Employee was operating a rock truck on 09/05/2025 and felt pain in lower back after hitting a pothole. EE went to the Emergency Room on 10/05/2025 and the CT Scan showed bulging disc. EE had a follow-up appointment on 10/13/2025 and they want Pre-Approval for PT and MRI. Employee began missing work on 10/08/2025 and estimated return to work is 11/24/2025.

The employee was mowing weeds with a Tractor and EE stepped out to use the restroom and bumped EE's knee on the step of the tractor causing a laceration in which EE had to go have stitches.

Approximately at 4:20pm radio traffic was heard about smoke in the area of the idle D11 dozer. The agencies and Fire department where contacted. This D11 dozer has been idle for 2.5 weeks. No injuries.

Employee was pulling HWM cable with J hook and felt a pop in right elbow.

Closing the magazine door when it slammed shut on ring finger (laceration)

A fire cracker was let off near EE's Lube Truck causing pain in EE's ear.

Employee was not aware that EE's hand was on door frame behind curtain when another employee shut door on fingers.

Employee was guard of Arms security Services and was contacted by radio at 6:00pm. At 8:00 pm an attempt was made to contact EE by radio EE did not respond. The night shift traveled to the guard shack to check on EE and found EE deceased in the guard shack

Employee was operating grader when hydraulic hose burst spraying fluid on turbo causing fire. Employee jumped out of grader to ground injuring

Employee was driving bus to the pit when bus ran up on a hump causing fuel pedal to stick. Bus sped up stopping against rock.

Employee was passenger of man bus when it ran up on mound of dirt and throttle stuck and hit rock.

Working on HWM when slate bar slipped knocking tooth out.

High voltage line broke, dropped to ground and caught grass and hills on fire.

Employee was dismounting fuel truck at end of shift and as employee stepped down employee felt shoulder pop. On 2-24-2021 employee was taken off work and scheduled to see a specialist and an MRI scheduled.

Employee was filling water pump to prime pump and slipped and caught finger on clevis. Employee continued to work until 05/20/2020 where employee was taken off work and scheduled to see orthopedic surgeon.

Pulled a muscle in right shoulder while cleaning out dirt from the rod wiper on the drill.

Employee was driving ground rod into ground with post driver and hit self in the head causing laceration requiring two staples.

Employee was bleeding airline on endloader and during the process employee strained neck.

Employee was helping other employees to change out a roller on a D10R Dozer. The old roller was lying on the track and was sharp from wear. Employee lost footing, which caused employee to place a hand on the old roller, resulting in a laceration to the left pinky which required 12 stitches.

Surveying pit, went to let bottom coal shot. As the employee was stepping down, the top edge gave out and their footing slid on the loose coal. Their knee popped and began to swell.

Employee was helping lift Drill Pontoon into back of truck when they felt pain in the right side. Employee continued to work until 06-06-18 when family Dr. took them off work and scheduled them to see a specialist.

The tires on the Komatsu 630 Haul Back Truck, were deflated first and while removing a lug nut, the tire fell on EE. Several lug nuts remained in place when tire suddenly fell.

EE was hitting a bearing with a sledge hammer and a shard of metal came off and entered right forearm.

Employee was working on bucket teeth and adapters on a shovel bucket. When attempting to remove the G.E.T. lock out the keeper pin fell out and pinched employee's hand between the keeper and a 2 inch plate. The plate was being used to press the wedge up using a ram and hydraulic pump. Injury required stitches to close the wound.

Hydraulic ram broke free from pressing pin out of bores on CAT 011R Dozer and struck employee on the head.

Removing wheel unit from 785C using a pry bar and crane on service truck. Hub came off spindle and struck employee on left hand causing employee to loose balance didn't fall but twisted employee's left hand and knee in the process.

Employee was removing an oil pump from a dozer with a chain hoist. As EE was holding it steady with EE's hand, the pump slipped from the chain hoist, pinching EE's finger. This caused a laceration that required 9 stitches to EE's ring finger

Filing a claim of simple pneumoconiosis, category s/t, 1/0, lower 4 zones

Steam cleaning equipment, coupling blew off steam hose causing hot water to hit chest (left side chest wall)

Employee stated while changing a drill bit in a hand held drill, while tightening the drill chuck the employee stated he felt something snap in his upper right arm. an accident report was filed and the employee continue to work. The Employee went to Med Express on 6-27-14 and put on restricted duty. employee Notified his employer of seeking medical attention on 7-1-14

This is to file for an occupational illness of pneumoconiosis.

Employee has been diagnosed with Occupational Pneumoconiosis

Employee was looking for the end of a hydraulic hose. He placed his hand on a pipe, his hand slipped and caused him to get against the hot hydraulic pump.

Employee was prying on one side of the belly pan(pan was hooked to the boom, holding the weight)He was pulling against it with a slate bar to line up the hinge pin holes. He felt a kink in his lower right back and he thinks he pulled a muscle. The muscle got tighter as the shift went on. This injury was not ruled work related until June 8th, 2013.

