Employee has been diagnosed with occupational Pneumoconiosis.

While setting Heizman roof jacks, Employee stepped on a piece of pipe that had been cut off, causing Employee to fall injuring his left knee. Employee had not missed work until 8/6/14.

An unplanned roof fall occurred in an intersection in the #1 entry of Tilley Mains in front of the #2 seals at spad location 0275. The fall measured 65' long X 18' wide X 7' high.

After his shift ended, employee was driving up Blue Pennant Road from the mine site. He noticed two vehicles approaching Blue Pennant Road from Progress Coal. The vehicles did not stop at the stop sign. The second vehicle ran into the employee's right front fender area. He was transported to Boone Memorial. Doctors gave him two days off work.

A unplanned roof fall occurred in the #1 entry on the #9 belt line at spad 4423. There is not a producing section at this location. Size of fall 50' L. X 20' W. X 7' H. Ventilations and passage of person was not affected. Fall will not be rehab. Area was initially supported with 60"" Resin Bolts. federal and State Agencies notified immediately.

EE was walking from his track jitney past the rock box at #3 tail piece to check a v-scraper. The top lowers at this point and the employee struck his head on the roof resulting in pain to the neck and between the shoulder blades. He missed no work until August 24, 2011 when doctors took him off work for a month for evaluation. He suffers from a degenerative disc disease.

While driving nails in the process of making belt splice, belt man felt pain in his left shoulder. He missed no work until July 1, 2011. His doctor took him off work to rest the shoulder until he could perform rotator cuff surgery.

EE,stopped to open airlock doors on track. He slipped and fell, hitting his right forearm on the rack rail. He was wearing rubber boots and the tread could have been worn. He continued working. He later went to Med-express where he was diagnosed with a hairline fracture to the radius soft cast applied and he has not missed work.

An unplanned roof fall occurred in the #4 entry at 1 break outby spad #2200. Fall measured 20'long X 20'wide X 6'thick. No equipment or injuries. Roof fall did not affect ventilation. Roof fall did affect passage of persons due to being in track entry. There is no active section at the location fall will be rehabilitated. Federal and State notified immediately.

An unplanned roof fall occurred in the #3 entry on the #2 belt @ spad 0567. Fall measured 25' long x 25' wide x 8' thick. No equipment or injuries, area was developed 6/97. Roof fall did not affect ventilation or passage of persons. The fall will not be rehabilitated. Roof support consisted of 48"" resin. State & Federal agencies were notified immediately.

An unplanned roof fall occurred in the #2 entry on #9 Belt Panel at spad 4402. Fall measured 20' long x 20' wide x 6' thick. No equipment or injuries. Roof fall did not affect ventilation and/or passage of persons. The fall will not be rehabilitated. Area was developed 3/2010. Federal & State agencies notified immediately. There is no active section at this operation at this time.

An unplanned roof fall occurred in the #3 Entry at spad #4408. This fall is 6 Breaks inby #9 Belt Head. Area was developed 3/2010 and was supported with 4 foot resin bolts. Fall measures 25' long x 20' wide x 6' high. At this time mine is in a non-producing status. Federal and State Agencies were notified immediately.

An unplanned roof fall occurred in the #6 entry of the 001-0 section, this fall occurred at spad #4405, 2350' from active face. Area was developed March, 2010. Fall measures 35'L x 20'W x 5'High, area was supported by resin bolts. All entries have been breakered off. Passage of persons and ventilation was not affected. Fall will not be rehabilitated. Fed agencies notified immed.

An unplanned roof fall occurred at spad #4413 in the #1 entry return. The fall is located approx 2000 ft from active section in the return airway. Area was developed Feb 2010. Fall measures 20'L x 20'W x 6'H. Ventilation & passage of person was not affected. Area was supported by 48"" resin bolts initially. All entries have been breakered off. This fall will not be rehabilitated.

