Employee, and six other employees, was traveling from the west mains section to the surface at end of their shift on a ten person diesel mantrip when a 50-ton roof jack struck the employee on the right side of the head. Employee was immediately transported to the surface and flown to Ruby Memorial Hospital. Employee passed away on the evening of 6/5/18.

Allegedly the employee stated EE was walking backwards while hand dusting a beltline and later felt a pain in EE's left knee. Upon investigation and review of EE's medical history the employee has suffered repeated trauma to this knee over EE's career, along with a previous surgery to this knee in 2002.

1B Panel intake BK 21 #6 entry roof fall approx. 80'L x 20'W x 10'H. Was not in the escapeway entry. 5' fully grouted bolts, also timbered on all four sides.
Apprentice Electrician was setting power up to S/C cathead, when the cathead blew up causing flash burn to eyes.
Mine Foreman was in the south 1 intake Bk33 to BK 34 and discovered a roof fall in the primary escapeway. Approx. 30' long x 16' wide x 5' thick at deepest point.
West section BK 53 509 scoop charger 2B belt Between #3 & 4 entry. Employee was setting up the fire ext. suppression tank back up on the charger and lost control of the tank dropping it on EE's left middle finger tip causing finger nail to be torn off. Went to ER and had a small tuft fracture and nail avulsion.
Employee was in #6 entry went to follow center dump across to put scoop on charge. The scoop became stuck in the rib when making turn and EE said side dump shuttle car traveling up #6 struck the operators compartment of the scoop hitting employee in left shoulder.
Operator was drilling hole and was back spinning the drill steel while EE was bringing the steel back out of the hole. When the pusher steel came out of the top, the steel spun around striking EE in the mouth. Causing a small laceration to EE's right upper lip.
Employee was operating #2 roof bolter while trying to move rock out of the way to set outside bolt employee had head down looking at the rock to be able to move the drill pod. Employee had left hand on the canopy and the operator on the helper side swung boom in to set the inside bolt causing movement of the machine pinching left hand between rib and canopy.
Employee was at 6 BK on Old 1-C Panel lacing 1000' of belt. When EE reached down to pick up the belt clamps, EE felt sharp pain in their back.
Roof bolter operator spun the machine to take it up #3 entry. The machine struck the scoop operator's left foot causing the employee to fall to the ground and pinned employee's foot & ankle under the roof bolt machine. Employee had swelling and a laceration to the inside of left leg just above the ankle bone.
West section 003-MMU unplanned roof fall at or above anchorage zone in active workings 6' to 7' high for length of 42' between 52 & 53 Block
Was bolting #8 face when the drill steel stopped up. EE took the bottom piece of steel out of the drill chuck and the top piece of steel fell out of the hole then bounced off the drill pot hitting EE in the mouth breaking EE's tooth.

1C panel 002 MMU BK33 8R second cut fell in after miner finished mining. When the top fell it road back through the last row with deep cut bolts in it.
Employee was bolting second cut out of #5 entry, first row outside bolt. Piece of rock 3'L x 2W'x 5 1/2""T fell and hit canopy, then hit the employee on the outside of left ankle. Causing puncture to boot and ankle. Small cut and swelling to Left ankle area.
Roof Fall old 1B panel #9 entry BK 20 in the return air course. Was bolted with 5' 3/4"" fully grouted bolts. Fall was approx. 17' high 28' long 18'wide. Fall was dangered off from all entries.
Employee was cutting height for overcast and a piece of the miner bit came off and hit EE in the left bicep, causing laceration to EE's arm.
Driver lost control of loaded coal truck going down hill at the 9 post causing truck to wreck at the bottom at the 8 post. Driver went to the hospital and received stitches to left ear. Returned to work next day.
Employee was attempting to remove rock from boom/dump end of shuttle car. The rock slid pinching right pinky finger between the rock and the metal of the boom. Causing cut and break to top knuckle of pinky finger.
Roof fall 1B belt BK 6 in track entry. Fall approx. 18' high. Also towards #3 entry in Bk 6.

