Employee was operating a Fletcher Roof Bolter, at which time a piece of loose metal on the roof bolt snagged the employee's glove. When the employee moved the joystick on the roof bolter, the bolt twisted the employee's glove dislocating two fingers.

While laying track rails, employee was using a slate bar to flip a rail when the slate bar sprung back and hit EE across the shoulder and collar bone.

Employee was performing normal job duties cleaning and maintaining belts. Due to continued pain and discomfort, employee sought medical attention. Initial exam indicated no injury. Employee was referred for MRI. Results were received on 03/16. Results indicate occupational illness in the form of early degenerative disc disease of the lower 5 lumbar levels.

Employee was attempting to wash out rock box at #5 Belt Head. When Employee turned on the fire hose the pressure knocked him down causing injury to his left shoulder area. Employee was transported to Raleigh General and then due to the nature of injury transported to CAMC General Division and was diagnosed with a broken clavicle and admitted for observation.

Employee was diagnosed with Occupational Pneumoconiosis and has exercised his rights as a Part 90 Miner on March 23, 2012. Employee was diagnosed in 1987 by another Employer.

Struck by flying object

Employee was welding and grinding on #5 Tail rock box. Employee was removing slag with a sledgehammer which resulted in a small piece of metal getting into left eye. Employee was treated at MedExpress and was instructed to follow-up with a specialist. The metal was slightly embedded in eye and was removed.

Employee was installing a clutch assemble and while holding it in his hand it slipped out and fell on his right hand.

Injured worker was stepping across to a beam. A rock rolled under his grounded foot, causing his leg to bend the wrong way and pop.

Over-exertion in lifting objects

While making a splice on the #1 Overland Belt EE bent over and attempted to pick up a set of chain ratchets and lift them over his head. He stated that he felt a sudden pain in his lower back. After seeking medical attention he was diagnosed with a strain. EE had scheduled a personal day to be off on 06-29-10 before this accident. He returned to work on 06-30-10.

EE was traveling on the roadway to the mine to begin work. As he started up the hill his truck lost traction . He attempted to get out of the truck, slipped on the slick roadway and fell, causing him to slide under his truck, hitting his chest.

Caught in, under or between a moving and a stationary object

While transferring mine timbers from a flat car to the forks of the outside forklift, one of them shifted and mashed EE's left hand. This resulted in a fracture to his left ring finger.

EE attempted to turn up a 1000' spool of communication and felt a pain in his back. EE continued to work until 03-08-2010. On 03-09-2010 he notified the Safety Department that due to continued pain he has seen a doctor and was dianosed with a lumbar protruding disc and could not work.

While using a railroad jack to put a mantrip that wrecked back on the track, the bar suddenly released hitting employee in the mouth. On 5-13-08 Marfork Safety was notified that both upper and lower front teeth were cracked and required dental work.

TRYING TO GET TWIST & KINK OUT OF 1 1/2"" WATER LINE HIGH PRESSURE & GOT HIS LITTLE FINGER CAUGHT IN IT. HE WAS TRANSPORTED TO RALEIGH GENERAL HOSPITAL WHERE HE WAS INFORMED THAT DUE TO DAMAGE A PORTION OF THE FINGER WOULD HAVE TO BE AMPUTATED.

EE WAS ATTEMPTING TO REPLACE A HYDRAULIC HOSE ON THE DRILL HEAD ON THE LEFT SIDE OF BOLTER. THE BOLTER WAS PARKED NEXT TO THE ROADWAY, WAITING FOR THE CONTINUOUS MINER TO FINISH MINING 5 FACE. WHILE REPLACING THE HOSE AN UNKNOWN AMOUNT OF (CAP COAL) FELL FROM THE MINE ROOF STRIKING EE ON HIS HARDHAT RESULTING IN SORENESS TO HIS NECK.

