EE was putting straps together on conveyor chain. EE put a chisel between press and the conveyer chain. The chisel came out hitting EE in the left wrist.
EE was carrying bits to the miner and stepped around the boom of the miner twisting his back.
EE was bolting top and the pin steel was caught/fouled in the drill pot. EE was removing the pin steel, the pin steel came lose and struck EE underneath the right eye causing a laceration.
A piece of rock fell out between ATRS and hit the operators canopy. A piece of the rock broke off and hit EE on the right wrist and arm.

There has been a fall found in the primary escapeway. All approaches to the fall have been cribbed and dangered off to keep workers from entering the area of the fall. The primary escapeway has been re-routed. We have had safety meetings with all three shifts discussing and going over on the map the re-routing of this entry.
EE was setting a timber, the ee did not have it wedged completely and the timber fell hitting ee in the mouth causing a broken tooth.

The EE was going under mining cable, lifting sagging cable up to cross under, his left elbow caught top and pulled his shoulder down.
The injured employee was recovering belt structure. He was carrying a piece of top structure to load it into the scoop bucket, the injured employee tripped catching his left wrist between the scoop bucket and structure, cutting his wrist on the structure.

The injured person was traveling outside on a 3 wheeled personal carrier and hit a sink hole, which resulted in the injured person hitting his head against the mine roof, injuring his neck and back. We were made aware that he was going to see a doctor on 12/02/2010, resulting in this filing date.

Injured man broke cut off valve for deluge fire spray. He turned the water off on the 6 inch line, when he bled the pressure off the line, it blew his hand into a pizza pan which was part of the bearing surface for roof support and recieved a cut to his finger.
