Employee has been diagnosed with Occupational Pneumoconiosis. Employee was terminated due to reduction in work force on March 25,2016.
Injured was walking to the warehouse when EE tripped on a down drill that had been covered by the recent snow fall.
Employee was bending a roof bolt in #8 entry when the bolt slipped and smashed EE's right pinky finger between the bolt and drill chuck

An unintentional roof fall has occurred in #4 entry spad number 3516 across from 2 belt conveyor. No injuries reported and no equipment involved. the fall measured approximately 70`L X 20`W X 18`H.
Employee was changing sides with miner to clean up. Shuttle car operator had backed up to allow room. As he was trammimg the miner back to the face, the shuttle car op. started advancing and struck miner op. in the back.
Employee was installing a rail tie, Using a scoop to raise the rail and a slate bar to install the tie, The chain on the rail causing it to fall. The rail and slate bar struck employee's right foot
While Employee was walking over to 5 head through the crosscut he slipped in the mud and twisted his left knee. Did not go to the DR. till the 26th
Miner in #5 entry MMU 004 while replacing Rub rail on RT miner rub rail slipped off bracket and fell on Employees right foot. Did not miss any work until 10/06/2015.
As Employee was bending a bolt in the #2 entry he felt a pop in his back.
While installing the inside bolt in the first row in the Number 7 entry the employee had set the starter steel and as he was removing his hand a rock fell from the top landing on his hand smashing it in the Drill Pod resulting in a laceration and ten (10) sutures to his right index finger.
Injured along with 2 (two) other employees was hanging miner cable, When Injured felt a pull in lower back.
On 8/5/2015 Employee was attempting to collapse a grease jack on a shuttle car. While prying on the conveyor chain with a pry bar, bar slipped out of the hole, came back and his hands hit the bridge of his nose and front teeth resulting in a broken tooth and a small laceration on his nose. Employee did not miss any work due to this incident. Dental repairs were made on 8/12/15.
On 6-8-15 Employee was coming back from the miner with a load of coal employee hit two holes and jarred his back. On 7-16-15 Employee was going up #6 entry to the miner he hit a hole in the intersection causing his lower back to slam into the shuttle car seat. The safety department was notified on 8-17-15 Employee was under doctors care. Employee began missing work on 8-20-15.
While pushing 6' bolt, using the kicker on the side of the drill pot. The bolt bent in the center, smashing his right little finger. He received two (2) stitches in his fingernail bed.
While rock dusting, using a 4-wheeler and a pull behind duster. The duster became lodged on a timber near the roadway pulling the duster off the 4-wheeler. Employee while attempting to reconnect the duster lost his footing and fell with his left hand under the goose neck causing a laceration to his left index finger. Resulting in amputation from surgery.
Injured was hanging miner cable with 2 other employees, when he felt what he described as a pop in his groin area.
An unplanned roof fall has occurred at 34 break on the number 1 belt crosscut between spad number 171 and 173 in the secondary escape way measuring approximately 20' in length 11, in width and 4' in height. there were no injuries or equipment involved.
Employee has been diagnosed with Occupational Pneumoconiosis. The Safety Department received notification of his diagnosis on May 28, 2015. Employee is currently employed.
Employee lacerated his left thumb while cutting a shovel handle with a hawkbill knife which resulted in three (3) sutures.
While building return stopping injured stepped in hole and heard something pop in left knee. Continued to work performing normal duty's until 8/11/15, when he began experiencing complications, Elk Run conducted an investigation and concluded that the complications are contributed to the Feb 24, accident.
Employee has been diagnosed with Occupational Pneumoconiosis.
Employee was struck with the miner boom while making adjustments to the ventilation controls in the #5 entry. Employee was dragging ventilation curtain through the adjacent cross cut toward the miner which was located in the #5 heading when Employee contacted the moving boom.

