While pillaring the PCC Mains mining in the #4 entry second lift on the left side a large rock fell out and pinned the head. While trying to work the miner pivoting back and forth the breaker knocked on the side of the miner. A retrieval plan was implemented to use a retriever to pull the miner out from under the rock.

While conducting his weekly examination the fire boss identified a roof fall in the #9 entry near spad #6887. The fall was approx. 20'W X 160'L X 4'-5'H the area was supported with 4' fully grouted resin bolts. The area will not be cleaned up and the area has been dangered off and timbers or cribs set.
Employee was tramming a Fairchild 35C scoop from outside to the section in the secondary escapeway (roadway), when dust flew in his eye. This is to report medical treatment only no days were missed.
Employee was bolting top when his drill steel got stuck; he reached to pull it out & it slipped through his hands.The drill steel hit the drill pot this caused the drill steel to bounce up & knock the employees safety glasses off.The hot drill bit then hit him in his eye & caused a burn which required medical treatment.This is to report medical treatment only NO days were missed.
Employee operated a CM for twenty years and noticed pain in his hands. He was later diagnosed with CTS and had surgery 4/30/13. He was placed off work on 5/1/13 and continues to be off.
Employee was helping lap belt when the fork fell off the fork loader. The employee picked up the fork and put it back on and felt a sharp pain in his lower back. The employee was treated and released with a referral to a specialist for an MRI. It was later determined the employee had a bulging disc and remains off.

The employee was building a stopping when a 4-wheeled rubber tired mantrip suddenly drifted off. The injured twisted his right ankle when he attempted to stop the mantrip. This resulted in a f ractured bone above his right ankle.
While walking quickly to turn off the remote switch the employee tripped and fell on the moving belt which caused his left leg to get caught between the belt structure. This caused a fracture of his left foot, fracture of his third toe and a laceration on his fourth toe. This is to report medical treatment only no days were missed.
During the preshift exam the fireboss identified a roof fall on the 2nd Lt. Section 450 outby the face in the #5 entry. The fall measured 20'W X 100'L X10'H.The area was supported with 5'resin bolts and the intersection with 8' cable bolts. The area was dangered off and cribs set. The fall will not be cleaned up and the section will be moved.
While operating a Roof Ranger II from the operator side installing the last row of bolts in the number #7 entry a piece of draw rock fell striking the employee on his right hand. Employee was tramming bolt machine from #2-#1 entry from the deck. The T-bar hit the rib he placed his right hand on the canopy to raise himself to see. Just as he did the bolter shifted due to a downhill grade causing the fingers on his right hand to be pinched against the roof. This resulted in lacerations that required sutures. No days were missed however he is restricted duty.
EE had scooped up an insert full of bolts to take to the section. While turning to back out of the crosscut he pinched his left hand and little finger between the deck and the coal rib. This caused a laceration that required sutures. This is to report medical treatment only NO days were missed.
Employee was tramming scoop on roadway he was looking the opposite direction he was traveling. When he turned around to look in the direction of travel the area was lower and he hit his right cheek on a short metal strap causing a laceration that required sutures. During a fireboss run a roof fall was identified in the #8 neutral entry inby spad #6997. The fall was approx. 50' L X 20' W X 4'-5' H. The area was supported by 4' fully grouted resin bolts. The area was mined approx 5/06. Cribs have been set and the area dangered off.
Employee was using a set of come-alongs to raise a metal skirt off the tailpiece. The hook slipped off and struck him in the mouth causing a tooth to be chipped. No days were lost, this is to report medical treatment only.

A roof fall occurred on the N. Mains Section in the face of #8 entry inby spad 10399 for 80'. The fall extended outby the intersection of spad 10399 for approx. 90'. The total area was approx. 170'x20'x8'. Area was mined 1/22/2010 & was supported w/5' fully grouted torq. tension bolts. The area will be partially cleaned to retrieve CM. Roof bolter operator of 25 years has developed pain in both wrists while roof bolting from repetitive motion and vibrations associated with roof bolting and has been diagnosed with carpal tunnel syndome. This is to report an occupational illness.
WHILE CONDUCTING HIS WEEKLY AIRWAY EXAMINATION THE FIRE BOSS IDENTIFIED A ROOF FALL IN THE OLD NORTH MAINS RETURN. THE FALL MEASURED 19'L X 50'W X 4'H. THE AREA WAS MINED APPROX. 12/8/06 ON 80' X 80' CENTERS. THE AREA WAS SUPPORTED WITH 4' RESIN BOLTS. THE AREA HAS BEEN DANGERED OFF AND TIMBERS HAVE BEEN SET.
Foreman/Examiner was traveling the airway on a A.L. Lee 2-man ride. The front tires hit a rut and the steering wheel spun out of his hand and his right hand was struck by the wheel. This resulted in a fracture to the 5th metacarpal bone of his right hand. This is to report medical treatment only, no days were lost.
This is to comply with violation #8075775 at the Josephine #3 Tunnel Mine. On 3/15/09 EE, 3rd shift Fire Boss, found loose rashy material had fallen from the highwall on the inby side of the fan house. The material measured approx. 9'H X 4'-6'L X 9'W.

