Employee stepped on unknown object at the tail of R4 Ref Belt. The object came up and hit employee in the mouth busting their lip and breaking (3) teeth.
Employee was diagnosed with pneumoconiosis. Safety was notified on 05/30/2025 of this condition.

While removing impact rollers employee smashed left index finger resulting in a dislocation and laceration resulting in 6 sutures.
While getting dressed in the bathhouse preparing for work shift, floor fan was running and blew debris into employee's eye. Injured had eye flushed at medical facility and was given prescription eye drops.
Employee was lifting air compressor head to spin around when right thumb became pinched between the ratchet hook and air compressor head. Resulted in an avulsion that required surgery for repair. Employee began missing work on 02/16/2024.
While moving dirt with a mini excavator, the operator struck a supply line that provides Anhydrous Ammonia to the plant. The operator was overexposed to the Anhydrous Ammonia and was hospitalized for precautionary observation and treatment.

While observing the plant clean coal belt, employee stepped on a pallet and rolled ankle resulting in a small fracture.
Employee was coming down embankment, slipped on loose dirt and twisted right ankle.
Smashed finger between rock box and slate bar. Resulted in five (5) stitches.
Employee smashed right ring finger between door and frame on D-10 Dozer. Resulted in a fracture to the tip of the finger.
Employee was handling tin. Resulted in a laceration requiring three (3) stitches.
Was washing down floor. There was a small pile of magnetite in floor. Tripped over pile and tried to catch self. Little finger on left hand went into (through) a fan guard. Cut the end of finger off through the gloves.
Employee was carrying materials up and down a ladder. Employee missed the 2nd riser from the ground and fell backwards. Employee's left foot got caught in between risers and twisted it.
Employee was installing a chute on the back of a concrete truck and the chute came down on EE's middle finger cutting the tip of the finger.
Employee was welding a piece of metal on a sled when the tractor trailer started backing up and ran over employees right leg

While handling a piece of sheet steel that was leaned up against a rock box on metal runners, the bottom of the sheet steel slid out causing the steel to fall flat on injured's foot resulting in fractures. Steel measured approximately 5' x 12' x 1/4"".
Employee was standing on the 7G-3980 handrail on right side of radiator guard of D10R attempting to install mounting bolts on right lift cylinder, EE's foot slipped on railing and went down between the railing causing EE to fall toward front of machine tracks catching right leg inside of railing causing a fracture of leg
Replacing bolts in air actuated feed valve, pressure released, pinching finger causing possible fracture.
Employee had raised up a new section of decking using a chain fall to install a bolt. The hook on the chain fall slipped and struck the employee in the mouth causing a chipped tooth. The employee had the tooth repaired.
Employee was using a nylon sling to lift boom of a WA320 when the strap failed. This allowed an arm of the WA320 to fall upon employee's left foot. Employee was wearing safety toed shoes with metatarsal protection.
As dozer operator was coming off the #4 Clean Coal Pile, EE trammed over a 3 foot ledge EE did not see causing EE to hit EE's head on the inside of the cab. Injury resulted in a laceration requiring 9 staples.

While lifting a panel door to gain access to the engine compartment, the hinge pin on the door broke off dislodging the door and causing employee to lose balance and fall down between the push arm and the cats.
Employee was stepping off of the dozer when employee rolled ankle.
While installing a belly pan on 345 CAT Excavator, the corner of the pan came loose striking injured at the top of EE's forehead causing a laceration. Two staples were required.
Employee was attempting to anchor EE's clothes basket after pulling it up, the latch did not connect securely to the anchor and the basket dropped striking the employee in the head. Resulted in two staples.