Before the start of Employees on-coming shift Employee was bending over to pickup his boots. When he stood up he struck his head on the door of his locker causing a laceration which required eight (8) staples.
Employee was moving draw head by hand when he felt a pain in lower back. Internal investigation waiting on medical documentation before reporting due to a knot on his back.
While bolting a piece of rock fell and smashed Employee's left middle finger against the pot. Employee was treated at Raleigh General Hospital where he received seven (7) sutures.
While Employee was installing roof bolts his shoulder came out of socket. Employee has stated this has happened several times before. Therefore the operator has determined this is a medical condition resulting in occupational illness.
Employee has been diagnosed with Occupational Hearing Loss. Employee retired on April 24, 2014.

An unintentional roof fall has occurred Outby #2 Section 50ft inby spad 3977. The roof fall was approximately 60ft x 16ft x 5ft. There were no injuries and no equipment was involved.

Employee was spoting up for a stopping on the #1 section. While backing the scoop out of break, the door on the scoop opened. He attempted to shut the door while continuing to back the scoop up when his fingers were caught in between the door and coal rib.
Injured was attempting to close bolt drawer when he felt something pull his Right Arm. NOTE: Did not miss work untill 10-29-13
Injured was unloading chuck bolts from the back of the bolt machine on the Number 2 section when his left arm caught a 4' strap causing a laceration his forearm the acting. Injured went to medical facility and received ten sutures.
Employee was hauling a shuttle car wheel unit outside when the unit rolled part way off the car. He stopped the car and used a slate bar to pry wheel unit back on to car. He felt pain in his lower back. Employee did not miss work until March 13, 2014 due to back surgery.
An unplanned roof fall occurred on the #2 section in the #3 entry 1 break inby spad 4359 in the intersection and extends 20' over to entry 4 measuring about 35' long 20' wide and 8'-9' high

Employee was loading track rails from the old works and loading them on a rail car. They had a couple rails on the rail car that needed adjusted to lay right in the rail car. Employee grabbed the rails to move them, he grabbed the bottom rail to slide it down when the top rail rolled over pinching his left middle finger.
Shuttle car operator turned left out of #2 entry heading to #1 face. Flyboard at #2 face hung down slightly. Board caught canopy and hit right hand in the hand-hold. He was not wearing gloves. Board cut two fingers,requiring fifteen stitches. Given full release and worked next day. On 9/19/2012 he missed work, went to another doctor that took him off work.
Emloyee got out of the mantrip at #2 head where the #1 section was located. He was walking toward his toolbox when he stepped on a rock and twisted his right knee. The area is well rockdusted, dry and lights around. Employee continued to work his shift and refused medical treatment. After work he drove to Raleigh General and was given seven days away from work.
Track men were pulling rails to be installed using a forklift. They used braided nylon pull ropes looped through the rail and tied to the fork bar. A rail became fouled and the rope pressure broke the fork locking lever off. The lever hit one man in the left abdomen. Doctors placed him on restricted duty for three days due to the contusion he received.
Employee was bolting on MMU003 in #1 entry, spad #2770, when a 28""x4"" piece of draw rock fell from the roof onto his right hand. The height was about 6 1/2'. He received a fracture to his hand and was placed on restricted duty.
Roof bolt operator was pulling the machine cable from the reel to hang when he tripped over a timber and fell. No injury was reported. Jan.18,2012 he sought medical attention and was taken off work for a dislocated shoulder.

