EE and co-worker were removing stopping when a rock measuring 22"" in length by 15"" in width and 5"" thick fell and struck him on the head causing a fracture in his T-3 vertebrae.

Member was putting a nylon hanger around 4 inch waterline. The waterline fell catching his right hand int he j-hook on the hanger.

EE was watching the roof bolter cable as the bolter was backing up. He stepped in a hole, causing his left ankle to twist.

Unplanned roof fall occured old works, mouth of 2nd right panel between spad #308 & #317, #3 entry. Fall measured 65'long x 19'wide x 7-8'high. No accidents reported. The area was developed in December 2005.

He was installing 10' cable bolts and the roof bolt plate came down and struck his left wrist causing a 7 in. laceration.

Employee had a piece of drill steel hung n chuck. He took a hammer and hit drill steel to try and loosen it. He accidentally bumped joystick and rotated steel striking him in the left wrist.

Member was in process of moving up flypads. He went to get his hammer off continuous miner. A piece of rib rolled out hitting him in his back causing him to stumble and strike his forehead.

Struck by falling object

Member was coming out of #4 into #5 when shuttle car hit top and broke the plate off a roof bolt. Bolt plate came down and struck his left hand causing a laceration.
