Caught in, under or between a moving and a stationary object

While cleaning the off-side walkway on #4 belt, 2 Heintzmann jacks became dislodged and a large rock struck employee pushing him into the belt line. The accident is currently under investigation by the WVOMHS&T, MSHA and the Company.

Accident type, without injuries

On 12/7/2015 while performing examinations, the examiner discovered an unintentional roof fall near the intake seals. There were no injuries, or damage to equipment. The fall measured approximately 4' x 4' x 16'. The M.S.H.A. was notified and an investigation was conducted.

Over-exertion in lifting objects

Employee stated he bent over and moved the continuous miner cable slack out of the roadway, and when he stood up he felt pain in his back. Employee was diagnosed with a back strain and taken off work. The operator of the coal mine at the time of the incident was Gateway Eagle Coal Company, LLC.

Employee stated he was attempting to hang a shuttle car cable, when he felt pain in his back. This resulted in a back strain.

Employee stated he was performing fire boss duties along the #1 belt line. As he attempted to cross under the belt line at break 10, he struck his head against the water line, resulting in a neck strain.

Employee stated he was operating a two-person mantrip along the roadway at break 159 when the brakes failed and the mantrip went down the inclined roadway, striking into the back of another mantrip and the rib. This resulted in strains from a jarring injury and one (1) restricted work day.

Employee stated he was operating the roof bolt machine and as he placed the drill steel in the chuck his left hand was pinched between the steel and chuck, resulting in a laceration. Sutures were required to close the wound.

On 5/27/2015 at approximately 7:12 a.m. an unintentional roof fall occurred in the intake (primary escapeway) at spad no.6. There were no injuries, or equipment involved.

Struck by falling object

EE was roof bolting when a piece of rock fell from the top, struck the RBM canopy, broke into pieces, then struck him on the face & right wrist. This resulted in a cheek laceration & a wrist contusion. Upon seeking medical evaluation the EE complained of back pain. On 7/15/15 the results were received by the operator & the lost work days were attributed to the incident.

On 5/14/2015 at approximately 4:55 p.m. an unintentional roof fall occurred in the intake (primary escapeway) at break 40. There were no injuries, or equipment involved.

Struck by rolling or sliding object

Employee stated he parked and exited his shuttle car. As he walked between the shuttle car and the rib, the rib rolled/sloughed off and a small piece of coal/rock either struck the employee in the knee, or caused him to fall. This resulted in a knee strain and restricted work days.

Over-exertion in lifting objects

Employee stated he was attempting to move a rock by hand measuring approximately 24"" x 18"" x 5"" out of the roadway on the #1 Section when he felt pain in his lower back. On 3/20/2015 he was diagnosed with a lumbar strain.

Fall to the walkway or working surface

Employee stated that he was replacing impact rollers on the tail of the stacker belt when the incident occurred. He was using a slate bar to pry the belt structure into place and the bar slipped, causing him to lose his balance and fall to the ground. This resulted in a left knee and shoulder strain.

Employee stated he was stepping through the rear door of the surface shop when his left foot slipped off the door jam, causing him to fall to the ground. This resulted in hip strain and restricted work days.

Employee stated that he bent over to move the roof bolt machine cable out of the roadway and felt a sharp pain in his lower back. The incident became reportable on 02/18/2015 when information was recieved diagnosing the employee with small posterior disc bulges.

Employee stated that he was pulling the rock duster behind the battery mantrip and as he turned out of the belt entry and onto the roadway the duster came unhooked. This caused the duster to separate from the mantrip. The employee was holding the on/off cable for the duster in his right hand. The cable pulled his right arm backwards resulting in a strain and restricted work days.

Employee stated that he was walking across the section and struck his head against the mine roof. This resulted in a neck strain. Incident was reported to Gateway Eagle Coal Co, LLC on 11/12/14, employee sought medical evaluation on 11/13/14, and medical was received on 11/21/14.

Employee stated that he was working around the #3 belt tail. He bent over to shovel around the belt and when he stood back up he struck his head against the water line. This resulted in a neck strain.

Employee stated that he was in the process of assisting the RBM operator as the roof bolter was moved into another place. As he bent over to pick up the cable the reel was engaged, which caused the cable to be pulled down from the mine roof and strike him on the lower back. This resulted in a back strain and restricted work days.

Employee stated that he was shoveling belt along the #8A belt line. He bent down to get on his knees and felt pain in his left knee. This resulted in a strain and restricted work days.

Employee stated that he was shoveling belt along the #5 belt line. As he lifted the shovel to empty gob onto the belt line, he felt pain in his left forearm. This resulted in a strain and restricted work days.

Employee stated that he was operating the scoop in the outby intake air course. The scoop road up onto a rock and slid to the side. This caused the employee to extend his hand to brace himself. The employees left middle finger was caught between the scoop frame and the mine roof. This resulted in a laceration that was repaired with sutures.

Employee stated that he was operating the scoop on the #2 Section. While assisting with the maintenance of the continuous miner, the scoop slid on a crib block, causing the employee to be bounced into the canopy of the scoop. This resulted in a head/neck strain and two (2) restricted work days.

