Member was operating the offside of the right roof bolter in 5 right. He went to remove his drill and a piece of draw rock fell. The rock hit him on his right arm, and bounced and hit him in the chin causing a laceration.

The crew had loaded the mantrip and started outside. The mantrip hit a jack that had been unloaded beside the track. The jack came around and hit employee in the left forearm causing a laceration.

An unplanned roof fall was discovered app. 1,200 ft. from the active section 1 x-cut inby S.S.#6088. The fall measured app. 32' in length X 19' in width X6' in height. The area was bolted with 6' torque tension bolts. The area is dangered off and will not be rehabilitated. MSHA inspector investigated the fall.

Member was getting out of a scoop and had his right hand on the corner of the canopy, when a piece of draw rock fell, stiking him on the right hand.

On 4-18-08 EE ceased work & went to ER w/back pain. Referred to chiropractor, he was dx'd w/a simple spasm & released to return to work. EE was placed in a different position soon after. He later went to a chiropractor & was sent for MRI on 5-4-08. Results were interpreted as degenerative. He worked till 6-9-08. At that point he had a dr slip stating unable to perform job

EE stated that a piece of rock fell from the rib and hit his right leg in the calf area causing a contusion and a sprain to his ankle. He took off and has not called or provided any dr. reports as to a diongnosis. This report is being filed as a reportable injury with no days lost till a determination can be made as to the reason for injured being absent.

Member stated that while bolting top he thought he got something in his eyes. He finished his shift and went to the eye doctor on Monday, 3/24/08. The doctor washed his eyes out and prescribed him anti-biotic drops.

A unplanned roof fall occurred in the # 4 entry on the 001 section at S.S. # 5694. The fall was 1250 ft. from the face. The fall will be rehabilitated and the size was 30ft in length X 19ft. in width and app. 10 ft. in height. The area was inspected by M.S.H.A inspectors and State.

EE was installing a jack on a roof bolter and while taking off a hydraulic fitting his hand slipped and he stated he hit something sharp causing a laceration to his right index finger. He recieved 4 stiches.

EE contacted mine management stating he was going to the Dr to have his hand checked. He stated he thought he had gotten a splinter in his hand a few days before. EE reported to management on 11/9/07 that he was in the hospital and had a foreign object removed and was staying in the hospital overnight for observation on which meds work best on injury.

Employee stated he was using the boom of the bolter to push bolt into a hole and his thumb got caught between the bolt head and the boom causing a crushing injury to his left thumb.

Employee was removing a guard from a Joy continuous miner and while he was laying the guard on the ground his glove got caught in the handle causing him to fracture his right ring finger and middle finger.

Employee was on his knees in a scoop bucket throwing crib blocks from the scoop to the back of the feeder. Employee ceased work and requested an ambulance.

Member was operating a continuous miner in the 4 left crosscut when a rib roll measuring 9' long x 42"" wide x 1"" to 5"" thick struck him resulting in a fractured right foot.

Fall onto or against objects

Member was getting on mantrip on the surface and triped over the track switch that was covered with water injuring his left knee. 1st day away from work 11/2/06.

An unplanned roof fall was discovered at 34 cross cuts inby the portal on the #1 belt. This mine is in non producing status. Time of fall is unknown. Fall measured approximately 25' long x 30' wide x 10' high.

Employee was making a belt move up. The scoop operator was pulling the tailpiece up. One of the chains slipped causing the tailpiece to go side-ways catching employee between the rib and the tailpiece.

He was installing a roof bolt when a piece of rock fell striking his left arm resulting in a laceration requiring stitches. Employee continued working until 8/16/06 and was taken off work.

EE was operating a shuttle car traveling toward the miner. The shuttle car struck a hole in roadway causing his right arm to hit the hand hold on the canopy injuring his right elbow.

