Stepped out of truck onto gravel lot at mine site. Slipped and fell - felt back pain.
Employee states, injury and other ee were loading the roof bolter, when he cut the bands on the bolts the band srung back striking him on the left hand. He was wearing gloves.
Lifting blocks, to build brattice and pulled his groin. He began missing work on 10/10/08. He had surgery.
While carrying belt structure to track entry, EE got his foot stuck in mud and twisted his back.
6/25/08 EE called to say he was unable to work. He said on 6/23 he was loading the TC sprayer (plaster machine) & liquid shot into face & eyes. I asked about safety glasses. He said he broke them, they were on his head but not on his eyes. His eyes were washed at work. He returned next day though his eyes were a little swollen. Today they're swollen worse & he is going to dr.
Prying on jack pin, slate bar slipped twisting his right shoulder. He fell to the ground on same shoulder. Began missing work on 10/12/08. He had surgery.

Fall occurred at 1:52 am. Fall secured. Located 4 cc iby portal in the intake. Size 17*20*10. Escapeway blocked.

Employee was trimming belt; knife slipped cutting left wrist.
Employee was walking checking belts, stepped on coal or rock and twisted right knee.
The coal hauler had a sharp piece of metal; employee brushed his right hand against it. Went to hospital rec. 5 stitches

NOT SURE, WENT TO DR DUE TO BEING SICK. THEY ASKED IF HE HURT, HE SAID HIS BACK HURT FROM RUNNING HYDRAULIC DRILL. HE THEN TOLD ME IT WAS FROM MOVING BELT STRUCTURE.
HE WAS GATHERING UP OLD SPLICES, CLEANING AROUND HEAD, SLIPPED, FELL, TRIED TO CATCH HIMSELF. HE HIT HIS HAND ON ROCK, CUTTING LEFT HAND.
CUTTING BELT FOR SPLICE, HE CUT HIS LEFT LEG (FOUR STITCHES).
ROOF FALL 6' X 18' X 40' IN FACE AREA OF PANEL. PULLED OFF PANEL. FALL WAS 2 BREAKS INBY 1408.
NORMAL CONDITIONS HE RAISED UP, GOT BACK AGAINST ROOF PLATE, CUT BACK.
EMPLOYEE, SITTING ON A SHUTTLE CAR, TOOK HIS FEET AND PUSHED THE MINER CABLE OUT OF THE WAY. HE SPRAINED HIS KNEE PUSHING ON THE MINER CABLE.

EE WAS DUMPING THE GOB OUT OF THE SCOOP BUCKET IN ORDER TO LOAD SUPPLIES. WHEN HE SWUNG THE SCOOP AROUND HIS HAND GOT BETWEEN THE SWING STOPS OF THE SCOOP.
EMPLOYEE WAS HELPING REPAIR THE R-BOLT MACHINE. HE WAS USING A LARGE WRENCH TO REMOVE A CONNECTION ON THE MACHINE, AND IN ORDER TO DO THAT HE PULLED UP ON THE WRENCH FOR MORE LEVERAGE. WHEN HE DID THAT HE FELT PAIN IN THE GROIN AREA. EMPLOYEE JUST RETURNED TO WORK FROM A SIMILAR INJURY AT ANOTHER JOB SITE. HE RE INJURED AN OLD INJURY.