Employee stated that he was loading belt structure into the scoop bucket. He lifted a 48"" top structure and turned to place it into the bucket. As he stepped into the insert to place the structure, he felt pain in his left knee. This resulted in a sprain.

Employee stated that he was bent over pulling on the continuous miner cable to remove a knot when he felt pain in his groin area. This incident became reportable when the employee began receiving physical therapy for a groin strain on 8/27/2014.

Employee stated that he was in the process of installing roof bolts in the top after a belt channel had been cut. He was installing the second row of bolts when a rib/rock rolled out and struck him on the left leg and ankle, resulting in a fracture.

EE stated he was prying on the take up dolly with a slate bar, when the bar slipped, his left thumb struck against the I-beam. Medical treatment when the thumb was set with a splint. X-ray reports received on 8/12/14 showed ""no evidence for fracture"". This injury was submitted on 08/13/14, but on 09/18/2014, there is no record of the submission. Therefore, it is being resubmitted.

Employee stated that he was tramming the scoop outby on the #2 section roadway when he ran over a rock in the road. This caused him to bounce in the seat and land on his left hip, resulting in a strain. This incident became reportable when the employee began missing work on 8/4/2014.

Employee stated that he was helping change the tire on the roof bolt machine. As the tire was removed, his left ring finger was caught between the rim of the tire and the hub of the roof bolt machine. This resulted in a fracture of the distal tuft of the left fourth digit.

Employee stated upon arriving to work he exited the cab of his truck and slipped on a rock falling to the ground. As he fell he extended his left arm to catch himself. This resulted in a radial head fracture of the left elbow. The employee has worked each scheduled shift at full duty, but on 7/28/14 he attended a doctor's appointment, which resulted in one lost work day.

On 8/21/2014 employee stated that on 7/20/2014 he was shoveling belt and felt pain in his back. The incident became reportable on 8/25/2014 when medical received confirmed the employee had sought medical treatment for a work related incident on 7/20/2014.

Information diagnosing the employee with occupational pneumoconiosis was received on 6/18/2014 by Gateway Eagle Coal Company, LLC.

Employee stated that at the end of the shift he staged the shuttle car up at the feeder and dismounted to walk to the mantrip. As he bent forward to walk, he felt pain in his back. This resulted in sprain.

Employee previously worked for Gateway Eagle Coal Company, LLC and retired on 06/27/2013. On 05/30/2014 information was received diagnosing the employee with occupational pneumoconiosis.

Employee stated that he was attempting to reset a timber in the No. 2 entry when a piece of rib/rock dislodged from the mine roof/rib and started to fall. As he stepped away from the rib he fell backwards onto his back and the timber fell across his right forearm. Employee was diagnosed with a contusion.

Over-exertion in lifting objects

Employee stated that he was changing a bottom roller on the stacker belt and as he lifted up on the roller he felt pain in his right side. This resulted in a chest strain. Employee began missing work on 2/24/14.

Employee was operating the continuous miner and stated that he took the 1st cut on the curtain side and then backed the C.M. up to trim top. After repositioning the C.M. employee began trimming top and walked up beside the C.M. The process of cutting top caused the rear of the C.M. to bounce and bump against the employee's right knee. This resulted in right knee effusion/strain.

Employee stated that he crossed the overland belt at the crossover and as he dismounted the last step his foot slipped and he felt something pull in the rear of his right ankle. This resulted in ruptured right Achilles tendon. Employee continued to work until 12/2/2013.

Employee stated that he was removing the block from behind the tire on the rock-duster. As the block was removed the duster rolled back and caught his hand between the tire and the block. Employee states that he jerked his hand back and felt pain in his elbow. Medical received on 11/21/13 diagnosed the employee with an elbow strain and the incident became reportable.

Employee was loading roof bolt plates onto the roof bolt machine and as he handled the plates on of the sharp edges cut him on the left index finger. This resulted in a laceration that required sutures to repair.

Employee was operating the shuttle car. As he exited the car to turn around and drive to the feeder, a piece of rock fell from the mine rib and struck him on the lower legs. This resulted in bruises and abrasions.

Employee stated that he was riding in the back of the mantrip as it came outside. As the trip passed another trip that was coming inside, his left knee struck against the incoming trip. This resulted in a left knee sprain. Employee continued to work until 08/12/2013. Incident became reportable on 09/10/2013.

Employee stated that he was assisting with moving the CM cable out of the roadway. As he lifted the cable and twisted to throw it out of the way he felt pain in his right hip. Employee continued to work until 08/08/2013 when he was diagnosed with a fracture of the right femoral head.

Employee stated that he was helping drop the CM cable. As he pushed up on the cable to unhook it, he felt pain in his left arm and left shoulder. Medical received on 08/14/2013 diagnosed the employee with tears of the left shoulder.

EE was backing the miner out of the cut of coal when the cable went under the miner. He went up to remove cable from under the miner and accidentally bumped the remote control box against the cable stand-off, activating the tram lever and then the panic bar. He was caught between the coal rib and the stand-off of the machine.