While attempting to lift and connect flat car tongue to locomotive, the car tightened up against the scotch and moved 3-4"". The movement caused a sharp pain in abdomen.

Injured was driving across the Blue Pennant access road on his way to work at Twilight MTR. The access road is not an active haulroad. The mine ID for this road is abandoned, however, the property is owned by Elk Run Coal. He fell asleep while rounding turn, at which time he drove his Dodge pickup under a parked 42' flatbed trailer parked on the side of the haul road.

A unplanned roof fall occurred in the #2 entry at spad #4424 approx 210' from the active face. Fall measured 28'Lx20'Wx6 1/2"" thick. Area was previously supported with 5' Torque Tension Roof bolts. area was developed on 3/1/2010. Ventilation and passage of personnal was not affected. Federal and state agencies were notified immediatly.

EE was struck with 2"" water line while it was being pulled with scoop. The rope broke causing waterline to strike EE causing him to fall against the rib.

Was using a hammer to remove a bolt from a track fishplate. When the rail freed up, the fish plate flew and hit his right knee causing him to fall down and injure himself.

Ventilation and passage was obstructed. Fall measures 20 feet long x 30 feet wide x 8 feet high. Area was developed 5/2009. Roof support consisted of 48"" resin roof bolts. State & Federal agencies notified immediately.

An unplanned roof fall occurred in the #8 Entry, 2 breaks inby spad #3155. The fall measured 20'LX18'WX4-5'High. This area was developed in August 2001 and suppored with 48"" resin bolts. Ventilation was not interrupted and passage was not hindered. Fall will not be rehabilitated. Jacks will be used to danger off area. Located approx 7000' from active section in return airway.

An unplanned roof fall occurred in #5 entry, spad # 4155 on the intake airway approximately 950 feeet from the working section. The fall measured 20' long, 40' wide and 5-8' thick. Ventilation was not interupted and no persons were affected. The strata is comprised of shale and sandstone, laminated with no rider seam present.

Injury decided to move his cable anchor which was loacted on the corner inby section dumping point. A piece of brow fell and struck him on the right foot. Tests revealed three fractures to the right foot.

While riding Brookville mantrip to section, EE fell asleep & allowed his foot to dangle outside trip. His foot was caught between rib & trip resulting in a bruise to right foot. He was released to return to work but did not show & was reported waiting to see specialist. After talking to him on 5/29/09, he said he had been under doctor's care & was receiving therapy & pain meds.

An unplanned roof fall occurred on 60-2 transfer belt, between 2&3 crosscut at spad 0270. The fall measured 8'X16'X30' on converyor belt and total fall was 60'X20'X4'. This area was developed in June 1997 and supported with 42"" resin bolts. A fall occured previously in the area in Aug 2005. Ventilation was not interrupted and passage was not hindered. Will be rehabilitated.

Employee was prying a bundle of 4' straps off of a supply car with a slate bar. The straps slid down the bar pinning his ring finger and little finger of his right hand between the bar and straps. This happened in # 3 entry, 2 breaks outby spad # 3792.

EE crossed the belt to remove a flag pin. He stepped off belt onto belt rail. His foot slipped off the rail & his leg went between the belt rubber & rail twisting his right knee. He finished his shift with a swollen knee & worked lt duty till 11/13/08, when he notified safety dept he had a torn meniscus & would have surgery 11/24/08. Off a minimum of 2 wks after surgery.

Injured was attempting to free a stuck shuttle car using a scoop. The shuttle car slipped off the bucket causing the scoop to bounce, He hit his head on canopy. He was taking too Raleigh general hospital. Result x ray were negative but he was giving 4 days off and referred to family dr.

EE was loading tools into scoop bucket, he had his hand on ejector. Co-worker didn't see his hand and sucked ejector back in smashing the tips of EE's right ring and pinky fingers resulting in a laceration that required 23 stiches.

an unplanned roof fall occurred in the #1 entry between spad 02716 and 02728 in the tiller return. This is an idle mine, with no production. No injuries, no equipment involved. Passage of persons & ventilation were not affected. Fall measured 20'l x 20'w x 48"" h. Fall was inspectted by wv inspector.

