A roof fall discovered in the 5th left panel # 6 belt entry during an exam. It measured 30' long, 20' wide, and 7' thick. Fall occurred due to laminated shale and coal above the 4' resin roof bolts. It did not impede travel, affect ventilation, no injuries occurred as a result of the fall. The fall occurred in an area developed Jul 05 12,000' from the active section.
A roof fall measuring 40' long, 18' wide, 7.5' thick was found in the # 3 entry adjacent to the #5 north belt approixmately 1700' outby the active section. The area was developed May 06 with 5' torque tension roof bolts. Laminated coal and rock streaks above the anchorage zone was the cause. Ventilation and the passage of personnel was not affected. No injuries occurred.
Employee was struck on the left shoulder by falling rock while operating the continuous miner in the #4 heading 111' inby spad 56357 on the 004 section. Employee suffered contusion to the left shoulder.
A roof fall 300' long 19' wide 8' thick occurred 600' outby the section. The fall occurred in the #2 return entry of #5 belt 60' outby spad 56074. 5' torque tension roof bolt supports was in use. No injuries occurred as a result of the fall, passage of men was not affected, ventilation was affected as 4 stoppings were damaged. Fall was due to laminated coal/sandstone streaks.
Roof bolter was struck by a falling rock fracturing the middle finger left hand. While raising drill steel up to the mine roof a piece of roof rock fell striking the employee on the left arm. As he pulled back the rock struck the left hand pinching it against the drill boom.
Employee was operating a Johnson 4 man personnel carrier in reverse when the front tires struck the coal rib. The sudden stop of the tires against the rib caused the steering wheel to spin and the knob on the wheel struck the employee on the left hand causing a fracture.

AREA WAS DEVELOPED OCT '02. 5' TORQUE TENSION ROOF BOLTS WERE INSTALLED IN THE AREA. THE FALL OCCURRED DUE TO LAMINATED ROCK & COAL ABOVE THE PERMANENT ROOF SUPPORT. FALL WAS APPX. 5200' FROM WORKING FACE. VENTILATION & TRAVEL NOT AFFECTED.
An unintentional roof fall occurred in the # 6 neutral entry along the # 8 belt at crosscut # 3. The fall measured 25' long, 19' wide, and 5' thick. The fall occurred due to coal streaked top. The type of roof supports which was used in the area was 5' torque tension roof bolts. The area was developed May 2005 and was 3300' from the working face.
EMPLOYEE SUFFERS FROM A PARTIAL HEARING LOSS DUE TO REPEATED TRAUMA OVER AN EXTENDED PERIOD OF TIME. HE HAS MISSED NO TIME FROM WORK DUE TO THIS ILLENSS. HE HAS WORKED 35 YEARS IN THE INDUSTRY IN VARIOUS CAPACITIES AND OPERATIONS
Employee and a co-worker were pulling belt on a flat car. The injured employee complained of pain in the groin area. On May 31, 2005 Brooks Run Mining recieved medical documents indicating that the employee suffers from an bilateral ing. hernia. Employee failed to notify management on seeking medical treatment for this injury originally classified as a first aid report.
An unintentional roof fall was discovered 5000' outby the working section in the intake entry inby spad 49941. The fall measured 20' long, 20' wide, 6' thick. The fall occurred due to laminated rock with coal above the 5' torque tension roof bolts. Ventilation and the passage of men was not affected. Fall access points were barricaded to prevent travel.
On 05-09-05 Brooks Run recieved medical documentation on the following, employee slipped on wet muddy mine floor at the section power center, fell causing an non displaced fracture to the tail bone. Prior to the medical documents this case was not reported by the employee. Employee missed no time and x-rays conducted showed the fracture.

The fall 1100' outby the 003 mmu and did not impeade travel or affect ventilation. The fall perimeter was resupported and barricaded to prevent travel into the fall.
An unintentional roof fall was found (2) breaks inby spad 49794 in the neutral entry 2400' outby the 003 MMU. The fall measured 40' long, 30' wide, and 5.5' thick, 5' tension bolts were installed in the area. The fall did not impead travel or ventilation, no personal injuries occurred due to the fall. The fall was barricaded to prevent travel.
EMPLOYEE WAS TRYING TO GET S/C ON THE OUTBY SIDE WHILE IT WAS LOADING. HE TRIPPED ON TRAILING FALLING DOWN. WHEN EE GOT BACK UP ON HIS FEET THE S/C BEGAN TO TRAM TOWARDS THE FEEDER, STRIKING HIM ON BOTH LEGS BELOW THE KNEE, KNOCKING HIM ON HIS BACK. EE was plastering a stopping and got some plaster in his eye. He said that it came through the side of his safety glasses.

8/11/04 EE WAS TAKING PRESSURE OFF THE WATERLINE AT THE END OF THE LINE. EE STATED THAT THE PRESSURE HOSE SLIPPED OUT OF HIS HAND & HIT HIM BEHIND THE LEFT EAR. WE WERE NOT NOTIFIED OF THIS ACCIDENT UNTIL 8/20/04.
Employee was struck by a rock measuring 42"" wide 41""long and up to 3"" thick while operating a continuous miner. The rock fell from between permanent supports and the right rib. Injured employee complained of pain in back and ribs and was later Diagnosed as having NON life threatening injures.