Employee was raising low boy trailer deck by using the hydraulic levers located near the cab end of the trailer. A chain was hooked to the gooseneck of the trailer to secure it, the chain came under tension while adjusting deck causing clevis to break and struck employee in knee. Operator was notified of PT on 07/17/2019.
Employee was diagnosed with pneumoconiosis.
Miner was riding elevator from 3rd floor to 2nd floor. Miner reported that the elevator slipped approximately 4 feet causing miner to fall fracturing left radius.
Miner was putting belts on the #9 Slurry Pump. While rolling the belt onto the sheave EE's right pinky finger became caught between the sheave and the belt. Received seven stitches to the end of the pinky finger.
Employee was welding from an aerial lift when the basket bounced from movement and caught their hand between the handrail and adjacent piece of structure.
While conducting pre-shift on D10 dozer, miner slipped rolling right ankle. Accident resulted in fractured ankle.
While working on the Raw Coal Classifying Pump, miner was holding key steel for the bushing when EE's finger was hit by another miner attempting to use a hammer to drive key steel in.

Employee was adding transmission fluid to a dozer. As employee was pulling hose for slack it became entangled causing employee to lose balance and fall from the dozer.
Employee was stripping pump cable to enter it into the J-Box. EE was holding the hawkbill knife with left hand and pulling the outer jacket with channel locks with right. When the jacket came off, right arm hit the blade of the knife.
Employee was mounting D10R Dozer from right side. EE laid lunch bucket on track, grabbed handrail and when EE raised up EE's left leg to get on the step of the Pusharm employees right knee gave out.
Employee was carrying a 12""X12"" square 1"" thick plate roughly 25lbs. up stairs of the breaker building when EE felt a pain in upper back.
Employee was assisting in thawing out the dryer in the plant with a propane heater through the inspection door, When the dryer was ""bumped"" to try and start it. Dry material fell on to the propane heater and ignited. Injured jumped out of the way and injured EE's back.
Employees were trying to thaw a centrifuge with a propane torch. While attempting to bump the dryer to get it started; dry material was suspended in the dryer which came in contact with the propane torch causing a quick fire and burst of flames. Employee was transported to CAMC General then to Cabell Huntington Burn Unit for EE's injuries.

Employee was walking across 2nd floor of plant looking down at the floor, Employee walked into a piece of pipe hitting his head resulting in a chipped front tooth. Employee's tooth was repaired on 11/24/2015 during holiday/vacation week.
As Employee was walking he slipped on ice. Employee was treated at CAMC General Division.
While changing a seat base in truck, employee needed to unhook the wiring harness which had a cable tie on it. As Employee cut the cable tie the knife slipped resulting in a laceration to his right index finger. Employee was treated at CAMC General Division where the right index finger was amputated below the finger nail.

During shift change a head on collision involving two (2) personal vehicles happened at marker 28 on the Marfork Haul Road. A small hill in the road blocks visibility in both directions. This is a Surface Facility ID and the individuals involved work at an underground operation but must cross the Surface Facility ID to enter and exit the property.

Employee stepped on a 3"" to 4"" hump of slate made from dozer blade (slate was dry and hard) resulting in sprain/strain to his upper back.
EE called his supervisor at the end of his shift and said that he was going to stop at the hospital on his way home because he thought he was having a heart attack.(He had open heart surgery one year prior). The ER dr told him that his heart looked ok but he should see his cardiologist. The dr wants to do further tests. He wants to see if he strained himself on job.
The 4-A Raw Coal Feeder washed in on Raw Coal Reclaim belt. Employee was using water hose to wash material. Where Employee was standing, material had built up over his feet, and when he stepped out, he felt pain in his lower back and legs.

Employee was climbing into #1 Prewet Screen from the right side. Employee slipped off of the side and fell backwards across the stabilizing bar inside the screen. Employee was treated at MedExpress.
Employee has been diagnosed with Occupational Pneumoconiosis.
Employee was cutting trees and ground gave way causing him to fall coming in contact with saw. Causing a laceration to the right hand.
Employee was helping close doors on railroad cars and while putting pressure on bar to latch door the bar slipped out of the latch causing Employee to fall backwards injuring his lower back. Employee was treated at Raleigh General Hospital and the CT Scan was negative and received a full release. Management was notified Feb 20,2014 he would begin physical therapy.
As Employee was traveling on main haul road (on his way home) he fell asleep causing him to wreck his truck. He received minor injuries to his upper body.
As Employee was cleaning out the tripper chute on the plant clean coal chute while using a 10ft piece of conduit; the material broke loose causing the conduit to slip Employee pulled on the conduit and felt sharp pain in upper back and right shoulder. Employee was treated at Raleigh General Hospital and received a full release. Employee started physical therapy on 4/29/2013.

