EMPLOYEE STATED DROVE A WAYS AWAY FROM THE DUMPSTER TO GET OUT AND USE THE BATHROOM AND TRIPPED OVER SOMETHING. EE STATED NOT AWARE OF WHAT TRIPPED OVER. STATED EE HIT EE'S HEAD AND EE THOUGHT EE WAS OUT A MIN. OR TWO AND ARM FELT NUMB AND LIGHT HEADED.

INJURED HAD PUT UP EE'S SECOND BOLT IN THAT ROW AND NOTICED THAT IT WAS WIDE TO THE RIB. EE SWUNG THE BOOM OVER TO INSTALL AN EXTRA BOLT. EE STARTED DRILLING AND A SECTION OF ROOF FELL ON EE. STRIKING EE ON HEAD AND SHOULDERS.

INJURED WAS TRAVELING TO SECTION ON A LIL MAC THREE WHEELER WHEN A PIECE OF DRAW ROCK FELL STRIKING EE ON THE UPPER PART OF EE'S BODY. EE SUFFERED A LACERATION BESIDE EE'S LEFT EYE, REQUIRING 8 STITCHES. A CT\SCAN WAS DONE AND FOUND NO OTHER INJURIES.

INJURED WAS PUTTING HYDRAULIC OIL IN ROOF BOLTER. WHEN THE OIL CAN WAS EMPTY HE PULLED THE FILL HOSE OUT OF CAN, GRABBED THE OIL CAN WITH HIS RIGHT HAND AND THREW IT TO THE RIB. A SHARP EDGE ON THE CAN CAUGHT HIS GLOVED FINGER AND SHAVED THE SKIN FROM IT. IT WAS THE MIDDLE FINGER ON HIS RIGHT HAND.

Employee was backing up on rubber tired ride when he ran over rock / debris which caused him to hit ledge in roof and drag him off of ride.

Injured was operating a roof bolting machine in very low conditions. He was drilling a hole and was having trouble with the drill steel. While trying to get the steel in the pot, he caught his left thumb between the steel and chuck. Resulted in a cut/mashed and broken left thumb.

Injured was bolting top when a piece of rock fell from the top, striking him in his upper body, pushing him to the ground. He was setting up to install the outside bolt. After emergency room visit, was reported that nothing was broken, but he would be sore.

Injured was installing roof bolts. he put the resin up in the drilled hole, stuck the roof bolt in the hole, then slid his hand down the bolt in order to put the bolt in the chuck. His hand came into contact with a sliver of steel, from where the bolt was punched in manufacturing. the sliver of steel went through the side of his glove and into the small finger on his right hand.

INJURED WAS CHANGING A WHEEL BEARING ON A DAMASCUS LIL MAC BATTERY RIDE. WHILE KNOCKING THE WHEEL BEARING RACE INTO THE REAR-END THE RACE BROKE AND SENT A PIECE OF THE RACE INTO HIS LEFT ARM. THIS LEFT A PIECE OF STEEL UNDER THE SKIN ON HIS LEFT ARM.

INJURED WAS TRAMMING A SCOOP, HIT A HOLE, AND GRABBED TO CATCH HIMSELF FROM HITTING THE BACK OF THE SEAT AFTER THE BOUNCE. HIS LEFT HAND WENT BETWEEN THE DECK AND REAR SECTION OF THE SCOOP WHEN IT WAS CLOSING TOGETHER. THE TIPS OF HIS RING FINGER AND MIDDLE FINGER ON HIS LEFT HAND GOT CAUGHT BETWEEN THE TWO SECTIONS. STITCHES WERE USED TO CLOSE THE ENDS OF HIS FINGER TIPS.

INJURED WAS TRAMMING A BATTERY POWERED SCOOP HAULING SUPPLIES. AROUND BREAK 94 HE HIT A POTHOLE THAT JARRED HIM AND SLAMMED HIM INTO THE DECK OF THE SCOOP. INJURY WAS TO THE BACK AND NECK.

Employee was changing a tire on a forklift at Rockhouse Creek #2 Mine. He stated he was pulling on the tire when he slipped (lost footing) and he stated he twisted his lower back. Safety dept administered first aid treatment until ambulance arrived

Crew was traveling to section, pulled over to get out of the way of another ride. Crew pulled under cable the EE put his hand up to keep the cable away from his face, a rope hanger came loose and snagged EE pointer finger causing a laceration.

EE was bolting in No.1 heading, he had already put up his first bolt. He swung over to put up his second bolt, when he placed his steel against the top, a piece of coal broke loose hitting the him on the back.

Riding in a battery powered mantrip,at a belt drive boom hole, the mantrip driver decided to turn the mantrip around,the victim did not keep his head down in the low seam height and he hit his head on the mine roof.

EE was going to clean No.2 Entry when he turned the corner he hit an oncoming production scoop bucket to bucket. The impact of the collision caused the EE to be thrown into the top.

The EE was on a Mac 12 traveling to the outside. The EE had his hand on the side of the ride and his hand was caught between the ride and the rib.

The injured employee was crawling by the scoop. The scoop operator thought he was clear and already by him. The scoop bucket was steered toward the offside of the scoop catching the injured's leg, pinching it.
