Member was using a knife to cut some belt to make a belt splice with, while cutting the piece of belt, the knife slipped and cut his left index finger. The cut required three stitches.

While in the process of changing the shaft and nut on the short arm of the offside drill boom, the ratchet slipped off, causing hand to hit the roof bolter frame. This caused a laceration to the right thumb and required 4 stitches.

While tramming continuous miner into #4 entry the operator stopped miner to take slack rope off of cable, while doing so a loose piece of the rib rolled out and struck the miners right pinky finger causing a laceration that required 3 stitches.

A roof fall measuring 50' X 16' X 5'-6' fell approx 1500' from face between entries 2 & 3 at spad #7519 & 7509. It was timbered and dangered off. There were no injuries.

While attempting to start roof bolting, the 15' cut in #5 crosscut to the left, employee noticed that the right rib was loose. As he started to get a slate bar, the rib fell on his right foot, fracturing it. The rib was 18' long and 18"" to 5"" thick.

A roof fall occured at approximately 2:40 pm at White Queen Mine in the # 1 entry in-by spad #7790. The fall measured App. 4' to 6' thick and 90' long. Their were no injuries and no equiptment involved. Fall has been cribbed off and will not be cleaned up.

A fall was discovered on 6/15/2006 in the #2 entry 2400' outby the active working section. The area was mined in Janurary 2006. 48"" resign bolts were being used with 6' torque tension bolts. Fall will not be cleaned up. All approaches are being timbered or cribbed. No passage of men was obstructed. No injuries reported. Fall was 23' long by 20'wide by7' high.

On May 20, 2006 due to degenerative changes caused by repeated trauma over a period of several years, employee was diagnosed by medical records to have degenerative conditions causing severe back pain.

On 2-22-2006 employee was getting into the Catipilliar IT-28. As he pushed off the ground he felt a pop in his right knee. He continued to work until 2-27-2006 and due to increased pain he took off work.

Fully Grouted Resin Bolts were being usedin this area, fall will not be cleaned up. All approaches have been supported. No injuries occured. Fall was 13,200 ft from working section. Area Examined on a weekly basis.

Area Examined on a weekly basis.

On 1/27/2006 a roof fall occured on the #1 working section of white queen mine. the fall measured 90' long X 18' wide X 10' thick. no injuries were reported. The fall was cleaned up and re-bolted with 6' fully grouted bolts, 8' and 12' cable bolts.Additional bolts were installed on all sides of fall.

While unloading 48"" Continental belt structure, employee felt a pain in his back. He continued to work until 4/23/05. Due to pain he visited the hospital and was referred to a specialist. The Marfork Safety Dept. was notified on 5/4/05 that he could not work due to a muscle strain.