Employee was removing screws from a tin building. As he attempted to take screws out the tin slipped and Employee grabbed the tin causing a laceration on his left hand between index finger and thumb. Employee was treated at Raleigh General Hospital where he received eleven (11) sutures.
EE was installing a blade liner in a D10 dozer. There were 4 plate dogs and (3) 6"" weld stitches holding it. He laid down under it to place some weld on the bottom when all of the welds and dogs broke loose and it fell on him.
As Employee was standing on lower counter weight of a D10R Dozer bolting transmission into place he lost his footing and fell on the right side of counter weight striking left elbow. Employee was treated at MedExpress and xrays were negative.

Employee was in the process of closing out-riggers on a 2001 Kenworth Service Truck when his left finger was pinned between the out-rigger and tool box causing a laceration. Employee was treated at Boone Raleigh Medical Center where xrays were taken and received six (6) sutures.
While employee was tightening bolts on the Plant Feed Belt he felt a sharp pain on his left side. Employee was elevated and tied off with his safety harness. He was using his right hand to hold ratchet and left hand holding onto belt structure. Employee was treated at Raleigh General Hospital and was diagnosed with bruised ribs.
EE was installing a spray bar into 4A Feeder skirts. Employee and an electrician were rotating the bar. The electrician lost his grip, causing the bar to flip up, smashing Employees right middle finger into the top of the tunnel. Employee was treated at Raleigh General Hospital and diagnosed with a small fracture in the tip of his right middle finger and received 13 stitches.
Member was standing on 1' x 2' platform raising flop gate at head of reclaim belt when he lost his footing and slipped on the platform. Rain and slick conditions caused member to slip off of the top step and bump his shin and knee. Member saw a doctor where he was told to remain off work for a few days due to excessive swelling on his knee and fluid building up around the area.
Employee was working on the #2 clean coal screen. After completing the repair work to the screen deck, Employee was climbing across the mechanism when his arm slipped out from under him. Employee landed on the screen on his back and felt pain in his neck. Employee was seen at Raleigh General Hospital on 3/6/11 and xrays were taken. He is to follow up with a specialist on 3/16/11.
Member was tightening a plug in the rear end of the Kenworth Mud Truck and the ratchet slipped and hit him in the mouth, causing him to break a tooth. Member immediately ceased work to go have his tooth repaired by a dentist.
The employee was moving a spreader pipe out of a coal screen and caught his right middle finger in a pinch point between the spreader pipe and the screen. Infection has now become present in the wound so EE's physician requested he remain off work until infection clears up.

EE was lifting Oxy-Act bottles and pulled something in right arm and elbow.
Employee was inside Midd's Product Screen replacing a screen panel. He turned around to get a bolt and felt a sharp pain in his lower back.
Member was in the process of making a metal guard for a pump when the guard fell. Member tried to catch the guard and the piece of metal caused a laceration requiring 13 sutures in left index finger.
While in the process of installing a new belt head to extend the refuse belts, the forks of a IT28 Catapillar forklift fell off when they were raised up, stricking EE and causing a concussion.
As employee was exiting a door coming from the thickner into the plant his right foot hit the door causing it to stop opening and he ran into the door. This caused one of his front teeth to chip requiring dental work.

Employee loosened set bolts on a two piece light pole. In doing so the top section slid inside of the bottom smashing his left hand requiring 3 stitches.
While removing a side board wedge from #1 Midds screen with a 4lb hammer when a piece of metal went into his left hand the index finger
While pushing coal on the #4 Clean Coal Stockpile in a D10 Dozer, the dozer was pulled in a cavity. No injury occurred. The dozer and member were safely freed at 5:46 AM.
EE was looking in a metal tool box for welding rods. As he bent over, the lid fell hitting him in the back of the head, causing a laceration that required 3 stitches.
EE was walking across the parking lot near the preparation plant office when he hit his head on a metal skirt board which was protruding about 3' outby the raised tailgate of a parked truck, caused a cut that required 7 stitches to close.

