Employee was assisting in moving the bolter to load supplies, the operator engaged the reel and shut the bolter off but did not disengaged the reel. when they were ready to move the bolter employee picked up the cable, the slack took up and jerked his arm. he completed his shift and did not miss work until 3/18/2014.
Employee states that he and his bolt partner were backing the roof bolter out of the #5 entry. As he pulled up the cable slack back, the bolter turned and began to roll up the cable to the spool. The cable had a loop in it and as it tightened up, his foot was in the loop. This caused a fracture to his (L) ankle.

Employee states that a piece of draw rock broke out between bolts and struck him in the head. He completed his shift and went to the ER afterwards. He was given 1 day off. On 8-22-2013 he went back to the Dr. where he was taken off work until further notice.
Employee states that while working on the continuous miner a piece of draw rock fell and struck him on the (L) shoulder. Employee finished his shift. On 8-14-2013 he went to the Dr. who took him off work for testing and P.T..
Employee states that as he was attempting to set a Hinzman jack he lost his grip in the jack body. The head of the jack came down and caught his Left hand between the jack head and body.
An unintentional roof fall occurred in the 1st west mains at or near spad #158. It was approx. 20' x 30' x 9' and located in the cross cut. It is a slick sided wet shale.
Employee was racking miner cable on the rib when he slipped and fell on a rock.
Employee was carrying a ladder to hang curtin. he states that he felt something pull in his groin area. Empoyee continues to work. He was diagnosed 02/26/2013 with a hernia. He was then placed on modified duty.
Employee was building a return stoping. The man that was holding the ladder reached for another block letting go of the ladder, when he let go of the ladder it became unstable causing the employee on the ladder to fall. Employee fell approx. 8 foot to the ground.

Employee states that while re-installing a shield on the #12 continuous miner, he slipped and fell off. When he landed on the ground, he felt something pop in his right foot and ankle. Employee continued to work until he went the Dr. on 1-10-13 when he was diagnosed with a fracture in his (R) foot. He continues to work with modified duty.
EE stated he fell off ladder while hanging ventilation curtain.
Employee states that while dusting drift at mannings branch and 3A head, I picked up a bag of rock dust and felt something pull around my groin area. It then started to swell.
Employee states that while operating the section scoop the chain holding down rock duster broke and a link from the chain struck him in the right eye. The safety glasses that he was wearing prevented the link from directly striking him in the eye, but the safety glasses caused a laceration to the eye brow area.

An unintentional roof fall was found while traveling the intake air course. It is located at or near spad #157. It appears to be approximately 20ft. x 30ft. x 10ft. 1st. North West mains, between #7 and #8 entries.

EE states the he was dragging miner cable away from the CM when he tripped over miner cable that was on the ground causing him to fall and injure his lower back.
Employee states that while putting up rib boards, the bolt spacer fell from the T-Bar striking him in the right shoulder and neck. He also states that it knocked him to the ground.
Employee states that he was bolting top in the #4 entry when the drill steel hung in the top. It came down and caught my finger between the steel and the pot.

Wet slicky sided slip formation - Unintentional roof fall occurred 8,600 ft from 001 MMU. Fall measured 40' long by 18' wide by 8' thick. Fall will not be cleaned up. This fall does not affect ventilation or prevent us from doing out weekly examinations.
Employee states that as he engaged the drill pot on the bolter, his (R) hand glove was caught between the drill steel and pot and was twisted by the rotation, it removed the glove from his hand.

An Unintentional roof fall occurred 6000' from 001 MMU Intersection spad # 171 1st left 2nd South West Mains. Approx. 30'w x 40'l. x 8' h. Area was bolted with 6' tork tension bolts with 8' rope bolt in intersections. This area has been cribbed off and no plans to clean up fall.

EE was plastering on stopping he stated he threw a hand full of plaster on stopping & rib and it splashed black in his eye, he also stated his glasses were scratched up and he couldn't see to plaster, so he took them off.
EE was walking towards face from power center, when a piece of rock 1/2 inch thick, 7 inch x 8 inch fell from the roof, striking him in the left side of his head. 90' inby spad #1691 (area of accident is 10'high)
EE climbed up on ladder to hang bolter cable, when leg of ladder broke causing him to fall striking his shoulder on the ground and leg twisted up in ladder. He on eveshift reported putting damaged ladder with trash to be brought outside. He didn't know ladder was damaged and used it.
Employee was assisting shuttle car operator move his anchor. Employee started the S/C. The anchor and cable took. The end of the anchor turned striking injured in the left hand and left ring finger.
Employee was pushing bolt up in hole when the plate cocked sideways on the sissors. He reached up to straighten plate and hit the up lever before removing his hand pinching his thumb between plate and bolt.

Employee was hooking chain up to tool car and scoop to pull tool car closed to section. He told the scoop operator to lower the scoop bucket. The operator dropped the bucket and injured screamed that the bucket was on his foot.

Employee was carrying block while building stopping, when he tripped & fell on some block hurting his left hip. He continued walking for 9 hours. (Employee was in a car accident prior to returning to work preventing him from returning to work for 39 days.)
Was holding shuttle car cable up so scoop could pass under, when a piece of draw rock fell out between rib and bolts. Struck on top of head & neck. Rock was 4' x 2' x 3 1/2"" thick.
He was hanging an angle curtain in #6 intersection and fell off a ladder and hurt his right rib when he fell. Employee went home at 8:00 AM called back to work and said he went to hospital & had x-ray. Said rib was cracked.

Loading supplies on roof bolter. When loading straps on bolter, 1 man was sliding straps on stack. Employee's hand was stuck and cut left hand.
Operator was hanging cable on corner of 5 right break. Cable being hung was roof bolt cable when a piece of rock 3"" x 8"" x 6"" in diameter fell and hit employee on top of the head knocking him to the ground. Saying he had got shocked.
Spray roof and ribs with pyro-cem sealant when it splattered in his eye under safety glasses.
HE WAS CLEANING THE SCRUBBER ON THE MINER WHEN ROCK FELL FROM THE RIB ABOUT 21""X 1"" IN DIAMETER & HIT HIM ON THE HEAD, NECK, & RIGHT ELBOW. WE PUT A NECK BRACE ON HIM & PLACED HIM ON A BACK BOARD & TRANSPORTED HIM OUTSIDE TO EMS. ON 8-12-05 HE INFORMED US THAT HE WENT TO THE DR & THEY FOUND A CYST BEHIND HIS LEFT EYE. THE EE STATED THE CYST WAS PRESENT DUE TO HIS INJURY ON 5/17/05.
EE WAS TRAVELING ACROSS HAUL ROAD ENROUTE TO MINE SITE. EE LOST CONTROL OF HIS VEHICLE IN A CURVE ON SLICK ROAD AND HIT A COAL TRUCK TRAVELING IN OPPOSITE DIRECTION. (PRE-SHIFT ACCIDENT; EE DID NOT SEEK MEDICAL TREATMENT - INJURED HIS LEFT KNEE.)
