner. Open fracture of left ring finger.

Walking to truck, slipped & fell in mud. Bruised ribs. Went home, changed clothes & came back to work. Hauled 2 loads of coal, complained about ribs hurting. Went to United Hospital Center & got doctor's excuse to take rest of day off work. Called in next day & said he was too sore to work. Returned to work next day.

EE was talking to the foreman and turned to walk away and hit his head on the shuttle car canopy.

Jacking supply car on track, blocked car to free up second jack. Reached for jack and car slid off jack and smashed his finger.

It is beleived that employee had a seizure, fell and struck the back of his head on 3"" gravel. Doctors excuse ran from 6/25/08 thru 6/27/08. He was not allowed to return to work until 7/1/08 pending a drug test and results by orders of company official.

Maintenance Foreman was using end loader forks to apply pressure to a bent drag link, trying to straighten it. Smashed his left thumb.

Injured himself lifting timber out of mud at head drive. He did not miss any work until 5/19/2008 - HERNIA SURGERY.

An unintentional roof fall has occured at Spad sta. 31 in the Mains. Height - 8' Width - 18' Length - 30' Support - 8' torque tension bolts and Diawadag trusses. Mine has been idle since 8/2006.

An unintentional roof fall has occurred at spad stations 56 & 57 on the Mains. This included the intersections. Length - 80 ' Width - 18' Height - 8' Support - 8' torque tension.

Injured was building a Kennedy stopping, as he lifted a metal stopping panel it slipped through his hand, the edge of panel cut through his glove and lacerated his right thumb. He received 6 stitches.

At approx. 1:20 pm on the miner unit 003-0 the continuous miner cut into an unchartered gas well there was an inundation of 7% CH4, 17.3 02 and 105 PPM Co.

While installing roof bolt, loose top started falling, injured started to move out of the way and tripped on a rock striking his elbow on the bottom.

While pulling charger cable, injured caught foot in another cable and fell, pulling groin muscle.

While tramming shuttlecar, injured hit a hole in the bottom causing him to bounce out of the seat and strike his head on bottom of canopy, hurting neck.

Pulling roof bolt off of roof bolter, bolt suddenly came loose causing injured to strike bolt and hand against rib. Injured received 3 stitches in left little finger.

Bending roof bolt while installing into top, injured felt pain in lower back. This became a lost time accident 5-23-06.

Sycamore Mine #2 had a roof fall on the mains section at cross-cut 4 to 5 in the last break. The fall measured 8' high x 15' long x 16' wide.

INJURED WAS DRAGGING 2 BOXES OF RESIN UP #6 ENTRY ONE BOX CAUGHT IN THE BOTTOM AND CAUSED A STRAIN TO LOWER BACK.

Walking in #7 entry towards power center, walked into wire screen, cut left ear and left side of neck. He has ringing in ear and stiff neck.

Injured was operating the helpers side of the roof bolter in the # 6 entry, he pinched the palm of his right hand between the pusher and the finisher steels, causing a laceration. Injured received 3 stitches.

INJURED WAS LIFTING A ROLL OF WIRE SCREEN OUT OF A CATERPILLAR LOADER BUCKET. THE ROLL OF SCREEN FELL SMASHING THE LITTLE FINGER ON HIS LEFT HAND. HE RECEIVED 7 STITCHES TO HIS FINGER.

Injured was moving a ABS 8 hp pump in the #3 entry. While moving pump injured slipped straining his lower back.

INJURED STATED HE WAS GETTING INTO FORKLIFT, HIS BOOTS WERE MUDDY AND HE SLIPPED TWISTING & HITTING HIS LEFT KNEE ON FORKLIFT.