Injury was working on the #2 clean coal O&R screen installing dripedge onto the side plate the piece of drip edge came loose due to the vibration of the plant running and fell onto his hand causing a laceration the required stitches to repair.
EE was performing maintenance on the #1 CC DNR Screen. A hold down was setting against the sideplate of the screen. The hold down fell over onto his hand, causing a laceration to the index and middle fingers of his right hand, required stitches to repair.
Member was shoveling material away from tailpiece and through a door to outside as he threw a shovel full of material he felt pain in hiw lower right side below his belt he continued to work until 5/22/08 when a swollen lump developed on his abdomen he went to the dr and was diagnosed with a hernia and is scheduled for surgery on 5/22/08.

While replacing spring pedestals in the #1 refuse dewatering screen, ee's feet slipped causing a strain to his abdomen. Marfork Safety was notified that he did not return until 11/19/07 on 11/12/07.
While trying to remove a piece of nylon band from a tail roller, it caused a laceration requiring 9 stitches on the left little finger.
Individual was using a slate bar to turn roll on rock sizer when the slate bar broke causing him to fall on left leg resulting in a fracture.
While working on a dozer starter EE lost his grip causing the starter to fall on his right knee. This is only 1 of the 3 traumas that has occurred to his right knee while working at Marfork. He continued to work until 7-24-07.

Employee was climbing onto beam and slipped. While attempting to catch himself his right hand came in contact with a hold down with a sharp edge resulting in a deep cut to the palm of his hand requiring stitches.
Member was taking discharge line off at pond pump. While jerking on line coupling broke loose and struck member in mouth upper lip requiring 7 stitches.
Member was holding a primary belt scraper while it was being cut with a reciprocating saw. The scraper started vibrating and he moved his hand closer to the saw blade when doing so the saw blade jumped out of the cut and punctured his hand between his little finger and ring finger at the knuckle. 3 stitches were required to treat the injury.

EMPLOYEE WAS WELDING LINER PLATES IN A TRUCK DUMP BIN ALONG WITH TWO OTHER EMPLOYEES. PROPER WELDING HOODS WERE WORN, BUT THE FOLLOWING MORNING HE WOKE UP WITH IRRITATION TO HIS EYES WHICH WERE DIAGNOSED AS WELDING FLASH.
EE WAS CHANGING OIL IN COAL REFUSE SCREEN EXCITERS ON REGULAR MAINTENANCE. HE WAS TRYING TO LIFT THE USED OIL OUT OF THE SCREEN WHEN HE FELT A SHARP SEVERE PAIN IN HIS LOWER BACK. TAKEN TO HOSPITAL, X-RAYS NEGATIVE & NO DIAGNOSIS COULD BE GIVEN. 10/15 MRI RESULTS SHOWED 2 HERNIATED DISCS & 1 BULGING DISC. SAFETY WAS NOTIFIED OF MRI RESULTS ON 10/21/2005
CHAINING DOWN A D10 ROLLER FRAME THE BINDER SLIPPED AND EMPLOYEE FELL FOWARD AND STRAINED BACK
Employee was attempting to close the door of a railroad coal car when the bar he was using slipped, causing him to fall and fracture his right ankle.
EMPLOYEE WAS GETTING OUT OF BOOM TRUCK & LOST HIS FOOTING, TWISTING HIS ANKLE.
EE WAS PULLING ON 1 1/2"" WATER LINE WASH DOWN HOSE STUCK IN MUD. WHILE PULLING ON HOSE, FELT SHARP PAIN IN LOWER BACK. DUE TO SCHEDULING AT PLANT, FIRST DAY OFF WAS 1/13/05.