Employee has been working in and around coal mines for 28 years. Has been diagnosed with carpal tunnel syndrome in the left hand. Surgery was performed on 4-21-05.

An unplanned roof fall occurred at one break outby spad 2826 in #3 outby above 7 belt. approx. 2200 feet from active face fall is approx. 40 feet long 20 feet wide and 7 ft high.

AN UNPLANNED ROOF FALL OCCURRED ON THE #2 BELT LINE, #4 ENTRY NEAR SPAD 212, NO ACTIVE WORKING FACE. DEVELOPED FEB. 1997. AREA WILL BE REHABILITATED. FALL MEASURED 50' LONG X 20' WIDE X 7' HIGH. NO ACCIDENTS REPORTED.

AN UNPLANNED ROOF FALL OCCURRED AT THE MINE ON THE TILLEY SECTION. ENTRY NO. 2 SPAD #0325 NUMBER 8 BELT. THE SIZE OF THE FALL WAS 60' IN LENTH, 20' WIDTH & 5' HIGH. THERE WERE NO ACCIDENTS REP ORTED.

A ROOF FALL WAS DISCOVERED AT THE BLACK KNIGHT MINE FOR BLK RUN COAL COMPANY. THE LOCATION OF THE FALL WAS ON THE MAINS, ENTRY 5 AND 6 CROSSCUT SPAD #3402. THE SIZE OF THE FALL WAS LENGTH 70' WIDTH 19' HEIGHT 10' ABOUT 320' FROM THR ACTIVE FACE WITH NO ONE INJURED.

AN UNPLANNED ROOF FALL OCCURED IN THE #6 ENTRY AT SPAD #2220 APPROX 6200' FROM FACE. THE FALL MEASURED 18'L X 16'W X 44"" H. ALL ENTRANCES TO THE FALL HAVE BEEN BREAKERED OFF & WILL NOT BE REHABILITATED.

AN UNPLANNED ROOF FALL OCCURRED ON THE TILLEY SECTION IN THE #3 ENTRY, 1 BK OUTBY SPAD 3150. THE FALL MEASURED APPROXIMATELY 60' L X 20' W X 7' H. THE FALL AREA HAS BEEN DANGERED OFF AND WILL BE REHABILITATED.

EMPLOYEE WAS DRAGGING A CONTINENTAL 48"" TOP IDLER FROM THE TRACK TO THE 48-4 BELT, WHEN HE FELT A POP IN HIS BACK, RESULTING IN A MUSCLE STRAIN.

EE WAS USING A BUMPER TYPE JACK TO SET SECTION TAIL PIECE. WHILE JACKING UP THE TAIL PIECE, THE HANDLE ON THE JACK DID NOT LATCH, THE JACK WAS UNDER PRESSURE AND THE HANDLE OF THE JACK STRUCK HIM ON THE RIGHT SIDE OF THE FACE.

AN UNPLANNED ROOF FALL OCCURRED IN THE #5 ENTRY 1 BRK INSPAD #2796 ON THE TILLEY MAINS 3,600 FT FROM THE ACTIVE FACE. THE FALL MEASURED 60' L X 20' W X 6 1/2' H. THE FALL AREA WILL BE BREAKERE D OFF BUT WILL NOT BE REHABILITATED.

AN UNPLANNED ROOF FALL OCCURRED IN THE NO.6 ENTRY ALONG THE #7 BELT LINE AT SPAD #2842. THE FALL MEASURED 20'LONG X 20'WIDE X 48'HIGH. VENTILATION WAS NOT AFFECTED. ALL ENTRANCES TO THE FALL WILL BE BREAKERED OFF, THE FALL AREA WILL NOT BE REHABILITATED.

