Weathered stack shale, mixed into stack sand stone., seperated in L line of x cuts, allowing roof to fall. Fall was found on a weeklt exam.
Unintentional roof fall occurred on 1-a northwest mains, #4 entry, spad #3332, 920' outby 005 mmu, 30' long x 19' wide x 8' high. Bolted with 6' torque tension and 8' point anchor bolts, fall was breakered off.
2nd Northwest Mains, #4 entry between spad #341-345, 9000' outby section, unintentional roof fall occurred. 30' long x 18' to 19' wide x 6' high, wet weathered soap stone, bolted with 6' resin bolts fully grouted.
EE had just helped block up scoop to cross track. When his weight was put on blocking it flew out striking him in face. Was crossing track to set tail piece for section pulling back to new set up to start pillaring.
An unintentional roof fall occurred. 30' long x 20' wide x 9' to 10' high. Was bolted with 6' tension & 8' point anchor bolts at spad #3838, #6 entry 005 mmu, 1-A northwest mains, 2200' outby section. Primary escapeway was blocked, production was stopped. Everyone was brought outby till escapeway was rerouted.

Unintentional roof fall occurred in intersection of #3 entry at spad #3831 1700' outby 005 MMU 3rd SW Mains. Fall was 18' wide x 18' long x 8' high, bolted with 6' tension rebar & 8' point anchor bolts; area is cribbed off.
Unintentional roof fall occurred 10000' outby working section 40' long x 18' wide x 6' high. Between #1 & #2 entry at spad #480 2nd Northwest Mains. Fell on top of service bolter; cribbed off, will not be recovered at this time. Found during weekly exam.

Unintentional roof fall occurred on 2nd northeast mains, #1 entry, 100' outby spad #2567. 30' long x 20' wide x 10' high. 2000' outby 003 section. Bolted with 6' torque tension bolts & 8' point anchore bolts in return air course. Found making weekly exam.
Employee was tightening bolts on a hyd. pump on a feeder when socket & ratchet slipped off the bolt head causing him to strike his hand on a shield & cutting his hand.
He was installing a bolt, the bolt head broke off and the plate and pan struck him on his left forearm and cut a 3"" cut.

Employee was walking in #4 Entry approx. 50' inby the feeder and slipped on loose, slick rocks. He fell and twisted his right ankle.
EE was attempting to roll rock out of track when it rolled back the wrong way pinning his leg betwene motor and rock, He started having trouble with his knee and went to dr and dr profomed microsopic surgery on his knee 4/9/07 causing him to be off work accident occured 100' outby spad #572 2nd northwest mains.

Miner was backing out of lift when shuttle car went down behind miner, allowing rock to fall on miner and causing miner to get stuck at 4:30pm. Before we could get s/car running and miner moved 7' of rock fell on the miner, causing us to use retriever on 003 section.

EE was building an intake stopping on 005 section. He was standing on ladder finishing wedging the stopping when a piece of draw rock fell and hit on his head and down his back causing him to fall off ladder to the ground. He was about three steps on ladder when he fell landing on his back.
EE fell behind tool sled, loading material on sled, stumbled on rock, fell on piece of metal and cut right thigh.

An unintentional roof fall occurred on 2nd Northwest Mains, #4 entry spad #573 20' x 20' x 6' to 8' high. Appeared to be shalie weather roof in intake aircourse. Not affecting escapeway. Type of roof bolt used was 6' resin.

He was working on #2 belt head drive chains. He was separating the straps on the chain by driving a chisel between the link & strap. He was going to move the chisel to the other side & hit it sideways & chisel flew up & hit him in the lip.
EE was standing behind bolter dropping trailing cable when bolter backed up to miss s/car cable, while doing so smashed EE foot between bumper and floor.
Employee was using a slate bar, assisting miner oper. in removing rock off of miner, when slate bar rolled smashing hand between slate bar & miner.