member was attempting to close door on railroad car while prying his bar slipped causing member to loose his balance and fall and broke his ankle.
Member was using sledge hammer to drive bearing on middling's shaft. Sledge hammer hit something else, causing member to hit left index finger on plate welded on four inch pipe, causing laceration on finger requiring 10 sitches.
Member was prepairing to weld on a d-10 cab pan he went to move a 1/2"" impact gun from dozer track it slipped and hit member in mouth knocking his upper left front tooth loose.
Member was using impact gun. It was not working correctly, it was bypassing on occasion. Member was holding extension & socket in hand and hit trigger. Gun started roll pin in extension, cut ring finger on left hand.
MEMBERS WERE INSTALLING WALKWAY TO REFUSE CRUSHER. THE PLANT FEED BELT HAD SHUT OFF. INJURED WAS WELDING ON THE WALKWAY WHEN THE BELT RE-STARTED. A PIECE OF ROCK FELL OFF AND STRUCK HIM ON THE HEAD. MARFORK SAFETY DEPT. LEARNED OF THIS ACCIDENT REQUIRING STITCHES ON 7/14/2004.
While trying to open a void on the #5 clean coal stockpile, employee while operating a Caterpillar D9 dozer fell into the void. Recovery was successful with no injuries.

EE WAS ATTEMPTING TO GET A CRIB BLOCK OUT FROM UNDER A PIECE OF METAL PLATE MEASURING 1/2"" X 4' X 5'. IT HAD A SHARP EDGE ON IT LACERATING HIS LEFT INDEX FINGER REQUIRING 17 STITCHES. EE WAS N OT SCHEDULED FOR WORK UNTIL 11-3-03.
EE WAS CARRYING A 5 GALLON CAN OF HYD.OIL OUT OF THE SUPPLY TRAILER. WHEN HE SET IT DOWN HE FELT A PAIN IN HIS LOWER BACK.
EMPLOYEE WAS RAISING THE DOOR ON A LIGHT PLANT TO CHECK THE FUEL LEVELS WHEN THE DOOR SLAMMED SHUT ON HIS RIGHT THUMB.
EE WAS CUTTING A PIECE OF BELT WITH A UTILITY KNIFE AND THE KNIFE SLIPPED RESULTING IN HIM LACERATING HIS LEFT THUMB REQUIRING THREE SUTURES.
EE WAS RELACING A SCREEN IN #2 MIDDLINGS CLEAN COAL SCREEN. HE DREW BACK A 4 LB HAMMER AND STRUCK HIS FINGER ON A PIECE OF ANGLE IRON RESULTING IN A FRACTURE TO HIS RIGHT LITTLE FINGER. HIS FI RST DAY AWAY FROM WORK WAS 6/26/03.
EE WAS PUSHING RAW COAL FROM BELOW THE #2 COAL TRUCK DUMPING SITE AT MARFORK PROCESSING. AS A TRUCK BEGAN TO DUMP HE QUICKLY BACKED THE DOZER UP STRIKING THE HIGHWALL WITH THE COUNTERWEIGHT ON THE REAR OF THE DOZER RESULTING IN A JAR TO THE DOZER'S CAB PRODUCING TWO HERNIATED DISCS.
INJURED AND CO-WORKER WERE IN THE PROCESS OF UTILIZING A CRANE HOIST TO LOWER A 19"" VALVE WITH AN AIR ACTUATOR ON IT TO THE BASEMENT FLOOR. AS THE VALVE APPROACHED THE FLOOR INJURED REACHED OU T TO TURN IT AROUND WHEN A WIRE CHOKER SLIPPED ON THE VALVE LACERATING HIS LEFT RING FINGER RESULTING IN 12 SUTURES.
EE WAS ROTATING A #2 SCREEN BOWL BY HAND AND A METAL BURR ON IT CUT HIS LEFT THUMB. HE RECEIVED9 STITCHES IN THE LACERATION.
EE WAS ATTEMPTING TO REPLACE A ROLLER ON A TAKEUP CARRIAGE WHEN IT SUDDENLY MOVE, STRIKING HIS LEFT KNEE CAUSING AN ANKLE INJURY WHEN IT HIT HIS LEFT ANKLE. HE WAS RELEASED ON RESTRICTED DUTY.
EMPLOYEE WAS CUTTING A DRYER DISCHARGE CHUTE WHEN A PIECE OF HOT SLAG WENT BEHIND HIS GOGGLES AND ENTERED HIS LEFT EYE. HE WAS GIVEN A PRESCRIPTION FOR HIS EYES.