While roof bolting in the last row in the face of #3 entry, employee bent a bolt. While drilling a new hole a piece of rock that came loose on the bent bolt broke at his right forearm. He received a small contusion without breaking the skin. Doctors gave him two days of restricted duty. He returned to work on 11/11/2011.
Roof bolter operator had finished setting two 6' torque tension bolts & waited for his offside partner to finish his, when a rock 3'x 3'x 2"" thick fell from between bolts & knocked his hard hat off, hitting his right hand. A second piece of rock fell and lacerated the back of his head. (the roof is mixed & 7.5 feet.) The laceration required 8 staples. #2 Section, 4R, inby spad 2579
Roof bolter operator was bending bolts and as he stood up his back began hurting. He reported that he had several back surgeries in 2009. He was transported to CAMC General. He is off work receiving physical therapy.
The move crew was outby the section loading belt structure into a scoop bucket. EE bent over to pick up a top 48"" piece of belt structure, when he felt a sharp pain in his groin area. The incident was not reported to mine management until a month later when EE took off to have hernia surgery. Employees working with him confirmed the incident. 1st day away was 10/13/11
Third shift crew was replacing a bad pump located in the creek on the surface. The crew foreman strained his back and later sought medical attention. He was given medications and taken off work. He missed 3 days work during the following week. He RTW on 9/19/11 and has missed no more work. The safety department learned of this accident on 9/19/11 and began investigating.
He was pulling on an old fire hose to replace it. When the hose broke, causing him to fall back on the frame of a scoop. His elbow was slammed into the frame causing swelling and discomfort to his elbow.
Roof bolter operator was bolting in #5 right. He was operating the offside drill and as he swung it out to drill a hole, a rock (11""x10""x3"") fell off his canopy, striking his left shoulder. He complained of numbness but received no abrasions or bruising. He refused ambulance care and drove himself to Raleigh General where tests were negative. The doc gave him 2 days off work.
An unplanned roof fall occurred in the #6 entry of the 001-0 section. This fall occurred at spad #2297, 500' from active face. Area was developed July 2011. Roof did not affect ventilation and/or passage of persons. The fall will not be rehabilitated. Area was initially support with 48"" resin bolts. Size of fall 18w x 25L x 4'H. Federal & State Agency's were notified immediately.
EE was bolting #6 right on 002 MMU when a piece of draw rock measuring 18' x 8' wide fell and struck his right forearm. He was not wearing the Kevlar arm sleeves provided. He received a laceration. He refused ambulance care and drove himself to the VA hospital, where he received seven sutures.
An unplanned roof fall occurred in the face of #7 entry. Forty feet inby spad #2331. Federal and State Agency's were notified immediately.
An electrician placed his dinner bucket on the ground outside the parts building at the start of his shift. He twisted his back, instead of facing the location he chose to place the bucket. He felt a ""pop"" in his back and had to be immobilized immediately. He was transported to the hospital and is off work for treatment. He has a history of back injuries.
The miner operator was backing the machine out to set over when a piece of rock approximately 4' X 4 1/2' X 3/4 to 3"" thick fell from between bolts onto the miner and broke apart. The rock slid off the machine striking his left knee area. Doctors place him on restricted duty for two working days.
EE was bolting in #1 entry when a small piece of roof fell, striking his left index finger. He received a laceration and ceased work. He drove himself to the hospital and received five stitches. He returned to work on his next shift.
Employee was bolting #5 left crosscut on #2 section when a piece of draw rock (1' x 2' x 1"") fell onto the operator's side tray. It struck his left forearm and left a 4"" laceration. He was treated and sent to the Raleigh Boone Clinic where he received six stitches. He returned to work and continued bolting for the remainder of his shift.
Operator was bolting #3 entry on #2 section when small pieces of rock fell from the top, striking his right arm. He ceased work and drove himself to the hospital where he received three stitches for a small laceration to his arm. He returned on his next shift.
EE was squaring up belt to splice he was cutting toward himself the knife slipped ,came off the belt and cut a 1.5"" place above his right knee.EE drove himself to the Hospital after the shift ended and rec. three stitches. EE RTW on his next shift with instructions to review the accident and task train his crew on the correct way to use a knife.