South Mains 1B panel BK 11 1/4 to 12 3/4. Cribbed and dangered off are.
South Mains 1B Neutral #3 entry BK 13 1/2 to 14 1/2. Cribbed and dangered off area.
West Mains 2B mains BK 40 #3 entry Roof fall in return, ventilation still good but no travel in this area.
Employee was taking drill steel out of the hole they drilled, rock fell and hit left hand pushing it against the drill pod. Causing laceration to left index finger.
Airway person found the fall on EE's examination of the airway. It's on old 1B panel in #2 entry BK 53 1/2 39' outby spade #3654. 15' high and about 40' long. It was also bolted with 4' bolts.
While cleaning out wet duster on scoop, the employee got water hose to spray it out and when they turned it on it slipped out of hand. The nozzle then flew back up striking the left upper lip. Cutting lip.
Old West 2 Intake Bk 16 #8, Mine foreman was inspecting discharge line and found that roof fell in the intersection on top of discharge line.
Fall was found in right return #8 entry at BK 44 on south 5 panel. 13 Breaks outby section tailpiece.
On 6-3-20 employee received 9.9% PPD award for hearing loss. The employee still works at this operation.
Employee was placing belt clamp on belt when clamp slipped and flipped up, striking EE in EE's mouth.
Roof fall 4 south #3 entry BK 33 1/2 to 34 1/2 in the neutral. Did not affect ventilation or travel.
While miner was in #7 face, the miner operator reached to get wash down hose to spray off miner. Was struck in right hand with a roof rock, causing laceration to top of hand that required stitches.
Employee was changing scoop batteries on scoop. As employee went to shut down the lids on the batteries, employee dropped the lid down on right hand.
While traveling airway the examiner came across a roof fall on 5 south left return. Break 7 1/2 to 9 1/2 in #2 entry. Did not bother ventilation or travel. No one was injured.
Said EE slipped on Rock and fell on EE's right side. When EE landed EE hit EE's ribs. Pain in EE's chest and right ribs. Was having trouble breathing.
Employee was drilling EE's hole with starter steel, went to add EE's pusher steel to finish the hole, couldn't get the steel in EE's chuck. Went to swing EE's drill pod to get steel in chuck, EE swung the drill pod the wrong way smashing EE's hand between the steel and the canopy of the bolt machine.

Long-term exposure to dust.
Roof fall was located at 31 block on south 1 belt line in the intake #6 entry and fell approx. 60' long x 18' wide x 8' high. The cause is believed to be a geological anomaly
Driver went to step out of the truck and slipped off the bottom step.
Employee was checking antifreeze and as employee was taking radiator cap off it blew off from pressure causing the liquid to burn employee's right hand, right side and right hip. 1st & 2nd degree burns.
Employee checked the top before bolting and noticed a loose rock and tried to pull it. When the rock would not come down, EE drilled the hole for the bolt and when EE inserted the bolt into the hole and went to push the bolt up into the roof, the rock fell and hit EE's left pinky finger causing a laceration.
Roof Fall on West section at 59 blk, #3 entry. No injuries occurred.
Employee had just arrived on west section and was down on knees waiting to begin work, when employee stood up employee's body twisted and employee fell to right side, which resulted in a broken fibula in right leg.
Employee was making pre-shift run and struck the back of their neck against a belt hanger (monkey face). This resulted in the employee receiving 5 stitches at the hospital.
Employee was loading a timber on the ride, the timber started to roll off the back side of ride and EE reached out with right hand to try to catch it. This resulted in a fracture of EE's middle finger on the right hand.
Employee was installing a fly board with the bolter, drilled first 3 feet and was removing drill steel when the fly board fell off of canopy and hit pinky finger between the drill and board that fell.
Employee was helping load miner head onto low boy underground. Employee was in the process of centering the miner head up on the low boy. Employee reached under the miner head to get a chain and the crib block rolled over and struck employee's hand.
Employee was installing boar in the face of #3 entry on south mains. Employee was pushing the 5' bolt up into the hole with the pusher, and the head of the bolt slid off the pusher causing the stored energy from the bolt to make contact with the palm of their left hand resulting in a broken left wrist.

Driver stepped up onto tire to clean windshield and slipped & fell breaking wrist.
Employee was trying to repair the stab shaft on the South Mains Feeder. EE was attempting to pry the sleeve off the stab shaft. The slate bar slipped causing ee to slip and twist right leg.
Employee was skinning a cable in the outby area of South Mains when the knife slipped and cut employee's left thumb.
Truck driver was traveling Ball Hooter Hill when they saw 2 vehicles that were pulled over to side of road. Went to gear down, got truck out of gear, tried to put back in gear but didn't know what gear it was. Tried to use brakes but they didn't work. Decided to put truck in the ditch line to stop it. Truck turned over on its passenger side.
Roof fall in the left return area of 1 section between 29 and 30 block. Also roof fall in the right return,#9 entry between 34 and 35 block. The cause of the fall was adverse geological conditions.
This injury occurred back on 7-23-18. The employee later had complications because of the incident and was scheduled for knee surgery on 12-21-2018. The initial incident occurred on #2 beltline, where the employee stated EE slipped on slick bottom and felt a pain on the inside of EE's right knee.
Rock fall was found at 2:45 PM outby our #1 section at 33 block between 8 and 9 entry. The cause of the fall was adverse geological conditions.
Employee was helping lap belt, they had pulled one pull of 52 feet. They started to pull the second pull and the belt was frozen and when it broke loose the stored energy resulted in the belt striking the employee in the left knee.

A fire was discovered in the office change room by dispatcher at approximately 9:00 PM Saturday Nov. 18 2017. The location of fire was in storage room where the IWT tracking and communications components were stored. The local fire department extinguished fire when they arrived. The fire remains under investigations by MSHA, State and company officials.
