Employee was operating an excavator above an embankment, when the excavator began sliding down the bank. EE tried to swing the boom around to stop the slide, but the excavator rolled onto its side. EE was taken to the hospital where the x-ray revealed a compound fracture of the T-12. EE was given meds and sent home; EE is awaiting an appointment with a neurologist.
Employee was operating a tractor trailer coal truck when they hit a rough spot in the road. Lower Right back began hurting, EE was taken to PMC hospital. The employee was x-rayed, given medicine and taken off work for a strained back.

Employee was working on personal vehicle in the mine parking lot. While changing the brakes, the vehicle fell off the car jack and onto EE's right lower leg. EE was transported to the hospital by ambulance and received stitches, x-ray (nothing broken) and treated with medication.
The employee was climbing back onto the Dozer, EE's foot slipped off the step next to the engine and EE's left leg fell between the track and the frame. EE's knee struck against the track causing a fracture. EE was taken to the hospital by ambulance and treated for the fracture.
Employee was operating an 830E Komatsu Rock truck. While being loaded a rock slipped out of the bucket hitting the top side rail of the truck bed jarring the Employee. The employee went to the hospital complaining of back pain and tingling in right foot.
Employee was conducting a pre-operational exam of EE's truck. EE raised the hood and felt a sharp pain in EE's back. EE continued to work for a few hours and then went to the hospital. EE was treated for a strain and given a couple days off to rest.
Employee was removing Struts on a rock truck. The left strut had been removed and EE was cutting the bottom pin out from the right Strut. The right strut moved when they cut through the pin. They stepped back and tripped over the new struts laying on the ground behind them. They landed on the second strut cracking a rib and vertebra.
Drill was snow covered and turned into ice overnight. While working on drill, the back door wouldn't open, so the technician moved to the back left corner to continue working, when standing up from back left corner, feet slipped on ice causing technician to twist left knee.

While Employee was cleaning up rock in the pit where the rock trucks where backing in, unaware that Employee was on an unstable rock under the excavator Employee exstended the arm out to move a rock and the excavator tipped over on its right side. The operator recevied two cuts to head and cuts to right hand. Employee was wearing seat belt.
Employee was checking the antifreeze in a loader, when employee started to remove the cap the raidiator was still hot and the antifreeze scalded employee's left hand. The employee went to the doctor and was treated for burns.
Employee was hauling across the mine road, the derate indicator light came on so EE stopped the truck. The stop engine light came, smoke and flames began to rise from the drivers side hood. The flames had gotten to intense before EE could raise the hood so EE backed away until help arrived. The trailer was saved but the Tractor was a total loss. No injuries reported.

Employee was exiting an excavator, EE placed right foot onto the step that had dirt and mud on it. EE's foot slipped striking knee against the step causing EE's knee to swell.
Employee was lifting a boring bar and trying to slide it in the bearing. As they were lifting and pulling they felt a sharp pain in the groin area. EE was transported to the hospital where they were diagnosed with a hernia. The boring bar weighs approximately 75lbs. EE returned to work the next day and hasn't missed any work yet. Restricted to no lifting over 15 lbs.

Changing conveyer belt on rock crusher, one mechanic manually rolls the counterbalance wheel, as the belt was being rolled the injured technician failed to remove a hand and sustained a laceration to the 3rd finger on right hand.
Employee was in a Komatsu 730E rock truck getting loaded by a Komatsu 5500 Excavator, the excavator operator had a large rock to put in the bed of the truck. The rock slipped out of the bucket hitting the bed and jarred the truck driver straining EE's lower back.
Employee was attempting to connect a man-basket to a crane. When employee turned a screw type lever that adjusts the basket the arm fell on employee's left hand breaking the ring finger.

Employee was getting off a dozer and stepped on a small rock on the first step causing them to slip. Employee attempted to catch themselves and hit right hand on the railing causing a fracture to a finger.
Employee was picking up a track pad and felt a pull in EE's left groin area. EE didn't go to the Dr until 3/4/2020 and was diagnosed with a hernia. EE didn't receive any treatment and has not missed any days of work.

Employee was traveling out of the parking lot in a pickup truck beside a rock truck, when the rock truck made a right turn, hitting the front end of the pickup. It pushed the pickup truck to the right, jerking the employee around.
Employee was scraping mud off the haul road with a front end loader when EE hit a rock with the bucket. It stopped EE immediately and the loader twisted sideways resulting in a strain to EE's lower back.
Employee was walking from pump room to engine room on PC5500 Komatsu Excavator when they tripped on the catwalk. As they were falling they grabbed a rail and pulled a muscle in upper right arm. EE continued to work and followed up with a doctor shortly afterward. The doctor let employee continue to work until the doctor took them off. The first day missing work was 2/5/2020.
Employee was exiting cab of dozer. When they stepped on the deck it had oil on the deck and they slipped and fell hitting an elbow on the deck and hit their tailbone on the dozer track.
Employee was walking to the back of the end loader on the counterweight level when EE tripped and fell to the ground landing on EE's feet. EE fractured right leg in the fall.
Employee was descending the access ladder of a Komatsu 5500 Excavator when they lost grip on the hand railing and fell approximately 6' to the ground landing on left foot resulting in a fracture to the ankle.
While driving an 830e rock truck and being loaded by a 5500 excavator, the excavator was loading a large rock that slipped out of the bucket and hit the bed of truck jarring the employee resulting in pain to neck and back.
Employee was walking across the hot start line when they tripped on the steps. They attempted to break the fall by grabbing the hand rail with their left hand. When they grabbed the rail it pulled their shoulder out of place.
Employee said EE tripped on some metal strips and fell on the Low-boy deck and hurt back.
Employee was getting on an excavator when foot slipped while stepping onto the track. Employee hit right leg on the edge of the track causing a bruise and swelling. Employee continued to work and didn't start missing days until 4/1/2019.