WORKING ON REFUSE SCREEN IN AN AWKWARD POSTITION HE LIFTED SCREEN OUT AND STRAINED HIMSELF.
EE HAD FINISHED HIS SHIFT AND WAS TRAVELING HOME. THE PROCESSING PLANT WAS MOVING A D-10 DOZER FROM THE CLEAN COAL TO THE RAW COAL STOCK PILES. THEY HAD THE ROAD BLOCKED WITH A PICKUP WITH THE EMERGENCY FLASHERS ON. EE CHOSE TO DRIVE AROUND & COLLIDED WITH THE DOZER.
WHILE ATTEMPTING TO REPLACE A BEARING ON A DRYER CIRCULATING FAN HE PLACED A PIPE WRENCH ON THE FAN'S SHAFT TO SECURE IT. THE OTHER FANS CREATED A DRAFT CAUSING THE SHAFT TO TURN CATCHING HIS HAND BETWEEN IT & THE DRYER WALL FRACTURING HIS RIGHT THUMB.
COMING DOWN THE ACCESS TO THE SCALPING SCREEN HE HAD BEEN CHANGING OUT THE MECHANISM ON WHEN HIS FOOT SLIPPED. HE LOST HIS BALANCE AND FELL BACKABOUT 2 1/2 FEET TO THE GRATED FLOOR. HE STUCK O UT HIS LEFT ARM TO CATCH HIMSELF.
THIS EE INFORMED HIS SUPERVISOR OF INJURY IN DEC. 2001 BUT DECLINED MEDICAL TREATMENT. NO ACTION WAS TAKEN BY THIS EE UNTIL 8/27/02, WHEN INJURY CAUSED HIM TO STOP WORKING. INJURY REPORTS COMP LETED AND SUBMITTED TO THIS OFFICE 9/20/02. MEDICAL TREATMENT REQUESTED BY DR. ON SEPT. 4TH AND SEPT. 27TH, 2002. NO PREVIOUS REPORTS WERE GIVEN TO THIS OFFICE PRIOR TO THE ABOVE DATE.LT. DUT
EE CLAIMS THAT WHILE USING A CONCRETE TROUGH OR CONCRETE ""COME-A-LONG"" TO SMOOTH OUT CONCRETE, HE FELT PRESSURE ON HIS LOW BACK. HIS SUPERVISOR WAS NOT INFORMED UNTIL 7/22/02 UPON CALLING TO F IND OUT WHY HE WASN'T AT WORK.
EE STATES THAT HE HAS HAD PAIN FOR SOME TIME, BUT NOW IT IS GETTING WORSE AND SPREADING. EE HAS MISSED NO WORK AND HAD NO MEDICAL TREATMENT TO DATE. EE WAS LAID OFF ON 5-4-02 AND EMPLOYER WAS NOT NOTIFIED OF ALLEGED INJURY UNTIL 6-4-02 WHEN THE WORKER'S COMP. FORM WAS RECEIVED. EE STATES THAT HE DOES NOT KNOW WHEN INJURY OCCURRED.
EE WAS CUTTING PIPE AND AS HE COMPLETED HIS WORK 1/2 SECTION OF 10-INCH VICTIM COUPLING WAS DROPPED FROM 15 FT ABOVE, STRIKING HIS RIGHT FURE ARM. IT WAS DISCOVERED THAT ANOTHER CONTRACTOR LOS T CONTROL IF THE COUPLING AND DROPPED IT FROM HIS WORK AREA ABOVE.
EE WAS LOADING STEEL WITH A BOOM TRUCK AND GOT RIGHT RING FINGER CAUGHT BETWEEN LOAD AND OUTRIGGER.
EE WAS MOVING BELT SECTION AND PINCHED HIS THUMB BETWEEN TWO PIECES OF CHANNEL.

