EE was unlocking a gate. When EE unlocked gate the chain got stuck in the gate. EE pulled on chain to release it and chain stuck EE's middle finger causing it to bruise. EE went to hospital and had finger x-ray the finger was not broke. Hospital put a splint on finger and released EE with no restrictions

On May 26, 2015 this mine was notified that this employee has been diagnosed with pneumoconiosis. This employee worked from 12-05-1990 to 03-04-2013 when he was terminated.

Fall from machine

As employee was descending the front steps of the truck his hand slipped from the handrail and he fell backwards to the ground, landing on his hips.

Employee was diagnosed with occupational hearing loss. Employee last worked December 18, 2013. Company was notified on 11/10/2014 of the claim.

Employee was checking oil levels in hydraulic tank. When he bent over and raised up, his foot slipped causing him to fall from the deck of the bulldozer, landing on the ground.

Employee was operating a 785 rock truck when the boom of the 992 G loader struck the side of the 785 rock truck's bed, shaking the employee causing back and shoulder pain.

Employee slipped and fell on black ice while walking to guard shack to begin his shift.

Employee has been diagnosed with a degree of Black Lung. This employee worked from 10-5-1998 to 9-30-2010 when he retired. On 11-21-2012 this mine was notified that he was diagnosed with pneumoconiosis.

Fall from machine

Employee was getting out of the rear of a service truck when he had one foot on the bumper and the other on the rear step. He slipped & fell striking his head on a concrete pad used to service equipment.

Employee was diagnosed with pneumoconiosis on 3-5-2014. Company was not notified of this event until 6-20-2014.

Employee was attempting to turn on his water valves on the side of his dozer engine when his finger came in contact with the fan belts resulting in the amputation of the right index finger at the 1st joint.

Working on installing a power link pin in the bucket at RH200 shovel. He had a 1400# power link pin secured to boom of his service truck & was swinging the bin with the boom on his service truck to the power link & he got his lt little finger caught between the pin & power link of machine while guiding the pin in the hole of the power link & mashed the end of lt little finger.

Over-exertion in wielding or throwing objects

EE used an 8lbs hammer and struck a ratchet to break a volt loose. When he did he felt pain in his right shoulder. EE worked until 8/27/10 and had surgery for a torn rotator cuff. It was initially diagnosis as a shoulder sprain. I am classifying this as an occupational illness due to the report provided by the treating physician. See attached.

While trying to set an anchor pin for the highwall miner, the pin shifted, pinning the miner's hand between the pin and a box on the miner.

EE released a chain ratchet which had tension on it. When the ratchet released it resulted in a piece of metal striking his left index finger.

Employee was spooling his cable back on his trucks boom, when he used his left hand to guide the cable. He got his left index finger caught with the cable, causing a skin amputation to the tip of his left index finger.

EE was in the process of giving his pre-shift to the fueler, when he fell to the ground from his dozer. He received 3 fractured ribs and a laceration to the right side of his head, requiring 2 staples.

Employee was doing his normal duties working on hooking beams together when he noticed a catch in his back and he had trouble straightening up in an upright position.

EE was cutting trees on a slope. While walking between trees, he slipped on an unseen rock. He fell onto the chain of his saw, saw brake was engaged and chain was not moving, cutting his left arm.

Employee cut a tree. As the tree was falling, it hit another tree breaking a limb out of the cut tree. When the limb hit the ground, it came back and hit him in the arm.

Employee was striking a wear plate on the shovel bucket. When he struck the plate a piece of metal broke and struck him in the right upper leg.

Employee cut a tree. When the tree fell it kicked up a stick that came back and hit him in the face shield. The face shield cracked and hit him in the eye.