A ROOF FALL OCCURRED AT LOCATION #3 ENTRY 1 BREAK INBY SPAD 3772 WITH AN INACTIVE FACE AREA. THE FALL WAS 65' X 19' X 8' TO 10' IN SIZE. SUPPORT WAS 4' RESIN BOLTS IN MIXED STRATA WITH NO RIDER SEAM. THERE WAS NO PERSONAL INJURY OR MATERIAL DAMAGE. THIS AREA WILL BE BREAKERED OFF AND NOT REHABILITATED.

A roof fall occurred at Diamond Energy possibly the day of 5/17/2004 or before. The time is unknown. This fall occurred in the area of spad 2385 in an entry between headings 3 and 4. and measured 16' x 18' x 6'. and was originally secured with 8' cable bolts. This area is being rehabilitated as part of it is in the track entry area.

EE WAS MOVING A SCOOP CHARGER & A CABLE. HE PUT IT ACROSS THE BELT (ON THE BELT) INSTEAD OF HANGING IT ACROSS (OVER) THE BELT WHICH HE SHOULD HAVE DONE. THE CREW STARTED THE BELT. THIS ACTION PULLED THE CABLE & PULLED HIM TO THE GROUND ON HIS RIGHT ELBOW RUPTURING HIS BURSA IN THE ELBOW.

EE WAS IN THE PROCESS OF LIFTING A BELT ROLLER. HIS INTENTIONS WERE TOGET IT TO THE OTHER SIDE OF THE BELT WHERE THE TRACK IS SO THAT IT COULD BE TRANSPORTED ON A FLAT CAR. HE LIFTED THE ROLLE R TO HIS WAIST LEVEL AND EXPERIENCED LOWER BACK PAIN. HE WORKED 3 MORE SHIFTS IN THE WEEK AND BEGAN MISSING WORK THE FOLLOWING WEEK. THE SAFETY DEPT WAS NOT INFORMED OF THIS UNTIL 11-07-02.

IN THE EVENT THAT HE DOES NOT RECEIVE ANY BENEFITS DUE TO INELIGIBILITY, AN AMENDMENT WILL BE SUBMITTED. (NOTE: RECEIVED IN THE SAFETY DEPT. 9/19/02).

EE WAS GOING TO SECTION FEEDER IN A LOADED SHUTTLE CAR WHEN HE HIT SOME BAD BOTTOM IN MINE FLOOR. HE SAID IT CAUSED HIM TO BE EJECTED OUT OF THE 21SC ONTO MINE FLOOR.

THE CONTINUOUS MINER WAS DOWN WITH NO WATER AT THIS TIME. A PIECE OF ROCK MEASURING 18""X 12""X 1 1/2"" FELL FROM BETWEEN ROOF BOLTS. IT STRUCK EE ON THE SMALL OF HIS BACK. EE IS PRESENTLY STI LL NOT WORKING SINCE THE DAY OF THE ACCIDENT.

EE WAS INSERTING A PIN BACK INTO THE RUB RAIL OF THE CONTINUOUS MINER. THE REMOTE CORD WAS BEHIND THE RAIL, SO A CO-WORKER MOVED THE RAIL TO REMOVE THE CORD. THE RAIL FELL ON HIS LEFT FOOT AND BROKE HIS TOE.

A BOLT FELL BEHIND THE SHIELD & VICTIM TRIED TO RETRIEVE IT WHILE AT THE SAME TIME HIS CO-WORKER PUSHED DOWN ON THE SHIELD & IT FELL ON VICTIM'S RIGHT INDEX & MIDDLE FINGERS. (NOTE: THE SAFETY DEPT. WAS NOT INFORMED UNTIL 2/27/02 THAT HIS INDEX FINGER HAD BEEN BROKEN).

EE WAS BUILDING A STOPPING. WHILE BUSTING A CAP BLOCK, A CEMENT BLOCK FELL OUT OF THE PILE ONTO THE BUCK OF HIS HAND. HIS HAND WAS BRUISED THROUGH THE METACARPAL GLOVES HE WAS WEARING.

IT WAS BOLTED WITH 4'RESIN B OLTS AND WAS CHECKED BEFORE THE MINER MAN ENTERED. EE'S POSITION WAS THE 3RD ROW OF BOLTS OUTBY THE CROSSCUT.