Unintentional roof fall ocurred in Return Air Course. 7000' outby 005 on 2nd Northwest mains in #2 entry, spad #476. 30' x 19' & approx. 6'thick. Appeared to be weathered & shale roof. Fall will be cleaned up & bolted back with extra support added.
UNINTENTIONAL ROOF FALL OCCURRED AT THIS TIME BLOCKING 2 MRS UNITS. 20' X 20' APPROXIMENTLY 6' THICK. IN INTERSECTION AND APPEARED TO BE A SLIP ON THE 003 SECTION 5TH LEFT 3 WEST MAINS #2 ENTRY SPAD 1476. DEVELOPED RECOVERY PLAN, SECURED AREA, RECOVERED MRS UNITS AND TIMBERED AREA OFF.
Setting spad in #1 entry, fell off ladder. Sliced finger on Roof Bolt that employee was holding.
SHUTTLE CAR LOST POWER WHILE ON FEEDER AT SECTION DUMPING POINT WHILE OPERATOR WAS USING MANUAL PUMP OFF BRAKE AND SCOOP WAS PULLING ON CAR. HE WALKED ALONG SIDE OF CAR AND WAS CAUGHT BETWEEN RIB AND SHUTTLE CAR
Employee was cleaning on walkway side of his belt. While lifting on a piece of draw rock, he felt something pull in his rt. upper arm.
Stack Shale Roof weathered separated allowing intersection to fall, no plans to rehab, area has been cribbed off.
EE was operating roof bolter, while pushing roof bolt up with drill pot roof bolt bent and roof bolt plate struck ee in rt. forearm just below the elbow causing laceration.
Roof in this area is a shale sandstone mix with streaks of coal, and some formations shale sand mix separated at app 5 to 6 ft allowing intersection to fall. Area had been bolted with 4 ft long bolts, fall has been cribbed off; roof control plan has been changed since this was developed.
Stack shale area with thin coal streaks present; weathered shale separated allowed to fall loose from main roof.
THIS IS A STACK SHALE AREA WITH THIN STREAKS OF COAL AND WEATHERING CONDITIONS, WHICH ALLOW THE THIN LAYERS TO SEPARATE & PULL AWAY FROM THE MAIN ROOF.

While installing roof a piece of thin shale rock fell out and struck him on left side of face.
STACK SHALE ROOF WEATHERED & DETERIORATED ALLOWING SHALE TO SEPARATE & SLIP AWAY FROM MAIN ROOF. FALL DID NOT AFFECT VENTILATION OR TRAVELWAY. FALL CRIBBED OFF ON ALL SIDES, NO PLAN TO REHABILITATE THIS AREA.
The lowest point in a swag area with wet shale roof with slick backs. Shale weathered and alloyed shale to pull loose from main roof.
WET SHALE AREA WITH SLICK BACKS. SLIP AREA PULLED LOOSE ALLOWING ABOUT 20-50' X 19'W X 6-7'THICK. THIS AREA HAS BEEN CRIBBED OFF NO AIRWAYS OR TRAVEL WAS AFFECTED.
Wet area with slikk backs and formations in roof with rider sham, allowed roof to separated above roof bolt machine and fell, fall has been cribbed off and travel way remarked, no plans to rehab this area.
EE WAS INSTALLING 8-FT-LONG CABLE BOLTS IN #8 ENTRY INTERSECTION. WHILE PUSHING THE ROOF BOLT UP, IT GOT STUCK. EE TRIED TO PULL THE BOLT BACK OUT OF THE HOLE. THE BOLT CAME OUT OF THE DRILL POT ON THE ROOF BOLTER, AND THE ROOF BOLT PLATE STRUCK THE EE ON THE NOSE AND IN THE FACE.
Fall was found during a weekly exam of area. Area was developed 3-19-01 and is approx 6000' from face. Fall is 6' high by 40-45' long by 19' wide between #8 and #9 entry. Roof control plan was being followed and 6' glue bolts used.

WHILE WALKING TO BACK OF ROOF BOLTER TO GET SUPPLIES, EMPLOYEE WAS STRUCK ON THE HEAD BY A ROCK APPROXIMATELY 4' SQUARE. IT SLID DOWN HIS BODY, STOVING HIS NECK, SHOULDER AND BACK.
EE WAS BOLTING THE ROOF WHEN A PIECE OF ROCK FELL STRIKING HIM ON HIS LEFT ARM AND CUTTING IT IN 2 PLACES.
AN AREA OF ROOF 35'LONG - 8'HIGH FELL OUTBY SPAD 563 IN #5 ENTRY, NEUTRAL. THE ROOF IS SLATE, ROOF IS DRY, A RIDER COAL SEAM 8""THICK IS 8'ABOVE COAL SEAM. AREA WAS BOLTED WITH 4,6 & SOME 8' FULLY GROUTED BOLTS & SOME CRIBS. AREA WAS DEVELOPED 9/2001 & IS 1200'FROM SECTION. AREA HAS BEEN BREAKERED OFF. FALL WILL NOT BE MOVED.

ROOF IS SLATE WITH A RIDER COAL SEAM AT 3'; ROOF IS DRY. FALL AREA WILL BE MOVED & REBOLTED. RIDER COAL SEAM IS 8"" - 12"" THICK.