EMPLOYEE WAS ASSEMBLING A BOLT STRUCTURE AND A PIECE OF CHANNEL PINCHED HIS FINGER BETWEEN IT AND ANOTHER PIECE OF CHANNEL.
EMPLOYEE WAS COMING DOWN A COLUMN & HIS LEFT FOOT HIT A CLIP THAT WAS BOLTED TO X-BRACING.
EE WAS DRIVING A CHISEL BETWEEN THE B PRESS FEED PUMP AND THE DISCHARGE FLANGE. WHEN HE STRUCK THE CHISEL IT BECAME AIRBORNE, STRIKING HIM IN THE MOUTH, CUTTING HIS LOWER LIP AND CHIPPING TWO OF HIS TEETH.
EMPLOYEE WAS OPERATINGA JACKHAMMER ON THE 3RD FL OF THE PLANT. A FIRE EXTINGUISHER LOCATED ABOVE HIM WAS SHOOK LOOSE AND FELL, STRIKING EMPLOYEE ON RIGHT ARM.
EE WAS OPERATING COAL TRUCK ON A HILL WHEN THE TRUCK STARTED BACKWARDS. EE LOST CONTROL AND FLIPPED TRUCK ONTO SIDE.
EE WAS OPERATING A WEED-EATER GROUND THE #Z REFUSE BELT WHEN IT THREW A ROCK STRIKING EE ON THE L WRIST. EE FIRST MISSED WORK ON 9-14-01.
EMPLOYEE WAS COMING DOWN 2ND FLIGHT OF STAIRS APPROACHING BASEMENT. HE WAS CARRYING 3 BUCKETS. TWO OF THE BUCKETS WERE EMPTY, BUT ONE CONTAINED SOME OIL. HE FELL APPROX 5 STEPS LANDING WITH HI S RIGHT LEG UNDER HIM. THIS RESULTED IN TORN LIGAMENTS TO THE RIGHT KNEE.
EE WAS PREPARING A CONVEYOR BELT FOR A VULCANIZED SPLICE, BY PULLING ONE INCH STRIPS OFF OF CONVEYOR BELT. THE STRIP THAT HE WAS PULLING BROKE. HE LOST HIS BALANCE AND FELL BACKWARD HITTING TH E BACK OF HIS HEAD ON OUR 1-TON TRUCK CUTTING HIS HEAD BEHIND HIS LEFT EAR REQUIRING 5 STITCHES.
EE WAS HELPING ANOTHER EE REPLACE A CHAIN ON A COAL FEEDER BREAKER AND GOT HIS LEFT RING FINGER CAUGHT IN THE PINCH POINT BETWEEN THE CHAIN & SPROCKET. EE WAS WEARING METACARPAL GLOVES AT THE TIME.

MEMBER WAS ATTEMPTING TO REMOVE BACK PLATE OFF OF A SLURRY PUMP OUT OF THE PREP PLANT WHEN THE SLATE BAR HE WAS USING TO PRY WITH SLIPPED HE TRIPPED & THE BAR FELL STRIKING HIM ON THE EYE ABOVE
EE WAS WASHING DOWN THE PLANT. HE LIFTED A GRATE COVERING A DRAIN AND FELT A PAIN IN HIS LOWER BACK.
EE WAS CUTTING A PIECE OF BELT RUBBER INSIDE THE STACKER CHUTE AND THE RAZOR KNIFE SLIPPED & CUT HIS LEFT HAND IN THE PALM AREA.
EMPLOYEE ASSISTED ANOTHER EMPLOYEE WITH THE LOADING OF STEEL BEAMS--THAT WERE LYING FLAT ON PAVEMENT THAT MORNING--ONTO A FORKLIFT. LATER THAT DAY, HIS BACK BEGAN TO ACHE & BECAME WORSE OVER THE NEXT COUPLE OF DAYS. HE WORKED 6/25 AND 6/26 AND WENT TO THE DOCTOR ON 6/28/00.
INJURED WAS GETTING OUT OF TRUCK TO LOAD HIS TRUCK WITH LOADER. AS HE GOT OUT OF HIS TRUCK HE BEGAN EXPERIENCING PAININ HIS LOWER BACK. NO MECHANISM OF INJURY.
PROLONGED EXPOSURE TO HES WHICH CAUSED CHEMICAL BURNS.
