EE was helping the continuous miner operator move the miner from the #5 entry to the #6 entry the miner operator stopped the miner to hook up a slack rope the miner operator hooked the rope to the boom of the miner went to turn around and stumbled and somehow hit the boom swing lever and somehow EE's hand was where the boom swung into the rib smashing his left hand.
Spot bolting went to move the lifeline out of the way and cut finger with roof bolt plate against the top.
EE was using his right foot to rake material onto the line curtain to tighten the line curtain and strained his right knee.
There has been a roof fall in the intake escape way. The fall has been dangered off in all directions and will not be cleaned up at this time.

There has been a fall located in the No.6 entry between break 39 and 40. This fall measured approximately 40' in length X 20' wide X 4' high. The escape way has not been affected. The area of the fall has been dangered and cribbed off. The nearest spad # is 2329.
The injured employee was attempting to tighten lead on battery and the battery blew up injuring right eye.
An unplanned roof fall was found 6 breaks inside the mine @ No.31 survey station in the No. 4 belt entry. The fall was found by Ricky Carter. This fall will be cleaned up and rehabilitated.
The injured was changing bits on miner and hit his thumb resulting in a laceration and a fracture to the left thumb.

The belt was in the process of coming to a complete stop, when the injured person reached toward the drive roller pulling him into the moving belt, causing him to strike an I beam and guarding around the belt head.
Employee had finished installing roof bolts and was lowering ATRS to advance bolter, when a piece of draw rock fell out between coal rib and first installed roof bolt striking employee on the right side of his body causing injury.

The employee was installing last row of roof bolts, tripped and fell on corner of roof bolt plate, lacerating left arm.

An untentional roof fall has occurred in the 3rd East Main no. 3 entry outby 90 ft inby survey spad no. 2431. The fall measured 180 ft long, 18 ft wide, and 5 ft thick. The fall was above the anchor of installed roof bolts. The fall does not impair air movement, or impede travel in the area.
Injured employee was moving miner cable with his left foot and injured his left knee.
Employee walked into a shuttle car anchor bolt, which resulted in a small cut to the forehead.
A piece of rock rolled out of the rib striking the injured employee in the right lower leg.
Employee was putting scoop batteries on charge when a piece of draw rock fell striking his left hand.

Injured was servicing the miner when a piece of draw rock fell out striking him under the right eye. There was a small cut at the top of his cheek.
Rolling rock away from belts and injured back.
Taking miner cable down to pull slack when miner pulled slack the cable fell striking the injured in the left shoulder knocking him into rib.
Turning corner to haul up #2 when rib and corner fell out and into the deck of shuttle car, injuring operator's right knee and lower leg.
Putting up outside bolt and rib roll fell on right leg.
Miner's hand was on drill pot when right middle finger was caught between uneven top and drill pot which cut end of finger off between finger nail and first joint closest to fingernail.
While loading tools on Mac 8, the Mac 8 started to roll and ee hit left elbow on frame of Mac 8.

EMPLOYEE WAS GETTING ROCK OFF OF THE MINER AND CAUGHT HIS HAND BETWEEN 2 ROCKS.
Injured was hooking up miner cable to 3-wheeler with rope hanger. Said foot hit tram pedal before he could get fingers out of rope hanger.

Employee was loading blocks on flat car and twisted lt knee.

Working on miner, rub rail came off and mashed left index finger.
PUT DRILL STEEL AGAINST TOP, PIECE OF ROCK FELL AND CAUGHT HIS THUMB ON POT.

HE WAS PULLING ON THE MINER CABLE AND SAID HE FELT SHARP PAIN IN HIS GROIN AREA. THIS WAS NOT REPORTED WITH 2-5-02.