Injured was trimming a speaker mount with a razor knife, it slipped cutting wrist slightly, first aid was rendered. EE transferred to hospital for professional care.

EE was helping put a cutter chain back on the sprocket when the chain slipped and pinched his left hand between the links. This resulted in a fracture to the top of his hand behind his little finger.

Employee was exiting a truck when he slipped on the step and hit his right side rib area on the cab. He finished his shift, worked the next day and sought medical attention on 9/30/2012 and was diagnosed with a strained muscle in the rib area.

Utilizing 1"" air operated impact wrench EE was tightening fasteners on the right hand spindle assembly of a Komatsu 930E Haul Truck. EE felt a searing pain in his abdomen possible hernia

Employee was a passenger in the HD2500 chevy portal truck when the driver lost control. They were traveling to the pit. The truck veered off the road, striking a rock embankment. EE's head hit the windshield post on impact.. He was not wearing an seatbelt

Employee was operating a Chevrolet 2500 hd pickup enroute to the pit. He said he heard a noise, panicked and hit the gas pedal, veering into and striking the embankment. His head hit the steering wheel. He was not wearing a seat belt.

Employee stated that he hurt his back while walking away from his truck.He went to the hospital and was released. He came back to work and worked two days then went back to hospital and was put off work with a strain. We are disputing his comp claim due to the fact he told other employees that he hurt his back off of the job at home.

Employee was making a grease line for RL4510,when he used a razor knife to cut a zip tie. The razor slipped and cut his middle and ring finger on the right hand.

Employee was lifting old ""h- bracket"" with boom when brace got hung. He then lowered the brace back down when a chain came unhooked, dropping the brace down approx. 12 inches on his left foot.

mine mechanic was assisting, Injured mechanic install a steel bushing in a link using a metal hammer, when mine mechanic struck the bushing it fragmented creating a projectile which penetrated EE's lower left calf. The fragment was to deep to be removed locally, required trip to emergency room for removal.

Employee was standing between two pump motors. He and another employee was attempting to move them. Each motor weighed approx. 800 pound each. As the first one was lifted, the second one became unstable and fell, pinning EE's leg under it.

Robert states that he was using a box cutter to cut a zip tie when the knife slipped causing a cut to his left thumb.

A pin keeper that holds the tooth on the adapter was being removed when a piece of the adapter broke off and flew through the air approximately 5 to 6 feet. An employee was walking past and was struck in the nose.

HWM pad man was washing the walkway off when a piece of coal became dislodged and flew from the belt, striking him above the left eye. This cut required 4 stitches.

Employee stated that the A/C was to cold in the cab. He then inserted a rag into the vent and shoved his finger into the A/C fan cutting his left index finger.

EE was standing on the drill deck and bent down and his knee locked up.

EE had left the office going home and was traveling on the mine access road when a deer ran out in front of her personal vehicle causing her to hit the berm and flip her jeep.

Member was using a 100 ton ram to straighten out a piece of 3/4 inch metal when the jack broke causing the metal to shift hitting him in the ankle.

The employee was cutting two leads apart when the knife cut his hand between the thumb and fourfinger. This resulted in Three Stitches.

EE was coming to work when vehicle he was in slid into another vehicle. EE hit the rear view mirror with his head and caused a laceration to his head. EE was taken to Raliegh General Hospital where he received 11 stitches to the left side of his forehead.

As the 5500 excavator was pulling overburden there was an unplanned detonation of an explosive material. There were no injuries. There were no damages to the equipment.

As the 3600 excavator was pulling some wall down there was an unplanned detonation of an explosive material. There were no injuries. The damages were only to the windshield and paint of the excavator. This event is still under investigation.

EE was loading the powder truck, standing on top, when the ANFO at the top of the bin broke loose causing the bin to shift abruptly hitting him on the right shoulder. At the same time he felt pain in his lower back.

While doing his normal job position, employee started having symptoms of lower back pain. After extensive testing the results show he is suffering from the occupational illness of degenerative discs. Per 2/5 document 220102230004, EE was doing walk around his truck, when he slipped and fell on flat ground.

Employee jumped off shovel and sprained ankle while landing on a rock.

Employee was welding while working off of scaffolding at a height of 6 feet 3 inches. He fell between two scaffold boards and landed on his knees, resulting in a back injury.

Employee was following other employee up the ""Woods Road"" to the Twilight Mine. At the 31 curve. He stopped to let other employee go up the hill 1st, almost at the top he (other employee) spun out sliding down the hill striking his truck on the front drivers side(fender and door).The roads were snow covered and slick.

Employee was putting the hose back on the pulley of the repump truck. After finishing he went to climb down the ladder, when he twisted he felt a burn in back. He finished the shift and decided to seek medical attention on 2/4/10.

Member was walking up a snow covered bank when he slipped and fell. His head struck the ground resulting in a laceration requiring two stitches.

