EE was operating a roof bolter when a rock fell from roof, striking his left ring finger.

Employee was spot bolting top in an outby area. A rock 23"" x 26"" x 6"" thick fell from the rib hitting him in the back.

Employee was hauling a load, when his scoop ran over some draw rock and jammed his head in the top.

EE on #1 belt was working with another EE, the victum was in a pinch point area and was struck by battery operated 4-wheeler, pinning EE against Rib and mantrip.

A 30' x 18' x 4' thick roof fall occurred in No. 5 track entry one break inby spad No. 12822.

A 25' x 15' x 1.5' high roof fall occurred in No. 2 entry of 1 West Mains, 4 breaks inby spad No. 84.

Roof fell between #1 & #2 Entry near spad # 13640 in the Spruce Laurel Mains. The fall was 30' x 20' x 5' thick and will not be cleaned up. Will set cribs, build new stoppings and re-route life line.

A roof fall, 20'x20'x5' thick, occurred in the No. 1 entry of the Cazy mains 2 breaks inby spad 13050. The area has been breakered off.

A 40' x 20' x 5' high roof fall occured in the #6 entry intersection of the Old Cazy section. The area will be breakered off

A 25' x 20 x 8' thick roof fall occurred in No. 2 entry, one break inby spad No. 15846. The area will be breakered off and air will be rerouted into the no. 1 entry.

A 20'x20'x4' roof fall occurred in the intersection of 4 right off 2 west mains in No. 3 entry at spad No. 13919. The fall has been dangered off and breakers set. The fall will not be cleaned up.

Roof fall in #7 intake entry of Spruce Laurel Mains, 3 breaks inby spad #13367, 25' x 20' x 5' thick.

Roof fall approximately 50 ft. x 20 ft. x 6 to 8 ft. thick occur in the No. 5 intake entry, 90 ft. inby spad No. 13622. Area is dangered off, lifeline and stopping line will be moved/re-routed.

Roof fell in No. 3 entry at intersect 2 breaks outby spad 15918. Fall was 20 ft. x 20 ft. 5 ft. thick.

Roof fall occurred in #2 entry at spad # 105. Fall was 17' x 40' x 4' thick.

Roof fell between No. 1 and No. 2 entry inby spad No. 13640. Fall was 50' long by 18' wide by 8' thick.

Roof fall in Winifrede No 15 mine occurred 3 breaks underground at intersection from No. 4 entry to No. 3 entry, 1 break inby spad No. 21. Fall was 70 ft. long by 6 ft. wide and 5 ft. thick.

Roof fall occured in the Spruce Laurel Mains, Sycamore Section, No. 5 Entry 1 break inby spad No. 1365. Fall was 60 ft. long by 20 ft. wide and 8 ft. thick. Set jacks at fall and will be moving stopping to reroute air and lifeline.

Unintentional roof fall near EP #11. 8'(T)x 30'(L)x 19'(W).

Unintentional roof fall measuring 60'(L) x 20'(W) x 10'(T) located one break inby #7 Belthead.

Employee stated that he was riding 3-wheeler mantrip to #6 head when he decided to cross the track in a place he says he probably wasn't supposed to cross. He stated that he had to go really fast to cross there and when he hit the track it caused his head to bump top.

Employee was carrying a H. Jack and stepped on a rail & slipped.

EE was trying to put a bottom roller in the #1 Belt when he tore a laceration in his chest where his ICD was located. He did not start missing work until 12/1/2006.

EE was in man trip, when person driving jeep got too close to rib. EE's arm got caught and pulled over his head, pulling EE out of jeep.

Employee was crawling by the bolter when rib rolled out & piece of cap coal hit hip & lower back.

Unintentional roof fall measuring 35' x 19' x 6' on Cazy Section near spad #15809.

Pulling belt splice out with a hammer & piece of hammer splintered & went into right index finger by center joint.

Unintentional roof fall measuring: 40'(L) x 19'(W) x 8'(T) -occurring near spad # 15830.

Unintentional roof fall measuring 35'(L) x 21'(W) x 6'(H) on Cazy Mains 1st Right- 2 breaks inby spad #13065.

Employee was riding in a mantrip. He rose to see where he was and hit his head on a steel beam.

