Employee was helping change a busted brake hose on Co. No. 956 Tub (Rover) when the tub suddenly moved, striking left leg, causing them to twist their left ankle. Employee was seen by Orthopedic Surgeon on 1/16/2024 and was taken off work for 4 weeks. Diagnosis still TBD due to swelling of the foot but is believed to be an Avulsed Fracture. This is a Lost Workdays Incident.
On November 6, 2024, the operator was notified that the employee was diagnosed with occupational pneumoconiosis. The employee retired from the company on 1/6/2024.

The fall is located on 1 Belt Mains in the track entry at break 18 at spad # 123. MSHA and WVMHST authorized representatives have arrived on scene and investigated the roof fall. A fall clean-up plan has been submitted for review and approval.

The fall is located on 2 Belt Mains in the track entry between break 26 & 27, inby spad 6481 and is approx. 20' long x 20' wide x 5' thick. A clean-up plan has been submitted for review and approval.

The intersection at Break 48 on #2 Belt Line fell approximately 20' long by 20' wide and approximately 7' thick. The affected area is supported with fully grouted 6' torque tension bolts. The roof fall is located in the belt entry and does not impede any travel of miners.
The intersection fell out in the track area at BRK.61 at number 3 track entry the fall is 19' wide 12' long 8' thick.
A roof fall was discovered in the track entry between break 25 and 26 on # 2 belt. The fall is located between spad 6478 and 6481. Its approximately 20'long X 18' wide x 8 foot high.

Roof fall was discovered in the belt entry between break 28 and 29 on # 2 South belt. All approaches to the fall is breakered off.
The employee stated was riding # 5 EMU down the track when a draw rock struck the employee on right shoulder, the piece of draw rock was 1/2""x2""x1' long. The employee sought medical attention on 8/30/2017 and was taken off work. This incident didn't become reportable until 8/30/2017. This incident will be a lost time incident.

The employee stated - was raising a 6"" discharge line up that had fallen down with the line shifted and fell striking the top of head and straining neck. The employee sought medical attention and was placed on restricted duty for 5 days.

The employee stated he had carried supplies to a roof bolt machine and on his way back stepped on a rock twisting his left ankle. The employee continued to work, but sought medical attention on 1/27/15 and was diagnosed with a strained ankle. The employee returned to work at his normal job class, but this incident is being reported due to medical treatment.

On 9/9/2014 a roof fall was discovered in an intersection 3 cross cuts outby the 1st Right #1 Seal in the Southwest Mains. The fall occurred in an intersection and was approximately 7' high, 20' long, and 19.5 feet wide.
At approximately 1:00 p.m. a roof fall was discovered just inby the mine fan canopy.
On 7/25/14 at approximately 8:00 a.m. the operator was notified that this previously employed individual had been diagnosed with occupational pneumoconiosis.
On 7/8/2014 at approx 2:00pm the operator was notified that this employee which was last employed on 10/16/2009 had been diagnosed with black lung.
On 7/7/2014 at 7:15 a.m. a roof fall was discovered at break 28 in the track entry. The fall was approximately 65' long and 20' wide.

A roof fall was discovered in the South Mains located at break number 5. The fall was approximately 20' long and 5' thick at the highest point. The fall occurred in number 6 entry. The fall was dangered off.
During the examination of a weekly aircourse a cont. of an old fall was found. The fall occurred in the Ellis Main at break 27 and now extends from #5 entry to #4 entry. The travelway was not affected.
On 4/8/2013 a roof fall was discovered 150' inby the mine fan in the intake air course. The fall measured approximately 30' in length and fell up to 7' in height.
The employee has been awarded 10% permanent partial disability for occupational pneumoconiosis

Injured was checking pump when piece of rock fell striking head.
Injured was tramming scoop backwards when he ran over board causing it to come into deck.

A piece of rock fell (about 20"" long, 12"" wide, 1"" thick) and some coal hit him. He was building an intake stopping. NOTE: We are suspicious of this alleged accident. There were no witnesses and no visible marks on his body.
While getting tools from the tool box, the lid of the tool box fell down onto his left hand.

Employee had a utlity knife, trimming on a roll of 48"" belt. Knife slipped, cutting right arm. Cut was about 2"" to 2 1/2"" long.