INJURED AND CO-WORKER WERE IN THE PROCESS OF PUTTING CUTTER DRUMS ON CONT MINER. AS INJURED BEGAN PLACING THE END DRUM RING ON, AS HE WAS STRIKING IT WITH A SLEDGE HAMMER, A PIECE OF METAL CHIPPED OFF THE HAMMER AND STRUCK HIM IN THE FACE BELOW RIGHT EYE.

AN UNPLANNED ROOF FALL OCCURRED INT HE #6 ENTRY, SPAD 2840 ALONG THE #7 BELT. THE FALL MEASURED 20'L X 30' W X 5' H. VENTILATION AND PASSAGE OF PERSONS WAS NOT AFFECTED. THE FALL AREA WILL BE BREAKERED OFF BUT WILL NOT BE REHABILITATED.

AN UNPLANNED ROOF FALL OCCURRED ALONG THE #7 BELT IN THE CROSSCUT FROM 5 TO 6 NEAR SPAD #02797. THE VENTILATION WAS NOT AFFECTED. THE FALL MEASURED 40'L X 20'W X 48""H. ALL ENTRANCES TO THE FAL L WILL BE BREAKERED OFF & THE FALL AREA WILL NOT BE REHABILITATED.

AN UNPLANNED ROOF FALL OCCURRED IN THE NO. 7 ENTRY ALONG THE NO. 7 BELT LINE NEAR SPAD #02813. FALL MEASURED 20'L X 20'W X 48""H. VENTILATION WAS NOT AFFECTED AND THE FALL AREA WILL NOT BE REH ABILITATED. ALL ENTRANCES TO THE FALL HAVE BEEN BREAKERED OFF.

AN UNPLANNED ROOF FALL OCCURRED ON THE TILLEY SECTION #6 ENTRY NEAR SPAD #2801. THERE IS NO ACTIVE FACE, TRANSFER SYSTEM ONLY. THE FALL MEASURED 40' L X 20' W X 5' H. VENTILATION WAS NOT AFFEC TED. ALL ENTRANCES TO THE FALL HAVE BEEN BREAKERED OFF AND WILL NOT BE REHABILITATED.

AN UNINTENTIONAL ROOF FALL OCCURRED IN THE #2 ENTRY, 1 BREAK OUTBY SPAD 2832. IN A NEUTRAL ENTRY. THE FALL MEASURED 30'L X 30'W X 8'H. THE ENTRANCES TO THE FALL WILL BE BREAKERED OFF AND THE AREA WILL NOT BE REHABILITATED.

AN UNINTENTIONAL ROOF FALL OCCURRED IN THE #5 ENTRY ON THE TILLEY MAINS SECTION, NEAR SPAD #3072. THE FALL MEASURED 20'L X 20'W X 5'H. VENTILATION OR PASSAGE WAS NOT AFFECTED. ENTRANCES TO THE FALL WILL BE BREAKERED OFF. THE AREA WILL NOT BE REHABILITATED.

EMPLOYEE WAS PICKING UP A CRIB BLOCK OFF OF THE FLAT CAR. CO-WORKER ROLLED A CRIB BLOCK TO HIM & IT STRUCK EMPLOYEE ON THE RIGHT HAND.

AN UNINTENTIONAL ROOF FALL OCCURRED INT HE CROSSCUT BETWEEN 5 TO 6 CROSSCUT. NEAT SPAD #20801. ON THE TILLEY SECTION INTAKE VENTILATION WAS NOT AFFECTED. THE FALL MEASURED 10'L X 20' W X 4' H. AREA HAS BEEN BREAKERED OFF AND WILL NOT BE REHABILITATED.

AN UNINTENTIONAL ROOF FALL OCCURRED IN THE #5 ENTRY ON THE TILLEY SECTION MAINS, 1 BREAK INBY SAPAD #2756. THE FALL MEASURED 45'L X 20'W X 4'H. THE FALL AREA WILL BE REHABILITATED.