Unintentional roof fall 70'(L) x 22'(W) x 10'(H). 1 break inby spad 15918 on Cazy.

Employee said bolt was bent & he had a hold of the bolt trying to get it out of drill pod & it kicked back on his left wrist.

Doing bolting rotation, twisted wrist and bolt came back and hit his left wrist.

Employee was hoisting a belt with a set of come-alongs when the come-alongs slipped & struck him in the face. THIS IS NOT A LOST TIME ACCIDENT, BUT SUTURES WERE REQUIRED.

Employee was struck by a rib roll while walking towards a MRS.

While tramming a scoop in #4 Entry, employee contacted a metal sign & lacerated his face. THIS IS NOT A LOST TIME ACCIDENT, BUT REQUIRED SUTURES.

EE states he was rock dusting belt line, when it caught his finger on accident on roof bolt plate, and roof bolt plate cut his finger. Note: EE did not report accident until 11/16/06, when EE said it got infected.

Unintentional roof fall 100'(L) x 19'(W) x 4'(T) on 1 West Mains.

A piece of drawrock fell out from between the roof bolts, striking employee on his head & left forearm.

Unintentional roof fall 20'(L)x 19'(W)x 10'(T)- two breaks outby spad 15822.

Unintentional roof fall 70'(L) x 19'(W) x 10'(T) located @ 21 Xcut- Spad #15813.

Unintentional roof fall 40'(L) x 18'(W) x 3.5'(T). The fall was on 1 West Mains, 6 breaks outby intake overcast on 2 Belt.

The slate bar that the EE was using struck him in the mouth when the material that he was pulling down broke loose.

Employee injured his back while lifting a foam filled tire out of a mantrip.

Employee injured himself while squaring a belt. The razor knife slipped & cut his leg when it cut through the belt.

EE was driving 3-wheeler to the outside with co-worker following on another 3-wheeler pulling the duster when they went thru swag. Co-worker asked injured employee to wait at the top of the swag in case he needed him to pull him up, which he did. Injured employee went to back up and the brakes went out causing him to flip the 3-wheeler.

Employee was struck by a piece of rib 3'(L) x 8""(T) while roof bolting. He did not start missing work until 7/24/2006.

Employee inadvertently cut his finger while cutting belt. THIS IS NOT A LOST TIME ACCIDENT.

Unintentional Roof Fall measuring 80'(L) x 20'(W) x 4'(T) found @ 12:30 PM on 6/7/06. It was located @ spad 299 in # 3 Entry.

EE stated he was going out on the 4-wheeler transport and hit a bump, hurting tailbone and lower back.

Bridge operators were going up swag & making right hand turn & they slid into 4x4 posts that turn belts on/off causing 4x4 to fall into EE's left shoulder.

Employee injured his back while sliding an 8' timber around in a scoop bucket. He was preparing to saw the timber. He did not start missing work until 6/14/2006.

While walking across the section, employee felt a burning sensation behind his kneecap(L) & his knee kept locking up.

Unintentional roof fall measuring 40'(L) x 20'(W) x 6'(T) on #7 Belt in #5 Entry.

EE was getting ready to move a piece of curtain out of the roadway when he was struck on his ankle(R) by apiece of rock that fell out of the rib. EE returned to work on 3/28/06, but will be doing light duty until his ankle heals.

Roof fall in the intake. Measured 30'(L) x 16'(W) x 3'(H) in the #3 Belt Intake.

Ee was shoveling wider the belt in mud and twisted his back the wrong way as he was shoveling mud onto the belt.

Employee was taking belt tools on ride to a co-worker when rock fell from top and struck him in head and shoulders (in middle of shoulders).

Unintentional roof fall occurred in #5 Entry- 1 break inby spad #15127. Measurement: 20'(L) x 20'(W) x 6' to 8' (T).

EMPLOYEE WAS FILLING IN FOR SCOOP OPERATOR & WAS TAKING SCOOP ACROSS SECTION, GOT STUCK UP IN SWAG. CABLES IN ENTRY; WENT TO THROW OUT OF THE WAY (AGAINST RIB) & A PIECE OF ROCK FELL ON HIS LOWER LUMBAR REGION, HIS LEFT SIDE & HIP.

Employee pulled his knee(L) while working on a trainer in a dolly. He didn't start missing work until 3/6/2006 when his knee was operated on.

