Accident type, without injuries

Unintentional roof fall discovered on this date by company and MSHA officials. Fall was located at North West Mains, #2 hdg, 1 block outby spad 4471, block 8-9. Fall measured 20'L X 7'H 18'W. Area previously supported with 6' full grout resin bolts. Area was mined on or about 01/20/2006. Overburden about 260'.

Fall onto or against objects

The employee was in an upright position walking down the mine pit access road, when he stepped on a piece of ice, causing him to slip. He was carrying a plastic dinner bucket and when he fell, his left chest area fell upon the dinner bucket, causing him to fracture the 5th and 6th ribs on the left side of his chest.

Unintentional roof fall discovered during weekly exam. Fall located in 5 zone, 8 entry, 31 block, station 4363. Fall measured approximately 20'W X 7'H X 30'L. Area previously supported with 6' full grout resin bolts. Area was mined on or about 3-29-06. No injuries or property damage.

The area was previously supported with 6' fully grouted resin bolts. The area was mined between 8/3/06 and 8/8/06. There is 340' of cover.

Unintentional roof fall discovered at 7:40AM. Fall measured approximately 20'L X 20'W X 10'H. Intersection was previously supported with 6' full grout resin bolts and 10' long cable bolts. Area was mined the week of 03/15/2007. At the time of this report, the 4 approaches to the fall will be cribbed off.

Unintentional roof fall discovered at above date and time. Fall measured approximately 4'H X 8'L X 12'W. Supported with 5' torque tension bolts. Fall located 1 block inby station 5100, #6 entry. Fall caused a hazardous condition in the intake escapeway and inby section workers were withdrawn.

Unintentional roof fall located on the Northwest Mains #2 Section, #6 entry, Station 5211. Fall measured approximately 18'L X 18'W X undetermined height. No injuries or equipment involved. Area previously supported with 6' fully grouted resin bolts and cable bolts. Overburden approximately 290'.

Unintentional roof fall discovered while checking dewatering pump. Fall measured 18'L X 18'W X 5'H. Area previously supported with 5' torque tension roof bolts. Area was mined 12/7/04. Fall was in 7 hdg, 3 belt, station 3479. Fall will be cleaned and supported.

Unintentional roof fall. Area was heard to be working prior to falling. Fall measured 40'L X 18'W X 10'H. Area was mined on11/14/06. Overburden is 360' in this area. Unintentional roof fall discovered at 5:50 AM. Fall measured 6' to 10'H X 19'W X 65'L. Originally supported with 6' full grout resin bolts in entry and 6' full grout and 10' cable bolts in intersection. #5 track between 36 & 37 block, Main Hdgs, to right of stations 4441 - 4448. Reported to C. Mosley at 5:55AM 11/13/06.

Struck by falling object

Injured was installing 5'W X 15'L with 4"" opening wire screen against the mine roof, with a roof bolter, when a small piece of shale fell from the roof and struck him above the right eye causing a laceration requiring 9 sutures to close. Injured was released to normal duties beginning 11/14/2006.

Unintentional roof fall discoverd on abandoned #1 Section. Fall measured approximately 70'L X 18'W X 10'H. Area previously supported with 6' resin grouted bolts and 10' cable bolts. Fall was from #1 entry to #2 entry just left of station 4981. Fall was discovered during examination of intake and return airways.

Over-exertion (Not Elsewhere Classified)

Employee was in a standing position operating a Joy 14-15 radio remote controlled continuous miner. The immediate roof began to fall between the roof bolts and the employee turned to move from the rock when his feet became lodged in the muddy bottom causing a strain to his lower back. Did not miss work until 11/9/06.

The injured employee was drilling a hole in the mine roof when a small/med piece of rock fell next to the rib, striking the injured on top of right shoulder & back of right leg.

Over-exertion in lifting objects

The injured was assisting hanging a miner cable in the #4 entry on the #1 section when he felt a ""pop"" & pain in his right shoulder.

The injured was installing a roof bolt into a drilled hole when bolt being installed slid against edge of hole causing small piece of slate to break away from mine roof striking injured on upper right lip. Injured returned to work same shift.

Injured was working at the #6 belt drive when he slipped in a thin layer of mud twisting his right knee. Did not become reportable until 6/11/06.

Injured was walking along #2 main belt conveyor when he stepped on uneven mine floor causing him to fall on left knee. This did not become LTA until 6-1-06.

INJURED WAS PULLING MINER CABLE AS C.M.O. WAS BACKING CONTINUOUS MINER OUT OF CUT WHEN A COAL RIB FELL STRIKING LOWER RIGHT BACK.

INJURED WAS WALKING AROUND END OF S.C., PUT HIS LEFT HAND AGAINST THE LOAD END OF THE CAR & A SHARP PIECE OF STEEL CAUSED A LACERATION TO HIS LEFT PALM.

THIS AREA WAS SUPPORTED WTH 5' TORQUE TENSION BOLTS, ROOF MAT & OVERSIZED BEARING PLATES (8"" X 8""). THIS AREA WAS DEVELOPED 7-29-04.

Injured was attempting to break loose a fitting on the valve bank of the fletcher roof bolter when the hydraulic fitting broke, causing him to strike his knuckle and middle finger on the valve bank.

While operating the Joy continuous miner on the 2 left section, in x-cut #3 & #2 entries, a piece of rock 36"" x 28"" x 10"" thick dislodged from the mine roof between the rib and rib bolts. The rock struck the employee on the right side of the head, middle and lower back and right foot. This rock had slick surfaces

WHILE WORKING AT THE FACE, EMPLOYEE TOLD CO-WORKER THAT HE FELT ""SOMETHING PULL"". HE FINISHED SHIFT AND TOLD FOREMAN THAT HE HAD SOME GROIN PAIN. ON WAY HOME, HE STOPPED AT HOSPITAL WAS EXAMIN ED AND INFROMED HE HAD A HERNIA. AFTER TAKING WITH EMPLOYEE IT WAS APPARENT THAT HERNIA RESULTED FROM PRIOR PULL AT SNOTHER MINE. PULLED AREA FINALLY FAILED WHILE WORKING A SPRUCE NO. 2 MINE.

AT 11:10 P.M. ON 1-17-00 A FALL WAS DISCOVERED IN 5 ENTRY AT 3 CROSSCUT OF THE NORTHEAST MAINS. IT WAS APPROX. 1 CROSSCUT INBY SPAD 6 AND 3 CROSSCUTS FROM THE SURFACE. IT MEASURED 6'HX18'WX35' L AND LOCKED THE INTAKE ESCAPEWAY. ESCAPEWAY WASREROUTED THROUGH A 5' HIGH X 8' WIDE DOOR IN #4 STOPPING AND CONTINUED OUT #4 ENTRY TO SURFACE. FALL WAS REPORTED BY PHONE TO STATE AND FEDERAL
