EE was helping the continuous miner operator move the miner from the #5 entry to the #6 entry the miner operator stopped the miner to hook up a slack rope the miner operator hooked the rope to the boom of the miner went to turn around and stumbled and somehow hit the boom swing lever and somehow EE's hand was where the boom swung into the rib smashing his left hand.

Spot bolting went to move the lifeline out of the way and cut finger with roof bolt plate against the top.

EE was using his right foot to rake material onto the line curtain to tighten the line curtain and strained his right knee.

There has been a roof fall in the intake escape way. The fall has been dangered off in all directions and will not be cleaned up at this time.

There has been a fall located in the No.6 entry between break 39 and 40. This fall measured approximately 40' in length X 20' wide X 4' high. The escape way has not been affected. The area of the fall has been dangered and cribbed off. The nearest spad # is 2329.

The injured employee was attempting to tighten lead on battery and the battery blew up injuring right eye.

An unplanned roof fall was found 6 breaks inside the mine @ No.31 survey station in the No. 4 belt entry. The fall was found by Ricky Carter. This fall will be cleaned up and rehabilitated.

The injured was changing bits on miner and hit his thumb resulting in a laceration and a fracture to the left thumb.

The belt was in the process of coming to a complete stop, when the injured person reached toward the drive roller pulling him into the moving belt, causing him to strike an I beam and guarding around the belt head.

Employee had finished installing roof bolts and was lowering ATRS to advance bolter, when a piece of draw rock fell out between coal rib and first installed roof bolt striking employee on the right side of his body causing injury.

A roof fall occurred in the #6 entry, 80"" inby survey station #3422 measuring 40' long X 18' wide X 6' high.

The employee was installing last row of roof bolts, tripped and fell on corner of roof bolt plate, lacerating left arm.

An untentional roof fall has occurred in the 3rd East Main no. 3 entry outby 90 ft inby survey spad no. 2431. The fall measured 180 ft long, 18 ft wide, and 5 ft thick. The fall was above the anchor of installed roof bolts. The fall does not impair air movement, or impede travel in the area.

Injured employee was moving miner cable with his left foot and injured his left knee.

Employee walked into a shuttle car anchor bolt, which resulted in a small cut to the forehead.

A piece of rock rolled out of the rib striking the injured employee in the right lower leg.

Employee was putting scoop batteries on charge when a piece of draw rock fell striking his left hand.

Injured was servicing the miner when a piece of draw rock fell out striking him under the right eye. There was a small cut at the top of his cheek.

Rolling rock away from belts and injured back.

Taking miner cable down to pull slack when miner pulled slack the cable fell striking the injured in the left shoulder knocking him into rib.

Turning corner to haul up #2 when rib and corner fell out and into the deck of shuttle car, injuring operator's right knee and lower leg.

Putting up outside bolt and rib roll fell on right leg.

Rock fell and hit left hand against pot.

Miner's hand was on drill pot when right middle finger was caught between uneven top and drill pot which cut end of finger off between finger nail and first joint closest to fingernail.

While loading tools on Mac 8, the Mac 8 started to roll and ee hit left elbow on frame of Mac 8.

An unplanned roof fall occurred in the return at spad No. 451. The fall measured 45' long x 18' wide x 6' thick.

Cutting bands off bundle of roof bolts. Band flipped back and cut right wrist.

Employee was installing belt hanger, a piece of drill steel fell out of the pot & hit him in the mouth.

An unplanned roof fall occured 20' Inby spad No. 1351 in the #3 Entry. The fall measured 70' long, 20' wide and 4' to 6' thick.

An unplanned roof fall occurred 1 break inby spad# 1346 in the crosscut between #5 & #6 entry. The fall measured 70'in length & 30'wide & 5-15' thick.

An unplanned roof fall occurred 2 breaks outby spad# 1350. The fall measured 18'W X 60'L X 4'T. In center fall was a horses back.

Loading in left hand lift in #7, a piece of rock fell out and hit EE in the lower back.

An unplanned roof fall occurred on #2 section, entry #4 at spad 2135. The fall was approx. 2900' from the working face. The fall measured 30'long, 20'wide and 3'-6'thick. The fall was called in on Saturday, 6/24/06 & left a message on an answering machine.

An unplanned roof fall occurred outby #2 section 2400' at spad no. 2134 in the #3 entry. The fall measured 30' wide x 30' long x 5' thick.

Loading hi line in car and got rt knee under wheel.

An unplanned roof fall occurred at the #6 set of seals at seal #7. The fall measured 20' wide, 40' long and 5' thick.

Injured was waiting on other shuttle car to go dump so he could go to the miner and load. A piece of rock fell and hit him in the right shoulder and right foot.

Moving miner back and went to move cable and felt pain in upper back.

EE was bolting top in #6 heading on #2 section, drill steel fell out of hole and landed on his left middle finger and ring finger.

AN UNPLANNED ROOF FALL OCCURRED IN #3 ENTRY, 1 BREAK INBY SPAD NO.0648. THE FALL MEASURED 18'WIDE X 70'LONG X 6'THICK. AREA WAS DEVELOPED 05/22/03.

An unintentional roof fall occurred 1 break outby spad #2130. The fall measured 18' wide, 20' long and 6' thick.