Installation of part (snubrite) on mining shovel bucket was being performed. Lifting sling failed, causing a component of the snubrite to strike the employees left finger. Employee was diagnosed with fractures to left index finger.

Employee was exiting the 495 shovel when the ladder shifted and he fell approx. 3 feet to the ground. His left knee struck a rock and caused a small laceration. He was taken to the Boone Memorial Hospital where he received 8 stitches to his knee.

Employee was spotting for another employee that was operating a forklift. The forks on the fork lift got in a bind on the load that was being picked up. The fork came off the lift and smashed the spotters finger between the fork and the load that was being moved.

Injury was removing a bolt when the wrench slipped of the bolt and his wrist, struck another bolt head causing a cut to his left wrist.

Employee was assisting another employee with lifting a piece of plate (1""x6""x10'). He felt a sharp pain in his Rt. Shoulder and was sent to the doctor. The doctor released him back to work, but set up further testing. On 8/18/2009, the specialist put him on light duty restriction and set up an appt. with a surgeon.

On 7/24/09 the intial accident occured, employee was attempting to mount a D-9 Cat Dozer. As he was mounting, he heard a pop in his knee. He was taken to the hospital where he was referred to a specialist for an MRI. He was returned back to work full duty. On 8/7/09, he went for the MRI reading and found a tear in his meniscus and was taken off work.

Employee was in the feeder tunnel removing material from the conveyor chain. A rock fell from an undetermined location striking him on the right side of his head.

Employee was walking off of the catwalk at the truckwash when the bottom of walk broke. He stepped off of the side of the walk and onto a rock in turn, causing him to twist his ankle.

Muscle strain to lower back and groin resulting in restricted duty for employee.

Injury was cutting a zip tie for wires with a knife. The knife slipped and cut his palm of his left hand. he was putting wires in a piece of conduit.

Employee was walking on job site with welding equipment in hand and fell on a loose rock. When he fell, he hit his head & neck on a welded support on a service truck. Initially medical evaluation came back negative and he was referred to a specialist. On 6/2/2009 specialist prescribed physical therapy & meds making the injury reportable.

Employee was using a pocket knife to remove a name tag from a uniform and cut her left middle finger requiring 5 stitches.

Employee was loading truck when he struck his right elbow on a part of the bin structure. Caused laceration believed to require stitches.

Employee was cleaning the fabrication yard and some rust blew into his eye.

Employee was doing his preshift exam on his 930E haul Truck, where he found glass in the cab of his truck, due to a window installation. He then began to clean it up when he felt a slight burn in his right eye. He then reported to his foreman and was transported to Boone Hospital. He was treated with eye drops & returned to work on his next shift.

Cleaning up & putting tools away in muddy conditions. EE lost footing & fell off truck landing on right hip and elbow.

EE was stacking crib blocks on truck, when a block fell down and smashed his finger.

Employee was using a steel bar trying to knock off a bearing on a swing motor shaft. The steel bar slipped from the employee's hands; as he went to bend down and catch the bar he felt a pain in his lower back.

Employee was cutting out a steel donut to weld onto the dump ropes that attach to the dragline bucket. While cutting he noticed the oxygen hose on the torch had a pin hole. He attempted to taped the hose instead of replacing or fixing the hose. Employee continued to use cutting torch and his gloves became saturated with oxygen gas and a spark set his gloves on fire.

Working on D11R installing sheet metal over transmission dipstick. Door on sheet metal fell and struck left index finger at nail bed.

An employee was attempting to change out a compressed gas cylinder on the Boom Service Truck. As the employee was attempting to manually lift the bottle he lost his footing in the mud and strained his right shoulder.

EE states that he was climbing into the ex200 excavator, when his right foot slipped off of the first step(approx 18 inches from the ground) and landed on his knee.

Employee was changing out Oxygen bottles on the Service Truck and felt a sharp pain in his abdomen area.

Injured employee was replacing dump ropes on the 8050 dragline bucket. The rope had formed a kink in it and the employee overexerted himself while trying to pull the kink out, resulting in a sprain/strain his right shoulder, arm & neck.

Two employees were grinding on the center pin of the 495 BI shovel. The injured employee walked 2 to 3 feet away from the work area to plug in another grinder. He removed his safety glasses while plugging in the grinder and a foreign object flew into his eye.

Employee was repairing the Emergency Brake bracket on one of our service trucks. He was using a 3/8"" electric drill with a 1/2"" drill bit to bore out the bracket mount, when the bit got jammed. The drill twisted and hit the employees hand, resulting in a spiral fracture to the right hand.

EE stated that he was sitting in his truck waiting on the road to get cleared of snow. He then took out his pocket knife and cut off some zip ties that was holding his radio antenna. His knife slipped and cut his left hand between his thumb and index finger. One stitch was required.

Employee slipped while exiting truck and injured back on the 22nd. He did NOT seek medical attention or miss work. On the 24th, his vehicle slid into berm in snow, reaggravating injury from the 22nd. He did not seek medical attention until the 28th, at which time he was off until 2/2/08.

