Fall to the walkway or working surface

Employee slipped and fell to the ground landing on right elbow while collecting water samples at a spillway location. Event was not reported to management until day after he received medical treatment on 3-20-2012.

Employee was walking down road from drill bench to service truck and slipped causing a sprain to his right knee. On the 26th didn't report to work & contacted doctor. Reported back to work on the 27th. Went to emergency room on 28th due to pain and swelling.

Contractor was dismounting Cat D-11 dozer. As he got down to the lift arm, he slipped, fell backwards, struck the ground, injuring back and right wrist, sustaining a fractured thoracic vertebrae and right wrist.

Employee was working on dissassembled bed of a rock truck that was on the ground. He stepped off the bed to the ground, stepped on a rock and twisted his ankle. First aid was applied and he was transported to the hospital. He was diagnosed as having a chip fracture of his right ankle.

Employee was loading his 2007 Mack fuel truck with diesel fuel at the diesel fuel station. He stepped in the cab on the passenger side to warm his hands. As he got out of the vehicle, he slipped on a step, suffering a compound fracture to his right leg.

While moving a Cat. D11 single flange roller onto a pallet, the middle finger of the right hand was pinched between the roller shaft and another roller causing a 3 cm laceration in the finger. Employee received 9 stitches and returned to work following the medical treatment.

EMPLOYEE WAS LIFTING A TABLE BUSHING OUT OF THE MECHANIC'S TRUCK. BUSHING SLIPPED & CAUGHT HIS FINGER BETWEEN THE BUSHING & THE TRUCK BUMPER.

Employee was working on dozer and felt a pop in his right shoulder. Went to doctor next day and diagnosis was rotator cuff injury.

Employee was helping lift a piece of feeder chain when he felt a sharp pain in chest and lower back. Employee had suffered a severe burn injury of the upper chest from an earlier accident.