Roof supports in use was 5' torque tension bolts. Fall did not impeade travel or ventilation, no accidents resulted from the fall. Information was recived on the fall 06/21/04.
Employee stated that he struck his head on the scoop canopy as he was transporting a power center in the scoop bucket. The power center became dislodged when the scoop trammed over a hill and caused the scoop to bounce. Employee recieved a laceration when he struck the canopy
Employee stated that he was removing rock from the top of the continuous miner by hand when a piece turned and caused a laceration to the right forearm. Wound required sutures only.
EE WAS OPERATING A CONTINUOUS MINER WHEN A ROCK MEASURING 3' WIDE 3' LONG AND 8"" THICK FELL FROM THE TOP STRIKING THE TOP OF THE CONTINUOUS MINER. THE ROCK TRAVELED ALONG THE CONVEYOR BOOM AND ROLLED OFF THE END OF THE BOOM STRIKING THE MINER OPERATOR ON THE LEFT KNEE.
I was notified on 06/01/2004 that employee was losing time from the following accident. Employee stated that while he was hanging miner cable he slipped on the wet mine floor and twisted his right knee. Employee started missing time on 05/17/2004.
(B) UNDERGROUND LOCATION: SECTION BELT TAIL. EE WAS ATTEMPTING TO REMOVE A CRIB TIE FROM UNDER THE FEEDER AS IT WAS BEING TRAMMED BACK ONTO THE TAIL PIECE. WHEN THE EE PLACED HIS HANDS ON THE CRIB TIE THE FEEDER TILTED BACK ONTO THE CRIBS. THE EE'S 3RD & 4TH FINGERS R. HAND WERE CAUGHT BETWEEN CRIB TIE AND MINE FLOOR - TIPS SEVERED. EE DID NOT HEAR THE ALL CLEAR SIGN.
Employee stated that he felt a pop and noticed pain in his left arm while helping lift a side shield on the continuous miner. Employee is losing time.
EE REPORTED THAT A ROCK FELL FROM THE TOP OF THE ROOF BOLTER BOOM CANOPY, SLID DOWN THE BOOM AND STRUCK A BOX OF ROOF BOLT GLUE. EE STATED THAT HIS RIGHT HAND WAS BETWEEN THE GLUE BOX AND THE BOOM. EE RECEIVED MEDICAL TREATMENT FOR HIS INJURIES.
EE WAS RIDING ON A MANTRIP TO THE SECTION AND TURNED TO WARN OTHER WORKERS TO BE AWARE THAT THE MANTRIP WAS CROSSING UNDER AN OVERCAST. AS THE EE TURNED BACK HE STRUCK THE OVERCAST WITH HIS FO REHEAD RESULTING IN A LACERATION. SUTURES WERE APPLIED TO CLOSE THE WOUND.
EMPLOYEE WAS PREPARING TO CHARGE THE BATTERIES ON THE SCOOP WHEN THE LID FELL STRIKING THE SMALL FINGER OF HIS RIGHT HAND. EMPLOYEE DIDN'T USE THE METAL STAND PROVIDED FOR PROPING THE LID UP. HE SOUGHT MEDICAL TREATMENT AND THE AFFECTED FINGER WAS AMPUTATED TO THE FIRST JOINT. EMPLOYEE IS MISSING DAYS FROM WORK.
EE WAS MOPPING OFFICE FLOOR AND A LARGE SPLINTER FROM MOP HANDLE WENT FIRST FINGER ON HIS LEFT HAND.

EE COMPALINED TO SEVERE PAIN IN THE LOW BACK AFTER TURNING HIS UPPER BODY. HE DID NOT REPORT ANY EVENT TO DESCRIBE CAN ACCIDENT NO INSTANCE WERE DISCOVERED DURING THE INVESTIGATION WHICH WOULD CAUSE AN AGGRAVATION FROM A PREVIOUS CLAIM.

A LARGE PIECE OF ROOF ROCK WAS LOADED ONTO THE UNAHAULER DURING THE MINING CYCLE AND WENT DOWN BEHIND THE RAM OF UNAHAULER, PREVENTING THE RAM FROM RETRACTING. THE FOREMAN GOT UP IN THE HAULE R AND ATTEMPTED TO MOVE THE ROCK OUT FROM BEHIND THE RAM. THE ROCK SLID AND CAUGHT HIS FOOT BETWEEN THE RAM AND ROCK.
EMPLOYEE WAS USING A CRIB BLOCK FOR LEVERAGE TO MINE A BATTERY STAND BACK FROM COAL RIB, FELT PAIN IN NECK AND CHEST AREA.

EMPLOYEE WAS OPERATING A ROOF BOLTER, DRILLING A HOLE. A PIECE OF UNBOLTED ROOF FELL NEAR HIM. HE TURNED QUICKLY AWAY FROM HE FALLING ROCK, CAUSING HIM TO TWIST HIS KNEE.
EMPLOYEE WAS HANGING A PIECE OF CURTAIN. WHEN HE FINISHED HE TURNED AROUND AND RAN INTO THREADS OF A PAINT ANCHOR BOLT.