EE states that the lights went out on his dml 50 drill, he got out to check the alternator, cut his right hand on a metal band that holds the exhaust insulation in place.

Employee was removing a jack stand from under the machine and as he tilted it to where the jack could be removed his hand slipped from the stand. The jack stand fell over and caught the employee's hand between the jack stand and the track pad.

Employee was assisting with extinguishing a fire that had occurred around the collector rings on the 495 Bucyrus shovel and as a result inhaled smoke.

EE HAD A 1/4"" X 6FT FACTORY WIRE PULL ROPE ATTACHED TO OVERHEAD CRANE HOIST. IT WAS ATTACHED TO A NEW WEAR PLATE. WHEN THE PLATE WAS HOISTED TO THE BUCKET, THE CRIMPED END OF THE CABLE GOT STUCK IN THE HOLE. BILL THEN TOOK A SIX FOOT PRY BAR AND CLEARED IT. THE SUDDEN RELEASE OF ENERGY FROM THE BAR PULLED THE SLATE BAR TO THE BUCKET, SMASHING HIS LEFT THUMB.

EE stated that he had made a push with the rd630 -d10 rock dozer and was backing up and backed over a rock and felt pain to his back. EE refused medical treatment. He did not cease work. On Oct 7,2007, EE called in and stated that he went to the doctor and he had a strained back muscle.

WE WILL CONFERENCE THIS INCIDENT: Employee twisted ankle jumping out of pickup truck bed while in process of unloading 4-wheeler during a personal trade before shift started.

Following a fuel delivery to customer tanks, EE was returning his 3"" x 20' hose to the storage tube when he slipped on the large stone drive surface. In a attempt to break his fall, he stuck out his arm and felt a ""pop"" in his right shoulder. Reported to Progress Coal.

Employee was changing out the dump ropes on the right side of a dragline bucket. They had the rope threaded through the socket and were getting ready to install the wedge, when another employee lost his grip on the rope and it fell out of the socket, striking the injured employee on the right side of his neck and collar bone.

The employee was working with the dump ropes on a dragline. He was working on the rigging sheaves while trying to thread the dump ropes through the sheave. He completed what he was doing and noticed he had a puncture wound on his right elbow. He was sent to a clinic where they cleaned the wound, removed a sliver of steel and closed the wound with 1 stitch.

Employee stated that while removing the air box on a 930 Komatsu rock truck, he was putting pressure on the lifting eye to remove the air box. The lifting eye broke and cut left ring finger. He received 10 stitches .

Employee was jacking upper and lower track pads together with a come-along to remove a pin. The upper hook slipped off and struck employee above the right eye resulting in a cut/laceration that required six stitches.

EE stated that he tripped on the cable going to the 495 shovel and fell into the mouse house, striking his right hand against the side of the transformer.

Employee said he hit a hole in the road and it caused him to go numb from his chest to his feet.

Employee was stemming a hole when a rock in the spoil pile gave way. He slipped and a foot went into the hole, twisting his back.

EMPLOYEE ALLEGED HE WAS STEPPING OFF A D 10R DOZER WHEN HE SLIPPED AND HIS LEG WENT BETWEEN PUSH ARM AND ROLLER FRAME.

Employee was lowering the boom on the light plant and the crank slipped from his hand. He tried to hold the crank back; while doing that the crank hit his right hand middle and ring fingers. He found that he had fractured the fingers when he went to the hospital.

Employee was exiting his service truck when he stepped on gravel and twisted his ankle. He was sent to a specialist to find a broken ankle.

EE was cleaning out the bed of a service truck, when a piece of steel plate fell off the tailgate and landed on his left foot. The EE was wearing meta-tarsal boots, but they lacked sufficient coverage, resulting in a fractured left pinky toe. A contirbuting factor was that there was a power outage, due to lighting, that resulted in poor visibility.

Worker was using a sledgehammer to dislodge ANFO stuck in the powder bin. When he swung the hammer back to hit the bin again, he smashed his finger on the beam of the powder bin.

Employee was welding and some welding slag flew into the employees ear. The employee continued to work and went to the doctor on a scheduled off day. The employee did not report the injury at the time it happen or when he returned to work. The first notice of injury was received on the day that we received paperwork from the Worker's Compensation Company (Brickstreet).

Ee stated that he was removing jack stands from no# tt212 as he exited the truck, the jack stands slipped and the truck fell off the stands, causing him to land on his left foot twisting his ankle. Fracture found by specialist dr on 3/6/07

Employee was air arcing all day and had another employee welding within 2 feet of where he was working. The employee was wearing an automatic tint welding helmet that may not have been adjusted correctly. The employee suffered from flash burns to the eyes & face.

Driving his rock truck, employee saw a 930 rock truck in the same pit was having problems out of the retarter system in which he saw smoke and sparks. He then went to help with his fire extinguisher. He was exposed to smoke and fire extinguisher dust about 10 - 15 minutes before smoke was gone.