AN UNINTENTIONAL ROOF FALL OCCURRED IN THE #4 ENTRY ONE BREAK INBY SPAD #675 ON THE SOUTH PORTAL SIDE. THE FALL MEASURED APPROX. 90'LX 20'W X 15' H. THE FALL WILL BE BREAKERED OFF AT ENTRANCES TO THE FALL BUT WILL NOT BE REHABILITATED IN THIS AREA.

AN UNINTENTIONAL ROOF FALL OCCURRED IN THE #5 ENTRY ON THE TILLEY SECTION. INTAKE 1 BREAK INBY SPAD #3140. THE FALL MEASURED 20'L X 20'W X 4 1/2'H. ALL ENTRANCES TO THE FALL HAVE BEEN BREAKERE D OFF. THE FALL WILL NOT BE REHABILITATED. VENTILATION OR PASSAGE WAS NOT AFFECTED.

AN UNINTENTIONAL ROOF FALL OCCURRED IN THE #3 ENTRY NEAR SPAD #0271. THERE ARE NO ACTIVE FACES IN THIS MINE, TRANSFER OF COAL ONLY. SIZE OF FALL WAS APPROX. 40'L X 20'W X 8'H. AREA WILL BE BRE AKERED OFF AND TIMBERED AND WILL NOT BE REHABILITATED.

AN UNINTENTIONAL ROOF FALL OCCURRED IN THE #5 ENTRY ON TILLEY MAINS. (THERE IS NO ACTIVE FACE.) NEAR SPAD 2791. THE FALL MEASURED APPROX 50'L X 20'W X 4 1/2'H. ALL ENTRANCES TO THE FALL HAVE B EEN BREAKERED OFF. THIS AREA WILL NOT BE REHABILITATED.

AN UNINTENTIONAL ROOF FALL OCCURRED IN THE INTERSECTIONS OF ENTRIES 2 TO 3, 4 TO 5 NEAR SPAD #849. THERE IS NO ACTIVE FACE AT THIS TIME. ALL ENTRANCES TO THIS FALL HAVE BEEN BREAKED OFF. THE F ALL MEASURED 160' L X 20' W X 10' H IN 2 TO 3. 60' L X 20' W X 10' H IN 4 TO 5 ENTRIES. NOTE: DID NOT FALL IN BELT ENTRY.

EE WAS DRILLING TOP. HE HAD DRILLED A HOLE AND LOWERED THE HEAD. AS HE LOWERED THE DRILL HEAD, HIS RT. HAND WAS ON THE STEEL, THE STEEL CAME OUT OF THE DRILL POT STRIKING EE'S RT. THUMB, BETWEEN THE DRILL STEEL AND DRILL HEAD.

EE SPERARTED THE AC JOINT IN HIS RIGHT SHOULDER WHILE UNLOADING BELT STRUCTURE FROM A SCOOP BUCKET.

WORKING IN AND AROUND COAL MINES FOR A TOTAL OF 26 YEARS. EE HAS BEEN DIAGNOSED WITH NOISE INDUCED HEARING LOSS. ON 12-27-01.

AN UNINTENTIONAL ROOF FALL OCCURRED ON THE TILLEY MAINS 1 BRK INBY SPAD #3142. APPROX. 1300FT. FROM THE ACTIVE FACE, IN THE RETURN ENTRY. VENTILATION WAS NOT AFFECTED. ALL ENTRANCES HAVE BEEN BREAKERED OFF TO THE FALL. THE FALL MEASURED 20'L X 20'W & 6'H. THE FALL WILL NOT BE REHAB-ILITATED.

EE WAS KNEELING ON TOP OF A CONTINUOUS MINER REPLACING A SPRAY BAR INSIDE OF THE SCRUBBER. WHEN HE STOOD UP HE LOST HIS FOOTING AND FELL ON TOP OF THE MINER, FRACTURING HIS LEFT LITTLE FINGER.