EE was building a stopping between #5 and #7 entry. A block was stuck in the mud and when he tried to pull it out he felt pain in his back. He was transported to ARH in Beckley. Tests were negative. He returned to work until 10/4/2010 and went to a different doctor who has him off work.
EE was moving charger with fork lift when a fork fell off. He said he tried to replace the fork and dropped it on his foot. He continued to work until 11/8/10 when doctor excused him from work until he gets an MRI. The forks were checked by electrician and he found the forks locked in place and they could not fall off. New forks had been recently installed.
EE & co-worker were removing a defective conveyor tail roller on #34 cont. miner. The roller was stuck & couldn't be removed w/slate bar. EE used torches to cut ears off shaft when roller grease ignited in his face. He was transported to the hospital with 1st degree burn to face & difficulty breathing. He was intubated & sent for more testing at a specialized burn unit.
EE was helping co-worker tighten up a check curtain in the intake on #2 section. While reaching up to put a nail in the curtain, a small rock hit him on the left side of his face & forced his safety glasses into his face below the eye. A small cut on his cheek was cleaned & a butterfly bandage applied. He later went to see a dermatologist & received sutures for cosmetic reasons.
An unplanned roof fall occured in the #2 entry one break outby spad #1377. Fall occured 700 feet outby the working face. No persons or equipment involved. The fall measured 20' x 20' & a height of 7-8 feet. There is a rider seam 4' above the seam which measures 8-10"" thick. Area was supported with 6' torque tension bolts. Area will be rehabilitated. Agencies were notified by phone.
An unplanned roof fall occurred in the #5 Entry at spad #1384. This fall is 500 feet from the active face. Ventilation or passage of persons was not affected. Fall measures 40'L x 19'W x 10-12'High. Area was supported by 60"" Torque Tension bolts initially. All entries have been breakered off, area will not be rehabilitated. Federal & State Agencies were notified immdediately.

Area was supported by 60"" torque tension bolts. All entries have been breakered off, area will not be rehabbed. Federal and State Agency's were notified immediately.

Area was initially supported with 6' torque tension roof bolts and star pattern cable bolts. Fall measured 50'L x 40'W x 8-10'H. Federal & State Agency's were notified immediately.
While changing clothes, EE set down for 2 or 3 minutes talking to the guys when a clothes basket fell from the top of the bathhouse onto his head creating a 1"" cut. He refused an ambulance and drove to the doctor where he received two stitches. He returned to work on his next shift.
An unplanned roof fall occurred in the #7 entry, 1 break outby Spad #1383. This fall occurred 80 feet from the active face. Ventilation or passage of persons was not affected. Area will be rehabilitated. Area was initially supported with 6 ' torque tension roof bolts. Fall measured 30'L x 19' W x 6' high. Federal and state agencies were notified immediately.
EE took a lunch break and retrieved his bucket from the first aid car. A rock 3' x 2' x 4"" fell onto his right knee. X-rays were negative but made folow-up with specialist on June 3rd, when he received orthoscopic surgery for a torn meniscus. (two staples)
EE knelt down to check oil in 4 wheeler when he felt a pop in his left knee. EE stood upright then knelt down again and felt another pop. He was treated and released at Raleigh General with a follow-up on May 21, 2010 with an orthopedic. He determined that the EE had a torn meniscus and performed a procedure to insert two staples.
Supplyman was loading a motor with the aid of a chain hoist & slate bar. The clevis broke on the hoist & he fell backwards. He said he strained his back. He stated he was ok & continued working. It was learned on 6/10/2010 that he had seen a doctor & was working in a different job capacity. He was taken off work on 6/10 until the doctor releases him for his regular job.
Member was pulling rib and was hit when the rib rolled and pushed ee into the roof bolter.
Employee was preparing to go underground and was walking across supply yard to speak to the motor crew. As he was walking across yard he stepped on track rail and lost his footing due to the ice/snow that had fallen over the weekend, He fell and tried to catch himself with his right hand and lacerated his right hand in the palm area. Laceration required 6 stitches.
While using a hose w/fire nozzle to wash gob away from belt drive, EE said he felt his wrist snap. He refused an ambulance & drove for an x-ray which was negative. He had surgery on the wrist in May & returned with restricted duty until 6/10/2010. He is scheduled for release by the end of July. NOTE: Safety Dept was not told of his surgery or light duty until 6/10/2010.

Individual was picking trash out of scoop bucket. He slipped and fell in scoop bucket hitting his right elbow and falling on his self rescuer on his back. Scoop bucket was wet and muddy. X-rays were negative and he returned on his next scheduled shift. He worked until July 14, 2009 when he went to the doctor. Doctor gave him off until July 27, 2009 for back pain.