Employee was checking holes where he was drilling when he slipped on a rock and fell to his right, falling on an old broken steel pipe. Upon falling he cut the outside right side of his knee.

Employee stated that he was traveling off the property at the end of his shift when a vehicle coming in on the property hit the vehicle he was a passenger in. At contact he stated that he hit his head on the rear view mirror cutting his forehead & above his right eye.

Employee states that he shut down #7 stacker belt to remove a rock from snub rollar. Once belt stopped, he began removing rock when rock slipped and caught his right index finger, cutting it. He was wearing his gloves.

While trying to hook a chain to the bucket of a 988F Loader to move a 495 dipper door, the loader bucket curled down pinning employee's foot 9 inches into the ground.

Employee was uncoiling 1"" wire cable. The cable end recoiled due to tension, striking him in face. Transported to local E.R. Treated for minor cuts and abrasions. Returned to work next scheduled shift.

When starting to fuel, the nozzle separated from the hose. He jumped back to avoid the whipping hose & fell in the shot rock. He got fuel in his eyes & on his clothes. He struck his leg, knee, buttocks, hip, neck & head against the rock. He retrieved the hose & went to the maintenance area where he flushed his eyes, changed shirt, repaired hose & continued his shift until 7 am.

Employee was working around the 495B shovel and stepped on a rock in heavy mud and water. Mud and water was 8 to 12 inches deep. Employee twisted his left ankle and knee.

Employee was grinding on boom when a piece of slag went in his eye after he raised his hood up (slag was on hood).

Employee was returning a tray of 630 amp fuses to the parts trailer, tripped over the corner of a box of parts on the floor. He fell into a shelf striking both arms against the shelf. Sustained injuries to both arms.

Departing the porta-john, EE stepped on an uneven surface, twisting his ankle. Investigation continuing, this injury is contested as having occurred off site.

Employee was assembling a drill stand, the top plate slipped off and struck employee across the top of his right foot, causing a bruise.

Employee was welding on the roof and fan house of the dragline and had too much exposure to welding rays.

According to employee, he was lifting rails.

EE was welding and received flash burns to his eyes.

Employee was lifting a full bottle of gas when he felt a sharp pain in his lower stomach/groin area

Employee was grinding a pipe over his head when a piece of metal dropped in his eye.

Employee working outside in the heat using a come-a-long to move machining and he got hot and dizzy.

Employee was grinding welds on structure and got a foreign object in right eye.

Employee was grinding overhead and some debris fell into his eye.

While installing a prop shaft, cable was making a noise, employee raised up quickly to move and lost his hard hat then struck a piece of angle iron with the top of his head.

While lifting a pedestal cover, the EE lost his footing allowing the cover to rotate and slip dislocating his right ring finger.

Employee was welding/chipping overhead on revolving frame when a piece of slag fell into his eye.

Employee was working on the swing bores of the 8050 dragline, was air chipping slag and old weld when a piece of metal went in his eye.

Two welders were working on lib back to back. Reflection of other welder flash burned eyes.

Employee was welding/gouging in a confined space and a piece of hot slag went into his boot and burned his ankle.

Stepped off dozer onto frozen ground and twisted ankle. Referred to specialist who removed him from work.

Electrician was trouble-shooting IGBT tower and failed to allow capacitor banks to discharge completely and was shocked by the capacitor banks.

EMPLOYEE WAS WALKING ON TUB SECTION OF 8050 AND SLIPPED ON ICE AND FELL IN A MAN HOLE.

USING AIR CHIPPER TO REMOVE SLAG FROM WELD, A PIECE FROM THE SLAG FLEW INTO HIS EYE.

Employee was attempting to close the discharge gate on an overhead storage bin. He over-extended and pulled a stomach muscle. Treated at ER, given 2 days off work and additional restricted duty.

While conducting a time study, the employee's chevy was struck by a 730E Komatsu rock truck. The Suburban he was in came to rest on its top with the rock truck on top of it. Extraction took approximately 1 hour. Employee was treated and released from Caml Gen Div. and returned to work on his next scheduled shift.

While operating a Komatsu 730E rock truck, an employee left the haulroad striking the time study on the right side. The rock truck came to rest on top of the time study vehicle. This vehicle was parked 223' from the haulroad. Employee was terminated due to not adhearing to company policy.

While conducting a time study, the employee's chevy was struck by a 730E Komatsu rock truck. The Suburban he was in came to rest on its top with the rock truck on top of it. Extraction took approximately 1 hour. Employee was treated and released from Caml Gen Div. and returned to work on his next scheduled shift.

on 9-22-05. Ee stated that he slipped and fell on a mat uner a mine phone and landed on his backside, He returned to work on his next shift after being released from physician to do so. On 10-7-05, he ceased coming to work and did not Mgt aware the he was seeking another dr opinion. The second dr removal him from work and place dhim on the port, mgt was not make aware of 2nd dr.

Lost balance after cutting tree, tripped and fell and rolled onto stob.