Unintentional roof fall: 125' (L) x 19' (W) x 6' to 8' (T). Casey Section 1 break inby spad 14978.

Employee was checking bridges when his right hand was struck by a rock. The wound required sutures.

Unintentional roof fall was found @ 7:45AM. It was 95'(L) x 19'(W) x 7'(T). This fall occurred on the Cazy Section approx. 200' inby spad #14979.

Unintentional roof fall 60'(L) x 18'(W) x 7'(T)- one break outby spad # 13379. Found 10/24/2005.

Unintentional roof fall 1 break inby Spad #13379 in # 5 Entry. Dimensions: 50'(L) x 20'(W) x 6'(T). Roof fall was found @ 3:15PM.

Employee was injured by a low hanging roof bolt that caught him on the right side of his face while he was riding a 3 Wheeler.

Employee was bolting top when the drill pot hung on a bolt & dropped on his knee (R).

Employee was trying to pick up a rock to clear walkway.

An unintentional roof fall was discovered @ 12:05 PM. Measurements: 120'(L) x 19'(W) x 8'(T). One break inby spad #13800 in # 5 Entry.

Cleaning wet rock dust from the auger with a screw driver. His hand got pulled into auger.

Employee cut his arm(L) while removing a band off of a bundle of steel straps. He struck the band with a hammer & the band struck his arm.

Employee was struck by a rock while trying to get a chain that was behind a scoop charger. A rock 8"" x 6"" x 5"" fell from the mine roof & struck his hand(L) & wrist(L).

The rachet broke that employee was using causing him to slip & injure his right shoulder. He started missing work on 12/9/2005.

Employee injured his wrist(L) when the rachet he was using stripped out. He did not start missing work until 8/13/2005.

Employee was awarded 10% ppd benefits for occupational pneumoconiosis.

EE was injured while riding a mantrip. While passing a disabled mantrip, He reached out of the mantrip he was riding to get a SCSR on the disabled mantrip. He became caught between the two mantrips.

Employee was picking up a rock and sprained lower back and neck.

The 3-Wheeler that employee was operating slipped and jerked the steering wheel causing his elbow to strike a piece of metal.

Employee's glove caught on the roof bolt that he was trying to install resulting in his left hand being fractured.

EE smashed his finger while cleaning a belt sensor.

EE re-injured his back while pulling a miner cable.

Employee was helping to pull on a miner cable. His foot got caught in loose coal and mud- twisting his knee.

EE WAS LIFTING LID OVER BATTERY AND LID FELL ON RIGHT HAND

The miner that EE was working on slipped off of blocks. EE's finger was smashed between the ground and the bar that he was prying with.

Unintentional roof fall found on 2/22/05 @ 5:10AM. Location: Spad # 133368 #2 entry to #3 entry. Dimensions: 90'(L) x 20'(W) x 6'(H).

EE was awarded an additional 5% occupational pneumoconiosis. (Please make sure that this was not submitted earlier).

EE injured his toe as he ran from a scoop when its batteries started popping. He slipped & fell in the mud. He fell on the end of his boot which bent his toes back.

Employee injured his left eye while he was letting pressure off of a grease jack on the #3 mobile bridge conveyor. The grease fitting blew out and struck him in his left eye.

Employee was awarded 10% for occupational pneumoconiosis.

Ee alleges to have injured his back on August 5, 2004, while he was pulling a miner cable. He reported this accident to mine management on August 25, 2004, & did not start missing work until September 7, 2004.

Employee claims to have injured his back while straightening a roof bolt for insertion. The bolt rotated with him while he was applying pressure- thus injuring his back. MINE MANAGEMENT BECAME AWARE OF THIS ACCIDENT ON 8/31/04.

EE was rewarded 5% ppd benefits for occupational pneumoconiosis.

Employee was putting a 6"" water line together when he felt pain in his low back. Employee has a history of back problems. Employee stated that his back would ""go out"" every so often.

Employee said that he was shoveling belt and sprained his back.

A ROOF FALL WAS DISCOVERED AT SPAD #13364 IN THE #5 ENTRY OF THE S. MAINS. THE FALL MEASURED 30' X 20' X 6' - 7' THICK.

