Employee was helping change a busted brake hose on Co. No. 956 Tub (Rover) when the tub suddenly moved, striking left leg, causing them to twist their left ankle. Employee was seen by Orthopedic Surgeon on 1/16/2024 and was taken off work for 4 weeks. Diagnosis still TBD due to swelling of the foot but is believed to be an Avulsed Fracture. This is a Lost Workdays Incident.

On November 6, 2024, the operator was notified that the employee was diagnosed with occupational pneumoconiosis. The employee retired from the company on 1/6/2024.

An unintentional roof fall was discovered at approx. 06:58 a.m. and was called in and reported to MSHA and WVMHST Officials. The fall is located on 1 Belt Mains in the track entry at break 18 at spad # 123. MSHA and WVMHST authorized representatives have arrived on scene and investigated the roof fall. A fall clean-up plan has been submitted for review and approval.

An unintentional roof fall was discovered at approx. 11:00 a.m. and was called into MSHA and WVMHST Officials. The fall is located on 2 Belt Mains in the track entry at break 26 and is approx. 30' long x 20' wide x 8-10' thick.

An unintentional roof fall was discovered during the day shift pre-shift examination at approx. 6:53 a.m. and was called in to MSHA and WVMHST Officials. The fall is located on 2 Belt Mains in the track entry between break 26 & 27, inby spad 6481 and is approx. 20' long x 20' wide x 5' thick. A clean-up plan has been submitted for review and approval.

An unintentional roof fall was discovered during the day shift pre-shift examination at approx. 7:30 a.m. and was called in to MSHA and WVMHST Officials. The fall is located on 2 Belt Mains in the neutral entry at Break 51, located at Spad 6564 and is approx. 20' Long x 20-25' Wide x 5' Thick. A clean-up plan is being submitted for review.

An unintentional roof fall was discovered during the day shift pre-shift examination at approx. 7:00 a.m. and was called in to MSHA and State officials. The fall is located on 2 Belt Mains in the neutral entry at Break 49, located inby Spad 6555 and is approximately 25' Long x 20' Wide x 6' Thick. A clean-up plan has been submitted for approval.

An unintentional roof fall was found during the pre-shift exam for the evening shift, the fall was located in the track entry at BRK 42. The fall was approximately 20' in length X 8'-10' in width X 4.5' in thickness. A clean up plan was submitted and approved.

The intersection at Break 48 on #2 Belt Line fell approximately 20' long by 20' wide and approximately 7' thick. The affected area is supported with fully grouted 6' torque tension bolts. The roof fall is located in the belt entry and does not impede any travel of miners.

The intersection fell out in the track area at BRK.61 at number 3 track entry the fall is 19' wide 12' long 8' thick.

A roof fall was discovered in the track entry between break 25 and 26 on # 2 belt. The fall is located between spad 6478 and 6481. Its approximately 20'long X 18' wide x 8 foot high.

Roof fall was discovered in the belt entry between break 28 and 29 on # 2 South belt. All approaches to the fall is breakered off.

The employee stated was riding # 5 EMU down the track when a draw rock struck the employee on right shoulder, the piece of draw rock was 1/2""x2""x1' long. The employee sought medical attention on 8/30/2017 and was taken off work. This incident didn't become reportable until 8/30/2017. This incident will be a lost time incident.

The employee stated - was raising a 6"" discharge line up that had fallen down with the line shifted and fell striking the top of head and straining neck. The employee sought medical attention and was placed on restricted duty for 5 days.

The employee stated he had carried supplies to a roof bolt machine and on his way back stepped on a rock twisting his left ankle. The employee continued to work, but sought medical attention on 1/27/15 and was diagnosed with a strained ankle. The employee returned to work at his normal job class, but this incident is being reported due to medical treatment.

On 9/9/2014 a roof fall was discovered in an intersection 3 cross cuts outby the 1st Right #1 Seal in the Southwest Mains. The fall occurred in an intersection and was approximately 7' high, 20' long, and 19.5 feet wide.

At approximately 1:00 p.m. a roof fall was discovered just inby the mine fan canopy.

On 7/25/14 at approximately 8:00 a.m. the operator was notified that this previously employed individual had been diagnosed with occupational pneumoconiosis.

On 7/8/2014 at approx 2:00pm the operator was notified that this employee which was last employed on 10/16/2009 had been diagnosed with black lung.

On 7/7/2014 at 7:15 a.m. a roof fall was discovered at break 28 in the track entry. The fall was approximately 65' long and 20' wide.

A roof fall was discovered in the South Mains located at break number 5. The fall was approximately 20' long and 5' thick at the highest point. The fall occurred in number 6 entry. The fall was dangered off.

During the examination of a weekly aircourse a cont. of an old fall was found. The fall occurred in the Ellis Main at break 27 and now extends from #5 entry to #4 entry. The travelway was not affected.

On 4/8/2013 a roof fall was discovered 150' inby the mine fan in the intake air course. The fall measured approximately 30' in length and fell up to 7' in height.

The employee has been awarded 10% permanent partial disability for occupational pneumoconiosis

Injured was sitting beside bolter when roof straps slipped off bolter striking left hand.

Employee walking down steps felt dizzy, caused by low blood sugar. Tripped on steps and fell, twisted right ankle.

70' X 19' X 7' fall in #4 entry on #2 belt near spad # 898.

EE was backing up miner and set the bumper down on his left foot.

Roof fall on 2nd South Mains in #6 entry near spad No. 977. Fall measured approximately 50'L x 18'W x 11'H. Entry was breakered off and will not be rehabilitated.