EE AND OTHER EE WERE MAKING A BELT SPLICE AND EE WAS SUQARING BELT UP. EE WAS MAKING THE LAST CUT ON BELT AND THE KNIFE SLIPPED AND CUT LEFT HAND (MIDDLE FINGER KNUCKLE WAS CUT TO THE BONE)

EE WAS REPAIRING LIGHTS ON TOP OF CAB. FOOT SLIPPED WHILE CLIMBING DOWN, HANGING ON WITH RIGHT ARM CAUSED STRAIN ON SHOULDER

Employee was prying a piece of metal when slate bar slipped and struck him in the face causing a laceration to lip and a fracture to his nose and cheek.

EE WAS ATTEMPTING TO LIFT BOXES OF EXPLOSIVES. AS HE WAS TURNING, EE FELT PAIN IN RIGHT LOWER BACK. EE VISITED ER DOCTOR - PRESCRIBED PRESCRIPTION MEDICATION AND LIGHT DUTY. (EE WAS LOADING BOXES INTO TRUCK; BOXES WEIGHED 40 LBS. EACH.)

Operator was standing on deck of dozer getting flashlight out of cab. He stepped backwards missing all the steps on the deck causing operator to fall backwards down between track and dozer. He struck his back on tracks and left leg went down between tracks.

A loaded 777D rock truck backed through the berm and overturned at the dumpster at the 4100 p&h shovel pit on south main ridge.

WAITING FOR MEETING TO BEGIN, EE COMPLAINED OF CHEST PAIN. SUPERVISOR TRANSPORTED EE TO WAREHOUSE WHERE EE WAS TREATED BY EMTS. WHILE BEING TRANSPORTED TO HOSPITAL, EE SUFFERED A HEART ATTACK.

We are contesting this injury pending investigation - employee stated that he was traveling down warehouse hill and hit a hole in road, his seat bottomed out and jarred him. He complained of numbness and tingling in his left arm & shoulder. He refused medical treatment and an ambulance.

EE WAS ATTEMPTING TO HOOK UP A MINE CAR ON THE HIGHWALL MINER WHEN HE ACCIDENTALLY STRUCK A MINE CAR PUSH-PIN WITH HIS LEFT SHIN.

EMPLOYEE WAS ASSISTING ANOTHER EMPLOYEE TO LOAD A TRUNION CAP FOR A D11R DOZER ONTO THE BUMPER OF A SERVICE TRUCK. WHILE PLACING THE CAP ON THE BUMPER, THE TRUNION CAP ROLLED OVER THE END OF T HE MIDDLE AND THIRD FINGER OF HIS RIGHT HAND.

WHILE CONSTRUCTING A 46KV POWER LINE, EE STEPPED OFF BUCKET TRUCK AND SPRAINED LEFT ANKLE.

WHILE PICKING UP A PART IN THE WAREHOUSE, EE WAS TRANSPORTING THE PART ON A HAND TRUCK. THE PART CAME OFF THE HAND TRUCK, EE PLACED THE PART BACK ON THE HAND TRUCK PINCHING HIS LEFT MIDDLE FIN GER BETWEEN THE PART AND THE HAND TRUCK. NOTE: EE DID NOT SEE A DOCTOR UNTIL 10-4-03. HE WAS TREATED & RELEASED W/BACK TO WORK SLIP. EE INFORMED PROGRESS OF THE NATURE ""BROKEN BONE"" UNTIL 10-

PORTAL VAN PULLED IN FRONT OF A 190 EUCLID ROCK TRUCK. THE ROCK TRUCK PULLED INTO THE VAN, CRUSHING THE VAN. IT WAS DARK AT THE TIME OF THE ACCIDENT.

EE WAS WORKING ON HIGHWALL MINER SYSTEM AND CHANGING BITS ON THE MINER. MINER WAS LOOCKED AND TOSSED AND THE EE WAS ROTATING THE MINER MANUALLY WITHOUT THE ASSISTANCE OF A PRYBAR. WHILE PERFORMING THIS WORK THE EE BEGAN TO EXPERIENCE PAIN IN HIS LOWER BACK. HE WAS TREATED AND RELEASED AT E.R. AND IS CURRENTLY AWAITING TO SEE AN ORTHOPEDIC SPECIALIST.

REMOVED BOLTS FROM FAN AND FAN SLIPPED OFF OF HUB. WAS HOLDING ONTO IT AND COULD NOT LET GO FAST ENOUGH, PULLED LEFT ARM, FELT IT PULL AND TEAR THE ARM. TWO MECHANICS WERE WORKING AT BACK OF T RUCK, PULLING GENERATOR, BELIEVE THAT MOVEMENT MADE FAN FALL AND TRIED TO CATCH.

