Injured employee while Pre-Op his shuttle car struck a sharp object and received a laceration to upper right arm and received 4 stitches.

Over-exertion in lifting objects

Injured member was lifting supplemental roof support timbers when he twisted his left knee. He did not cease work and continued to work his regularly scheduled shifts until May 25, 2011. Due to continued discomfort, injured was evaluated and excused from work by his doctor and awaiting an MRI on his knee.

Struck against stationary object

While walking to the part sled, he twisted his right ankle on a rock, resulting in swelling to the ankle.

Over-exertion in wielding or throwing objects

Injured was shoveling the off side of the belt near the rock box when he began to experience lower back pain. He finished his shift but later went for medical evaluation and was diagnosed with a lower back strain and he was excused from work.

Fall to the walkway or working surface

Injured member was carrying a feeder jack and as he was stepping over the feeder cable his knee twisted causing him to fall to the ground. He completed his shift and the following shift. On 03/08 he went to the ER and was told by the physician that he could possibly have a Meniscus tear and scheduled an MRI and excused him from work.

Caught in, under or between a moving and a stationary object

Injured employee was placing a crib block under the continuous miner cats when the operator raised the miner head and smashed the injured's left hand between the cats and block. Diagnosis: Fractured left little finger and 4 sutures on the posterior side of the left hand.

Injured employee was hanging a curtain when his arm contacted a roof bolt plate and caused a laceration. Injured received 4 sutures.

Injured was hanging a curtain when he lacerated a finger on his right hand on the curtain hook. Injured received 2 sutures.

Fall onto or against objects

Injured was assisting the scoop operator by placing the scoop on charge when he slipped and fell. Injured struck his left knee against a rock on the mine floor causing pain and swelling. He was transported to the ER. The ER physician excused from work and remains excused from work due to increased swelling and edema in the knee.

He was removing fly pads to move up to the next line. He cut the back of his left hand on a pizza pan, requiring 5 stiches.

Injured employee was assisting in the installation of a belt head. While sliding the head on the grid using a scoop, the head lunged forward and the employee turned to run. He struck his right leg against an I-beam. He continued to work his shifts until 03-15-10 when he received an MRI. On 03-22-10 MRI results; fractured fibula and a torn meniscus. Employee placed on light duty.

Member was picking up a 6x6 timber and felt pain in his back, MRI on 3-8-2010 reported a strain muscle and was placed on restricted duty.

While preforming a preshift exam of the mine a roof fall was observed at the #18 cross cut track entry along the #2 main belt. No injuries occurred from this fall.The fall measured about 25 feet long,20 feet wide and 4 feet high, This area was bolted with 4 foot resin bolts

Member states he was lifting belt structure and felt a pain in his back, he worked until 3--13-2010 when he went to doctor and was removed from work.

A roof fall was discovered in the #1 Entry approximately 80' inby spad 172 at the Cook Portal. The fall measured approximately 45 feet long x 19 feet wide x 6 feet high. No injuries resulted from this fall.

A roof fall was discovered in the #2 Entry: Inby sapd #250 at the Cook Portal. The fall measured approx. 16' long by 18' wide by 4' high. No injuries resulted from this fall.

Struck by falling object

Injured employee was unloading sand jacks from the motor when he struck the roof with one of the jacks breaking loose a rock which stuck him in the corner of his left eye lacerating his eye lid. He received 3 sutures. The rock measured approximately 6""x6""x2"".

A roof fall was discovered in Entry #4 (Cook Portal), the fall measured approxmately 80ft long by 20ft wide by 10ft high. No injuries resulted from this fall.

A roof fall was discovered in the number 3 entry, number 2 belt at crosscut 40. The fall measured approximately 18' wide by 30' long by 8' high. No injuries resulted from this fall.

Struck by falling object

Injured employee was walking the roadways prior to the start of his shift when a piece of draw rock approx. 26""x48""x1-3"" fell from between the roof bolts and struck the left top of his head and continued down the left side of his torso. He was airlifted to the ER and admitted to the ICU. ER Diagnosis: 4 Fractured vertebrae, small bleed on the brain and injuries to his left ribs.