EE was driving service vehicle and a piece of rock fell hitting his shoulder and causing a bruise.

Ee was lifting flight pump when he felt pain in his back. ee did not think he was injured and did not report accident until 6-15-04. he did not miss work or seek medical treatment until 7-15-04.

EE WAS CARRYING 60-TON JACK TO SET IN #2 ENTRY. HE FELT PAIN IN LOWER BACK. HE FINISHED HIS SHIFT BUT HAD TO GO TO THE DOCTOR (EMERGENCY ROOM).

Employee was awarded a 1.65% ppd benefits for noise induced hearing loss.

SHOVELING BELT - BAD SPLICE CAME BY AND CUT ARM.

EMPLOYEE WAS MOVING MINER CABLE WHEN HE TWISTED HIS RIGHT KNEE.

EE WAS CRAWLING IN ENTRY, MOVING THE CABLE OUT OF THE WAY, WHEN A ROCK FELL ON HIS LEFT HAND. THE ROCK MEASURED 12"" X 18"" X 1 1/2"" THICK.

EE was helping to install a cable bolt when a rock fell rib striking his ankle (L).

OPERATOR WAS DRIVING MANTRIP DOWN STEEP GRADE. EMPLOYEES HAD RAILS TIED OFF AT END OF TRACK. OPERATOR LOST CONTROL OF VEHICLE. EMPLOYEE JUMPED OFF MANTRIP & INJURED HIS RT SHOULDER.

ROOF FALL MEASURING 110' LONG X 19' WIDE X 10' THICK. LOCATION: NEAR SPAD #13392 ON SOUTH SECTION #'S 1-3 ENTRIES.

ROOF FALL IN THE NUETRAL ENTRY BETWEEN #4 & #5 ENTRIES NEAR SPAD #12479. THE FALL MEASURED 25' L X 10' W X 4' THICK.

EE WAS PULLING CABLE WHEN HE FELT PAIN IN HIS BACK.

A ROOF FALL OCCURRED IN #4 ENTRY OF #2 BELT OUTBY SPAD #1940. THE FALL MEASURED 50'L X 18'W X 4.5' THICK.

Employee was bolting top when he dropped the bolter head on his r. thigh.

EE STARTED MISSING WORK ON 9-18-03 IN ORDER TO HAVE CARPAL TUNNEL SURGERY. EMPLOYER WAS NOT AWARE OF REASON FOR ABSENCE UNTIL 9-29-03, WHEN NOTIFIED BY EE.

EMPLOYEE SLIPPED & FELL, TWISTING HIS LEFT KNEE.

MOVING A ROCK OUT OF THE WAY, IT ROLLED OVER HISHAND & INJURED HIS WRIST.

PUTTING STACK STEEL IN ROOF OBLTER HEAD TO DRILL A HOLE, GLOVE GOT CAUGHT WHEN HE STARTED ROTATION. BROKE LITTLE FINGER AND DISLOCATED RING FINGER ON LEFT HAND.

EE WAS WALKING UP THE BRIDGE LINE WHEN HIS HEAD HARD HAT STRUCK THE MINE ROOF AND SPRAINED HIS NECK.

WHILE OPERATING THE ROOF BOLTER, HE SAID HE RAISED FROM A SQUATTING POSITION TO INSET ROOGBOLT IN TOP AND FELT SOMETHING POP IN HIS LEFT KNEE.

WORKING ON A D6H PUTTING ON THE TRACK HAD CRANE HOOKED TO THE TRACK CHAIN SLIPPED ON TRACK & HITEE IN THE EYE & THE NOSE

PULLING MINER CABLE, HIS LEG GOT CAUGHT IN THE CABLE AND TWISTED HIS RIGHT KNEE.

INJURED WAS TRAVELING TO SECTION ON A 4 WHEELER MANTRIP. HE RAN OVER A ROCK IN THE ROADWAY AND FELT A PAIN IN HIS LOWER BACK.

EMPLOYEE JUMPED OFF THE TOP OF A BELT HEAD AND TWISTED HIS RIGHT KNEE.

HE WAS GROUND FAULTING BRAKER, RECEPTICAL ARCHED AND BURNT HIS LEFT HAND AND EYE BROW.

SAID HE HURT HIS BACK WHILE LIFTING BELT STRUCTURE.