Person was weed eating around office building and was found unresponsive and was taken to Logan General Hospital where ee was pronounced dead. Probable cause was natural death. We have not received the autopsy report as of yet.
Mechanic was trying to free the end of the hard bar that had dropped down and wedged into the roller frame. They were using a hyd. jack to push the roller frame out placing crib block under the bar when it came free from it's position as EE was placing a crib block under it and the roller frame shifted inward striking mechanic on the head on EE's hard hat causing a cervical strain

While using a chain block to line up a trunion ball on a grader, the hook came undone and struck the employee in the groin area causing swelling.
Injured pulled EE's personal vehicle within 5' of a contract welders service truck. Welder was pulling off start line when injured got out of EE's car (which was lower than the hood of the truck) the truck pulled off the start line striking the injured's personal vehicle door causing it to strike the injured resulting in fracture to the top of the tibia bone.
Driver said the truck jumped out of gear causing the truck to run away down the hill. Driver has 6 cracked ribs and a punctured lung and is recovering at home. MSHA investigators have inspected the truck and found no problems with the truck transmission.

Employee was operating a swivel truck and allegedly hit a hole in the haul road and allegedly jarred back.
While working on a scaffolding approximately 6' off ground, one corner ear of the scaffolding broke causing employee to fall hurting his left shoulder and back.
While attempting to loosen an oil filter under a 993 loader, employee strained his neck. Employee went to the doctor on 2/20/2015 and the doctor put him on restricted duty. Note: EE went to Dr on 3/13/15 and the Dr took him off work beginning 3/13/15.

While installing a jack on a loader injured was attempting to align jack eye with ears when the strap holding the jack slipped causing the eye to fall catching his finger between eye and ears of jack
Employee was cutting a tree, stepped back away from the tree and the chainsaw he was using slipped and cut his elbow.
We had an ice storm and the steps and hand rails where covered in ice. EE was descending down the stairs of a 789 rock truck and was on the third step when his feet slipped causing him to fall down the stairs hurting his shoulder. He continued to work until his Dr. told him to take off work beginning 10/1/2014 and return to work on 11/17/2014.
Driver tripped and fell over fuel piping causing injury to right shoulder. The fuel delivery driver was setting up to deliver fuel into tanks located at bulk storage fuel farm. Tanks are under shelter and enclosed. It was dark outside and inside of shelter is dimly lit. While gathering tank reading to determine ullage, the driver lost his where a bouts and tripped over piping.

EE STATES WHILE PUTTING A LOCKRING ON THE INSIDE WHEEL OF A ROCK TRUCK MY RIGHT SHOULDER POPPED. I HAD PREVIOUSLY HURT MY RIGHT SHOULDER WHILE PUTTING THE LAST BEAD ON A SKIDDER TIRE ON 6/7/13 BUT INJURY WAS NOT REPORTED TO MANAGEMENT.

EE got on his knees to check the oil in the water truck. When he tried to stand up his right knee locked up. We think this is an occupational illness because he didn't do anything to cause a tear in his knee. There is nothing he or anyone could have done to prevent this. There is no accident. This shouldn't be considered a work injury but an illness.
Standing beside coal truck while mechanic put air in tire and was stung by a bee.
Employee was changing cutting edges on a Cat 16M Grader using a line-up bar to position the bolt holes. He had his hand around the end of the line-up bar which was has a 90 degree flat, sharp edge. When his helpers bar slipped, the resulting jar caused the sharp edge of the bar to penetrate his leather glove resulting in a cut requiring 3 stitches.
On 4/20/11 employee was working on dozer. He placed his right elbow (which had a calcium deposit on it) on the grouser on the track and his arm rolled off and the calcium deposit moved. He told his supervisor what had happened and that this was not an accident. He refused to fill out accident report and he said no because it wasn't an accident.
While installing a pin in PC5500 bucket the bullet slipped while being removed and struck employee's finger resulting in a laceration.
While operating dozer the seat broke and bottomed out. EE reported this on 3/3/11 when he told his foreman that his back was hurting. Attached letter protesting this alledged injury. EE began missing work on 3/14/11.
Employee was shutting cab door of rock drill and he placed his hand in the pinch point area and closed his hand up in the door resulting in a laceration to two fingers resulting in stitches.

EE was on the back of Red Top and was walking around the two Miller welding machines and lost his balance and fell to the ground. EE fractured his right upper arm between the elbow and the shoulder and chipped a fragment of bone in the same area.

While stepping off manbus at quit time, EE twisted his ankle.
While descending down from fuel truck EE stepped on a rock twisting his right ankle. This resulted in a small fracture.
Injured was attempting to open the lid of a 5 gallon bucket of oil. The spout ring broke when he lifted it. He laid his left arm on the bucket lid and attempted to use a sharp chisel to pry open the spout when the chisel slipped and struck his left arm resulting in a laceration.
Employee used a steel sledge hammer to strike a bucket tooth on a loader and a piece of metal struck him on the leg resulting in a laceration.
789 rock truck - strips missing. Cut strips on back of welding truck then put strips on ground & welded brackets to lift metal with boom. Finished shift and went home. That night around 11:00 pm started hurting in left shoulder and neck. Went to ARH Hospital in Hazard Ky.

A SAFETY CHAIN IS LOCATED AT THE END OF THE WAREHOUSE LOADING DOCK. THE CHAIN WAS IN PLACE AND THE EMPLOYEE WAS LEANING ON IT WHEN THE LATCH OR HOOK BROKE CAUSING HIM TO FALL BACKWARD STRIKING THE BACK OF HIS HEAD ON A PVC PIPE.