FALL AREA SOFTENED UP & FELL OUT AROUND 6'BOLTS, PULLING THE 4'BOLTS OUT. AREA LATER FELL OUTBY FOR APPROX. 1 BK TO SPAD 826. FALL WILL NOT BE MOVED.
EMPLOYEE WAS GOING TO RAISE THE BATTERY LID ON A LOCOMOTIVE. THE LID WAS STUCK & WOULDN'T RAISE. HE STRAINED HIS LOWER BACK.

AREA WAS BREAKERED OFF. AREA WILL BE REHABILITATED. MEN WERE MOVED OUTBY FALL AREA & WORK ON FALL HAS BEGUN. FALL AREA IS 1000' FROM SECTION FACE.
AREA FELL BETWEEN #2 & #3 ENTRIES RETURN, 3,000' FROM ACTIVE SECTION. AREA DEVELOPED 5-10-01. ROOF IS SLATE BOLTED WITH 4'& 6' RESIN. FELL RIB TO RIB, 3-4' THICK. THERE IS A 1 INCH RIDER ABOUT 2'-3' UP. FALL IS DIRECTLY ACROSS FROM SPAD 299. FALL WILL NOT BE MOVED.

AREA IS BREAKERED & DANGERED OFF. ROOF IS SLATE, A RIDER SEAM IS PRESENT ON LEFT SIDE ONLY AT 4'. FALL WILL BE MOVED.

EE WAS STANDING AT FRONT WHEEL OPERATORS SIDE WATCHING OTHER INSTALL LAST BOLT IN ROW. WHEN A SLAB WATCHING OTHER INSTALL LAST BOLT IN ROW. WHEN A SLAB OR LOOSE PIECE OF SLATE ROCK DISLODGED S TRIKING HE IN THE HEAD AND SHOULDER PIECE OF ROCK FELL FROM BETWEEN BOLTS.

ROOF FALL BREAK 41, 1 BREAK IN BY SPAD 296. FALL IS 20 FT BY 20 FT BY 4 FT THICK. AREA BOLTED WITH 4' BOLTS (RESIN FULL GROUT) AREA DEVELOPED 5-1-01 4,000 FT OUT BY FACE AREA. BREAKERED OFF FALL. FALL WAS FOUND 11-9-01 ON WEEKLY EXAMINATION.
EE WAS MOVING THE MINER, NOTICEING A PIECE OF LOOSE SLATE, EE STOPPED THE MINER, TOOK A SLATE BAR AND PULLED THE LOOSE SLATE, WHEN A PIECE OF THE SLATE STRUCK EE ON THE FOOT.
EMPLOYEE WAS SHOVELING BELT LINE AND AT AROUND #3 BELTHEAD WHILE SHOVELING EMPLOYEE'S HAND GOT CAUGHT IN BOTTOM ROLLER, BREAKING HIS HAND.

EMPLOYEE WAS REMOVING A WIRE ROPE FROM A SHUTTLE CAR WHEN IT CAME LOOSE AND SPRANG BACK AND STRUCK HIS LEFT INDEX FINGER CUTTING IT.

A ROOF FALL WAS DISCOVERED BY THE FOREMEN AT APPROX. 12:30 P.M. ON 6-29-01 IN THE #8 ENTRY, A NETURAL AIRWAY. THE FALL WAS 1 BK OUTBY SPAD 323 IN THE INTERSECTION. IT WAS APPROX 30 FT LONG X 1 8 FT WIDE X 4 1/2 FT THICK. WET LAMINATED SHALE ROOF. NO EE OR EQUIPMENT WERE INVOLVED.
WHILE MAKING THE REQUIRED WEEKLY EXAM, THE FIRBOSS DISCOVERED A ROOF FALL IN THE RETURN AIR COURES, #1 ENTRY, 1 BK OUTBY SPAD #313. THE FALL WAS BETWEEN THE BLOCK IN THE ENTRY TO WAS APPROX. 3 0 FT LONG X 17 FT WIDE X 4 1/2FT THICK NO EQUIPMENT WERE INVOLVED.
EE SAID HE WAS THROWING MINE SLACK OUT OF ROADWAY & FELT PAIN IN HIS RIGHT SHOULDER. RESON FOR LATER REPORT: EE DID NOT INFORM MGMT. HE WAS OFF OVER AN INJURY UNTIL 7-13-01. 1ST L9OST WORK DAY 7-9-01.