EE WAS STANDING IN THE CONVEYOR OF A SHUTTLE CAR ATTEMPTING TO UNFOUL A ROCK THAT WAS TOO WIDE TO PASS THROUGH TO THE DUMPING END OF THE CAR. HE WAS USING A SLATE BAR TO TRY TO MANUEVER THE RO CK FREE WHEN HE SUDDENLY SLIPPED, FALLING TO HIS KNEES. WHEN HE FELL HE CAUGHT HIMSELF WITH HIS LEFT HAND, JAMMING HIS LEFT THUMB AGAINST THE CONVEYOR CHAIN.

EE WAS OPERATING CONTINUOUS MINER IN 3 RIGHT XCUT WHEN A PIECE OF SHALE 36"" X 18"" X 1"" FELL OUT STRIKING HIM ON HTE HEAD & RIGHT SHOULDER.

EE WAS IN THE PROCESS OF CLEANING THE SECTION BELT TAIL PIECE, WHEN HE TURNED TO THROW A SHOVEL FULL OF COAL INTO THE FEEDER HE FELT PAIN IN HIS RT. KNEE. UPON SEEING A DR. HE WAS DIAGNOSED WI TH A TORN MENISCUS.

EE ATTEMPTED TO LIFT A PIECE OF METAL OFF MANTRIP, WHEN HE SUDDENLY SLIPPED RESULTING IN HIM STRAINING HIS LOWER BACK. HE CONTINUED TO WORK. HE WAS ADVISED BY A DR. TO NOT WORK ON THE 7-16-02.

MEMBER WAS PULLING CABLES OUT FROM UNDER A RIB ROLL & FELT A PAIN IN HIS BACK. HE WAS EXAMINED & FOUND TO HAVE A PULLED MUSCLE. HE WAS TOLD TO REMAIN OFF FROM WORK 1 WEEK BY HIS DOCTOR.

MEMBER WAS BENDING OVER TO PUT A BOLT INTO THE RIB WHEN THE RIB ROLLED. HE JUMPED BACK, SLIPPED ON THE BOTTOM, FALLING AGAINST ROOF BOLTED AND HITTING HIS SIDE. HE WAS EXAMINED BY A DOCTOR AND FOUND TO HAVE A CRACKED RIB.

EE WAS OPERATING AN HDDR-13 WALK-THRU ROOFBOLTER IN THE RIGHT CROSSCUT OF THE #6 ENTRY WHEN A PIECE OF ROCK MEASURING 0-6"" THICK X 34.5"" LONG X 21"" WIDE FELL FROM BETWEEN THE BOLTS STRIKING HI M ON THE HEAD & NECK. HIS INJURIES WERE DIAGNOSED AS A CONTUSION/ABRASION & HE WAS INSTRUCTED TO MISS 2 DAYS.

EE WAS USING A HAMMER IN AN ATTEMPT TO REMOVE A WORN CUTTING BIT FROM THE CONTINUOUS MINER RIPPER HEAD. UPON STRIKING THE BIT A PIECE OF THE HAMMER HEAD BROKE AND PENETRATED THE SKIN, IMBEDDIN G IN HIS RIGHT THUMB. EE DID NOT SEEK MEDICAL TREATMENT FOR THIS INJURY UNTIL 4-18-02. THE METAL WAS SURGICALLY REMOVED.

EE WAS ASSISTING WITH THE ADVANCEMENT OF A ROOF BOLTER CABLE ON A POWER MOVE. HE WAS WALKING IN THE #6 ENTRY OBSERVING THE BOLTER CABLE AS IT WAS PULLED WITH A SCOOP WHEN SUDDENLY HE STEPPED IN A HOLE FALLING ON HIS LT. SIDE. THE MINE FLOOR IS SOFT, WHICH CAUSES IT TO BE UNEVEN ONCEEQUIPMENT HAS TRAVELED OVER IT. HE SOUGHT MEDICAL ATTENTION ON 1/28/02, DIAGNOSED WITH CONTUSION