EE STATES JE BEGAN EXPERIENCING PAIN IN HIS BACKWHILE OPERATING A WATER TRUCK. HE BEGAN TO EXPERIENCE PAIN AT 7:30 PM BUT DOES NOT KNOW WHY OR HOW THE PAIN BEGAN OR WHAT CAUSED IT. HE CONTINUE D TO WORK THE SHIFT BUT FAILED TO RETURN TO FOLLING DAY. HE STATES HE INFORMED HIS FOREMAN HIS BACK FELT STIFF WHILE THE FOREMAN STATES HE WAS NOT INFORMED THE PAIN WAS WORK RELATED. PROGRESS

REPAIRING WIRING BELOW FLOOR PLATE OF A D10 DOZER, BENT OVER WORKING IN A CONFINED AREA.

EE WAS STEMMING BLAST HOLES ON DRILL BENCH. HE SLIPPED ON A ROCK AS HE WAS WALKING AND TWISTED HIS BACK. EE REFUSED MEDICAL TREATMENT UNTIL 3-10-03. HE VISITED ER AND WAS PRESCRIBED MEDICATION AND REST.

EE WAS LIFTING A 1 1/2 INCH PULL CABLE TO A ROCK TRUCK STRUCK IN THE MUD. AS HE WAS LIFTING THE CABLE HE BEGAN TO EXPERIENCE PAIN IN HIS LOWER BACK. HE DID WORK THE NEXT DAY AND ON 1-31-03 HE ADVISED MINE MANAGEMENT HE WAS GOING TO SEE A PHYSICIAN. HIS NEXT SCHEDULED DAY TO WORK 2-3-03 HE FAILED TO REPORT TO WORK.

EE WAS CUTTING TIN WITH A SAW WHEN SOMETHING ENTERED HIS LT. EYE.

EE WAS REMOVING PLASTIC PLUGS FROM DRILL HOLE. PLUGS HAD FILLED WITH DRILL CUTTINGS. EE WAS LYING ON STOMACH AND LIFTED PLUG. HE FELT PAIN IN BACK & LEFT SHOULDER. VISITED E.R. ON 1/17/03. HE WAS PRESCRIBED MEDICATION AND TIME OFF WORK.

EE WAS HELPING ANOTHER EE WELD A PLATE ON A NEWLY INSTALLED CRUSHER OF THE TRUCK DUMP AND SUFFERED A FLASHBURN WHILE ASSISTING THE OTHER EE, HE WOULD RAISE HIS LOAD WITH ONLY CLEAR SAFETY GLAS SES ON.

EMPLOYEE WAS WALKING UP THE STAIRS WITH A BAG OF NUT & WASHERS, WHEN HIS METATARSAL BOOTS GOT CAUGHT ON A PIECE OF 3/4"" CONDUIT STICKING UP 4""-6"" ABOVE THE WALKING SURFACE. HE TRIPPED AND FELL , CATCHING HIMSELF IN A FORWARD FALL WITH BOTH HANDS, TWISTING HIS BACK.

EMPLOYEE WAS WELDING ON THE BUCKET DOOR OF A BE 495 SHOVEL. FOR COMFORT, HE HAD PLACED HIS FEET IN THE OPENING BETWEEN THE BUCKET AND DOOR. THE LATCH BAR SHIFTED ALLOWING THE DOOR TO CLOSE PIN NING HIS FEET.

EE WAS EXITING TRUCK WHEN HE SLIPPED & FELL INJURING LOWER BACK & BUTTOCKS.

EE HAD CLIMBED ONTO A D-11 TRACTOR TO FILL IT WITH HYDRAULIC FLUID. WHEN HE REACHED OUT TO REMOVE THE HYDRAULIC TANK CAP HE LOST HIS FOOTING AND FELL APPROXIMATELY 8' TO THE GROUND LANDING ON HIS RIGHT SIDE. HE POSSIBLEY SLIPPED ON SOME SPILLED HYDRAULIC FLUID.

EE WAS STANDING ON THE GROUND, PULLING THE WASTE OIL HOSE OFF THE REEL OF THE GREASE TRUCK, TO PERFORM SERVICE ON A ROCK TRUCK. AS HE WAS PULLING, THE END OF THE HOSE CAME OFF CAUSING HIM TO L OSE HIS BALANCE & FALL TO THE GROUND. HE ATTEMPTED TO BREAK HIS FALL WITH HIS LEFT ARM, LANDING ON HIS ARM INJURING HIS WRIST.

BLOWING OUT RADIATOR ON A D9R WHEN EMPLOYEE BROKE OUT IN A RASH ALL OVER.

EE WAS USING A PORTA-POWER RAM WEIGHING APPROX. 35 LB TO PRESS A BEARING ON A HARD BAR FOR A TRACTOR. HE WAS HOLDING UP THE RAM WHILE ANOTHER EE WAS RUNNING THE AIR PAD. HE STATES HE WAS IN A TWISTED POSITION CREATING A STRAIN AND FELT A TINGLING IN HIS BACK. HE WORKS A 4 ON 4 OFF SCHEDULE AND DIDN'T REPORT TO MANAGEMENT UNTIL 7/15/02

EE WAS REMOVING FRONT DIFFERENTIAL DRAIN PLUG FROM A 992 LOADER USING A 3/4 INCH BREAKER BAR AND 1/2 INCH SOCKET WHEN HE FELT A STRAIN IN HIS LOWER BACK. HE DID NOT CEASE WORK NOR SEEK MEDICAL ATTENTION UNTIL 8/15/02 WHEN HE NOTIFIED MINE MANAGEMENT HE WAS UNDER PHYSICIANS CARE AND ORDERED OFF WORK.