Injured was prying rail with bar when it slipped causing him to strain back and neck. Did not start missing until 9/28/09.

Injured was checking pump when piece of rock fell striking head.

Injured was tramming scoop backwards when he ran over board causing it to come into deck.

A piece of rock fell (about 20"" long, 12"" wide, 1"" thick) and some coal hit him. He was building an intake stopping. NOTE: We are suspicious of this alleged accident. There were no witnesses and no visible marks on his body.

While getting tools from the tool box, the lid of the tool box fell down onto his left hand.

Setting tail piece, Jack was setting on tail piece, fell off hitting ee on middle finger of right hand. tip

While taking the scoop off the charger, employee pulled the ground wire and the battery lid fell down striking his left hand between his thumb and finger.

Injured was operating scoop when he ran over rock causing injured to hit back and hip against SCSR.

While roof bolting, 2 piece steel got stuck, tried to pull apart, glove got stuck on steel and pinched hand.

Injured was lifting lid on tool box when he strained back.

Injured was operating shuttle car, when he hit hole in roadway and jammed back. Did not start missing work until 2/29/08 and was unsure it was related to injury.

Injured was changing scoop battery and took piece of drill steel to knock pin out. Pressure from pin releasing caused drill steel to hit injured in head.

Employee had a utlity knife, trimming on a roll of 48"" belt. Knife slipped, cutting right arm. Cut was about 2"" to 2 1/2"" long.

Roof fall on #3 belt, 2nd South Mains between #2 - #3 Entry, 1 brk outby spad 1031. Fall was 40' long x 20' wide x 8' high. Area was cribbed after fall.

Injured was going through mandoor to check pump when mandoor shut on right leg.

Injured was hanging miner cable when slack was pulled up causing cable hanger to puncture hand.

Injured was loading bolts on bolter when one slid off and fouled in drill boom. Injured lowered boom causing bolt to strike him in the face.

Injured was handling roof bolt plates when cut finger.

Injured was opening air lock doors when he strained back. did not start missing until 2/26/07

Injured was helping pump seals and had his hand on the pump. When the pump started an arm struck his hand.

Injured stated he strained back on 8-30-05. Started missing work on 9-19-05 and did not report account until 9-21-05. Injured stated he strained back lifting planetary for miner.

Injured was standing four rows of bolts, back from last row of bolts when a piece of rock fell between bolts and rib striking him on head and back.

Injured was putting breaker in on shuttle car (testing cable) when breaker flashed burning right hand.

Injured was removing trash from roof drill when rock fell striking his ankle.

30'L X 18'W X 6'H ROCK FALL ON ELLIS MAINS IN TRACK ENTRY 4 BRKS INBY SPAD 723. FALL WAS CLEANED AND BOLTED.

Injured was unloading belt rope on spool what spool slipped smashing finger against rod.

injured employee was climbing a ladder. the botton of the ladder kicked out, causing him to fall. employee received a fracture to his right arm, and pain in the rib and back area.

Injured was carrying timber, slipped and fell causing timber to fall on shoulder & neck.

Injured was leaving feeder area when shuttle car cable caught foot flipping injured and he landed on right ankle.

ROCK FALL (24'L X 23'W X 12'-16'H) ON SECTION #2 APPROXIMATELY 3700' FROM ACTIVE SECTION. FALL WAS LOCATED IN ENTRY #1 AT SPAD #621.

INJURED WAS CLEANING ROCK DUST AUGER WHEN PIECE OF METAL CUT END OF RIGHT HAND - FOURTH FINGER.

Roof fall (28'lx18'wx14'h) in #4 entry @ spad #714 on 2 section. fall was cleaned up and rebolted.

ROCK FALL IN #1 ENTRY ON #2 SECTION APPROX. 1100' FROM FACE (SPAD #682). FALL WAS 2.5'L X 17'W X 15'H AND WAS BREAKERED OFF.

Rock fell from top striking injured on head and neck.

Rock fall (40' L by 18' W by 8"" H) in #2 return on 2 section. Fall located at spad #505 between crosscut of #6 and #7.

Rock fall (70' length by 18' wide by 10-20' high) on 2 section approx 2400' from active face, fall located at spad #486 between #6 and #7 crosscut.

EE was loading roof bolt supplies when a piece of draw rock fell striking rt knee.

Roof fall (40'l x 17'w x 7'9"") in crosscut between #3 and #4 entry, fall on #2 section at spad 475 approx. 2300' from face. Area breakered off.

INJ WAS PULLING MINER CABLE WHEN HE STRAINED BACK. EE DID NOT REPORT ACCIDENT UNTIL HE RET'D TO WORK.

INJURED WAS HANDLING ROOF PLATES WHEN A PIECE OF METAL CUT HIS RT.INDEX FINGER.

Injured was removing load lock valve from cylinder on bolter when load lock came out under pressure hitting right index finger.

INJURED WAS BUILDING STOPPING WHEN PIECE OF ROCK FELL STRIKING INJURED ON TOP OF BOTH HANDS.

INJURED WAS LIFTING BELT STRUCTURE WHEN HE TWISTED BACK

INJURED WAS TO PORCESS OF MOVING BEAMS FOR OVERCAST WHEN BEAMS SLID CAUSING LEFT ANKLE AND LEG TO BE TRAPPED.

INJURED WAS CLEANING STOPPING THAT WAS KNOCKED WHEN BLOCK FELL STRIKING INJURED IN HEAD.

TOOK 5 BOLT BUNDLES OF BOLTS OFF MACHINE & FELT PAIN IN HIS SHOULDER, NECK & BACK.
