Unintentional roof fall discovered on this date by company and MSHA officials. Fall was located at North West Mains, #2 hdg, 1 block outby spad 4471, block 8-9. Fall measured 20'L X 7'H 18'W. Area previously supported with 6' full grout resin bolts. Area was mined on or about 01/20/2006. Overburden about 260'.
The employee was in an upright position walking down the mine pit access road, when he stepped on a piece of ice, causing him to slip. He was carrying a plastic dinner bucket and when he fell, his left chest area fell upon the dinner bucket, causing him to fracture the 5th and 6th ribs on the left side of his chest.

Injured was installing 5'W X 15'L with 4"" opening wire screen against the mine roof, with a roof bolter, when a small piece of shale fell from the roof and struck him above the right eye causing a laceration requiring 9 sutures to close. Injured was released to normal duties beginning 11/14/2006.

Employee was in a standing position operating a Joy 14-15 radio remote controlled continuous miner. The immediate roof began to fall between the roof bolts and the employee turned to move from the rock when his feet became lodged in the muddy bottom causing a strain to his lower back. Did not miss work until 11/9/06.

Injured was attempting to break loose a fitting on the valve bank of the fletcher roof bolter when the hydraulic fitting broke, causing him to strike his knuckle and middle finger on the valve bank.
While operating the Joy continuous miner on the 2 left section, in x-cut #3 & #2 entries, a piece of rock 36"" x 28"" x 10"" thick dislodged from the mine roof between the rib and rib bolts. The rock struck the employee on the right side of the head, middle and lower back and right foot. This rock had slick surfaces

EE backed miner out of face of #4 entry, shut off machine and walked towards a roof bolter parked in x cut #4 x #3 when rock dislodged from borw at edge of x cut of #4 entry. Rock struck EE on the neck area and upper back.

Employee was attempting to set a mine prop, when he bent over to get a cap wedge, the timber started to fall toward a co-worker. In an attempt to catch the timber employee twisted his left arm.

EMPLOYEE WAS SITTING IN A STOPPED MANTRIP WHEN ANOTHER MANTRIP FAILED TO STOP, SLIDING INTO THE STOPPED TRIP. THIS CAUSED THE EMPLOYEE TO STRIKE HIS HEAD AGAINST THE CANOPY OF THE MANTRIP, CAUSING PAIN IN HIS UPPER BACK & NECK AREA.

Fall was investigated by MSHA inspector at 7:10 AM 4-6-05, and by State inspector at 8:40 AM 4-6-05. The fall occurred in a 3-way intersection.

The injured employee was using a wrench to tighten bolts on the end of T&T pump. Wrench slipped off of bolt causing employee to fall back and pulled lower back muscle.

He felt this was due to an accident he had sometime the 1st week of Feb., as he was helping to install the no. 1 belt into the mine. He had belt structure at the time, felt a sharp pain in his groin and notified his supervisor of this on that day.

THE INDIVIDUAL WAS GETTING OUT OF THE MANTRIP, WHEN HIS FEET SLIPPED FROM UNDER HIM, CAUSING HIM TO HIT HIS LOWER BACK ON THE EDGE OF THE MANTRIP. THIS CAUSED HIM TO SPRAIN HIS LOWER BACK. IND IVIDUAL DID NOT HAVE TO BE OFF FROM WORK UNTIL 02/04/02.

AT 11:10 P.M. ON 1-17-00 A FALL WAS DISCOVERED IN 5 ENTRY AT 3 CROSSCUT OF THE NORTHEAST MAINS. IT WAS APPROX. 1 CROSSCUT INBY SPAD 6 AND 3 CROSSCUTS FROM THE SURFACE.
