Struck against a moving object

After his shift ended, employee was driving up Blue Pennant Road from the mine site. He noticed two vehicles approaching Blue Pennant Road from Progress Coal. The vehicles did not stop at the stop sign. The second vehicle ran into the employee's right front fender area. He was transported to Boone Memorial. Doctors gave him two days off work.

A unplanned roof fall occurred in the #1 entry on the #9 belt line at spad 4423. There is not a producing section at this location. Size of fall 50' L. X 20' W. X 7' H. Ventilations and passage of person was not affected. Fall will not be rehab. Area was initially supported with 60"" Resin Bolts. federal and State Agencies notified immediately.

Struck against stationary object

EE was walking from his track jitney past the rock box at #3 tail piece to check a v-scraper. The top lowers at this point and the employee struck his head on the roof resulting in pain to the neck and between the shoulder blades. He missed no work until August 24, 2011 when doctors took him off work for a month for evaluation. He suffers from a degenerative disc disease.

Over-exertion (Not Elsewhere Classified)

While driving nails in the process of making belt splice, belt man felt pain in his left shoulder. He missed no work until July 1, 2011. His doctor took him off work to rest the shoulder until he could perform rotator cuff surgery.

Fall to the walkway or working surface

EE,stopped to open airlock doors on track. He slipped and fell, hitting his right forearm on the rack rail. He was wearing rubber boots and the tread could have been worn. He continued working. He later went to Med-express where he was diagnosed with a hairline fracture to the radius soft cast applied and he has not missed work.

Roof fall did affect passage of persons due to being in track entry. There is no active section at the location fall will be rehabilitated. Federal and State notified immediately.

Fall measures 25' long x 20' wide x 6' high. At this time mine is in a non-producing status. Federal and State Agencies were notified immediately.

An unplanned roof fall occurred in the #6 entry of the 001-0 section, this fall occurred at spad #4405, 2350' from active face. Area was developed March, 2010. Fall measures 35'L x 20'W x 5'High, area was supported by resin bolts. All entries have been breakered off. Passage of persons and ventilation was not affected. Fall will not be rehabilitated. Fed agencies notified immed.

An unplanned roof fall occurred at spad #4413 in the #1 entry return. The fall is located approx 2000 ft from active section in the return airway. Area was developed Feb 2010. Fall measures 20'L x 20'W x 6'H. Ventilation & passage of person was not affected. Area was supported by 48"" resin bolts initially. All entries have been breakered off. This fall will not be rehabilitated.

Over-exertion in lifting objects

While attempting to lift and connect flat car tongue to locomotive, the car tightened up against the scotch and moved 3-4"". The movement caused a sharp pain in abdomen.

Struck against a moving object

Injured was driving across the Blue Pennant access road on his way to work at Twilight MTR. The access road is not an active haulroad. The mine ID for this road is abandoned, however, the property is owned by Elk Run Coal. He fell asleep while rounding turn, at which time he drove his Dodge pickup under a parked 42' flatbed trailer parked on the side of the haul road.

Area was previously supported with 5' Torque Tension Roof bolts. area was developed on 3/1/2010. Ventilation and passage of personnal was not affected. Federal and state agencies were notified immediatly.

EE was struck with 2"" water line while it was being pulled with scoop. The rope broke causing waterline to strike EE causing him to fall against the rib.

Struck by flying object

Was using a hammer to remove a bolt from a track fishplate. When the rail freed up, the fish plate flew and hit his right knee causing him to fall down and injure himself.

Ventilation and passage was obstructed. Fall measures 20 feet long x 30 feet wide x 8 feet high. Area was developed 5/2009. Roof support consisted of 48"" resin roof bolts. State & Federal agencies notified immediately.

An unplanned roof fall occurred in the #8 Entry, 2 breaks inby spad #3155. The fall measured 20'LX18'WX4-5'High. This area was developed in August 2001 and suppored with 48"" resin bolts. Ventilation was not interrupted and passage was not hindered. Fall will not be rehabilitated. Jacks will be used to danger off area. Located approx 7000' from active section in return airway.

An unplanned roof fall occurred in #5 entry, spad # 4155 on the intake airway approximately 950 feeet from the working section. The fall measured 20' long, 40' wide and 5-8' thick. Ventilation was not interupted and no persons were affected. The strata is comprised of shale and sandstone, laminated with no rider seam present.

Struck by falling object

Injury decided to move his cable anchor which was loacted on the corner inby section dumping point. A piece of brow fell and struck him on the right foot. Tests revealed three fractures to the right foot.

Caught in, under or between a moving and a stationary object

While riding Brookville mantrip to section, EE fell asleep & allowed his foot to dangle outside trip. His foot was caught between rib & trip resulting in a bruise to right foot. He was released to return to work but did not show & was reported waiting to see specialist. After talking to him on 5/29/09, he said he had been under doctor's care & was receiving therapy & pain meds.

An unplanned roof fall occurred on 60-2 transfer belt, between 2&3 crosscut at spad 0270. The fall measured 8'X16'X30' on converyor belt and total fall was 60'X20'X4'. This area was developed in June 1997 and supported with 42"" resin bolts. A fall occured previously in the area in Aug 2005. Ventilation was not interrupted and passage was not hindered. Will be rehabilitated.

Caught in, under or between a moving and a stationary object

Employee was prying a bundle of 4' straps off of a supply car with a slate bar. The straps slid down the bar pinning his ring finger and little finger of his right hand between the bar and straps. This happened in # 3 entry, 2 breaks outby spad # 3792.

EE crossed the belt to remove a flag pin. He stepped off belt onto belt rail. His foot slipped off the rail & his leg went between the belt rubber & rail twisting his right knee. He finished his shift with a swollen knee & worked lt duty till 11/13/08, when he notified safety dept he had a torn meniscus & would have surgery 11/24/08. Off a minimum of 2 wks after surgery.

Struck against a moving object

Injured was attempting to free a stuck shuttle car using a scoop. The shuttle car slipped off the bucket causing the scoop to bounce, He hit his head on canopy. He was taking too Raleigh general hospital. Result x ray were negative but he was giving 4 days off and referred to family dr.

Caught in, under or between running or meshing objects

EE was loading tools into scoop bucket, he had his hand on ejector. Co-worker didn't see his hand and sucked ejector back in smashing the tips of EE's right ring and pinky fingers resulting in a laceration that required 23 stiches.

INJURED AND CO-WORKER WERE IN THE PROCESS OF PUTTING CUTTER DRUMS ON CONT MINER. AS INJURED BEGAN PLACING THE END DRUM RING ON, AS HE WAS STRIKING IT WITH A SLEDGE HAMMER, A PIECE OF METAL CHIPPED OFF THE HAMMER AND STRUCK HIM IN THE FACE BELOW RIGHT EYE.

EE WAS KNEELING ON TOP OF A CONTINUOUS MINER REPLACING A SPRAY BAR INSIDE OF THE SCRUBBER. WHEN HE STOOD UP HE LOST HIS FOOTING AND FELL ON TOP OF THE MINER, FRACTURING HIS LEFT LITTLE FINGER.