An unplanned roof fall occurred on the Rt. return two crosscuts inby spad No. 2007 in the #7 entry where No. 3 north mains started. The fall measures 30' in length, 20' wide and 4' high.

EMPLOYEE WAS TRAVELING THROUGH THE PRIMARY ESCAPWAY ON INTAKE, A PIECE OF ROCK FELL BETWEEN ROOF BOLTS & HIT HIS RIGHT WRIST.

Removing drill steel from hole, got right forearm between boom and roof.

Cutting rockdust hose with utility knife and cut lt hand.

Pulling miner cable out of roadway and hurt back.

EE WAS LOADING SUPPLIES INTO SCOOP BUCKET AND TWISTED LOWER BACK.

EMPLOYEE WAS GETTING ROCK OFF OF THE MINER AND CAUGHT HIS HAND BETWEEN 2 ROCKS.

Injured was hooking up miner cable to 3-wheeler with rope hanger. Said foot hit tram pedal before he could get fingers out of rope hanger.

A roof fall occurred 1 break inby spad #1896 in the #4 entry. Fall measured 60' long, 18' wide, 0-8 foot thick.

Cleaning on intake-a peice of rock fell and hit shoulder & neck.

A ROOF OCCURRED 5 BREAKS INBY SPAD NO 1573 IN THE #5 ENTRY ABOUT BREAK 37. THE FALL MEASURED 50' LONG, 14' WIDE, 36"" THICK. IT PULLED 2 MIDDLE ROWS OF BOLTS.

Employee was loading blocks on flat car and twisted lt knee.

A piece of rock 9' long x 5' wide from 0-18"" thick fell from roof bolt to rib striking victim working on roof bolter.

ROOF FALL 3 BRKS OUTBY SPAD #335 ON INTAKE. FALL MEASURED APPROX 20' X 20' X 48"" THICK

OPERATING SHUTTLE CAR, RUNNING UNDER CABLE BOLTS. HIS BIBS CAUGHT A BOLT HANGING DOWN AND DRUG HIM UP ON FENDER OF SHUTTLE CAR (SPRAINED NECK).

Working on miner, rub rail came off and mashed left index finger.

PUT DRILL STEEL AGAINST TOP, PIECE OF ROCK FELL AND CAUGHT HIS THUMB ON POT.

Hanging cable and pulled knee. He did not cease work at that time. he worker for 4 consecutive days. went to emergency room 4-24-05. dr's excuse until 5-9-05.

EE WAS INJURED WHILE TURNING BOLT WITH RATCHET (PULLED & TWISTED SPINE).

A roof fall occurred at spad #1756 toward #1 section. Fall measures 20' wide x 15' long x 40"" high. area was cribbed off.

Roof fall occurred 8 crosscuts below ep2 no. 1 and no. 2 entry blocked. cribbed to be cleaned and supported.

A roof fall occurred at spad #348 to spad #351 blocking travelway to 2 left panel on @2 belt. fall meausred 100' long 20' wide x 5' high. cribbed off to be cleaned and supported.

A roof fall occurred at the #1 set of scals blocking the #3 and #4 seal. Fall measured 80'l x 20'w x 34' h. cribbed off-to clean up and supported.

EE WAS WELDING ON MINER REPORTED HE WENT HOME AND SHOWERED ADN HIS EYES BEGAN TO BURN.

LIFTING SHUTTLE CAR TIRE HE HURT HIS BACK.

LOADING COAL WITH MINER PIECE OF DRAW ROCK FELL FROM BETWEEN PINS HITTING HIM IN HEAD & RIGHT SHOULDER.

EE WAS CHANGING BITS IN MINER AND BUMPED ELBOW AGAINST MINER BIT - BRUISED RIGHT ELBOW.

ROCK FELL OUT INJURING SHOULDER.

STARTED TO BACK UP 3 WHEELER, 3 WHEELER GOT STUCK IN TRAM - RAN UPON GOB CAUSING HEAD TO HIT ROOF.

A PIECE OF ROCK BROKE OFF FROM THE RIB TO THE FIRST BOLT. EE RECEIVED A STIFF NECK.

PUTTING PINNER CABLE IN STEEL HANGER AND CABLE WAS PULLED & HANGER HOOKED HIS RT. INDEX FINGER.

SETTING BITS & CLEANING SCRUBBER WHEN ROCK FELL OFF & STRUCK EMPLOYEE IN HIP.

EE SAID HE WAS DIPPING UP WATERY MATERIAL BEHIND #2 BELT DRIVE AND NOTICED HIS BACK STARTED HURTING HIM. (INJURY IS QUESTIONABLE).

WENT TO ANSWER THE SECTION PHONE AND PICKED UP A PIECE OF ROCK AND THREW IT OUT OF WAY.

EE WAS OPERATING CONTINUOUS MINER IN #4 HEADING WHEN PIECE OF ROCK FELL STRIKING HIM BTWN SHOULDERS & LOWER BACK. HE WAS ABOUT 40' OUTBY. ROCK FELL OUT BETWEEN RIB (RIGHT RIB) & RIB BOLTS. MAY HAVE STRAINED MUSCLES IN NECK & LOWER BACK, ACC

HE WAS PULLING ON THE MINER CABLE AND SAID HE FELT SHARP PAIN IN HIS GROIN AREA. THIS WAS NOT REPORTED WITH 2-5-02.