EE WAS HOLDING A PIECE OF METAL IN PLACE TO BE WELDED. OVER THE TIME PERIOD EE WAS EXPOSED TO WELDING FLASHES. THEREFORE CAUSING FLASH BURNS TO HIS EYES.

MEMBER WAS WORKING ON CLEANING OUT #2 STACKER TUBE FROM THE TOP. MEMBER WAS IN THE TUBE BREAKING GOB LOOSE. GOB BROKE LOOSE WHILE HE WAS INSIDE THE TUBE ROCK BOX & FELL DOWN THE TUBE & WAS DIS CHARGED OUT THE SIDE CHUTE ONTO THE STOCKPILE.

BENT OVER TO INSTALL TORQUE CONVERTOR OUTLET VALVE ONTO THE OLD TORQUE TO SHIP BACK & FELT SHARP PAIN IN CENTER OF BACK AT WAIST RADIATING DOWN RIGHT LEG.

INJURED WAS RIDING IN HTE BACK SEAT OF THE SCHOOL BUS WHEN IT TRAMMED OVER A DIP OR ROCK IN THE ROAD CAUSING INJURED TO BE TOSSSED UP & OUT OF HIS SEAT, LANDING BACK INHIS SEAT IN THE SEATED P OSITION CAUSING HIM LOWER BACK PAIN.

FUELER APPROACHING DOZER TO FUEL. WALKING THROUGH MUD SLIPPED ON A PIECE OF MUD COVER SLATE AND FELL. ATTEMPTING TO CHECK FALL EXTENDED RIGHT ARM AND HYPEREXTENDED WRIST FINGERS. EE LATE IN R EPORTING INJURY AS ""ON THE JOB"".

EE WAS OPERATING THE 285 DEMAG, LOADING OVERBURDEN SHOT MATERIAL WHEN A ROCK FELL FROM THE TOP, STRIKING THE BOOM & THE BUCKET OF THE DEMAG. THE FORCE SHOOK THE EQUIPMENT, JARRING HIM.

EE WENT UPSTAIRS OF HIGHWALL MINER TO SET UP CAR BREAKER. HE ALLEGED WHILE WALKING DOWN STEPS HIS ANKLE WAS SPRAINED WHEN HIS FOOT GAVE OUT.

EE DID NOT REPORT INJURY UNTIL 2-18-02. MECHANICWAS LAYING ON HIS BACK LIFTING AND ALIGNING A DRIVE SHAFT TO THE TRANSFER CASE. IN DOING SO HE PLACED HIS BACK IN AN UNNATURAL POSITION CAUSING THE INJURY, SUBSEQUENT LIFTING ACTIONS CONTRIBUTED TO THE SEVERITY.

REPAIR WORK WAS BEING PERFORMED ON A LOADER TO THE CUTTING EDGE. EE WAS STANDING AT THE REAR OF HIS MECHANIC'S TRUCK ~30' AWAY FROM WHERE THE WORK WAS BEING PERFORMED. THE WELDER PERFORMING TH E WORK TO THE CUTTING EDGE WAS STRIKING IT WITH A SLEDGEHAMMER WHEN APPARENTLY A PIECE OF METAL BROKE LOOSE AND STRUCK EE IN THE LEFT TEMPLE AREA OF THE HEAD. THE PIECE OF METAL BECAME IMBEDD

EE WAS GREASING A WIDE BODY TRUCK. AS HE MOVED BETWEEN THE TIRES HIS FOOT CAME IN CONTACT WITH THE DUMP CHAIN CAUSING HIM TO FALL TO THE GROUND. HE LANDED ON HIS LEFT SIDE STRIKING HIS FACE, B ACK, LEFT LEG AND LEFT ARM ON THE GROUND.

EMPLOYEE WAS LANCING A PIN ON A 992G AND TOOK HIS WELDING HELMET OFF AGAINST CO EMPLOYEE'S ADVICE AND HIT A GREASE SPOT AND WAS SPRAYED IN THE FACE AND NECK WITH HOT GREASE. EMPLOYEE DID LEAVE HIS SAFETY GLASSES ON.

WHILE REMOVING PANEL SHIELD FROM CAR, HE CAUGHT THE TIP OF HIS MIDDLE FINGER BETWEEN SHIELD & C. MINER.

EE WAS CLIMBING STAIRS TO LOADING RACK WHEN HE SLIPPED AND FELL. HIS GLOVED HAND (RT) SLIPPED FROM THE RAILING AND IN ATTEMPTING TO CATCH/BREAK THE FALL HIS LEFT WRIST BENT CAUSING A SPRAIN.
