EE exited cab of articulating truck after he had raised the bed. He walked behind the truck to hook a chain to a pipe to drag up a hill. A piece of frozen material broke loose and rolled out of the truck bed and struck him in the left knee.

A non-fatal excavator accident occurred on 10/12/07 when the Hitachi 450 Excavator was clearing trees from below and above the high wall. While attempting to push them down, the excavator came too close to the edge and slipped over the high wall.

EE WAS PRESSURE WASHING THE STRUCTURE THEY WERE WORKING ON AND THE SPRAY GOT UNDER HIS FACE SHIELD. THE ALKALINE MIXTURE BURNED HIS FACE.

EE WAS WORKING IN AN ACCESS PANEL AND LOST HIS BALANCE CAUSING HIM TO HIT HIS ELBOW ON CRIBBING. EE WAS TREATED ON 1/4/02 AND RELEASED TO RETURN TO WORK, BUT HE LEFT THE JOB EARLY. WE HAD NOT HEARD FROM HIM AND WERE NOT ABLE TO CONTACT HIM SINCE THAT DATE. I RECIEVED AN OFF WORK SLIP FOR PERIOD 1/8/02 TO PRESENT FROM A DR. TODAY MAKING THE INJURY REPORTABLE.
