Employee was lifting an Omega block to the top row of a stopping and felt pain in his left shouder, but continued woking. He called the mine office on 8/18/09 and informed them that he had gone to the doctor and rotator cuff surgery had been scheduled and that he would be off work from that date.

This injury is being CONTESTED since he worked 2 weeks before having a problem.