EMPLOYEE WAS USING A MECHANICAL CRIMPING TOOL WHILE MAKING A HIGH LINE SPLICE. THE GLOVES HE WASWEARING WERE WET AND HIS HAND SLIPPED OFF THE CRIMPER CAUSING THE HANDLE TO HIT HIM IN THE EYE, CAUSING SMALL LACERATION.

AN UNINTENTIONAL ROOF FALL OCCURRED IN THE #15 ENTRY NEAR SPAD #3107 APPROX 1000' FROM THE WORKING FACE. THE FALL MEASURED 20' L BY 15' W BY 42"" H. ALL AREAS TO THE FALL HAVE BEEN BREAKERED O FF. THIS AREA WILL NOT BE REHABILITATED.

AN UNINTENTIONAL ROOF FALL OCCURRED IN THE #4 ENTRY NEAR SPAD 267 APPROX 7000 FT FROM THE ACTIVE FACE. THE FALL MEASURED 50' LONG 5' WIDE AND 4' HIGH. ALL ENTRANCES TO THE FALL HAVE BEEN BRE AKERED OFF. THE AREA WILL BE REHABILITATED.

HE WAS LOADING A SHUTTLE CAR WHEN A PIECE OF ROCK FELL FROM BETWEEN THE ROOF BOLTS HITTING HIM ON HIS RIGHT THIGH. ROCK WAS 14' L X 3' W X 1-5"" THICK.

WHILE CUTTING THE ANGLE FOR THE BIAS ON A VULCANIZED SPLICE HIS KNIFE SLIPPED AND HE STABBED HIMSELF IN THE LEG CAUSING A 1""-1 1/2"" CUT.

HE WAS SHOVELING BELT AND WHILE ATTEMPTING TO THROW THE MATERIAL UP TO THE BELT FROM THE BOTTOM, HE FELT SHARP PAINS IN HIS LOWER BACK. HE STOPED WORK AND WAS TAKEN OUTSIDE WHERE HE TRANSPORT ED HIMSELF TO THE HOSPITAL; HE HASN'T RETURNED.

WHILE INSTALLING WALKWAY AND HANDRAIL ON A COAL SILO, A SECTION OF HAND RAIL FELL 8 FT. STRIKINGEMPLOYEE ON LEFT SHOULDER, BRUISING SHOULDER. HE FINISHED SHIFT.

AN UNINTENTIONAL ROOF FALL WAS DISCOVERED WHILE TRAVELING THE NEUTRAL ENTRY AT SURVEY STATION #547 #3 ENTRY. THE FALL MEASURED 80' LONG X 20' WIDE X 42"" THICK. ALL ENTRANCES HAVE BEEN BREAKE RED OFF. PASSAGE OF PERSONS AND VENTILATION WAS NOT AFFECTED. AREA WILL NOT BE REHABILITATED.

AN UNINTENTIONAL ROOF FALL WAS DISCOVERED IN THE INTAKE AIRWAY ON 3-1-00. TIME AND DATE OF FALL IS UNKNOWN. THE FALL MEASURED 20 FT LONG BY 10 FT WIDE BY 42 INCHES HIGH. ALL ENTRANCES HAVE BEE N BREAKERED OFF. LOCATION IS APPROX 9500 FT FROMTHE WORKING FACE.

INJURED WAS REMOVING A METAL PANEL FROM A METAL STOPPING WHEN PIECE OF DRAW ROCK BECAME DISLODGED AND STRUCK HIM IN THE HEAD RESULTING IN A LACERATION REQUIRING 9 SUTURES.

AN UNINTENTIONAL ROOF FALL WAS DISCOVERED WHILE TRAVELING THE #2 NEUTRAL AIR WAY. THE FALL MEASURED 200 FT LONG X 20 FT WIDE X 4 FT HIGH. ALL ENTRANCES HAVE BEEN BREAKERED OFF. THE LOCATION IS AT SURVEY STATION 283 IN ADJACENT ENTRY 9700 FTFROM ACTIVE SECTION.