Roof bolter operator was traming scoop and pulled into a crosscut to turn with his left hand on the scoop swivel bumper block outside the operators compartment. His middle, index and ring fingers were caught between the swivel block and were crushed. The middle and ring fingers were damaged to the base of his finger nails and had to be amputated at the base of the fingernails.
Roof bolter was installing a roof bolt on the rib side and as he put two pieces of steel together a piece of draw rock fell from the mine roof and struck his right middle finger lacerating it. The laceration required (4) sutures. This is to report medical treatment only.
While traveling airways in the old 5Lt. Section with an MSHA inspector a roof fall was discovered at spad #8016 in #3 entry. The fall measured 20'L X 20'W X 4' high. The area was supported with 4' resin bolts. The area has been dangered off and timbers have been set. The area was mined approx. 4/07.
Employee was cleaning spillage in outside tunnel when a belt splice caught his clothing pulling him onto the #5 belt and he was then caught by the belt chute for the outside slate belt. He received contusions and lacerations to his upper back, chest, legs and arms.
Roof bolter operator was bolting in #2 entry on the off side of the machine, when a piece of draw rock 5.5""L X 18""W X 3.5"" thick fell from the top and struck the drill head and then struck him on the right forearm. He required (9) sutures. This is to report medical treatment only.
CMO was place changing the miner from #1 to #2 entry while positioned on the inby side of the intersection in #2 as he tramed the miner backwards towards #3 entry. As the machine went by he placed his right foot between the cable and the head to protect the cable and his foot was pulled under the head. He fell backward and fractured his r. tiba and fibula near his ankle.

A roof fall was discovered in the return on 4Lt. section about 3000' outby near spad #8034 this area was developed approx. 1/10/07. The fall occurred in the #1 & #2 entries and is approx. 4' thick X 20'W by 40'L. The area was bolted with 4' resin bolts. It has been dangered off and cribs set.
As the roof bolter operator lowered his canopy to swing over and set another bolt cap coal measuring approx. 5"" wide x7"" thick X 22"" long fell and struck him in the lower back.
While drilling a roof bolt from the operator side of the fletcher roof bolter the utility man laid his left hand on the drill head. This resulted in his left hand being caught between the drill head and the mine roof. His middle finger was amputated from the tip to the distal joint and his index finger from the tip to the base of his finger nail.
Employee was bending a bolt and he had his body (back) out of position and the end result was a pulled back.
A roof fall occurred in the face of #4 entry inby spad # 8334. The area was not bolted and when it fell it pulled a few rows of bolts out. The area that was pulled out was bolted with 4' double lock bolts. The fall was approx. 40'L x 20'W x 4'to 6' high. The area will be cleaned up and a clean up plan posted in the face area.
A roof fall was found in #6 entry 2500' outby the face, one break inby spad #7993. The fall was 20'W x 40'L x 4' to 6' high. The fall has been dangered off and timbers have been set. This area was bolted with 4' resin bolts and will not be cleaned up.
While making repairs to the #4 bridge; employee struck the back of his right hand on a piece of loose metal causing a laceration. This is to report medical treatment only.

The area was bolted with 4 ft. fully grouted resin bolts. The area has been dangered off and heintzman Jacks and cribs have been placed at access. The area was developed in 1992.
The employee was adjusting the belt hanger chain while the belt was being leveled. He thought the chain link was in the hanger and as the persons assisting him let the belt down his right hand was pulled into the hanger lacerating it. Twelve sutures were required. No Days were Lost.
Employee was using a utility knife to cut a piece of belt when it slipped and struck his left anterior thigh incising it. The resulting incision required 6 sutures. This is to report medical treatment only, no days were lost.
A roof fall was discovered 1 break outby spad 7781 in the No. 4 Entry on the mains section. After intentionally cutting an area that was bolted and sagging the mine resupported part of the area. They cribbed off the inby side, two rows of bolts were taken out by the falling material. The area was 20 ft wide x 10 ft. high x 10ft. long. Area mined on 2/2/07.
While traming a scoop in the #4 entry the operator ran over a crib block and struck his head against the mine roof causing pain in his neck. He continued working and did not seek medical care until 1-15-07. He was diagnosed with a fractured vertabrae (C-4) and is off work effective 1-15-07.

After moving the bolter up in a cut, the operator reached out past the canopy and laid his right hand near the head to help himself get in postion to bolt. A rock fell out and struck the top of his right hand lacerating his middle finger. He received 5 sutures. He was not wearing metacarpal gloves. This is to report medical treatment only. While installing a 6' resin bolt the resin begin to set up before the bolter operator got the bolt fully installed. When he begin to pull the bolt from the hole he caught his rt. thumb between the roof bolt plated the drill area. this resulted in a laceration requiring 5 sutures to case. this is the report medical treatment only.

Employee struck his hand(L) with a hammer while working on a continuous miner. HE DID NOT START MISSING WORK UNTIL 7/23/2005.
Employee injured his back when the scoop that he was operating hit a hole in the roadway. He did not start missing work until 7/18/2005.
EE was pushing a scoop that was stuck in a hole. The bucket of the scoop that EE was operating slid off of the scoop that that was stuck when it broke free, causing EE to strike his head against the scoop's canopy.

EE was loading belt structure onto a sled. He cut the bands on a stack of top rollers & removed a roller. While he turned with the roller, the stack of rollers fell against his right leg.
EE was injured as he was caught between the coal rib and the miner boom. There was a miscommunication between EE and the miner operator.
EE was walking around a scoop when loose cap rock fell. Employee turned suddenly to avoid rock- twisted his knee & fell.
