Employee was walking belt when a rock fell, knocking timber over on right ankle. This knocked employee into belt. The belt rubbed his left arm & left thumb and the belt rolled him, causing him to twist his back.

Employee was hitting rope dog knocking loose from tight rope; a piece of metal broke off the hammer and flew into his right bicep.

EMPLOYEE STEPPED OFF BOLTER, SLIPPED ON A BOARD, FELL AND HIT BUMPER OF THE BOLTER HITTING ON HIS LEFT SIDE. EMPLOYEE DID NOT START MISSING WORK DUE TO THIS INJURY UNTIL 03/08/2004.

EE WAS GETTING OFF MANBUS; TRIPPED AND FELL ON HIS FACE INJURING HIS NOSE. EE SUSTAINED A NON-REPORTABLE (NOSE) INJURY ON OCTOBER 22, 2003, WHILE AT WORK. HE HAS HAD MINO R OUT-PATIENT SURGERY ON 12-04-03 AT WHICH TIME WE ARE CHANGING THE STATUS OF HIS INJURY TO REPORTABLE.

EE WAS STANDING APPROX 6' FROM RIDE PUTTING TOOLS IN BOX AT THE END OF SHIFT. ANOTHER EE SAT DOWN ON RIDE, SAID THAT THE RIDE JUMPED BACKWARDS HITTING INJURED IN THE BACK OF HIS LEFT KNEE.

MINER WAS SET UP IN #2, EMPLOYEE WAS THROWING MINER CABLE OUT OF WAY IN XCUT WHEN PICKED UP ON TIGHT MINER CABLE, THE MINER MOVED CAUSING THE CABLE TO PUT STRESS ON EMPLOYEE AND HIS KNEE GAVE OUT.

EMPLOYEE WAS DOWN UNDER ROCK BOX 8 TAILPIECE CHANGING EOLLERS WHEN HE PULLED MUSCLE IN HIS RIGHT/ MIDDLE LOWER BACK.

EE WAS SPOTTING 10 FT BOLTS IN TWO WHILE DRILL WITH 2 PIECE STACK STEEL PUTTING ROTATION PRESSURE ONT HE STEEL. THE STEEL FAILED AT IT'S CONNECTION POINT & SPUN OUT STRIKING EE IN THE HAND, BR EAKING HIS HAND AND WRIST. FILED REPORT LATE DUETO BEING LEFT OUT OF OFFICE HOLIDAYS MINES WAS SHUT DOWN.

ROCK FELL FROM THE ROOF & HIT EE IN THE MOUTH. CUT THE INSIDE OF LOWER LIP CAUSING HIM TO GET STITCHES.

EE WAS CARRYING BELT ROLLER TO REPLACE ROLLER ON BELT. HE TRIPPED & FELL ON ROLLER, SPRAINED RT KNEE. 3/4/02 1ST DAY MISSED WORK.

EE STEPPED OFF THE BACK OF THE BOLTER, HIS RT. FOOT LANDED ON A PIECE OF COAL, TWISTING HIS RT. ANKLE.

EEWAS UNLOADING PLATES FROM SCOOP, THE BANDS WERE CUT AWAY FROM THE PALLET. A PIECE OF METAL WAS LACED IN THE PALLET, THE EE DID NOT KNOW THIS, AND WHEN HE PULLED THE PLATES FROM THE PALLET, A PC. OF METAL CUT THROUGH HIS GLOVE INTO THE PALM OF HIS LEFT HAND.

EE WAS TRAMMING MOTOR AND ROCK DUSTER TO DRIF TODUST BELTS. HE THOUGHT THAT HE NEEDED TO OPEN BOTH SETS OF DOORS TO LET THE DUST OUT QUICKER. EE WAS UNLOADING OXYGEN TANK WHEN ONE HAND SLIPPED CAUSING TANK TO FALL ONT HE OTHER HAND SMASHING FINGER.

EE WAS CROSSING TAILPIECE & CRIB WAS LAYING ON TOP OF IT. INSTEAD OF STEPPING OVER THE CRIB BLOCK & SECURING HIS HOLD, HE JUMPED ACROSS THE CRIBTO THE GROUND INJURING HIS KNEE. EE WAS SENT FOR MRI 3-3-01. DR HAS NOT READ REPORT AS OF 3-7-01.