A roof fall occurred in the return entry located at # 17 crosscut along the # 2 belt line No injuries from this fall. The fall measured about 18 ft wide 18 ft long and 6 ft high.

Struck against a moving object

Injured employee was riding the mantrip to the surface when the mantrip derailed. Employee struck against the inside of the mantrip injuring his left ribs causing bruising and a hematoma on his left rib cage. He went to the ER later that evening and the X-Rays were negative for fractures. He was excused from work until further notice.

Injured employee was shoveling the 4C tailpiece when he began to experience lower back pain. He was transported to the ER by EMS where he received a CT Scan and referral for an MRI. MRI revealed inflammation and swelling causing pressing against the sciatic nerve causing lower back and left leg pain. He remains excused from work.

Injured employee was using a belt knife to cut the belt. The knife slipped, cutting through his glove and causing a laceration to the tip of his left thumb. He received 4 sutures.

A roof fall was discovered in the left return entry at cross cut number 36. The fall measured about 30 ft long, 20 ft wide and 6 ft high, This area was dangered off and no clean up at this time. No injury from this fall

Injured employee was operating the roof bolter and installing belt hangers. He had ahold of the bolt wench when his glove became entangled in the wench and injured his left ring finger. He was transported to the ER by EMS and was diagnosed with a dislocation and lacertion. The dislocation was reset and the laceration sutured. Returned to work on restricted duty.

A roof fall occurred in the track entry located along the # 2 belt inby spad # 251 at the # 19 crosscut. The fall measured about 16ft wide, 10ft long and 5ft high. No injuries resulted in the fall.

Security guard was traveling up the hill to Allegiance mine bench. The transmission was slipping. He parked it and start to walk to the mine office. He heard a popping noise turn around and saw the explorer on fire.

A roof fall ocurred in the intersection of the 006section,The fall measured about 19 feet wide 19 long and about 8 feet high.

Flash burns (electric)

The injured employee, an outby electrician, was changing a blown fuse using a pair of channel locks. As the fuse contacted the connections it arced and blew, resulting in burns to the posterior side of both hands. He did not cease work at that time but later went to the ER and was diagnosed with first degree burns and has been placed on restricted duty.

A roof fall occurred on the #1 section, entry #6, spad # 372 of the Allegiance Mine Cook Portal. The fall measured 20ft long, 20ft wide and 6 ft high. No injuries resulted from this fall.

Injured was helping change tail roller bearing. He was using a slate bar to pry back on the roller when it slipped and struck him on his right ear causing a laceration. EMS transported him the the ER where he received 7 sutures in his right ear. He returned to work on his next regularly scheduled shift.

Injured bent over to sapd curtain against rib when a timber fell and struck him on the head and back. He was transported to the ER by EMS.

Injured was stocking his tray on the roof bolter when a rock approx. 41''x31''x4'' fell out of the roof and struck his left hand causing a laceration. Injured was taken to the ER by EMS. The laceration was sutured at the ER.

Over-exertion in lifting objects

Injured bent over to pick up a bundle of stopping panels when he injured his back. He had an MRI on 04/24/2009 and is awaiting results.

Injured was helping move the tool sled with a chain hooked to the scoop and the sled. When the chain tightened up, a bolt broke off of the chain and struck the injured above the right eye. The injury required 4 sutures.

Using hammer to remove a belt pin, while striking with hammer a piece of metal flew up and struck him on the right cheek requiring 5 stitches.

He was cutting a zip tie loose using a utility knife. He slipped and cut left hand requiring 4 stitches. He completed his shift and did not report this to his supervisor until he returned to work the next day.

INJURED WAS OPERATING MINER ON SECTION IN #2 LEFT BREAK. A ROCK MEASURING 0-3"" THICK X 18"" LONG X 14"" WIDE FELL FROM TOP STRIKING HIM ON HEAD & NECK. HE WAS TREATED BY CREW & TAKEN TO CAMC-GEN ERAL.
